Sonographic and Strain Elastographic Findings of a Clear Cell Hidradenoma that Looked Like an Epidermoid Tumor: A Case Report (표피 종양처럼 보이는 투명 세포 열선 종의 초음파 및 변형 탄성 소견: 증례 보고)

  • Clear cell hidradenoma (CCH) is a rare tumor of the sweat glands of eccrine or apocrine differentiation. It can occur anywhere in the body, but common sites of involvement are the head, face, trunk, and extremities. Although several reports have described sonographic findings of CCH, only one study on the axilla mentioned its strain elastographic findings. Here, we present a case of CCH in the right calf with its sonographic and strain elastographic findings in a tumor that looked like an epidermoid tumor.

Topiramate for the Treatment of Binge Eating Disorder or Bulimia Nervosa : A Systemic Review of Human Clinical Studies and Case Reports (Topiramate의 신경성 폭식증 치료효과: 국내외 보고된 임상연구결과 및 치험사례 중심으로)

  • The clinical investigations above suggest that topiramate may be an effective agent in the treatment of BED or BN by reducing binging/purging episodes, improving the HRQOL, and decreasing weight. The case report and case series also support these findings. However, there are several limitations in the above studies and cases. All these had relatively small sample size, and two of them were only 10-week-period studies. Optimal duration of treatment with topiramate in patients with BED or BN is unknown. As most clinicians treat the patient with BED or BN for 6 to 12 months and then reassess, at least 6 months period is needed to show its efficacy. One of studies included only women in the patient group. In the case series, all patients had severe comorbid mood disorders such as major depression and bipolar disorder besides BN. Therefore, notwithstanding its clinical usefulness, additional researches are needed to define the role and the benefits of topiramate in the treatment of BED or BN more thoroughly.

A Case of Late Onset Neonatal Bacteremia and Meningitis Caused by Streptococcus lutetiensis (Streptococcus lutetiensis 에 의한 지발형 신생아 균혈증과 수막염 1례)

  • Invasive infection of the Streptococcus bovis group in a neonate is rare. In cases reported to date, the pathogen of neonatal S. bovis infections is usually Streptococcus gallolyticus subsp. pasteurianus (S. bovis biotype II/2). Streptococcus lutetiensis (S. bovis biotype II/1) was identified using 16S rRNA and tuf gene sequence analysis of the isolates from blood and cerebrospinal fluid (CSF) of a fever-presenting 28-day-old male. Blood culture analysis was performed using automatic equipment (VITEK 2) and identified Streptococcus infantarius supsp. infantarius, yet we were unable to get accurate results from the CSF culture. The fever subsided on the second day of hospitalization, and the patient was discharged without neurologic complication after 14 days of antibiotic therapy. In this case, we were able to accurately identify the pathogen using molecular genetic methods. To our knowledge, this is the first case of late onset neonatal bacteremia and meningitis caused by S. lutetiensis.

First report on the non-native species, Western painted turtle (Chrysemys picta bellii), in the wild, Republic of Korea (국내 야생에서 처음 발견된 외래종, 서부비단거북(Chrysemys picta bellii)의 관찰 사례 보고)

  • On May 27, 2019, a non-native turtle was discovered at Ilsan Lake Park, Janghang-dong, Ilsandong-gu, Goyang-si, Republic of Korea. The turtle was identified as a western painted turtle (Emydidae: Chrysemys picta bellii) based on the external morphology, plastron color, plastron pattern, stripes on the side of the head, and shape of the carapace. In the lake park, another non-native turtle was observed sunbathing on the same rock. In this paper, we firstly reported the confirmed presence of a western painted turtle in the wild of Korea. Unfortunately, various non-native turtles are continuously reported in Korean ecosystems. Therefore, we highlight that continuous surveying and reporting of the non-native species is important to their management.

