Dysphagia Risk and Associated Factors among Community-Dwelling Elders (지역사회 재가노인의 삼킴장애 위험실태와 관련인자)

  • The present study investigated the risk and prevalence of dysphagia as well as related factors among free-living elders. Subjects were 419 elderly men (116) and women (303) aged 65 years and older ($74.49{\pm}4.70y$) living in Seoul, Gyeonggi-do and Chungcheongbuk-do area in Korea. Data were collected by personal interviews using questionnaires and analyzed by descriptive statistics, t-test, and chi-square test. Prevalence of dysphagia was 53.50% among participants. Risk of dysphagia was significantly different according to age (P<0.001), use of dentures (P<0.05), and activities of daily living (P<0.05). Moreover, dysphagia risk group had significant food intake problems, including poor appetite, smaller portions to reach satiety, frequent meal skipping, as well as poor taste compared to the normal group, accompanied by high risk of weight loss as assessed by simplified nutritional appetite questionnaire (P<0.01). Taken together, risk of dysphagia was more prevalent in older people, affecting the majority of those living independently in the community. This could indicate an association with undernutrition due to factors affecting food intake.

Characteristics of the Oropharyngeal Swallowing Impairment in Stroke Patient using the Modified Barium Swallowing Impairment Profile (MBSImP에 따른 뇌졸중 환자의 삼킴 장애 양상 분석)

  • Swallowing impairment is a frequent complication following stroke. The characteristics of swallowing impairment with stork patient can facilitate identification of individuals at risk of dysphagia would be of great helpful. The present study examined oropharyngeal swallowing impairment with subacute stroke patients using the Modified Barium Swallowing Impairment Profile(MBSImP). The 49 consecutive patients with the supratentorial stroke met inclusion criteria for the present study. A retrospective review was performed of patients who underwent the videofluoroscopic swallowing study(VFSS). Of Stroke patients, 95.9% exhibited abnormal function of lip closure. 98% and 57.1 % shown abnormal tongue function and lingual motion, respectively. Oral residue was present in 51% and delayed pharyngeal response was present in 89.9%. In addition, abnormal laryngeal and hyoid excursion was seen in 42.9% and 87.8%, respectively. Abnormal function of soft palate elevation was present in 18.4% and abnormal epiglottic movement was seen in 4.1%. 30.6% of 30.6% of these patients exhibited abnormal laryngeal closure. All of the stroke patients(100%) in this study exhibited abnormal pharyngeal stripping wave and pharyngoesophageal segment opening. Abnormal tongue base retraction and oral reside were present 91.8%, respectively. The results suggest that stroke patient is more likely to exhibit reduced swallowing functions including lip closure, tongue control, initiation of pharyngeal swallow, anterior hyoid motion, tongue base retraction, pharyngeal residue and pharyngoesophageal contraction. Therefore, these data could provide valid and precise information regarding physiological evidence to delineate symptoms of dysphagia in this stroke cohort. Future studies should explore the bolus effect in the swallowing impairment.

A Convergence Study of Surface Electromyography in Swallowing Stages for Swallowing Function Evaluation in Older Adults: Systematic Review (노인의 삼킴 단계별 삼킴 기능 평가를 위한 표면 근전도 검사의 융합적 연구 : 체계적 문헌고찰)

  • In this study, a systematic review was conducted to analyze the method of applying sEMG to evaluate the swallowing function of the elderly at each stage of swallowing, and to help objectively measure the swallowing stage of the older adults in clinical practice. From 2011 to 2021, 7 studies that met the selection criteria were selected using Pubmed, Scopus, and Web of Science (WoS). As a result of this study, the older adults and adults were divided into an experimental group and a control group and the swallowing phase was analyzed using sEMG only for the older adults. sEMG was used to evaluate swallowing in the oral and pharyngeal stages, and the sEMG attachment site was attached to the swallowing muscle involved in each stage. The collected sEMG data were filtered using a bandpass-filter and a notch-filter, and were analyzed using RMS, amplitude, and maximum spontaneous contraction. In this study, it was found that sEMG can be used as a tool to objectively and quantitatively evaluate the swallowing function in stages. Therefore, it is expected that this study will activate various studies that incorporate sEMG to evaluate the swallowing function in stages.

