  • Purpose : The incidence of type 2 diabetes in children and adolescents has been reported to increase recently. The aim of this study is to investigate the clinical features of type 2 diabetes developing during childhood and adolescent period. Methods : The medical records of 33 patients with type 2 diabetes were reviewed. We analysed clinical manifestations, demographic data, and modes and responses of treatment. Results : Age at diagnosis was $13.4{\pm}1.8$ years. Seventy percent of patients revealed pubertal signs at diagnosis. Half of the patients had BMI more than $25kg/m^2$. Seventy-three percent of patients had family history of type 2 diabetes. Acanthosis nigricans were found in 18% of patients. Nineteen(57.6%) patients were diagnosed incidentally by random urine or blood glucose test without any typical diabetic symptom or sign. The modes of therapy to control hyperglycemia were insulin alone(75.8%), oral hypoglycemic agents alone(9.1%), insulin and oral hypoglycemia agents(9.1%), and only diet with exercise(6%). At the time of investigation, 45.5% of patients were not using insulin. The typical diabetic symptoms at diagnosis were more prevalent in patients who required insulin for more than two years than patients who did not(P<0.05). Conclusion : The development of type 2 diabetes in children and adolescents is possibly related to puberty, obesity, family history, and defects in insulin secretion rather than insulin resistance. Many children and adolescents with type 2 diabetes required insulin initially and some of them could discontinue. More than half of the patients were diagnosed as diabetes without any typical symptom or sign, which might be one of the predictive factors of the prolonged insulin requirement.

  • Radiotherapy of rectal cancer requires a stabilized image but the movement of patients is almost unavoidable in radiotherapy. In this study, the setup error using the radiation treatment technique was compared according to the loading time and BMI(Body Mass Index) for 14 patients with rectal cancer. In addition, the variation of the dose by the average setup error was compared. Therefore, the technique of a selective standard was established. As a result, 3DCRT(3-Dimensional Radiation Therapy) and VMAT(Volumetric Modulated Arc Therapy) showed a similar time and error. In comparison, IMRT(Intensity Modulated Radiation Therapy) increased the time two fold and the error four fold. In BMI, a more pyknic patient showed a larger error for all techniques. Regarding the dose, IMRT and VMAT increased much more than 3DCRT in the average error at the small bowel. Therefore, 3DCRT of the short time will be applied to pyknic rectal cancer. Moreover, VMAT selects than IMRT in the overexposure of the small bowel.

  • Kidney stones are a common disease with an annual prevalence of about 30,000 people in Korea, and are deeply related to an increase in chronic kidney disease, high blood pressure, coronary artery disease, metabolic syndrome, type 2 diabetes, hyperlipidemia, and abdominal obesity. Therefore, in this study, 135 examinees who visited a general hospital in Dalseong-gun from May 2019 to June 2020 for a medical examination were examined. The relationship between kidney stones and factors related to obesity and metabolic syndrome were found in abdominal ultrasound. I tried to find out the relevance. As a result of the study, the risk of kidney stones in the abnormal group was increased by 4.255 times compared to the normal group in total cholesterol factor, and the risk of kidney stones in the abnormal group was increased by 2.072 times compared to the normal group in the low-density lipoprotein cholesterol factor. Total cholesterol factor and low-density lipoprotein cholesterol It was found that the factor affects the prevalence of kidney stones and metabolic syndrome. Since the risk of kidney stones is related to total cholesterol and low-density lipoprotein cholesterol factors, active attention should be paid to preventive purposes through health check-ups.

