Optimization for the Process of Osmotic Dehydration for the Manufacturing of Dried Kiwifruit (건조키위 제조를 위한 삼투건조공정의 최적화)

  • The developments of various processed foods and the high quality dried fruits, in particular, are urgently needed for the enhancement of fruit consumption and their competitive values. Therefore, in this study, three variables by three level factorial design and response surface methodology were used to determine optimum conditions for osmotic dehydration of kiwifruit. The relationships of moisture losses, solid gains, weight reductions, sugar contents, titratable acidities and vitamin C contents depending on changes with temperature, sugar concentration and immersion time were investigated. The moisture loss, solid gain, weight reduction and reduction of moisture content after osmotic dehydration were increased as temperature, sugar concentration and immersion time increased. The effect of concentration was more significant than those of temperature and time on mass transfer. Sugar content was increased by increasing sugar concentration, temperature, immersion time during osmotic dehydration. Titratable acidity and vitamin C content were increased by decreasing temperature, immersion time and increasing concentration during osmotic dehydration. The regression models showed a significant lack of fit (P>0.05) and were highly significant with satisfying values of $R^2$. At the given conditions such as $66{\sim}69%$ moisture content, above $24^{\circ}Brix$ sugar content and more than 23 mg% vitamin C, the optimum condition for osmotic dehydration was $37^{\circ}C,\;55^{\circ}Brix$ and 1.5 hour.

Comparison of Effectiveness and Adverse Reactions between Isoniazid 300 mg and 400 mg in Korean Patients with Pulmonary Tuberculosis (한국인 폐결핵 환자의 표준치료에서 Isoniazid 300 mg 및 400 mg의 치료 효과와 부작용의 비교)

  • Background : Isoniazid (INH) is one of the most effective anti-tuberculosis (TB) drugs. In Korea, the dose of INH normally used in patients over 50 kg is 400 mg/day, which differs from the dose recommended by other countries. Indeed, the metabolism of INH shows ethnic variations, and Koreans are predominantly rapid acetylators. However, two reports suggested 300 mg of INH might be sufficient to reach an ideal peak level in Korean patients over 50 kg. Therefore, the aim of this study was to compare the effectiveness and adverse reactions between INH 300 mg and 400 mg in Korean TB patients. Method : Patients who were culture-positive, susceptible to all 1st-line drugs, initially on HREZ, and weighed over 50 kg were selected from patients with pulmonary TB between April 2003 and March 2005. The treatment results and adverse reactions in the INH 300 mg and 400 mg group were compared. Since April 2004, most TB patients at Asan Medical Center were administered INH 300 mg irrespective of the body weight. Results : The study included 123 patients in the 300 mg INH group and 128 in the 400 mg INH group. There were no significant differences between the groups in terms of age, gender, weight, history of TB treatment, initial smear strength, and frequency of cavitary lesions. There was no difference in the treatment duration between the groups. One hundred eleven (90%) patients in the INH 300 mg group and 102 (80%) in the INH 400 mg group completed treatment (p>0.05). There were no differences in the frequency of modification of the initial regimen between groups due to any adverse reactions (300 mg : 9.0%, 400 mg : 13.7%) and hepatotoxicity (300 mg : 2.7% ; 400 mg : 7.8%). Conclusion : Considering treatment results and adverse reactions of two groups, 300mg of INH may be sufficient for treating Korean TB patients. Further studies comparing the frequency of relapse will be needed.

Pediatric Endoscopic Sedation in Korea: A Survey of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition (소아청소년 진정내시경: 대한소아소화기영양학회 학술위원회 조사 보고)

