A Case of Tracheobronchopathia Osteoplastica (기관기관지골형성증 1예)

  • Tracheobronchopathia osteoplastica(TPO) is a rare disorder characterized by submucosal cartilaginous or bony projections into the tracheobronchial lumen with sparing of the posterior membranous portion of tracheobronchial tree. The etiology of TPO is still unknown. A 44-year-old male was admitted to Seoul Paik Hospital Inje University due to left chest pain for 10 days. On the past history he had sufferred from symptoms of bronchitis for several months. He showed radiologically massive pleural effusion in left lung field. Pleural biopsy revealed chronic pleuritis with hemorrhage. Bronchoscopic findings showed multiple intraluminal portruding nodule from just below the vocal cord to carina and both main bronchi. Pathology of bronchoscopic biopsy showed abnormal proliferation of atypical bony and carilagious nodules in the tracheal submucosa. We experianced a case of tracheobronchopathia osteoplastica involving the trachea and main bronchus in 44-year old male, associated with massive pleural effusion. This report is a case of TPO with review of literature.

A Case of Pseudochylothorax Developed from Chronic Pleural Effusion after Treatment of Tuberculous Pleurisy (결핵성 흉막염 치료 후 잔류된 만성 흉막삼출에서 발생한 가성유미흉 1예)

  • A pseudochylothorax, a chyliform pleural effusion, is a rare disease of pleural effusion that contains cholesterol crystals or high lipid content that is not the result of a disrupted thoracic duct. Most of the cases were found in patients with long-standing pleural effusion due to chronic inflammatory disease, such as old tuberculous pleurisy or chronic rheumatoid pleurisy. We experienced a case of pseudochylothorax in a 74-year-old man, who was being treated for pulmonary tuberculosis and pleurisy 10 years ago. The diagnosis was confirmed on pathological study of the pleural effusion, which contained cholesterol crystals having a diagnostic rhomboid appearance.

  • Paragonimiasis is the infestation of lung flukes of the trematode genus Paragonimus. This disease is common in Asia, and the southern part of Korea has been known as one of the endemic areas of Paragonimiasis westermanii in Korea. Human infection is associated with specific dietary habits, such as eating freshwater crawfish or crabs. In a 6 1/2-year-old boy with pleural effusion and eosinophilia, paragonimiasis was diagnosed by skin test, serologic exam, and histologic identification of the parasites in a skin lesion. The same diagnosis was entertained in his elder sister with silent pleural effusion. We describe herewith these rare cases of paragonimiasis in two siblings who had a history of eating cooked freshwater crabs.

  • 이 연구의 목적은 악관절 내장증 환자에서 관절원판의 형태를 자기공명영상사진으로 평가하고 임상적 증상과의 관계를 알아보고자 하는 것이다. 악관절 내장증의 증상을 가진 90명의 환자에서 169개의 관절을 자기공명영상사진으로 관절원판과 하악과두의 관계를 기준으로 정상군, 정복성 관절원판 변위군, 비정복성 관절원판 변위군으로 분류하고 관절원판의 형태를 5가지로 나누었다. 임상검사는 측두하악관절통과 관절음의 유무를 확인하여 그 상관관계를 알아보았다. 실험 결과에서 측두하악관절통은 각 군에서 관절원판의 형태와는 정상군을 제외하고는 통계적인 유의성이 없었으며, 염증성 삼출액과 유의한 관계를 보였다. 관절음은 정복성 관절원판 변위환자에서 관절원판형태와 통계적으로 유의성을 보였으며, 비정복성 관절원판 변위환자에서도 상당수 관절음이 발생하였음을 알 수 있었다. 이 결과는 측두하악관절의 통증은 관절원판의 형태가 많이 변형될수록 통증이 심해지는 경우가 많았지만, 다른 여러 가지 원인들이 존재할 수 있으며, 관절음은 정복성 관절원판 변위환자에서 관절원판의 변형이 심할수록 관절원판이 정복할 때 발생될 수 있다는 것을 의미한다. 따라서, 임상적인 검사와 더불어 자기공명영상사진으로 정확한 진단을 함으로써 관절원판 변위의 적절한 치료가 행해질수 있으리라 생각된다.

A Deep Learning Model for Judging Presence or Absence of Lesions in the Chest X-ray Images (흉부 디지털 영상의 병변 유무 판단을 위한 딥러닝 모델)

  • There are dozens of different types of lesions that can be diagnosed through chest X-ray images, including Atelectasis, Cardiomegaly, Mass, Pneumothorax, and Effusion. Computed tomography(CT) test is generally necessary to determine the exact diagnosis and location and size of thoracic lesions, however computed tomography has disadvantages such as expensive cost and a lot of radiation exposure. Therefore, in this paper, we propose a deep learning algorithm for judging the presence or absence of lesions in chest X-ray images as the primary screening tool for the diagnosis of thoracic lesions. The proposed algorithm was designed by comparing various configuration methods to optimize the judgment of presence of lesions from chest X-ray. As a result, the evaluation rate of lesion presence of the proposed algorithm is about 1% better than the existing algorithm.

