  • Modern society gradually change into diversification and specialization, the field of medical service also is specialized in these days. According to changes, A medical institution stress the need of women's min space of the concept "Well Women Clinic" Therefore, this study aims to propose improve way and architectural data after analysis the developing circumstances and problem on the architectural planning on the ward of women's hospital by case studies on the ward of women's hospital builded recently in korea.

A Validation Study on the Translated Korean Version of the Edinbergh Postnatal Depression Scale (한국판 에딘버러 산후우울측정도구의 타당성 연구)

  • Purpose: The Edinbergh Postnatal Depression Scale (EPDS) has seen widespread use in epidemiological and clinical studies. The objective of this research was to translate and to test the reliability and validity of the 10-item EDPS in Korea. Methods: Subjects were 145 women who were 6 to 10 weeks post delivery. To test reliability, the internal consistency was assessed by Cronbach's alpha coefficient. Validity testing was performed using convergent validity by Pearson's correlation. Data was gathered at 3 hospitals during Oct. 2005 after obtaining an informed consent. Result: The mean age of the subjects was 29.9 years. and the mean parity was 1.46 times. The mean EPDS score was 18.4. Reliability analysis showed a satisfactory result (Cronbach's alpha coefficient=.84). There were significant positive correlations between EPDS and Mills postnatal depression and anxiety check list scores as expected(r=.69, p<.001) and thus supported convergent validity. Conclusion: The study findings showed that the Korean version of the EPDS-10 is a reliable and valid instrument that can be used for measuring postnatal depression in Korea.

Relationship Between Postpartum Depression and Body Image in Postpartum Women (분만 여성의 산후 우울과 신체상의 관계)

  • The purpose of this study was to identify the change and difference and relationship of postpartum depression and physical image. Method: The subjects consisted of 86 postpartum women at one general hospital in Seoul. The data was collected from September to November 2001. The instrument used for this study were SRD (Self-Rating Depression Scale) and Norris' Body Image Scale to evaluate depression and body image. The collected data was analyzed with frequency, mean, t-test, paired t-test, ANOVA and Pearson's correlation coefficient. Result: The result of this study were as follows: 1. The mean score of D2 was significantly higher than D1(p=.003). There was no difference significantly B1 and B2(p=310). 2. There was significant correlation between the two, D1-D2(r=.381, p<.01), B1-B2(r=.364, p<.01), D1-B1(r=.579, p<.01), D2-B2(r= .567, p<.01). (*D1: depression of postpartum 1-3days, D2: depression of postpartum 6-8weeks, B1: body image of postpartum 1-3days, B2: body image of postpartum 6-8weeks) Conclusion: There was very high postpartum depression in postpartum women, but body image was positive. Also, there was correlated to postpartum depression and body image. Thus it is necessary to implement nursing intervention focused on to decrease the postpartum depression and to enhance the body image of the postpartum women.

Experiences of Sanhujori Facility Use among the First Time Mothers by the Focus Group Interview (포커스 그룹 연구방법을 적용한 초산모의 산후조리원 이용 경험 분석)

  • Purpose: The purpose of this study was to examine the experiences of Sanhujori facility use among the first time mothers in Korea. Methods: A qualitative study was conducted, using focus group interview. Data were collected from the 24 first time mothers of 4 focus groups, who had given birth within 6 month and had used one of the Sanhujori facilities located in C province, Korea. After obtaining written informed consent from all participants, each session of the focus group was audio-taped and transcribed into verbatim. Data were analyzed using content analysis in order to identify significant themes. Results: Four major themes that emerged from the data were as follows. 1) Promoting postpartum physical recovery through a enough time with only focusing on herself, 2) Promoting postpartum psychological recovery through emotional and informational support with peer mothers, 3) Experiencing breast feeding difficulties and disappointing with unsatisfied help from health professionals, and 4) Lack of the professional education programs regarding parenting. Conclusion: Based on these results, it will be suggested that the various support programs by not only the peer mothers co-resided in Sanhujori facilities but also the health care professionals in the Sanhujori facilities should be developed for helping a "becoming a mother" of the first time mother in the Sanhujori facilities. In addition, qualified education and counseling program, especially for the successful breast feeding, should be provided by the health care professionals for improving mothering ability of the first time mother in the Sanhujori facilities.