  • Gastric metastasis from renal cell carcinoma (RCC) is extremely rare, occurring in 0.2% of all RCC cases. Owing to its low prevalence, metachronous gastric metastasis from RCC may be underdiagnosed, and the imaging findings have not been well-established. Herein we present a case of metastatic RCC manifesting as a gastric polyp in a 70-year-old female along with a literature review on the imaging findings of gastric metastases from RCC. In patients presenting with gastric hyper-enhancing polypoid masses, metastasis from RCC should be considered as a differential diagnosis.

  • Ischemic stroke is one of the manifestations of reversible cerebral vasoconstriction syndrome (RCVS). Many precipitants and associated disorders of RCVS have been suggested. However, few case reports have indicated an association between anemia and RCVS. Here, we report a case of a 66-year-old female with severe iron deficiency anemia (IDA), who presented with ischemic stroke and cerebral vasoconstriction, which gradually improved with conservative treatment. High-resolution vessel wall magnetic resonance imaging findings and reversibility suggested the possibility of RCVS. In patients with RCVS and ischemic stroke, IDA should be considered. Prompt management should be delivered to prevent disease progression and recurrence.

  • Brainstem infarction due to vertebrobasilar insufficiency is a rare initial presentation of systemic lupus erythematosus (SLE) patients and small-vessel dissection as the direct cause of infarction has not been reported. We report the case of a 20-year-old female with acute infarction on the right side of the pons due to a small artery (pontine perforator) dissection, identified on digital subtraction angiography and high-resolution vessel wall MRI (vwMRI). She was diagnosed with SLE based on the presence of neurologic disorders and relevant laboratory findings. The pontine perforator-dissecting aneurysm had occluded and the right distal vertebral artery had resolved on subsequent vwMRI. She had a modified Rankin Scale (mRS) score of 1 at discharge with mild symptom improvement, and exhibited no further aggravation of symptoms at 3 or 12 months, maintaining an mRS score of 1.

Large Dolichoectasia Concurrently Involving Both Anterior and Posterior Cerebral Circulation: A Case Report (전순환계와 후순환계를 동시에 침범한 긴머리확장증 환자의 영상 소견: 증례 보고)

  • Dolichoectasia is an uncommon disorder characterized by the presence of a dilated, elongated, and tortuous cerebral artery. Its main pathologic mechanism is the disruption of the internal elastic lamina. Risk factors for dolichoectasia are advanced age, chronic hypertension, and metabolic disease. It mainly involves the vertebrobasilar vasculature (or "posterior circulation"), but dolichoectasia can also be seen in the anterior circulation, particularly the anterior cerebral artery. There are no reported cases of dolichoectasia involving both anterior and posterior circulation in South Korea. Here we report an unusual case of dolichoectasia involving both anterior and posterior circulation in a young female without any underlying disease on the basis of prominent imaging findings.

The Effects of Near Miss and Accident Prevention Activities and the Culture of Patient Safety Management for the Patient Safety (Near Miss 사고 예방 활동과 환자안전관리 문화형성이 환자안전에 미치는 영향)

