Data were collected with structured questionnaires from 147 patients and 147 nurses from a university hospital in J city, Gyeongnam, from July 24 to September 23, 2019, and analyzed using SPSS 23.0. The result showed that the patient's safety perception was significantly higher than the nurse's perception. The significant factors affecting patient management activity were nurse's educational level, position, effective communication, patient safety perception, and perception of importance on patient safety management. • The purpose of this study is to analyze the difference of medical care between medical assistance and health insurance patients to evaluate the increase of medical care costs due to the moral hazard of medical care patients and to provide a basis for rational medical care policy decision. For this purpose, we compared health insurance benefit data for Seoul citizens by gender, age, and type of medical institutions. The results of the analysis are as follows. First, all of the hospitalized and outpatient use of the advanced general Hospitals, medical assistance patients were less than those of the health insurance patients, so that the medical assistance patients could not use the high cost medical services. Second, in general hospitals, patients with health insurance are often hospitalized. On the other hand, medical assistance patients use a lot of outpatient services because they are less burdened. Third, in hospitals and clinics, medical benefits patients often use inpatient and outpatient services. Therefore, medical assistance patients are likely to use unnecessary medical care of outpatient and hospitalization clinics and hospitals, outpatient of general hospitals. But, in hospitalization and outpatient use in advanced general hospitals and medical assistance patients can not use due to excessive medical burden. Therefore, the policy to reduce the burden of medical expenses for patients with severe illness will continue, and the medical care patients using clinics and hospitals should be careful not to use unnecessary medical services.

  • Korea's health care delivery system is based on the Medical law and the National Health Insurance Act. In order to efficiently operate limited medical resources, it is classified to use medical institutions according to the severity of the disease. The question is whether a tertiary care hospital can terminate a medical contract for a patient, if treatment for severe diseases has already been performed at a tertiary care hospital. In the case of termination of treatment, the Korean court has both a judgment that recognizes the right to terminate and a judgment that denies the termination. Among the U.S. rulings, there are rulings that order transfer to a skilled nursing facility or nursing home or home if acute treatment is no longer needed. Considering that medical resources are limited, it is necessary to acknowledge the right to terminate the contract of the medical institution when treatment by a medical institution is completed.

  • We examine the change in healthcare utilization pattern of cancer patients since the launch of KTX in 2004. It is found that during the year of 2004 and 2005 the rapid increase in the ratio of cancer patients' healthcare utilizations for the tertiary hospitals in Seoul to the total healthcare utilizations has accompanied the rapid decrease in the corresponding ratio for the tertiary hospitals in the local metropolitan areas under the influence of KTX while the corresponding ratio for the tertiary hospitals in the local cities with little influence of KTX on them has exhibited mild change. Since healthcare consumers' choice of hospitals can be characterized by "foot voting" action in the healthcare service market in Korea, such a phenomenon may have strong implication that the introduction of KTX may have affected substantially the inter-area healthcare utilization pattern by cancer patients by reducing various sorts of long-distance travel costs. Therefore, considering the potential contribution of KTX to the increase of cancer patients' accessibility to hospitals with higher qualities, support policies such as fare discount for low-income cancer patients in local areas may need to be taken into consideration where the well-designed fare discount program for low-income cancer patients may increase utilizations of KTX by lowincome cancer patients without incurring additional costs to running of KTX.

  • This study provides fundamental data in order to obtain improved medical services and competitiveness by analyzing the satisfaction level of patients receiving inpatient services at general hospitals. Data from self-administered surveys distributed to inpatient subjects at local general hospitals was collected from 09/19/2011 to 09/30/2011. Firstly, the results of 320 surveys show that the average level of satisfaction from inpatient services was 3.25 (maximum 4.0). Secondly, the results from general characteristics and satisfaction from medical care provider services showed that there is a statistical significance regarding medical treatment, health condition, the number of times admitted to the hospital, and hospital environment including hospital admittance/release proceeders. Also the satisfaction with other services is statistically significant with regards to age, medical treatment, health condition, and the number of times admitted to the hospital. Thirdly, there is a statistically significant positive correlation between inpatient services and levels of satisfaction. Finally, from the analysis of factors influencing inpatient satisfaction showed that physician services and other services significantly affect satisfaction. In conclusion, in order to increase inpatient levels of satisfaction the workforce involved medical treatment of patients need to be retained and a variety of programs need to be in operation that will satisfy patients while they stay in the hospital.