  • Purpose: The aim of this survey was to analyze the choice of sedative drugs and their side effects during pediatric endoscopic sedation in Korea. We also evaluated doctors, caretakers and patient satisfaction with the procedures. Methods: Between June 2006 and July 2006, a 16-item survey regarding current sedation practices, during one month, was mailed to 28 hospitals in Korea. The results of the survey responses were then analyzed. Results: Endoscopy performed under conscious sedation was reported in 89.0% of the responders and endoscopy under general anesthesia in 0.9% of 465 endoscopic procedures. Endoscopy under conscious sedation was performed in 89.1% for upper gastrointestinal endoscopy (GFS) and 88.1% for lower GFS. Midazolam was used for conscious sedation during the endoscopy in 84.5% of cases and propofol was used in two cases (0.5%). In addition, a bezodiazepine/opioid combination was used iin 84.6% (44 cases) for lower GFS. Patients were monitored with pulse oxymetry, EKG (91.4%) as well as automatic BP (5.1%). Transient hypoxia was the only side effect noted and was treated with supplemental oxygen (4.6%). Flumazenil was used in 2.71% of cases. The choice of sedation was made by the endoscopist (84%). The satisfaction rate for endoscopists was 68%, and for the patients and caretakers was 84% (as reported by the endoscopists). Conclusion: Midazolam was used only for the upper GFS and benzodiazepine/opioid combination was used for the lower GFS in Korea. The rate of satisfaction was relatively high and there were no significant side effects noted during the endoscopy under conscious sedation.

병원정보시스템 품질 항목에 대한 제안

  • 정보기술의 발달과 함께 소프트웨어 제품은 모든 산업에 필수요소가 되었고, 품질과 평가에 대한 관심도 점차 증가되고 있다. 하지만 일부 산업에서는 소프트웨어 품질 평가에 대한 사용자들의 만병 통치적 기대, 품질 표준의 부족, 측정을 위한 양질의 데이터 부족, 소프트웨어 분석과 디자인에 대한 공학적 한계로 소프트웨어 품질 평가에 대해 많은 문제점이 지적되고 있다. 국내에서도 의료산업 관련 정보시스템의 오류 및 사용자들의 운영 미숙은 매년 많은 금액의 사회적 비용을 증가시켰고, 병원정보시스템의 품질에 대한 관심을 초래하였다. 특히, 산업적 특성이 강한 병원정보시스템은 성공적 구축을 위해 사용자 중심의 소프트웨어 디자인과 다양한 전문가들의 지식 통합이 필요하며, 정보시스템 품질 측정으로 연구자들과 개발자들의 시스템 설계 혼란을 감소시키는 방법론이 필요하다는 연구들이 등장하고 있다. 대부분의 병원정보시스템이 단편적인 업무처리 위주로 개발 운영되고 있고, 장기적 경영전략이나 임상연구를 위한 분석적 정보처리 기능들은 결여되어 있다. 또한 소프트웨어 재설계나 추가적 개발 에 활용될 수 있는 객관적 품질 기준이 부족하고, 사용자들의 요구사항에 대해 소프트웨어 설계에 있어서 효율적으로 반영되지 못하고 있다. 이에 본 연구는 최근 발표된 병원정보시스템 품질 평가에 대한 연구 경향을 종합하고, 품질 평가에 대 해 효율적으로 활용되고 있는 사용성(Usability)을 기준으로 병원산업의 특수성을 포함한 품질 평가 방법과 품질척도를 제안하고자 한다. 국제표준기구(ISO:International Standards Organization)에서는 품질 특성을 기능성, 신뢰성, 사용성, 효율성, 유지 보수성과 이식성의 특성을 제시하고 있다. 특히 Folmer & Bosch(2004)가 정리한 ISO9126에서는 품질의 특성을 배움의 용이성, 운영의 용이성, 이해성과 매력성으로 분류하였고, ISO9241-11는 효과성, 효율성과 만족성으로 분류하였다. 또한 Shackel(1991)은 배움의 용이성(배움과 시간, 기억력), 효과성(오류, 직무시간), 유연성과 마음가짐으로 분류하고 있다(Shackel, 1991). Nielsen(1997)은 배움의 용이성, 기억의 용이성, 오류, 효율성, 만족성으로 분류하고 있고(Nielsen, 1997), Shneiderman(1998)는 효과성(직무시간, 배움의 시간), 효율성(기억의 지속시간, 오류), 만족도를 품질의 특성으로 분류하였다. 이와 같은 소프트웨어의 품질은 소프트웨어 계획, 개발, 성장과 쇠퇴의 모든 과정에 적용되며, 환경적 변화에 따라 사용자들의 정보욕구를 적절하게 반영하여 만족도를 높이 는 것이라고 요약할 수 있다. 그러나 현재까지 소프트웨어 품질 평가에 대한 연구들 은 보편적인 평가 항목들을 대상으로 측정하여 일반적인 품질기준을 제시하고 있고, 유사한 측정 내용들이 중복되어 있다. 이러한 경향은 산업별 특수성이 강한 소프트웨어에 대해서는 정확한 품질측정이 어려웠고, 품질측정에 대한 신뢰성을 떨어뜨리는 계기가 되었다. 이러한 한계를 극복하고자 나타난 방법론이 최종사용자들의 요구사항을 얼마나 적절하게 시스템에 반영했는지에 대한 사용성(Usability) 측정이다. 사용성에 대한 정의는 사용자들이 실질적으로 일하는 장소에서 직접 사용자들의 시스템 운용실태를 파악하여 문제점을 개선하는 것으로 요약할 수 있다. ISO9124-11에서는 사용성을 "어떤 제품이 구체적인 사용자들에 의해 구체적인 목적을 달성하기 위한 구체적인 사용의 맥락에서 효율성, 효과성을 만족함으로 사용될 수 있는 정도"로 정의하고 있다. 지난 10년간 병원정보시스템 평가에 대한 문헌들을 고찰한 결과 품질 측정의 효과는 정보화에 대한 동기유발과 의료품질을 높이는 게기가 되었으며, 질병에 대한 예방효과도 높은 것으로 조사되었다. 그러나 평가에 대한 인식의 문제, 평가 방법의 신뢰성 부족, 평가 지침과 부분적 평가에 따른 인증의 어려움 평가 결과에 대한 확산과 단편적 연구의 한계 등으로 연구결과에 대한 신뢰도와 활용도는 낮은 것으로 조사되었고, HIS에 대한 연구 빈도와 범위 가 매우 미약하였다. 특히, 품질속성은 같은 용어이지만 연구자에 따라 전혀 다른 측정 내용을 제시하고 있어 효율적인 품질 지표를 제시하는데 많은 혼란을 초래하고 있다. 이러한 품질 평가의 경향은 시스템 설계 및 개발자들에게 필요한 사용자들의 구체적이고 독특한 욕구나 병원정보시스템 환경의 특수성 파악에 한계를 보였으며, 평가 범위도 부분적으로 이루어져 전사적 시스템 설계 및 개발에 중요한 자료를 제공하지 못하고 있다. 이러한 문제점과 한계를 극복하고자 ISO와 같은 품질 표준 속성과 컨텍스트(Context)를 중심으로 사용자에 의한 평가 척도의 설정은 구체적이고 실용적이며 신뢰성 있는 평가 방법이 될 것이다.