  • The aim of the present study is to elucidate the anti-obesity effects of Doenjang with and without salt in C57BL/6J mice. For the analysis, a total of forty mice were randomly divided into four groups: normal diet group (ND), high-fat diet group (HD), high-fat diet supplemented with 20% Doenjang group (DJ), high-fat diet supplemented with 20% unsalted Doenjang group (NS). During the study period, food intake and body weight were measured daily and weekly, respectively. The animals were sacrificed after 12 weeks. Body weight gain, epididymal fat pad weight and serum triglyceride levels of DJ group were found to be significantly lower than those of the HD and NS groups. Serum total-cholesterol levels of DJ and NS groups were significantly lower as compared to the HD group. There were significant decreases in plasma insulin and leptin levels in DJ group compared with the HD and NS groups. We did not observe any significant changes in the expression of hepatic lipogenic-related gene $PPAR{\gamma}$ among the HD, DJ and NS groups. However, ACC expression was found to be significantly decreased in DJ group. Lipolysis-related gene ($PPAR{\alpha}$ and CPT-1) expression was significantly higher in the DJ group as compared to HD and NS groups. In conclusion, the results of this study showed that Doenjang supplementation lowers body weight gain and improves obesity-related parameters.

  • To evaluate the ischemic heart disease risk factors and analysis on the its relationship between risk factors and ischemic heart disease on EKG findings in a rural area, We conducted cross-sectional health screening test for 1304 persons aged over 30 years. Blood pressure, total cholesterol, HDL cholesterol, fasting blood glucose, BMI and Waist/hip ratio, smoking data and EKG data were collected. Hypertension was classified by the sixth report of the Joint National Committee on Detection, Evaluation, and Treatment of High Blood Pressure and the cutpoints of hyperlipidemia was used National Cholesterol Education Program. The cutpoint of hyperglycemia was used National Diabetes Data Group and those for obese was 30%. The results obtained were as follows; 1. Prevalence of definitive hypertension was 41.2% in males and 41.6% in females. The prevalence of hypertension showed Increasing tendency according to age increase(p<0.05). 2. Prevalence of hyperlipidemia was 20.6% in males and 20.4% in female. In females prevalence of hyperlipidemia showed increasing tendency according to age increase(p<0.001). 3. Prevalence of obese was 23.4% in males and 28.8% in females. Upper body type was predominant in females, but lower body type was predominant in males. 4. Prevalence of hyperglycemia was 11.0% in males and 12.1% in females. The prevalence of hyperglycemia showed increasing tendency according to age increase(p<0.01). 5. The smoking rate was 63.7% in males and 2.6% in females. 6. On the EKG findings, the prevalence of myocardial ischemia and myocardial infarction was 6.7% in males and 7.5% in females. 7. The prevalence of Ischemic heart disease was higher in hypertension than normal in females and higher in obese than normal in males. In males and females the prevalence of ischemic heart disease was higher in hyperglycemia than normal and higher in upper body type than lower body type. But there is no statistical association among them.

  • This study was intended to evaluate the weight control program for 18 obese female college students during 9 weeks. The weight control program was composed of periodical counsel for self-control of dietary attitude and exercise with personal/group program. The female college students were average 21.3 years old and average 161.8 cm of height. The subjects were classified into 2 groups based on BMI: overweight group $25\le{BMI}<27$, obese group BMI$\ge$27. The subjects were average 71.56 kg of weight and $27.25 kg/m^2$ of BMI before they joined the program. The prompt of joining was less self-confidence for appearance. After 9 weeks, the overweight group lost their weight about 3.28 kg and also reduced 1.61 percentage of body fat. The obese group also lost their weight about 3.0 kg but reduced only 0.8 percentage of body fat. The serum levels of total cholesterol and LDL-cholesterol dropped significantly (p<0.05) in the overweight group. The $VO_2$ max inclosed 2.71 mL/kg/min in the overweight group. The obese group reduced caloric intakes from $109.2\%\;to\;86.5\%$ of RDA. The scores of dietary attitude such as eating speed, snack frequency, watching TV or reading during the eating were significantly increased (p<0.05) in obese group. These overall results suggest this program would be effective in self-weight control of overweight people. But the obese group assumed negative attitude in self-exercise program. So it is necessary to manage weight control programs, as considering obesity degree of subjects.