Husbands' Awareness of Sanhujori, Needs for Education and Family Strength (남편의 산후조리 인식, 교육요구도 및 가족건강성에 관한 연구)

  • Purpose: We conducted a descriptive study to: 1) understand how husbands are aware of the importance of Sanhujoiri(i.e, Korean traditional postpartum care) and 2) identify their needs to learn how to take care of postpartum women and newborns and to strengthen family bonding. Methods: The subjects consisted of 123 husbands who had children under the age of 5 years. Data were analyzed with t-test, one-way ANOVA, $Scheff{\acute{e}}^{\prime}s$ test, and Pearson's correlation. Results: The mean age of subjects was $37.1{\pm}4.63$ years. The mean scores of awareness, needs for education on postpartum mother and newborn care, and family strength were $4.57{\pm}1.18$, $4.61{\pm}1.18$, $4.92{\pm}1.07$, and $4.01{\pm}0.95$, respectively, indicating higher scores. There were statistically significant differences in awareness (F=5.08, p<.05), newborn care (F=3.70, p<.05), and family strength (F=4.64, p<.05) by husband's role in Sanhujori There was a positive correlation among study variables. Conclusion: This study shows that even though husbands want to participate in Sanhujori, they do not have enough information on Sanhujori and mother/newborn care and adequate paternity leave. Realistic paternity leave system and effective Couple Centered Childbearing (from pregnancy to postpartum) Education Program CCCEP development are required to help husbands' participation in mother/newborn care with confidence and competence in home based Sanhujori.

Postpartum Depressive Score and Related Factors Pre- and Post-delivery (산후 우울 수준과 분만전후 관련 요인에 관한 연구)

  • Purpose: This study aimed to identify the scores of postpartum depression(PPD) on the first day, 1st week, and 6th week after the delivery and to explore their related factors before and after delivery in postpartum women. Methods: With a survey design, 293 postpartum women were recruited from a postpartum unit, Ilsin Christian hospital in Pusan via convenience sampling and were followed at 1st week and 6th week in the outpatient clinic. Results: Results showed that the scores of PPD(EPDS score) were low at postpartum 1st day, 1st week and 6th week but prevalence of PPD(EPDS ${\geq}13$)was 3.1%at 1st day, 8.2%at 1st week and 7.5%at 6th week, respectively. The pre-delivery factors were experience of depression, and the post-delivery factors were baby's sex(1st day), no caregiver for baby(1st week), and no help and concern for taking care of baby from husband and family(1st day and 6th week). The greater satisfaction with becoming a mother and her life, and greater maternal attachment were related to lower level of PPD at the three time points. Conclusion: Regular screening for postpartum depression and supportive and informative education is needed for postpartum women visiting the outpatient clinic for follow-up.

A study on Functional Status of Women after Childbirth (산후 여성의 기능적 상태에 관한 연구)

  • Recovery of functional status and affecting factors it were examined at 1 and 2 months after delivery in 111 women who defined as the resumption of household, self-care, social & community, and occupational activities, and assumption of infant care responsibilities. The data were collected by Tulman & Fawcett (1988)'s IFSAC questionnaire at 2 local hospitals & health center in Kwangju city & Chonnam province. The results were as follows : 1. Total mean scores of functional status were 2.3 points and increased in total functional status between 1 & 2 months after childbirth. The analysis revealed significant changes in 3 dimensions-household, social & community, and occupational activities-of IFSAC between 1 & 2 months after childbirth. 2. Mean scores of 5 dimensions of IFSAC were : self-care activities, 1.9 points : household activities, 2.7 points ; infant care responsibilities, 3.6 points ; occupational activities, 1.5 points ; and social & community activities, 1.6 points. 3. The job affected significantly the household, occupational activities and infant care responsibilities. And the complication of infant or mother affected significantly the infant care responsibilities in association with recovery functional status. Based on the findings and a review of the literature in regard to our understanding of recovery of functional status, the following recommendations were derived. 1. Future research needs to be policied concerning length of maternity leave after delivery on a firmer basis throught longitudinal study. 2. The data also nay be used to develop individual interventions to facilitate recovery from childbirth. 3. The IFSAC may be used to clinical assessments of functional status in the case of women cancer or obstetric and gynecologic operation.