Therapeutic Efficacy of Cefotaxime as an Empirical Antibiotic on Ascending Cholangitis after Kasai Operation for Biliary Atresia (Kasai 수술 후 발생한 상행성 담관염에서 경험적 항생제로서 Cefotaxime 치료의 적정성에 대한 평가)

  • Purpose : Ascending cholangitis is the most common complication after Kasai operations. The aim of this study is to evaluate the therapeutic efficacy of cefotaxime as an empirical antibiotic on ascending cholangitis after Kasai operations. Methods : Thirty-nine episodes of cholangitis in twenty-nine children who underwent Kasai operations at Seoul National University Children's Hospital from January 1991 to December 2000 were included in this study. Empirical cefotaxime treatments were divided into three groups : cefotaxime and amikacin treatment group(CA group), cefotaxime and gentamicin treatment group(CG group) and cefotaxime treatment group(C group). A diagnosis of cholangitis was made on the basis of unexplained fever(>$38^{\circ}C$) and either development of acholic stool or elevation of serum total bilirubin (>1.5 mg/dL). Therapeutic efficacy was judged by elimination of fever up to 72 hours, 120 hours, and 168 hours after antibiotic treatment. Results : There were therapeutic responses in 51%(20/39) up to 72 hours after antibiotic treatment : 54%(13/24) in CA group, 43%(3/7) in CG group and 50%(4/8) in C group. There were therapeutic responses in 69%(27/39) up to 120 hours, in 79%(31/39) up to 168 hours and in 82%(32/39) up to 2 weeks. There were no differences in therapeutic efficacy among the three regimens. In 12 of 39 episodes, the etiologic pathogens including Escherichia coli and enterococcus were cultured from the blood. Conclusion : Cefotaxime can be tried as an initial antibiotic in Korean children with ascending cholangitis after Kasai operation prior to the identification of microorganism on culture. However, further evaluation of pathogen and its resistant strain to cefotaxime should be done.

