플라즈마 방출광 진단법은 플라즈마에 특별한 영향을 주지 않으면서도 진단 정보를 안정적으로, 지속적으로 취득할 수 있는 우수한 진단 방법이다. 이러한 분광 진단의 신뢰성을 확보하기 위해서는 방출광의 정확한 측정과 해석이 중요하다. 방출광의 측정에 이용되는 분광 장비는 모노크로메터(monochromator)와 소형 스펙트로메터(spectrometer)가 주로 사용된다. 스펙트로메터의 경우 모노크로메터보다 분광 성능은 다소 부족하지만 가볍고 작은 크기로 인해 장비의 설치가 용이하고 가격이 저렴하다는 장점이 있다. 또한 모노크로메터에 비해 분광 성능이 낮은 대신 넓은 범위의 파장을 동시에 측정할 수 있는 장점이 있다. 따라서 스펙트로메터는 플라즈마의 모니터링에 주로 사용된다. 그런데 스펙트로메터의 기기적 선폭 증대(instrumental broadening)보다 조밀하게 위치한 스펙트럼들은 서로 중첩이 일어나 진단이 어려워진다. 특히 분자 띠 스펙트럼(molecular band spectrum)과 같은 경우 선 스펙트럼들이 매우 밀집된 형태를 이루고 있어 범용적인 스펙트로메터로 진단하기가 어렵다. 이러한 문제점을 해결하기 위하여 합성 스펙트럼 해석법(synthetic spectrum method)을 이용할 수 있다. 본 연구에서는 수소 플라즈마의 Fulcher-${\alpha}$ 띠 스펙트럼 해석에 합성 스펙트럼법을 적용하여 분자의 회전 온도가 측정 가능한지 확인하고, 고성능의 모노크로메터를 이용한 온도 측정 결과와 서로 비교하였다. 그리고 분자의 진동 상태(vibrational state)가 분자 회전 온도 측정에 미치는 영향과 이에 따른 측정의 한계 등을 제시하였다.
Unlike benign tumors, malignant tumors are capable of metastasis, easy to relapse, poor survival, and low quality of life. In Korea, here is a tendency to treat the tumors collectively according to the General Principles of Cancer Chemotherapy(GPCC) of the Health Insurance Review & Assessment Service (HIRA). But recently, companion diagnostics(CDx) is recommended rather than unilateral medication because biomarker-based molecular diagnostics is possible to predict the drug response of patients before drug treatment. Not only domestic but also overseas Food and Drug Administratio (FDA) recommends the development of the CDx system at the stage of drug development to ensure the responsiveness and safety of medicines. In this study, I focused on the necessity of CDx development direction as well as CDx development status through literature review. Furthermore I also discussed CDx types according to the molecular diagnostic technology such as immunohistochemistry (IHC), polymerase chain reaction (PCR), in situ hybridization (ISH), and next-generation sequencing (NGS) not only in the approved CDx but also in the developing one by US FDA. And I suggested the technology issue of CDx development process such as a selection of molecular diagnostics at the time of release, a clear understanding of the CDx mechanism, and a convergence of drug with CDx development. The necessity of social insurance system also was proposed for CDx development.

레이저 진단은 플라즈마 진단법 중 가정 정확한 진단법으로 알려져 있다. 특히 진단이 까다로운 고밀도, 자기장, 고압 환경에서도 그 측정 결과가 신뢰성 있다고 잘 알려져 있다. 본 특별 심포지엄에서는 중 성종을 측정 할 수 있는 레일레이 산란과 자유 전자를 측정 할 수 있는 톰슨 산란, 그리고 분자를 측정할 수 있는 라만 산란을 중심으로 발표 할 것이다. 또한 특정 준위를 갖는 원자 및 분자의 거동을 측정할 수 있는 레이저 유도 형광에 대해서도 간단히 발표 할 것이다. 그밖에 레이저 진단의 종류, 원리 등을 설명 하고 한국표준과학연구원에서 이루어 진고 있는 연구 결과에 대해 발표 할 것이다.

