An Ethnographic Study of Sanhubyung experienced by Women in Korean Postpartal Culture (한국의 산후 문화와 여성이 경험한 산후병에 관한 일상생활기술적 연구)

  • This ethnogrphic exploratory study sought to de-fine the meaning of Sanhubyung, as consequence from the perspective of the women who experienced it. A convenience sample of 9 elderly women in San Francisco. and 20 postpartal women and their 20 non-professional helpers during postpartum in Seoul, Korea were observed and interviewed for 23 months from January 1991 to December 1992 at the Human Development Center in San Francisco and at the hospital and their homes in Seoul, Korea. Sanhubyung was regarded as the consequence of "Doing a Sanhujori Wrongly," as a group of symptoms or sequelae which have two types of characteristics of symptoms . chronic and acute. It can be called a culture bound syndrome in the cultural context re-lated to childbearing phenomenon in Korea. If women violate the principles of Sanhujori, such symptoms can appear at various times : during the period of postpartum itself, at any time, periodically, especially at the anniversary of the child's birth, late forties, and in old age. Acute symptoms that can be classified into immediate and late types include painful and edematous gingiva, sensitive teeth, strange sensation and pain in the knees or backache. Besides, there is a localized sense of soreness and pain ; sense of being in a draft and cold, stomach upset, GI irritation, chilling, shivering, and tiredness, pain and dazzling in the eyes. Chronic symptoms occur in the head, neck, teeth, back, hands, knees, hands and feet, arms and legs, eyes, sinews and joints, bones, and in the body or as a whole. Generally these symptoms are pain, often accompanying a feeling of being cold and in a draft, regardless of actual weather conditions. In conclusion, this findings reflect the Oriental way of thought of causal relationship of women's health and illness based on the wholistic paradigm of harmony and balance of two forces, Yin(cold) -Yang(hot). It provides a challenge to the professional sector to rethink the effect of culture on health and illness. Finally, it suggests care providers use cultural assessment for the appropriateness of the intervention and quality of care for desirable health outcomes.

Three cases of postpartum low back pain patients treated by heating-conduction acupuncture therapy (전열침을 적용한 산후 요통환자 치험 3례)

  • Objectives : The purpose of this study is to evaluate the effect of the heating-conduction acupuncture therapy on the postpartum low back pain patients. Methods : Heating-conduction acupuncture therapy was administered three and four times on three postpartum low back pain patients. The improvement of clinical symptoms was evaluated by SF-MPQ(short form-McGill pain questionnarie), VAS(visual analogue scale) and ODI(oswestry disability index). Results : After treatment, each three patients' SF-MPQ, VAS and ODI score reduced respectively. Conclusions : Heating-conduction acupuncture therapy for postpartum low back pain patients was effective. Although this cases presented valuable result, further research is encouraged to confirm the effectiveness of this treatment.

The Changing Pattern of Physical and Psychological Health, and Maternal Adjustment Between Primiparas Who Used and Those Who did Not Use Sanhujori Facilities (산후조리원 이용여부에 따른 초산모의 신체적, 심리적 건강상태 및 모성역할적응의 변화양상에 관한 연구)

  • Purpose: The purpose of this study was to compare levels of postpartum fatigue, depression, childcare stress, and maternal identity according to postpartum period between primiparas who used Sanhujori facilities and those who did not. Methods: The research design was a longitudinal descriptive study using self-report questionnaires. Participants were 55 healthy primiparas who delivered at one of 3 hospitals in Chungnam, 21 using Sanhujori facilities and 34 not using these facilities during the first three weeks after childbirth. Data were collected from October 2008 to April 2009 at three measurement points, 2-4 days after childbirth (T1), 4-6 weeks (T2), and 12-14 weeks (T3). Data were analyzed using the SPSS 17.0 WIN program. Results: There was a significant difference in childcare stress between the two groups at 4-6 weeks after childbirth. Postpartum depression and childcare stress at 4-6 weeks were significantly higher than those of the other postpartum periods, while maternal identity was significantly lower. Conclusion: Child care stress is the most important issue among women who use Sanhujori facilities and the 4-6 week period after childbirth is very difficult to primiparas. These results indicate that nursing interventions for primiparas in Sanhujori facilities should focus on reducing childcare stress. Furthermore proper follow-up programs at 4-6 weeks are needed to decrease the difficulties in adjustment by new mothers.