A Study of Dose Stability at Low Monitor Unit Setting for Multiple Irradiated Field (다중 조사면 치료 시 기계적 입력치(MU)에 따른 선량적 안정성에 대한 연구)

  • Purpose : Many authors have been introduced field in field technique and 3-D conformal radiotherapy that increased the tumor dose as well as decreased the dose of abutting critical organ. These technique have multiple beam direction and small beam segments even below 10 MU(monitor unit)for each field. we have confirmed the influence of low MU on dose output and beam stability. Materials and Methods : To study the dose output, the dose for each field was always 90MU, but it divided into different segment size: 1, 2, 3, 5, 10, 15 segments, 90, 45, 30, 18, 9, 6 MU the measurements were carried out for X-ray energy 4 MV, 6 MV, 10 MV of three LINAC(Varian 600C, 2100C, 2100C, 2100C/D), in addition each measurement was randomly repeated three times for each energy. To study the field symmetry and flatness, X-omat V films were irradiated. After being developed, films were scanned and analyzed using densitometer. Results : Influence of low MU on dose is slightly more increase output about $1.2{\sim}2.9%$ in cGy/mu than 90MU, but may not changed beam quality(flatness or symmetry), Output stability depends on dose rate(PRF)rather than beam energy, field size. Conclusion : Presented result are under the limits(out put<3%, flatness<${\pm}3%$, symmetry<2%). The 3 accelerators are safe to use and to perform conformal radiotherapy treatments in small segments, small MU around 10MU. but Even if the result presented here under the limits, continuous adjustments and periodic QA should be done for use of small MU

  • 강재의 방식법 중 도장은 부식을 억제하는데 효과적이고 편리한 방법으로 선박 및 해양 강 구조물의 방식법으로 사용되고 있다. 한편, 강 구조물의 효율적인 유지관리를 위해서는 방식 도장의 도막 열화도를 평가하고 잔존 수명을 예측하여 최적 시기에 보수도장 혹은 재도장하는 것이 필요하다. 일반적으로 선박 및 해양구조물에 적용되는 도막의 방식 성능 평가 방법으로 해수 침지 시험, 염수 분무 시험, 옥외 폭로 시험 등이 있다. 그러나 이러한 시험들은 그 시험 방법에 따라서 정량적인 평가에 한계가 있음은 물론 장기간 소요되는 등 곤란한 문제점이 있다. 그러므로 선박 및 해양구조물을 비롯하여 교량, 각종 강 구조물의 도장 방식에 사용되는 방식용 도료의 성능을 단기간에 적절하게 평가할 수 있는 가속시험법이 제시되며 연구-사용되고 있다. 그 중 도막 방식 성능을 보다 효율적, 비파괴적, 정량적으로 평가할 수 있는 임피던스 분광법(EIS)과 같은 전기화학적 방법은 상대적으로 시험 기간을 크게 단축시킬 수 있고, 대상 방식 도장의 미세한 성능 차이도 분별 가능하다는 장점이 있다[1]. 따라서 본 연구에서는 선박 및 해양구조물 등 가혹한 부식환경에서 강력한 내구성을 가질 수 있도록 다양한 종류의 표면처리 도장 시편을 제작하여 자외선 조사-염수분무-침지환경 등의 열악한 환경조건 하에서 부식-열화 촉진 시험을 실시하였다. 그리고 그 촉진 열화 과정에서 도막의 외관 상태를 관찰 분석함은 물론 전기화학적 임피던스 분광법을 병행 측정하며 그 표면막의 부식 및 도막 열화도를 비교-종합 평가하였다.본 연구에 사용된 시편은 Al 및 Zn 도금 강판에 에폭시, 에폭시-실리콘 우레탄, 에폭시-우레탄 도장 시편으로 Scribe, No Scribe 및 비교재 Al 및 Zn 도금 시편으로 분류하여 각각 실험을 진행하였다. 즉, 도막 열화 시험은 복합 노화 시험법으로 UV 조사 36 시간(ASTM G53), 염수분무 32 시간(ISO 7253), 수분 응축 10 시간을 1 Cycle로 100 Cycle(7800 시간) 동안 실험을 진행하였다. 이때 도막 열화도 평가는 전기화학적 임피던스 분광법을 이용하여 각 실험 조건별로 주파수에 따른 임피던스(Z) 값을 평가하였다. 즉, 상온 $25^{\circ}C$의 3.5% NaCl 100 ml 수용액에 작동 전극(Working Electrode)과 구리 도선을 통해 연결하였고, 노출 면적은 $1cm^2$로 일정하게 유지 하였으며, 상대 전극(Counter Electrode)은 탄소봉, 기준 전극(Reference Electrode)으로 포화카로멜전극(Saturated Calomel Electrode)을 사용하여 측정하였다. No Scribe 시편의 경우에는 Al 기판 에폭시-실리콘 우레탄 도장 시편이 우수한 도막 저항성을 나타내었으며, 에폭시-우레탄 도장시편은 23사이클 이후의 저항값이 가장 낮게 나타났다. Zn 기판의 경우는 에폭시, 에폭시-실리콘 우레탄, 에폭시-우레탄 도장 시편 모두 저항 값이 유사하였으며, Al 및 Zn 도금 시편은 도장 처리된 시편에 비해 훨씬 낮은 저항 값을 보였다. 또한 Scribe 시편의 경우에는 Al 기판 에폭시-실리콘 우레탄 도장 시편에서 높은 초기 저항 값을 보였으며, 23 사이클 후의 저항 값은 세 종류의 도막에서 약 1~0.1 Gohm 으로 나타났다. 그리고 Zn 기판 에폭시-실리콘 우레탄 도장 시편에서 가장 낮은 도막 저항 값이 나타났다. 이상의 실험을 통해서 본 연구 내용은 실내촉진시험으로 선박 및 해양 강 구조물에 사용되는 다양한 종류의 도막의 열화도를 평가하는 기초 설계 지침으로 응용될 수 있을 것으로 사료된다. 한편, 도막은 노출 환경에 따라 방식 성능이 다르므로 실제 도막의 사용환경을 고려하여 도장 사양별 적용 부위에 따른 적정 가속 실험 방법을 선정할 필요가 있다고 사료된다.

