Effects of a Remote Sensory Integration Home Program on the Visual Seeking and Stereotyped Behavior of Children with Autism Spectrum (원격 감각통합 가정프로그램이 자폐스펙트럼장애 아동의 시각추구와 상동행동에 미치는 효과)

  • Objective : This study investigated the effect of a remote sensory integrated family program on the visual seeking and stereotyped behavior of children with autism spectrum disorder. Methods : This study was conducted as a single-subject study, a mid-baseline design for three children with autism spectrum disorder, and the intervention was conducted 4 times a week for 4 weeks. All three children confirmed the changes in visual seeking and stereotyped behavior after the intervention. Results : After the intervention, visual seeking and stereotyped behavior decreased compared with the baseline period. Visual seeking decreased on average after 7.59% at intervention, and stereotyped behavior decreased by 6.81 on average. Conclusion : Through this study, it was confirmed that the remote sensory integration home program is effective in visual seeking and stereotyped behavior of children with autism spectrum disorder. Addionally it is useful as a new intervention method a program that allows parents to participate in the intervention process. and it is emphasized that occupational therapists should create and promote cooperative relationships for children intervention.

Effects of a Tactile-Oriented Sensory-Motor Developmental Program on the Visual Attention and Stereotypical Behaviors of an Adolescent with Intellectual Disability (촉각중심 감각운동발달 프로그램이 지적장애가 있는 청소년의 시각적 주의집중과 상동행동에 미치는 효과)

  • The purpose of this study was to investigate the effects of a tactile-oriented sensory-motor developmental program on the visual attention and stereotypical behavior of an adolescent with intellectual disability. The subject was an adolescent with intellectual disability attending a special school located in B-gun, A-province of South Korea, with whom an A-B-A' design individual target study was conducted between August 27 and December 12, 2018. The intervention consisted of twelve 40-minute sessions, which were provided twice a week. For every session, two types of evaluation were conducted: a test that required staring at objects for visual attention, as well as an observation record for identifying the frequency and duration of stereotypical behavior. The results of this study showed that the tactile-oriented sensory-motor developmental program had positive effects on the duration of the participant's ability to stare at objects and the frequency and duration of stereotypical behavior. As this study confirmed and usefulness of this tactile-oriented sensory-motor developmental program as an intervention for improving the visual attention, and stereotypical behavior of adolescent with intellectual disabilities, the program may be employed to help members of this groups who have problems with sensory processing.

뇌중 dopamine 활성에 작용하는 뇌기능개선 약물의 효능검색

  • 뇌기능개선 약물들(nootropic drugs)은 항치매약물로 기대되나 현재까지 약리학적으로 공통점이 보고되어 있지는 않다. 최근 수종의 뇌기능 개선약물의 작용이 중추도파민신경 활성을 매개로 함이 보고된바 있다. 따라서 apomorphine 투여에 의해 유발되는 상동행동을 지표로 하여 현재 사용중인 뇌기능개선약물과 국내 자원생약 성분의 작용을 비교 검토하였다. 1. apomorphine 투여에 의하여 유발된 상동행동에 대하여 centrophenoxine 과 N-methyl -D-glucamine은 현저한 상승효과를 나타내었으나 Piracetam은 영향을 미치지 않았다. 2. 인삼사포닌 성분 중 panaxatriol 및 Rgl 성분은 현저한 상승효과를 나타내었으나 total saponine, panaxadiol 및 Rbl 성분은 거의 영향을 미치지 않았다.

일측성선조체의 6-OHDA손상 후 도파민효능약물 투여로 발현된 회전운동의 특성

  • 본 실험은 신경독성물질을 이용하여 선조체도파민신경찰성을 충분하게 손상시키고 도파민효능약물투여 후 유발되는 특정행동이 basal ganglia에서 발생되는 진행성이며 퇴행성신경질환인 파킨슨씨 질환(Parkinson's disease : PD)의 연구에 기여할 수 있는지의 타당성을 검토하고자 하였다. 흰쥐의 왼쪽선조체에 6-OHDA 8,16 and 24$\mu\textrm{g}$/2${\mu}\ell$(in 0.1% ascorbic acid)를 각각 투여하여 효율적으로 상동행동을 발현하는 신경독성물질의 투여 용량을 검토하고 도파민신경에 작용하는 약물의 투여방법(반복투여), 관찰기간(수술후 1주부터 4주간) 및 투여시기(7,21 및 35주령)에 따른 행동발현의 특성을 비교검토하고 아울러 성별의 영향도 검토하였다.