  • Purpose: Despite the rapidly changing healthcare environment, healthcare organizations have recognized the importance of patient safety management. But patient safety management has the problem of the lack of participation of members due to the process of focusing on the follow-up service and punishment. The department of nuclear medicine in Uijeongbu St. Mary's Hospital started this research to reduce the near miss and prevent patient safety accidents by both initiating the participatory near-miss-proof activities as an advance management and constructing a system without disadvantages of reporting. In addition, this research aims to establish a differentiated patient safety management system in the department of nuclear medicine. Materials and Methods: 1. Colleting cases of team members' past and present near miss and accidents(First data collection). 2. Quantifying the cases of near miss and accidents after identifying the degree of importance and urgency through surveys(Second data collection). 3. Quantifying cases and indentifying important points of contact through data analysis. 4. Making and standardizing a manual for important points of contact, and initiating participatory activities to prevent errors. 5. Activating web-based community for establishing the report system of near miss. 6. Estimating the result of before and after activities through surveys and focus group interviews. Results: 1) Quantified safety accidents and near miss in the department of nuclear medicine. About 50 near misses a month and one safety accident a year. 2) Establishing improvement measurements based on quantified data. About 11 participatory activities, the improvement of process, a manual for standardization. 3) Creating a system of safety culture and high participation rate of team members. Constructing a report system, making a check list and a slogan for safety culture, and establishing assessment index. 4) Activating communities for sharing the information of cases of near misses and accidents. 5) As the result of activities, the rate of near miss occurrence declined by 50% and the safety accident did not happen. Conclusion: The best service in the department of nuclear medicine is to provide patients with safety-guaranteed high-quality examination and cure. This research started from the question, 'what is the most faithful-to-the-basics way to provide the best service for patients?' and team members' common answer for this question was building a system with participation of all members. Building a system through the participatory improvement activities for preventing near miss and creating safety culture resulted in the 50% decline of near miss occurrence and no accident. This is a meaningful result from the perspective of advance management for patient safety. Moreover, this research paved the way for creating a culture to report and admit near miss or accidents by establishing a report system with no disadvantage of reporting. The system which sticks to the basics is the best service for patients and will form a patient safety culture system, which will lead to the customer satisfaction. Therefore, all members of the department of nuclear medicine will develop a differentiated patient safety culture with stabilizing the established system.

An Experimental Study on Water Level Change Around Bridge According to the Variation of Debris Size (유송잡물 크기변화에 따른 교각주변에서의 수위변동 실험연구)

  • 국내 소하천에 설치된 대표적인 수공구조물은 교량, 보, 암거 등이 있으며 소교량은 전체 소하천 구조물의 56%를 차지하고 있어 태풍 및 집중호우로 인한 피해사례가 다양하게 보고되고 있다. 소교량에서의 대표적인 피해는 교각기초의 세굴, 여유고 부족으로 인한 파괴 등이 대표적인 피해사례로 알려져 있다. 이와 더불어 유송잡물 집적으로 인한 피해사례가 발생하고 있는데 교량에서의 유송잡물에 대한 피해는 유송잡물 집적으로 인한 교량전도, 수위상승으로 인한 제방월류, 압력흐름 증가로 인한 하상세굴 등의 피해 등 다양한 피해가 발생하는 것으로 보고되고 있다. 이러한 유송잡물로 인한 피해를 저감하기 방법으로는 유송잡물 차단, 우회시설을 설치하거나 충분한 교량의 여유고를 확보하는 방법 등이 있다. 여유고 확보를 위해서는 유송잡물로 인한 교각에서의 수위변동 특성을 검토할 필요가 있다. 따라서 본 연구에서는 유송잡물의 크기변화에 따른 교각주변에서의 수위변동을 실규모-모형실험으로 연계하여 영향을 검토하고자 하였다. 수리실험은 소교량 대상 실규모 유송잡물 집적실험과 집적형태를 반영한 수리특성실험으로 구성하였다. 이를 위해 1/1 규모의 실규모 소교량 교각모형을 대상으로 유송잡물(목본류, 초본+목본류)에 대한 집적실험을 수행하고 집적형태를 파악하였다. 집적형태를 반영한 수리실험은 실규모 실험결과를 토대로 유송잡물 모형을 제작하였으며 잡물의 크기변화에 따른 수위변동 영향을 검토하였다. 실험결과 유송잡물의 집적형태는 전반적으로 직사각형 형태로 집적이 이루어지고 있는 것으로 나타났다. 이러한 형태를 반영한 수리실험결과 유송잡물의 높이가 교각두께의 1배 미만일 경우 상류단에서의 수위변동이 미미한 것으로 나타났다. 폭비와 길이비 변화에 따른 수위변동은 폭비변화에 비해 높이비 변화에 민감하게 영향을 받는 것으로 나타났다. Fr에 따른 수리실험에서도 높이비가 1이상일 경우 상류단에서의 수위상승이 발생하는 것으로 확인되었다.
