사이버나이프의 Targeting 에러의 평가

  • 사이버나이프를 이용한 두경부와 체부 등의 정위방사선수술에서는 목표점의 식별 및 위치 추적에 대하여 높은 정확성을 요구한다. 본 연구에서는 사이버나이프의 targeting의 정확성을 평가하고자 한다. 사이버나이프의 targeting의 정확성을 평가하기 위하여 두상 팬텀을 사용하였다. 환자 치료시와 동일한 CT 영상 프로토콜로 두상 팬텀의 영상을 얻어 치료계획을 세웠다. 팬텀에 radiochromic 필름을 삽입한 뒤 수립된 치료계획에 맞춰 팬텀에 방사선을 조사하였다. 방사선 조사시 위치 추적은 skull과 fiducial 추적의 두 가지 방법을 사용하였다. 방사선에 조사된 radiochromic 필름을 분석하여 정확성을 평가하였다. 본 연구를 통하여 실시간 영상유도 기술을 사용하는 사이버나이프는 약 1 mm의 targeting 에러를 보였다.

  • 사이버나이프 시스템은 독특한 형태의 정위적 방사선 치료를 수행하는 로봇 팔을 이용하는 방사선 수술 시스템으로 치료 중 환자의 호흡 등에 따른 신체의 움직임에도 1mm 이내의 오차 범위에서 정확하게 방사선을 조사해야하며, 여러 병소를 동시에 치료할 수 있다. 사이버나이프 시스템은 로봇 팔, 선형 가속기, 병소 위치 추적 장치, 방사선 조사 스케줄러와 같은 다양한 모듈들이 통합된 시스템으로, 이를 통합 제어하기 위한 주제어 시스템이 필요하다. 본 논문에서는 본 연구원에서 개발하고자 하는 사이버나이프 주제어 시스템의 설계에 관하여 기술한다.

Early Therapeutic Effects of Cyberknife Radiosurgery on Trigeminal Neuralgia (삼차신경통에 대한 사이버나이프 방사선수술의 조기 치료 효과)

  • Purpose: We evaluated whether Cyberknife radiosurgery is an effective and safe method of therapy for medically intractable trigeminal neuralgia (TN). Materials and Methods: We retrospectively analyzed the outcome of 26 patients, who failed to surgery or were not suitable candidates for invasive intervention and were treated by Cyberknife radiosurgery between March 2004 and May 2005. Radiosurgery doses of $60{\sim}64 Gy$ were delivered to the 80% isodose line prescribed to an 6 mm length of the nerve, sparing the most proximal 3 mm away from the trigeminal nerve root entry zone (median dose: 64 Gy). Results: Follow-up period was $3{\sim}15$ months (median follow-up period: 9 months) Preliminary results from a cohort of 26 patients undergoing Cyberknife radiosurgery for TN showed that pain relief was achieved in 50% (13/26) of patients within the first 24 hrs after treatment. At last follow-up, 96.2% (25/26) of patients reported early pain relief within 7 days. Treatment failure developed in 2 of 26. Poor response occurred in one patient and relapse was observed in the other patient. 3 patients had hypoesthesia (11.5%), which was the only complication observed with any of our patients. Conclusion: With these results, authors assumed that Cyberknife radiosurgery for TN could be one of safe and effective therapeutic methods.

A Fusion Study on the Selection of Cyberknife Technique according to the Location of the Pulmonary Tumors (폐종양의 위치에 따른 사이버나이프 기법의 선택에 관한 융합적 연구)

  • Depending on the location of the lung tumor, the choice of treatment technique should be considered when treating the Cyberknife. The 4DCT images of 18 lung cancer patients were analyzed, and location error values were extracted through application program. The evaluation result was lower than the average position error only in the upper and the inner. These results suggest that the Vertebral tracking technique is effective when it is close to the pulmonary attachment or near the vertebral body, and the Synchrony technique is effective at other positions. In the future, we would like to study cyber knife treatment technique according to the position of the tumor as well as the volume of the lung and the respiratory cycle.

Evaluation on the Usefulness of Lung Tumor Stereotactic Radiosurgery Applying the CyberKnife $Synchrony^{TM}$ Respiratory Tracking System (사이버나이프 $Synchrony^{TM}$ 호흡 추적 장치를 이용한 폐종양 방사선수술의 유용성 평가)

  • This study evaluated the motion of tumors during the entire period of therapy and the accuracy of radiosurgery among forty eight lung tumor patients who were underwent radiosurgery using the CyberKnife Synchrony Respiratory Tracking System. The motion of lung tumor was measured by the coordinates of a gold acupuncture needle inserted into the tumor or the area around the tumor using the CyberKnife image guided system. Then the accuracy of radiosurgery was evaluated based on the error of correlation computed with the motion tracking system. The lung tumor motion is Cranio-Caudal direction by an average of $2.63{\pm}1.87\;mm$, moved left-right direction by $1.13{\pm}0.71\;mm$, and anterior-posterior direction by $1.74{\pm}1.16\;mm$. The degree of rotational movement was $1.66{\pm}1.66^{\circ}$ on X axis, $1.20{\pm}0.97^{\circ}$ on Y axis, and $1.18{\pm}0.73^{\circ}$ on Z axis. The vector of translation movement was measured to be $3.78{\pm}2.00\;mm$ on the average. The results show that directions of Cranio-Caudal(p < 0.001), anterior-posterior direction(p < 0.029), and three dimensional vector value(p < 0.002) showed statistical significance, because the lower side of tumor showed more intensive movement compared to the upper side of tumor. The radiosurgery was carried out by compensating the motion of tumor after accurate investigation of the correlation error with the average of $0.95{\pm}0.62\;mm$ during the lung tumor radiosurgery with the CyberKnife Synchrony Respiratory Tracking System.
