PSR 모델을 적용한 근로환경조사 지표 개발 (A Study on Health-related PSR Model using Korean Working Conditions Survey Data)

  • 본 연구에서는 업종에 따른 다양한 근로환경 요인들을 파악하고 관련 분야의 산업안전보건 정책을 판단할 수 있도록 근로환경조사 기초자료를 분석하여 사회요인을 고려한 지수 및 지표 개발을 목표로 하였다. 지표개발을 위해 OECD에서 제시하는 지표개발 가이드라인과 해외의 지표개발 사례들을 벤치마킹하여 수행되었다. OECD의 압력-상태-대응 모형을 벤치마킹하여 건강과 관련 있는 지표들을 제시하였다. 근로환경조사의 압력-상태 건강관련 지표는 압력은 물리적 위험환경, 노동시간, 업무환경, 사회 환경으로 구성되었고, 이에 따른 상태 항목은 건강관련 항목으로 정신건강, 신체건강, 건강문제로 인한 결근, 업무 만족으로 구성되었다. 그 결과 임금근로자, 상용근로자, 관리자, 사무종사자, 전문가 및 관련 종사자, 50세 미만 근로자의 압력-상태 지표가 상대적으로 양호하였다. 반대로, 50세 이상, 자영업자, 일용 직, 농림어업 숙련직, 단순노무, 장치 기계 조립 근로자의 압력-상태 지표가 상대적으로 취약한 것으로 나타났다.

종합병원 일부 행정직원의 우울과 관련된 융복합적 요인 (A Study on the Convergent Factors Related to Depression among Some Administrative Staff in General Hospital)

  • 종합병원 행정직원의 우울수준(CES-D: Center for Epidemiologic Studies Depression scale)과 관련된 융복합적 요인들을 조사하였다. 설문조사는 2017년 7월 3일부터 2017년 7월 29일까지 임의로 선정된 J지역 9개 종합병원의 재직자 201명에 대하여 무기명 자기기입식 설문지를 사용하였다. 위계적 다중회귀분석 결과, 자아존중감(RES: Rosenberg Self-Esteem Scale)이 낮을수록, 피로(MFS: Multidimensional Fatigue Scale)가 높을수록, 사회심리적 스트레스(PWI-SF: Psychosocial Well-being Index Short Form)가 높을수록 우울이 높았으며 이들의 설명력은 32.5%이었다. 이상의 연구결과를 볼 때, 종합병원 행정직원의 우울수준을 낮추기 위해서는 자아존중감을 높이고, 피로 및 사회심리적 스트레스를 낮추는 노력이 필요하다. 이러한 결과는 종합병원 행정직원의 우울수준을 낮추는 조직인사관리 및 산업보건교육에 활용될 수 있다. 추후연구에서는 종합병원 행정직원의 우울수준에 영향을 미치는 구조방정식 모형의 분석이 필요하다.

서울지역 산업장의 보건관리 (A Study on the Health Care Management of Industries in Seoul)

  • The objective of this study was to analyze the status of health examination, and the health care management of industries in Seoul. This will provide effective industrial health care services. 46 industries were selected as a sample, and were given self administered questionnaire developed by the academic affairs of community health nursing. The results of this study are as follows: (1) The general characteristics of industries: The greater part of the industries were manu facturing companies, 300-900 workers, in a one shift system. (2) The general characteristics of health man ager: 96% of the companies had industrial nurses and the larger industries had comprehensive health managers. 90% of the industrial nurses were full-time employees and 77% of the industrial physician were part time employees. 80% of the industrial physician were age 50 or older, while 59% of the industrial nurses were 25-29 years of age. (3) Health examination and follow up care: The proportions of the workers who took general health examination was 94%. Of those, 12% took secondary examinations. In secondary examinations, 36% were evaluated at the C-grade, 20% were evaluated at the D-grade. The proportion of the workers who were required to take special examination were 29% and of that, the proportion of the workers who actually took the examination was 88%. 85% of the workers who were recommended for follow up cares were given follow up care while working. The physical agents among the special examination were done the most frequently to detect the examination effects. (4) Health care services: Industrial nurses performed health examination most frequently and health education was done the least. There were significant differences in health care services according to the size of the industries. Companies with 300 workers or less had higher health care services than other groups. The three primary reasons of health care management and examinations of workers are: First, industrial nurses are requires to accumulate professional knowledge and experience through continuos' activities. Second, systematic and concert examination for industrial workers should be performed periodically. Third, it needs developing health education strategies that are important workers for health promotion in industries.