Behavioral Characteristics Following Lesions of the Nucleus Accumbens Septi in Rats (측좌각의 부위별 파괴가 행동에 미치는 영향)

  • Caudal, rostral and both areas of the nucleus accumbens septi (NAB) were lesioned each in separate animal group by applying DC of 3.0 mA for 15 sec. in order to examine if any regional differences in the NAB with regard to the manifestation of locomotor activity, The r-NAB and cr-NAB-lesioned rats were significantly increased in locomotor activity but not in the c-NAB-lesioned rats and the effect following the NAB lesion was immediately produced and returned to control levels in about 7 days postoperatively. On the other hand, the locomotor stimulation produced by methamphetamine was significantly attenuated in all NAB-lesioned rats but also stereotyped behavior was significantly elicited simultaneously. These results thus suggested that NAB may be attributed to the inhibitory role in the locomotor activity, of which intensity differs from rostral site and caudal site, and stereotyped behavior may be shown higher sensitivity of the denervated striatal dopamine function.

Alterations of Binding Capacities of Dopamine Receptors After Treatment with Haloperidol and Sulpiride in Rat Brain (Haloperidol 및 Sulpiride 투여후 백서 뇌내 Dopamine 수용체 결합력의 변화)

  • The effects of chronic treatment with haloperidol and sulpiride on the binding capacities of dopamine(DA) receptor were examined in rat striatum and olfactory tubercle. Additionally, the stereotypy scores were assessed after apomorphine administration. Rats were treated with haloperidol(0.5mg/kg/day) or sulpiride(40mg/kg/day) for four weeks. Apomorphine(0.5mg/kg) was injected after three-day washout from neuroleptics, and stereotypy scores were assessed. Haloperidol group showed high scores of stereotyped behavior in comparison with control and sulpiride groups. With control group, sulpiride group displayed similar stereotyped behaviors. Saturation analysis of the binding of [$^3H$]spiperone to striatal membranes showed that the Bmax of haloperidol and sulpiride groups increased significantly in comparison with that of control group. The $K_D$ decreased significantly after sulpiride treatment in striatum. Although sulpiride produces the same proliferation of DA receptor, the low stereotypy scores of sulpiride group indirectly suggest that sulpiride acts differently from haloperidol in brain DA system. The Bmax increased remarkably following both treatment with haloperidol and sulpiride in olfactory tubercle. Also, the increase in $K_D$ was significant after treatment with haloperidol and sulpiride in olfactory tubercle. Moreover, the $K_D$ of control group in olfactory tubercle was more than twice the $K_D$ of control group in striatum. The $K_D$ was 86.2 in striatum and 37.5 pM in olfactory tubercle. The present finding indicates that sulpiride also induces the proliferation of DA receptor in olfactory tubercle and may interact with some DA receptor subtype with high affinity profile. The different affinities of the control groups of striatum and olfactory tubercle suggest that striatal DA receptor subtypes labeled by [$^3H$]spiperone could differ from those of olfactory tubercle.

Development of the self-diagnosis system for initial stage of developmental disability (발달장애 초기 자가 진단 시스템 개발)

  • Although developmental disabilities account for a relatively low number of the total number of disabilities, they are generally classified as severe disabilities considering the degree of disability. If these developmental disorders are discovered early, adaptability and early treatment efficiency can be improved, but most parents do not detect any signs from their children or miss the right time for treatment. In this paper, we conducted development of the developmental disorder diagnosis algorithm that can recognize hand-flapping, one of the early unusual behaviors of developmental disorders, for parents and early childhood care workers who cannot recognize signs of early developmental disorders based on specific behavioral characteristics as a pilot study. It was confirmed that the recognition area and fingers were accurately recognized, and the number of hand flapping was accurately counted. It is expected that research on algorithms that can diagnose various behavioral patterns will continue to be conducted and expanded all through algorithms advancement and expansion of functional performance using big data.

  • Autism is characterized by pervasive impairments in social interaction, verbal and non-verbal communication and restricted and stereotyped behavior. It is difficult for autism patients to receive dental treatment as lack of cooperation, so various behavior management method have been tried for dental treatment in clinic. The patients with mild autism can be managed by conventional behavior modification method and phamaco-logical conscious sedation. But the patients with severe autism and multiple dental caries can be treated by deep sedation or general anesthesia. The benefits of children's dental care in general anesthesia are full-mouth rehabilitation in one single appointment. We reported three cases of autism patients who were treated by various behavior management method as to severity of autism.