A Case Report of Korean Medicine Treatment for Pleural Effusion due to Metastasis of Breast Cancer (유방암의 폐전이로 인한 흉막 삼출의 한의치료에 대한 증례보고)

  • The survival time of breast cancer patients with lung metastasis has been prolonged as treatment methods have improved. However, there is yet no definitive therapy for the additional symptoms of lung metastasis to improve the quality of life of these patients. We report a patient who was suffering from pleural effusion due to metastasized breast cancer. She was treated with Korean medicine, including herbal medicine (Bojungikgi-tang-gami), combined with hormone therapy, for two months. We assessed her other symptoms, such as shortness of breath, generalized weakness, and lower extremity swelling, using a numeric rating scale (NRS). We also evaluated pleural effusion with chest X-rays. Shortness of breath was maintained at NRS 5, generalized weakness was slightly improved from NRS 6~7 to NRS 7~8 (10=healthy condition), and lower extremity edema was improved from NRS 3 to NRS 2 (10=highest score of discomfort). Malignant pleural effusion was maintained during the admission period. This report suggests that Korean medicine can help to maintain malignant pleural effusion and improve additional symptoms of lung metastasis.

  • Immunoglobulin G4-related disease (IgG4-RD) is a chronic inflammatory condition involving multiple organs, including the salivary or lacrimal glands, orbit, pancreas, bile duct, liver, kidney, retroperitoneum, aorta, lung, and lymph nodes. It is histologically characterized by tissue infiltration with lymphocytes and IgG4-secreting plasma cells, storiform fibrosis, and obliterative phlebitis. In the thoracic involvement of IgG4-RD, mediastinal lymphadenopathy and perilymphangitic interstitial thickening of the lung are the most common findings. Peribronchovascular and septal thickening and paravertebral band-like soft tissue are characteristic findings of IgG4-RD. Other findings include pulmonary nodules or masses, ground-glass opacity, alveolar interstitial thickening, pleural effusion or thickening, mass in the chest wall or mediastinum, and arteritis involving the aorta and coronary artery. Radiologic differential diagnosis of various malignancies, infections, and inflammatory conditions is needed. In this review, we describe the imaging findings of IgG4-RD and the radiologic differential diagnoses in the thorax.

Tuberculous Pericarditis Mimicking a Malignant Pericardial Tumor: A Case Report (악성 심막 종양으로 오인한 결핵성 심막염: 증례 보고)

  • Tuberculous pericarditis is an extrapulmonary manifestation of tuberculosis that is most commonly associated with pericardial thickening, effusion, and calcification. We present a case of tuberculous pericarditis mimicking a malignant pericardial tumor in a 77-year-old male. CT revealed an irregular and nodular pericardial thickening. MRI revealed high signal intensity on T1-weighted fat-suppressed images and peripheral rim enhancement after gadolinium administration. MRI can be helpful in determining the differential diagnoses in cases of tuberculous pericarditis with nonspecific imaging findings.

  • Electronic cigarette or vaping-associated lung injury (EVALI) is a disease defined by lung injuries caused by e-cigarette use. It predominantly manifests in forms of organized pneumonia or diffuse alveolar damage but rarely as acute eosinophilic pneumonia (AEP). This report describes a 34-year-old male with acute respiratory symptoms and a vaping history of only nicotine. Chest CT revealed peripheral distributing multiple patchy consolidations and ground-glass opacities dominant in both lower lobes, bilateral diffuse interlobular septal thickening, and bilateral pleural effusion without cardiomegaly. Bronchoalveolar lavage fluids showed increased eosinophilia levels, while infectious laboratory results were all negative, enabling the diagnosis of both AEP and EVALI. Herein, we report a rare case of only-nicotine vaping EVALI manifested as AEP.

Usefulness of echocardiographic findings in the early diagnosis of Kawasaki disease (가와사끼병의 조기 진단에서 심장초음파검사의 유용성)

  • Purpose : The early diagnosis of Kawasaki disease (KD) is difficult sometimes, especially in atypical or incomplete cases presenting 4 or less principal clinical features without coronary arterial lesions (coronary arterial ectasia or aneurysm). The authors investigated the incidence of echocardiographic abnormalities in patients with KD to discover whether abnormal echocardiographic findings might be helpful in the early diagnosis of KD. Methods : Echocardiography was done in the acute stage of 103 patients with KD (Kawasaki group) and 40 patients with other acute febrile illnesses (control group). Abnormal echocardiographic findings were classified into 4 categories and defined as follows; 1) significant pericardial effusion, 2) significant valvular dysfunctions, 3) left ventricular systolic dysfunction, 4) coronary arterial ectasia or aneurysm. Results : In the Kawasaki group, significant pericardial effusion was present in 24 patients (23.3 percent), significant valvular dysfunctions in 30 patients (29.1 percent), left ventricular systolic dysfunction in 10 patients (9.7 percent), and coronary arterial lesions in 27 patients (26.2 percent). In the control group, significant pericardial effusion was present in only one patient (2.5 percent). The number of patients with any one of echocardiographic abnormalities was 57 (55.3 percent) in Kawasaki group and one (2.5 percent) in control group. The sensitivity of echocardiography in the diagnosis of KD was 55.3 percent, the specificity 97.5 percent, the positive predictive value 98.3 percent, and the negative predictive value 45.9 percent. Conclusion : If abnormal echocardiographic findings, even if other than coronary arterial lesions, are confirmed in patients in whom KD is suspected, it seems desirable to inifiate specific treatment for KD.