산업장의 건강검진과 보건관리실태에 관한연구 -부산 지역을 중심으로- (A Study on The Periodic Medical Examination, and Health Care Management Programs of Industries -Busan City Province-)

  • The Purpose of this study was to explore the condition of periodic medical examination and the health care services of industries in order to offer some basic data on developing industrial nursing care. To achieve this goal a self - administered questionnaire (developed by the academic affairs of community health nursing) was provieded to the nurses in 56 industries from Dec. 10, 1992 to Jan. 20, 1993. The statistical computer package, SPSS, was used to manipulate the data along with T-test and ANOVA. The results were as follows : 1. General characteristics: The greater part of the industries were manufacturing company, and below 300 employees of industry were 55.4%. The shift system was mostly one shift(66.1%) and three shift(23.2%), and 50.0% them organized the Industrial Safety and Health Commitee. Average a number of employees was 631 person. 2. Periodic Medical Examination: Most of the workers were receiving periodic medical examination from the designated hospital (95.71%). From the about 12.89% were gone through a colse medical examination. In colse medical examination 58.41% were decided 'C' and 36.73% were decided 'D'. About 6.23% off those who had any clinical findings were work-time shortening(7.84%), work-transition(8.12%), recoverating at home. The majority of the workers receive the result of the periodic medical examination individually (78.5%). 3. Special medical examination: The rate of those who are receiving special medical examination were 76.82% and about 8.24% were decided 'C' and 1.23% were decided 'D' Those who had any health problems were receiving follow-up checking (9.10%) and medical treatment while working (15.04%). The health managers in the company can consult (85.7%) those who had any suspicious sign and symptoms of occupational disease. 4. Health care services: The average score of health care services were 17.57 point out of 28 point, and the score was lower in health assessment and environmental hygiene than medical diagnosis and health education. There were significant differences in environmental hygine (F=3.72, P=0.017), health care services(F=3.94, P=0.013) according to the size of the size of the industries The other's significancy is not shown by any type of industrial nurse. The level of health care services were higher in the wokers who had better health and showed no singificant differences(T=-0.73, P=0.470).

산업장의 특성별 건강검진과 보건관리 -광주.전남지역- (A Study on the Periodic Medical Examination, and Health Care Management Programs in Industries -Kwang-Ju city and Chonnam Province-)

  • The purpose of this study was to explore the condition of periodic medical examination, and health care services in industries. This will offer some basic data in developing industrial nursing care requirements. To achieve this goal a self-administered questionnaire (developed by the academic affairs of community health nursing) was provided to the nurses working in 40 industries from Dec 20, 1992 to Jan 20, 1993. The statistical computer package SAS, along with t-test, and ANOVA was used to manipulate the date. The results were as follows : 1. General Characteristics: The greater part of the industries studied were manufacturing company, with over 500 employees. The shift system was used with most companies using one or three shifts, and 75.0% of them were organized with Industrial Safety and Health Committees. 2. Periodic Medical Examination : Most of the workers were receiving periodic medical examination from a designated hospital (96.7%). Of those 15.8% had a close medical examination. In the medical examination 9.4% were evaluated at 'C' and 3.8 were evaluated at 'D'. About 55.0% of those workers received the result of the periodic medical examination individually(95.0%). 3. Special Medical Examination : The rate of those who were receiving special medical examination was 98.4% and about 11.7% were evaluated at 'C' and 3.9% were evaluated at 'D'. Those who had any health problems (54.2%) were receiving follow-up care, 52.4% of them had medical treatment while working. The health care managers in the company consulted 97.1% of them who had any suspicious signs and symptoms of occupational diseases. 4. Health Care Services : The average score of health care services was 13.8 out of a possible 28 point. The score of health education and health assessment, was lower than medical diagnosis and environmental hygiene. There were no significant differences in health care services according to the size of the industries(F=.95, P=.429). The score of health care services was higher in the worker who had better health and showed significant differences (F=4.50, p=.025).