Effects of Fatigue and Postpartum Depression on Maternal Perceived Quality of Life (MAPP-QOL) in Early Postpartum Mothers (초기 산욕기 산모의 피로와 산후우울이 지각된 삶의 질에 미치는 영향)

  • Purpose: This study was done to identify effects of fatigue and postpartum depression on quality of life in early postpartum mothers. Methods: The data were collected from 130 mothers at four general hospitals in J and M metropolitan cities. Instruments used to collect the data for the study were the Fatigue Scale developed by Pugh (1993); Postpartum Depression Scale developed by Cox, Holden & Sagovsky (1987), and the Quality of Life Scale developed by Hill, Aldag, Hekel, Riner, G., & Bloomfield (2006). Results: Results showed that the mean for fatigue was 56.74, the mean for postpartum depression was $8.00{\pm}4.37$ and mean for quality of life was 19.78. The quality of life variable showed statistically significant differences for the variable: age (F=3.20, p=.026). The relationship between fatigue and quality of life showed a significant negative correlation (r=-.44, p<.001). The relationship between postpartum depression and quality of life also showed a negative correlation (r=-.42, p<.001). The relationship between postpartum depression and fatigue showed a positive correlation (r=.59, p<.001). These factors explained 23% of the variance in quality of life. Conclusion: The results indicate that it is necessary to develop nursing intervention programs to improve quality of life in for early postpartum mothers.

Effects of Education on Primiparas' Postpartial care (초산모를 위한 산후간호 교육의 효과)

  • The study was to find whether the educational program contributed to increase of knowledge and self-efficacy of the postpartal primiparas. This study aimed at improvement of the educational effect for postpartal primiparas. The Subjects were 34 primiparas who were admitted to the obstetric ward in a University hospital from November 15th to December 9th, 1999. The Subjects were those who had no labor pain at the admission time, had no complications during labor and delivery and, gave birth to a healthy baby. They were tested on knowledge and self-efficacy two times, one at the admission time and prior to discharge. After the first test nurses in a maternity ward taught them on postpartal care. Two tools were developed by authors based on literature review. The test tool fr knowledge of postpartal car consisted of 23 items. The test tool for self-efficacy of postpartal care consisted of 16 items. Analysis of demographic data were analyzed with calculation of percentage. Score differences between the first test and the second test were analyzed with paired t-test. The Spss (Win 8.0) program was used for data analysis. The results are as follows. 1. There were not significant influencing general characteristics of primiparous to pre-educational knowledge. There were significant influencing general characteristics of primiparous to post-educational knowledge : occupation(t=13.04, p=0.00), postpartal education(t=5.51, p=0.02). 2. There were not significant influencing general characteristics of primiparous to pre-educational self-efficacy. There were significant influencing general characteristics of primiparous to post-educational self-efficacy : antenatal education(t=5.53, p=0.02) 3. Primiparas' knowledge of postpartal care increased significantly after education(t=13.04, p=0.00). 4. Primiparas' self-efficacy of postpartal care increased significantly after education(t=5.51, p=0.02). 5. Correlation between knowledge and self-efficacy was r=.360(p=0.03). We suggest follow-up studies to find whether primiparas' self-efficacy will last after discharge or not.

Types of Breastfeeding and its Predictors of Mothers in Twenty-four Months after Birth (산후 24개월 이내 어머니의 수유형태와 예측요인)

  • Purpose: This study was done to investigate the types of breastfeeding visits by mothers twenty-four months after birth and to identify the factors that increased breast feeding rates. Methods: The subjects of this study totaled seven hundred and seventy-four mothers with infants aged 2 years or less who had visited national medical institutions including Sanhujori service facilities, breastfeeding care service facilities, and community health centers. Data was collected from June 22 to July 31, 2009. Data was analyzed using descriptive statistics, $x^2$ test, ANOVA with a post hoc Scheffe test, and multiple logistic regression. Results: Major findings of this study were significant differences of planned length of breastfeeding and breastfeeding self-efficacy in relation to the differing types of breast feeding community visits by mothers. In multivariate logistic regression, baby's age, exclusive breastfeeding at hospital, utilization of breastfeeding care service facility, planned length of breastfeeding, and breastfeeding self-efficacy were independent predictors of the feeding type. Conclusion: In order to increase breastfeeding rates, programs provided by public health care services and medical facilities should start education on breast feeding in the hospital before mothers are discharged and then continue through the use of Sanhujori service facilities and workplace at 3 month and 6 month postpartum.