Effect of Oral Health Care on Swallowing Function and Swallowing-Quality of Life in Patients with Dementia (치매환자의 구강관리가 삼킴기능과 구강관련 삶의 질에 미치는 영향)

  • The purpose of this study was to evaluate the status of oral health care in dementia patients living in long-term care facilities and to investigate the effects of this on swallowing function and swallowing-quality of life. The subjects were 60 demented patients who were admitted to long-term care facilities in Gwangju and Jeonnam provinces. Their Oral Health Assessment Tool(OHAT), Clinical Dysphagia Scale(CDS), and Oral Health Impact Profile 14(OHIP-14) were measured. The results of this study were as follows: First, the effect of oral health care on dentition patients using long-term care facilities on swallowing function and swallowing-quality of life was significantly different according to oral care score (p<.05)(p<.01). Second, the relationship between oral health care and swallowing function of dementia patients and swallowing-quality of life. was examined. As a result, oral health care of dementia patients was affected by swallowing function (r=.405, p<.01) the swallowing-quality of life (r=.462, p<.01) and swallowing function were significantly correlated with swallowing-quality of life (r=.351, p<.01). Therefore, the oral health care of users of long-term care facilities was closely related to swallowing function and swallowing-quality of life.

  • 삼킴장애는 음식물이 입에서 식도로 가지않고 걸리거나 기도(Trachea)로 흡입되는 문제를 갖는 상태이다. 특히 노인이나 신경계 질환을 앓는 환자의 경우 기도로 흡입된 음식덩이가 폐렴을 일으키고 결국에는 사망으로 이어지기에 적절한 치료와 관리가 요구된다. 보통 영상으로 판단할 수 있는 삼킴단계는 구강준비단계(Oral Preparatory Phase), 구강단계(Oral Phase), 인두단계(Pharyngeal Phase), 식도단계(Esophageal Phase) 4가지로 분류하고 삼킴장애는 침습(Penetration)과 흡인(Aspiration)으로 크게 2가지로 분류한다. 본 논문에서는 이러한 6가지 클래스를 가지는 삼킴장애 환자 비디오 파일을 라벨링하기 위한 웹 애플리케이션을 제안한다. 이를 구현하기 위해서 대용량 멀티프레임 이미지를 수신해서 분리하여 저장하도록 개발하였다. 또한 음식덩이를 정교하게 분할할 수 있도록 GrabCut 알고리즘을 적용하여 라벨링할 수 있도록 하였다. 차후 라벨러와 전문의 간의 협업이 가능하도록 라벨링 데이터의 상태를 관리할 수 있도록 개발하고자 한다.

The Effects of Orofacial Exercises Program using Smart Phone on Swallowing Function and Tongue Strength in Acute Stroke Patients with Dysphagia (스마트폰을 사용한 구강안면 운동 프로그램이 삼킴장애가 있는 급성 뇌졸중 환자의 삼킴기능과 혀 근력에 미치는 효과)

  • The aim of this study was investigated the effect of orofacial exercise program using smart phone on swallowing function and tongue strength in acute stroke patients with dysphagia. This study participated in sixteen acute stroke patients with dysphagia. All subjects allocated that randomized each eight patients in experimental and control groups. Subjects of both group received to conventional dysphagia therapy during 30 min/day, 5 times per week, for 4 weeks. Experimental group performed additionally that orofacial exercise program using smart phone, supervised under caregivers, during 30 min/day. The outcome measures were the IOPI(: Iowa Oral Performance Instrument), VR(: Vellecula Residue), PSR(: Pyriform Sinuses Residue), MIP(: Maximum Isometric Pressures). In results of study. Both group showed significant improvements after intervention in all assessments(p<.05). In comparison of change score between two groups, experimental group showed significant improvements than control group in PSR and MIP(p<.05). Orofacial exercise program using smart phone suggested that expected to positive effects the reduction of residue in pharynx and improvements of tongue strength in acute stroke patients with dysphagia.