경남지역 일부 산업간호사의 보건관리 업무 및 관련요인에 관한 연구 (A Study on Health Care Activities of Some Industrial Nurses and their Related Factors in Kyungnam Area)

  • The purpose of this study was to assess the performance of the role and function of some industrial nurses and to characterize the factors affecting the performance of their activities. Thus the results could be used to suggest the direction in the performance of industrials nurses' activities effectively. During a period from January 10 to March 31, 1994, the data were collected from 87 industrial nurses, who were working as health managers in the plants, in Ulsan city and the vicinity in Kyungnam province, using a structured questionnaire. The results were as follows : 1. The general characteristics of industrial nurses in this study were 82.8% being 30 years old or less, 60.9%, being not married, and 93.1% having eduction levels above junior college. 2. With respect to general work conditions, 94.3% were working in a separate room provided for health care division, 40.2% working under the safety and health department, and 98.9% working as common-level staffs. And 60.9% were working less than 44 hours a week, 70.1% had work experiences less than 5 years, and 50.6% had annual incomes ranging 10 to 14 million wons. 3. As work conditions related to health care activities, 49.4% performed the activities not related to health care as always or occasionally, and 87.4% answered that occupational physicians were appointed in their plant and among them, however, only 6.9% worked on full-time basis and 52.8% perform little activities as occupational physicians. For a decision related to health care activity, 69.0% discussed the problems with the supervisors, and 19.5% made decisions by themselves. 4. As for attitude and perception to their activities as health managers, 66.7% moderately recognized the importance of health manager in the workplace, with 63.2% being satisfied their wages and treatment from the company, 57.5% being satisfied with their job positions and 51.7% having positive attitudes as being health managers. 5. The degree of performance at least in one of health related activities were very high in activities such as general medical care(100%), general health examination(98.0%) and specific health examination(100%), and relatively high in health education(72%), new employee health examination(60.9%), document handling(79.3%) and activity for work environment(70.1%). However, the performance rate was very low in preparing protective equipment (20.8%). 6. The levels of activities related to health care were significantly high when making decisions by themselves, when occupational physicians not being full-time, and when satisfying their job positions, and, on the other hand, significantly decreased as work hours increased. 7. In addition to some kinds of periodic education asked by all of the nurses, 89.7% wanted a specialized licensing system for industrial nurse, and 97.4% wanted to apply for the license test. As a conclusion, it is suggested that industrial nurses should be given more authority and placed in more self-controlled system to perform health care and other activities more efficiently, and the role and function of the occupational physician should be clearly distinguished from that of the industrial nurse as a health manager to avoid an unnecessary overlapping.

일부 병원행정직의 조직시민행동과 관련된 융복합 요인의 관련성 분석 (Analysis of the Relationship between the Convergence Factors Related to Organizational Citizenship Behavior of Some Hospital Administrative Staffs)

  • 본 논문은 병원행정직원의 조직시민행동과 관련된 융복합적인 요인을 찾아보고자 시도되었다. 설문조사는 무기명 자기기입식 설문지를 사용하여 임의로 선정된 J지역 22개 병원의 행정직원 274명에 대하여 시행되었다. 설문조사 기간은 2019년 7월 1일부터 7월 31일까지 약 한 달간 소요되었다. 위계적 다중회귀분석 결과, 조직공정성의 하위영역인 절차공정성이 높을수록, 집단내신뢰가 높을수록, 조직헌신도가 높을수록 조직시민행동이 높았으며, 이들의 설명력은 41.9%이었다. 이러한 분석결과를 볼 때, 병원행정직원의 조직시민행동을 높이기 위해서는 조직공정성의 하위영역인 절차공정성 및 분배공정성을 높이고, 집단내신뢰를 높이고, 조직헌신도를 높이는 노력이 필요하다. 이상의 결과는 병원행정직원의 조직시민행동을 높이는 의료기관의 인적자원관리 및 산업보건교육에 활용이 기대된다. 추후연구에서는 병원행정직원의 조직시민행동에 영향을 미치는 구조방정식 모형의 수립이 필요하다.

조선수리업종 석면노출 근로자들의 흉부방사선상 석면관련 폐질환 소견의 발생양상 (Survey of Asbestos-Related Disease on Chest Radiograph of Workers Exposed to Asbestos in Ship Repair)

  • 모 조선수리업에 근무하는 1,086명 중 석면에 노출되는 작업공정에 10년 이상 근무한 근로자 118명을 대상으로 단면조사연구를 수행하였다. 연구대상자들에 대해 흥부방사선검사, 폐기능검사와 미국의 산업안전보건청 석면표준, 의학적 감시 프로그램(OSHA asbestos standard, medical surveillance program)에 따라 국내 실정에 맞게 일부 편집한 설문조사를 시행하였고 아울러 이들의 작업장에 대한 작업환경측정 자료도 분석하였다. 흉부방사선검사상 석면관련 폐질환 소견의 유병율은 118명의 근로자중 16명으로 $13.6\%$였다. 흉부방사선검사상 정상군에 비해 석면관련 폐질환 유소견군에서 유의한 요인들은 폐기능검사상 이상소견, 기침, 가래, 호흡기 질환의 과거력, 근무력이었다. 본 연구 결과를 통해 볼 때 폐기능검사 소견상 이상소견, 기침과 가래의 임상증상을 호소하거나 호흡기질환의 과거력, 20년 이상의 근무력이 있는 근로자에 대해서는 석면관련 폐질환의 엄격한 관리가 필요할 것으로 생각된다. 그리고, 본 연구에서 관련성 여부를 완전히 배제할 수 없었던 요인들에 대해서는 향후 광범위한 전향적 연구를 통하여 연관성을 규명하여 석면노출 사업장의 의학적 감시 프로그램에 적극 활용되기를 기대한다.