The Effect of Parenting Stress and Depression and Fatigue on Quality of life in Early Postpartum Mothers (산욕기 산모의 양육스트레스, 산후우울, 피로가 삶의 질에 미치는 영향)

  • The study was investigate the effect of quality of life of mothers in the postpartum period. This study was a descriptive research study of 113 women who lived in D metropolitan area within 6-8 weeks after birth, Data were analyzed using SPSS 21.0 program ANOVA, t-test, Pearson's correlation analysis and multiple random analysis. Data collection was from August 2018 to September 2018. The results were as follows. The most influential factors on quality of life were parenting stress(${\beta}=-.646$, p =.000), fatigue(${\beta}=-.329$, p =.009). It is necessary to develop a nursing intervention program related to physical and mental health because maternal quality of life leads to lower quality of life when feeling emotional mood change such as emptiness and depression.

  • Acute puerperal uterine inversion is a rare postpartum obstetric complication; however, without rapid diagnosis and appropriate management, it is life-threatening. Substantial bleeding hinders the verification of a partially inverted uterus, possibly delaying the treatment. Herein, we present the report of a 32-year-old female presenting with massive postpartum bleeding managed by uterine artery embolization. The peculiar course of the uterine artery bowing inferiorly along the inverted fundus during embolization could uncover the uterine inversion, which was not diagnosed by physical examination and CT. In conclusion, uterine artery embolization is not only an effective therapeutic strategy for postpartum hemorrhage but also a valuable tool for diagnosing uterine inversion.

Concept Analysis of Sanhujori and Tool Development (산후조리 개념분석 및 도구 개발)

  • As one of the important purpose of the concept is mutual communication, the concept should be defined clearly concerned concept, differed from others concept, identified intersubjectiveness, and could communicated readily and easily between researcher and practitioner through concept analysis. By analysing and synthesizing the concept of "Sanhujori" according to concept analysis of Walker and Avant. The author has tried to establish the term "Sanhujori" which gas been widly used from the past till now in Korea as a scholarly terminoloy and to develop a tool for the assessment of "Sanhujori". The conceptional attributes of the "Sanhujori" were figured out such as (1) hospitality; (2) physical and emotional recovery; (3) caring; (4) maternal role attainment. The assessment tool for the "Sanhujori" was constituted of 40 items of the conceptional attributes and tested on the 30 mothers more than 8 weeks of postpartal period. The reliability was that Cronbach's $\alpha$ is .9238.

Simulation-based education program on postpartum hemorrhage for nursing students (산후출혈 산모 간호 시뮬레이션 교육 프로그램의 효과)

  • Purpose: This study was conducted to develop a simulation-based postpartum care education program for women with postpartum hemorrhage and to verify the effects of the program on postpartum care. Methods: This program was developed according to the ADDIE model of instructional system design, which consists of analysis, design, development, implementation, and evaluation phases. This quasi-experimental study used a non-equivalent control group pre- and post-test design, and data were collected from April 23 to May 4, 2015. To verify the effects of the program, 33 nursing students in the experimental group participated in a simulation program, whereas 31 students in the control group were given a case study. Results: The experimental group had statistically significantly higher scores for clinical performance (t=-4.80, p<.001), clinical judgment (t=-4.14, p<.001), and learning satisfaction (t=-10.45, p<.001) than the control group. Conclusion: The results of this study indicate that the simulation-based postpartum care education program for women with postpartum hemorrhage was effective for developing students' competency, implying that a similar program should be integrated into the clinical training component of the maternal nursing curriculum.

Canonical Correlation between Korean Traditional Postpartum Care Performance and Postpartum Health Status (산후조리 수행과 산후 건강상태간 정준상관관계)

  • Purpose: The purpose of this longitudinal study was to examine the relationship between postpartum care performance and postpartum health status. Sample: The study subjects were 82 mothers who delivered full-term infants at 3 hospitals at P city. Data were collected for their health status at the postpartum unit and the sample was followed up to 6 weeks postpartum to collect postpartum care performance and health status. Results: Mothers rated postpartum care performance as moderate to high and especially rated the maternal role attainment the highest. Mothers experienced 4 physical symptoms and moderate levels of fatigue. In addition, they experienced moderate levels of positive affect and low levels of negative affect at both times. Canonical correlation revealed that ostpartum care performance was related to postpartum health status with 2 significant canonical variables. The first variate indicated that mothers who performed hospitality, physical and emotional recovery, self-caring, and role attainment well showed higher positive affects, lower negative affects, fewer physical symptoms, and lower levels of fatigue. The second variate showed that the greater the performance of caring and physical and emotional recovery, the fewer physical symptoms and lower levels of fatigue. Conclusion: Although Korean traditional postpartum care performance was related to postpartum health status, the further study is needed to identify the causal relationship between them. Nurses need to integrate the perspective of westernized postpartum care and Korean traditional views of postpartum approach to maintain and promote women's health.