THE EFFICACY AND SAFETY OF RISPERIDONE IN CHILD & ADOLESCENT PSYCHIATRIC INPATIENT (소아 청소년 정신과 입원 환자에서 Risperidone의 효과 및 안정성에 관한 연구)

  • Objective : The purpose of this study was obtaining data on the efficacy and safety of risperidone in child and adolescent psychiatric patients. Method : Thirty one children and adolescents (males n=18, females n=13, age ranged from 5.4 to 17.3 years) treated with risperidone were selected among child and adolescent psychiatric inpatients of Seoul National University Hospital from January, 2001 to June, 2002, and charts for them were reviewed retrospectively. Results : The primary psychiatric disorders treated with risperidone were schizophrenia and other psychosis, bipolar I disorder with psychotic features, Tourette's disorder, autism spectrum disorders, mixed receptive and expressive language disorder, attention deficit-hyperactivity disorder, conduct disorder and obsessive-compulsive disorder. twelve of these had comorbid mental retardation. Primary target symptoms of risperidone were psychotic symptoms (n=13 or $41.9\%$), behavioral symptoms (n=10 or $32.3\%$) including aggression, impulsivity, hyperactivity, stereotypy nonresponsive to other psychiatric treatments, and chronic and severe tics (n=8, $25.8\%$). The efficacy of risperidone was measured by clinical global improvement (CGI) for target symptoms, $67.7\%$ of subjects showed moderate or marked improvements and its therapeutic effect appeared to be maintained during at least 7.5 months. Mean daily dosage of risperidone was $0.05{\pm}0.01mg/kg$, the group with psychotic symptoms had significantly higher mean daily dosage (0.07mg/kg) compared with other two groups (0.04mg/kg) with behavioral symptoms or tics. A variety of adverse events were reported in this study : weight gain (n=23) most commonly reported, extrapyramidal symptoms (n=15), autonomic symptoms (n=6), sedation (n=5) and symptoms related to hyperprolactinemia (n=2) etc. Although there was no drug change related to the adverse events of risperidone, and $90\%$ of subjects at their last visits were maintained on it, thus its tolerability appeared good. Conclusions Results suggest that risperidone may be relatively safe and effective drug in managing a wide variety of child and adolescent psychopathologies such as psychotic symptoms, behavioral symptoms including aggression, impulsivity, hyperactivity and stereotypy nonresponsive to other psychiatric treatments, and chronic and severe tics. Controlled and long-term studies of efficacy and safety of risperidone treatment for children and adolescents are recommended in the future.

A STUDY ON COMORBID DISORDERS AND ASSOCIATED SYMPTOMS OF PERVASIVE DEVELOPMENTAL DISORDER CHILDREN (전반적 발달장애 아동들의 공존질환 및 동반증상에 대한 연구)

  • Objective:The purpose of this study was to investigate the prevalence and characteristics of comorbid disorders and associated symptoms in pervasive developmental disorder(PDD) and to examine the correlation between associated symptoms and developmental characteristics in PDD children. Method:The sample consisted of 209 cases of PDD and 143 cases of developmental language disorder(DLD)(control group) who were treated at the Seoul National Mental Hospital from Jan. 1996 to Mar. 1999. The diagnostic work based on DSM-IV criteria was performed by one or two child psychiatrists, while the clinical feature was evaluated by doctors’s notes, occupational/speech therapy reports, and results of social maturity scale(SMS), childhood autism rating scale(CARS), and psycho-educational profile(PEP). Two groups were compared on a wide range of measures including comorbid disorders, associated symptoms, treatment drugs, and PEP. The relation between associated symptom & PEP was investigated in total(106 cases) and in each dignostic group. Sixty-four cases of PDD were divided into three groups by CARS and then compared on associated symptoms. Result:The prevalence of comorbid disorder was 19.6% in PDD, 41.2% in DLD. The rate of manifestation of 13 associated symptoms was 31.47% in PDD, 22.13% in DLD on the average. Associated symptoms significantly high in PDD were preoccupation, obsession, self-mutilation, stereotypy, sleep problems, and odd response. In total patient group, associated symptoms that significantly influenced PEP were preoccupation, self-stimulation, stereotypy, inappropriate affect, sleep problems, and odd response. But, in each diagnostic group, no associated symptom influenced PEP. Associated symptoms significantly different between the 3 groups of CARS were stereotypy, anxiety, and sleep problems. Conclusion:These preliminary results suggest that developmental characteristics may influence associated symptoms in PDD children and a realistic approach considering minute diagnosis by associated symptoms and comorbid disorders is required.