건강정보이해능력과 개인의 특성이 스마트 헬스케어 이용 경험에 미치는 요인 분석 (Factors Affecting Consumers' Experience of Using Smart Healthcare Focusing on Health Literacy and Personal Characteristics)

  • 헬스케어의 패러다임이 치료 중심에서 예방 중심으로 변화함에 따라, 질병 예방과 관련된 제품 및 서비스가 국내외로 많이 등장하고 있다. 스마트 헬스케어는 만성질환자 수의 증가, 의료비 부담의 증가 등의 보건의료 문제점의 대안으로 언급되어 기업 및 정부는 산업 확산 움직임을 보여주고 있다. 기업은 웨어러블 기기, 앱 등의 형태로 제품 개발 및 출시를 하고 있고, 정부는 보건소를 중심으로 모바일 헬스케어 사업을 시행하고 있다. 본 연구는 소비자의 스마트 헬스케어 제품 및 서비스 이용에 미치는 요인을 건강정보이해능력(Health Literacy)와 건강관련 개인특성을 중심으로 탐색적 연구를 진행하고, 정책적 시사점을 제공하고자 한다. 전국의 20대 이상 소비자 1,027명을 대상으로 온라인 설문조사를 실시하고 의사결정나무 기법으로 요인을 분석하였다. 연구 결과, 응답자의 대부분(76.9%)은 사용 경험이 없는 것으로 나타났지만 건강정보이해능력 중 '대중매체 정보의 활용'의 어려움 정도에 따라 사용 경험에 차이를 보였다. 그 밖에는 신기술 활용 의향 정도, 주변인의 조언 이해, 건강검진 유무가 관련이 있는 것으로 분석되었다. 스마트 헬스케어를 통한 자가 건강관리가 가능하려면, 건강정보를 대중매체로부터 탐색하고 활용할 수 있는 소비자의 건강정보이해능력이 제고되어야하며 이를 위한 정부와 기업의 노력이 필요하다.

디지털 중량제어기술을 활용한 고령친화운동기구의 인간공학적 분석 (Ergonomic Analysis for the Aging-Friendly Exercise Device Utilized on the Digital Load Control Technology)

  • 노쇠의 관리를 위해 근력운동의 중요성이 강조되고 있고 다양한 운동기구들이 개발되어지고 있다. 최근에는 전자기 저항을 디지털 중량으로 변환하여 제어하는 디지털 중량제어기술이 주목을 받고 있으나 국내에서 고령자를 대상으로 하여 디지털 중량제어기술 기반 운동기구의 효과성과 안전성을 검증한 보고는 전무하다, 따라서, 본 연구는 디지털 중량제어기술을 활용한 고령친화운동기구의 인간공학적 분석을 통하여 이후 개발방향의 지표 제시를 목적으로 하였다. 총 20명의 성인 남녀 (연령: 62.40 ± 2.09 세) 를 대상으로 적용 가능한 운동강도를 분류하기 위하여 운동기구의 부하를 레벨화하였고, 세가지 실험대상 동작시의 근 활성도와 심박수를 측정하였다. 또한, 국제 표준 ISO 9241-11에 준거한 설문지를 활용하여 소프트웨어부와 기기부의 안정성, 조작성, 효율성, 만족도 등을 평가하였다. 그 결과, 운동 부하, 근활성도, 심박수가 일관되게 증가하는 결과를 통해서 현장에서 적용 가능한 3단계의 운동 강도를 분류할 수 있었다. 모니터 사이즈의 확대, 메뉴의 한글화, 기구의 소형화, 소음의 최소화가 필요한 것으로 나타났다. 이러한, 결과들을 고려할 때 향후 신제품 개발시에는 기구 크기, 표시부, 소음의 보완을 통한 고도화된 고령친화운동기구 개발의 필요성이 시사 되어 진다.