Climate-Smart Agriculture (CSA)-Based Assessment of a Rice Cultivation System in Gimje, Korea (한국 김제의 벼 경작 시스템의 기후스마트농업 (Climate-Smart Agriculture) 기반의 평가)

  • The overarching question of this study is how a typical rice cultivation system in Gimje, Korea was keeping up with the triple-win challenge of climate-smart agriculture (CSA). To answer this question, we have employed (1) quantitative data from direct measurement of energy, water, carbon and information flows in and out of a rice cultivation system and (2) appropriate metrics to assess production, efficiency, GHG fluxes, and resilience. The study site was one of the Korean Network of Flux measurement (KoFlux) sites (i.e., GRK) located at Gimje, Korea, managed by National Academy of Agricultural Science, Rural Development Administration. Fluxes of energy, water, carbon dioxide (CO2) and methane (CH4) were directly measured using eddy-covariance technique during the growing seasons of 2011, 2012 and 2014. The production indicators include gross primary productivity (GPP), grain yield, light use efficiency (LUE), water use efficiency (WUE), and carbon uptake efficiency (CUE). The GHG mitigation was assessed with indicators such as fluxes of carbon dioxide (FCO2), methane (FCH4), and nitrous oxide (FN2O). Resilience was assessed in terms of self-organization (S), using information-theoretic approach. Overall, the results demonstrated that the rice cultivation system at GRK was climate-smart in 2011 in a relative sense but failed to maintain in the following years. Resilience was high and changed little for three year. However, the apparent competing goals or trade-offs between productivity and GHG mitigation were found within individual years as well as between the years, causing difficulties in achieving the triple-win scenario. The pursuit of CSA requires for stakeholders to prioritize their goals (i.e., governance) and to practice opportune interventions (i.e., management) based on the feedback from real-time assessment of the CSA indicators (i.e., monitoring) - i.e., a purpose-driven visioneering.