The Impact of SWAL-QOL on Overall Quality of Life in Normal Elderly People: Focusing on Jeonbuk Province (정상노인의 삼킴관련 삶의 질(SWAL-QOL)이 노인의 전반적인 삶의 질에 미치는 영향-전북 지역을 중심으로-)

  • The purpose of this study was to investigate the quality of life related to swallowing through Swallowing-Quality Of Life(SWAL-QOL) and examine the impact of SWAL-QOL on overall quality of life measured by Short-Form Health Survey(SF-36) and Geriatric Quality of Life(GQOL). For this purpose, the SWAL-QOL, SF-36 and GQOL have been conducted on 140 old people who were all over 60 years of age and physically and mentally normal. The collected data have been analyzed using a Windows SPSS 21.0 program. The results show that the SWAL-QOL are significant differences according to age(F=7.76, p<.01) and aspiration(t=-4.44, p<.01) and aspiration is ouly factor affecting the SW AL-QOL in elderly people. There are significant correlations between SF-36, GQOL and SW AL-QOL and the pearson product moment correlation coefficient is .39(p<.01) and .33(p<.01) respectively. The SWAL-QOL can predict approximately 36% and 25% of quality of life measured by SF-36 and GQOL. Therefore, this result will be able to utilize as basic data to improvement of quality of life related swallowing and overall quality of life related to health in normal elderly people.

Comparison of Chin Tuck Against Resistance and Shaker Exercise on Suprahyoid and Sternocleidomastoid Muscle Activity in Stroke Older Patients with Dysphagia (삼킴 장애가 있는 뇌졸중 노인 환자에서 저항성 고개 숙이기 운동과 Shaker 운동의 목뿔위 및 목빗근 활성 비교)

  • Chin Tuck Against Resistance Exercise(CTARE) and Shaker exercise is used to common for strengthening of suprahyoid muscle. No previous studies was compare of two exercises for patients with dyaphagia. This study investigated the effects of CTARE and Shaker exercise on suprahyoid, sternocleidomastoid muscle activity in stroke older patients with dysphagia. Ten stroke patients with dyaphagia voluntarily participated in this study. All subjects was performed in the order in the CTARE (isometric, isokinetic), Shaker (isometric, isokinetic), and repeated each ten trials. After CTARE was performed, subjects took a 5 min wash out period to minimize muscle fatigue. Activity of suprahyoid and sternocleidomastoid muscle during two training was analyzed using surface electromyography(sEMG). Wilcoxon signed rank test was used to assess differecences for muscles activity between the effects of the CTAR and Shaker exercise within group. CTARE and Shaker exercise showed no significant difference activity in suprahyoid muscle(p > .05). CTARE showed significantly lower muscle activity in sternocleidomastoid muscle than Shaker exercise(p < .05). CTARE in stroke older patients with dysphagia may be a effective intervention to improve swallowing function than Shaker exercise.

Intervention of Swallowing Disorder in Dementia Patient: A Systematic Review (치매환자의 삼킴장애 중재에 관한 체계적 고찰)

  • Purpose : The purpose of this study was to establish scientific evidence though a systematic review, of the existing interventional methods and their respective effects, for swallowing disorders among dementia patients. The interventional methods were based on patient characteristics. Methods : An electronic literature search was performed using five electronic databases from 2010 to 2020. The keywords were 'Dementia', 'Alzheimer disease', 'Deglutition disorder', 'Swallowing disorder', 'Dysphagia'. Five articles met the inclusion criteria. Results : One study with moderate-to-severe and four studies with severe dementia patients were included. The interventions consisted of one remedial intervention, three compensatory interventions, and one comprehensive intervention. Compensatory intervention, including behavioral and psychological management, were used in moderate-to-severe patients. For the severe patients, remedial, compensatory, and comprehensive intervention were used to address the feeding tube problem. The interventions used in each study had a positive effect on the outcome variables associated with swallowing ability. Conclusion : The purpose of the intervention and the intervention factors changed depending on the patient's dementia stage. The intervention was effective when considering the progressive stage of the patient. These results may serve as a guide in designing effective interventions for swallowing disorders in dementia patients.

Voice Change Associated with Swallowing Disorder Caused by a Stroke After Neuromuscular Electrical Stimulation (뇌졸중으로 인한 삼킴장애 환자의 경부근육전기자극치료에 따른 음성 변화)

  • The purpose of this study was to look into changes in voice using acoustic analysis during the process of neuromuscular electrical stimulation targeting dysphagia treatment. Fifteen man with dysphagia caused by stroke was treated neuromuscular electrical stimulation for two months and intensity of voice, $F_0$, Jitter, Shimmer, NNE were measured. The results of this study that improvement in functions of dysphagia and Jitter, Shimmer were stabilized. But there was not significantly changes of $F_0$. NNE was improved after the intervention, but still showed abnormal levels. This result suggests a possibility of effects that Neuromuscular electrical stimulation has on stabilization of Jitter, Shimmer and intensity of voice.