바이오칩의 대표 주자인 DNA 칩은 점차 분자생물학의 주요 도구로 인식되고 있다. 쓰임새 또한 다양해져 기초 생물학, 기능 유전체학 연구뿐만 아니라 임상 현장에서의 적용을 위한 연구가 활발히 진행되고 있다. 임상분야에서 최근 주목 받고 있는 분야가 DNA 칩을. 이용한 질병진단 및 예후 측정이다. 개별 환자 세포의 분자유전학적 상태는 DNA 칩의 유전체 프로파일링(genome-wide profiling)으로 상세히 파악될 수 있으므로, DNA 칩은 질병의 세부아형 진단, 약물에 대한 개인 민감도 측정, 정확한 예후 측정을 통한 환자의 세심한 관리 등 미래 의료의 핵심이라 할 수 있는 개인별 맞춤 치료(personalized medicare)를 가능하게 하는데 지대한 역할을 할 것으로 기대되고 있다. 특히 수많은 질병 중에서 현대인의 난치병으로 손꼽히는 암은 DNA 칩 분석의 주요 적용 대상이다. 암에 연관된 복잡한 메커니즘을 기존의 단일 표지자로 진단하는 데는 한계가 있기 때문에, DNA 칩을 이용해 질병의 특정 phenotype과 관련 있는 암의 특이 패턴을 전사체 수준에서 분석하여 새로운 형태의 분자유전학적 표지자(transcriptional molecular signature)를 발굴하는 것이다 본 발표에서는 이러한 연구에 쓰이는 DNA 칩 분석 방법들과 실제 위암 데이터에 적용한 사례에 대해 논의하고자 한다. 연세의대 암전이 연구센터의 17K cDNA 칩을 이용하였으며, 진단 및 예후 측정을 위한 여러 분석 방법을 수행하였다.
폐결핵의 진단은 주로 환자의 증상, 흉부 X-선, 객담의 균도말 및 배양검사에 의존하며 이는 과거와 크게 차이가 없으나 최근에는 좀더 효율적인 객담배출을 위한 방법, 분자생물학적 기법을 동원한 결핵균의 진단 및 결핵균항원 또는 항체를 혈액 등에서 측정하는 방법 등이 개발되어 결핵의 진단 율을 높이는데 기여하고 있다.
Many endocrine disorders have a genetic component. The genetic component is the major etiologic factor in monogenic disorders, while multiple genes in conjunction with environmental and lifestyle factors contribute to the pathogenesis in complex disorders. The development of the molecular basis of inherited endocrine diseases has undergone a dramatic evolution during the last two decades. The application of molecular technology allowed us to increase our understanding of endocrine diseases, and to impact on the practice of pediatric endocrinology related to diagnosis and genetic counseling. Identification of the mutation in the particular disease by genetic testing leads to precise diagnosis in the equivocal cases and prenatal diagnosis. However, clinicians should be cautious about determining therapeutic decisions solely on the basis of molecular studies, especially in the area of prenatal diagnosis and termination of pregnancy. This review describes an introduction to molecular basis of various inherited endocrine diseases and diagnosis by genetic testing.
Cytogenetic analysis plays an important role in examinations of a variety of human disorders. Over the years, cytogenetic analysis has evolved to a great extent and become a part of routine laboratory testing; the analysis provides significant diagnostic and prognostic results for human diseases. Microarray in conjunction with molecular cytogenetics and conventional chromosome analysis has transformed the outcomes of clinical cytogenetics. The advantages of microarray technologies have become obvious to the medical and laboratory community involved in genetic diagnosis, resulting in greatly improved visualization and validation capabilities. This article reviews how the field is moving away from conventional cytogenetics towards molecular approaches for the identification of pathogenic genomic imbalances and discusses practical considerations for the routine implementation of these technologies in genetic diagnosis.
Due to the highly contagious nature and severity of the respiratory diseases caused by COVID-19, economical and accurate tests are required to better monitor and prevent the spread of this contagion. As the structural and molecular properties of SARS-CoV-2 were being revealed during the early stage of the COVID-19 pandemic, many manufacturers of COVID-19 diagnostic kits actively invested in the design, development, validation, verification, and implementation of diagnostic tests. Currently, diagnostic tests for SARS-CoV-2 are the most widely used and validated techniques for rapid antigen, and immuno-serological assays for specific IgG and IgM antibody tests and molecular diagnostic tests. Molecular diagnostic assays are the gold standard for direct detection of viral RNA in individuals suspected to be infected with SARS-CoV-2. Antibody-based serological tests are indirect tests applied to determine COVID-19 prevalence in the community and identify individuals who have obtained immunity. In the future, it is necessary to explore technical problems encountered in the early stages of global or regional outbreaks of pandemics and provide future directions for better diagnostic tests. This article evaluates the commercially available and FDA-approved molecular and immunological diagnostic assays and analyzes their performance characteristics.
Preimplantation genetic diagnosis (PGD) has become an assisted reproductive technique for couples who are at risk that enables them to have unaffected baby without facing the risk of pregnancy termination after invasive prenatal diagnosis. The molecular biology and technology for single-cell genetics has reached an extremely high level of accuracy, and has enabled the possibility of performing multiple diagnoses on one cell using whole genome amplification. These technological advances have contributed to the avoidance of misdiagnosis in PGD for single gene disorders. Polymerase chain reaction (PCR)-based PGD will lead to a significant increase in the number of disorders diagnosed and will find more widespread use, benefiting many more couples who are at risk of transmitting an inherited disease to their baby. In this review, we will focus on the molecular biological techniques that are currently in use in the most advanced centers for PGD for single gene disorders, including biopsy procedure, multiplex PCR and post-PCR diagnostic methods, and multiple displacement amplification (MDA) and the problems in the single cell genetic analysis.