Comparative Study on the Regimens with Pyrazinamide or Ofloxacin in the retreatment of pulmonary tuberculosis (폐결핵 재치료에서 Pyrazinamide 복합처방과 Ofloxacin 복합처방의 효과에 관한 비교 연구)

  • Objective: In the early short-term therapy of pulmonary tuberculosis, PZA is used for the first two months on 6EHRZ therapy but PZA is not effective in the case of long-tenn use PZA for retreatment in the sensitive relapse or acquired drug resistance for PZA. But in the endemic area as Korea, if we can't use PZA in the retreatment of pulmonary tuberculosis, we can't expect the success for retreatment of pulmonary tuberculosis, therefore we need new drugs substituting for PZA. In these days, 4 - fluoroquinolone derivatives were investigated and only ofloxacin and ciprofloxacin of derivatives were known to be effective but the effectiveness was also not certain because the result was experimental or combined with other bacteriocidal drugs and datas on effectiveness of pulmonary tuberculosis were so little. Therefore these drugs should be use with other two or three strong-acting drugs in the last period of retreatment of pulmonary tuberculosis. The ofloxacin or ciprofloxacin is used in some area in Korea but randomly and needed more study. We did this study for proving the effectiveness of these drugs and establishment of retreatment regimen for pulmonary tuberculosis. Methods: Retrospective cohort study of 83 drug-resistant pulmonary tuberculosis patients at National Masan Tuberculosis Hospital from Jan. 1994 to dec. 1995 was made. All the patients taken medicine for 2nd ami-tuberculosis regimens for the first lime. We separated the patients by two groups.(Group I : OFX+ PTA + CS+PAS + Injection, Group II: PZA + PTA+ CS + PAS + Injection). We compared the difference between two groups and tested the confidence limit about results after treatment by $\chi$2-test and T-test. Results : 1. The age distribution was most frequent in fourth decade(29.2% in Group I, 37.1% in Group II) and the mean age was 43.9 year in Group I, and 39.0 year in Group II, but had no significant difference between two groups. The sex distribution was more frequent in the males(68.8% in Group I, 85.7% in Group II), but had no significant difference. 2. Family history was 29.2% in Group I, 28.6% in Group II, but had no significant difference. 3. In the respect of extent of disease, far-advanced stare was 60.4% in Group I, 74.3% in Group II, but had no significant difference. 4. The side effects for drugs showed in 58.3% in Group I and 65.7% in Group II, and the gastrointestinal trouble showed 25.0% in Group and arthralgia 34.3% in Group II predominantly respectively and had the significant difference(p<0.05). 5. The negative conversion rate on sputum AFB smear was 87.5% in Group I and 80.0% in Group II, but had no significant difference. But the negative conversion rate on sputum AFB culture was 83.3% in Group I and 57.1 % in Group II and had the significant difference(p<0.05). 6. The success rate of treatment was 87.5 % in Group I and 83.3 % in Group II but had no significant difference. Conclusion : In the retreatment of pulmonary tuberculosis, ofloxacin is useful drug for the patients who are not available to use PZA and can be use effectively substituting for PZA.

The Signal Distortion Due to the Electromagnetic Induction Inside a Cable (전선의 전자기유도 현상에 의한 신호 왜곡)

  • Electric cables such as multi-interphone cables and ribbon cab]os are commonly used for data aquisition in the DC resistivity survey. In general, electromagnetic induction may occur in the electric cables when electric current flows through them. In case of using multi-interphone cables in the DC resistivity survey, electromagnetic induction could take place due to the entangled wires of the multi-interphone cables, when the current flows through them. Then, the electromagnetic induction may cause measured DC resistivity data to be distorted. In this study, a monitoring system with PXI (PCI Extention for Instrumentation) was constructed to examine signal distortion on the DC resistivity data, attributed to the electromagnetic induction. Common electric cables used in the DC resistivity survey were tested to observe the waveforms of the electric voltages. The waveforms measured were compared to examine signal distortion due to the electromagnetic induction. The results may provide information on the resistivity data obtained using different electric cables in the DC resistivity survey. The distortion of waveforms attributed to the electromagnetic induction wat not observed when using ribbon cables for DC resistivity data aquisition, while the distortion were observed when using multi-interphone. Therefore, the ribbon cables provide better quality of data than other cables in the DC resistivity data aquisition.
