The Effects of Simulation-based Delivery Nursing Practice on Proactivity of Problem-solving, Clinical Performance and Team Efficacy in Nursing College Students (시뮬레이션기반 분만간호 실습이 간호대학생의 문제해결의 적극성, 임상수행능력, 팀효능감에 미치는 효과)

  • The purpose of this study was to test the effects of simulation-based delivery nursing practice on proactivity of problem-solving, clinical Performance and team efficacy in nursing college students. This study conducted with a quasi-experimental one group pretest-posttest design. The subjects of the study were those who participated in the pretest-posttest survey among third graders in nursing who took a simulation-based delivery nursing practice class at S university in D city, and finally, data from 78 students who completed the survey were analyzed. Data collection was conducted from August 30, 2022 to October 28, 2022, and data were analyzed using SPSS 27.0 program. The findings suggest that simulation-based delivery nursing practice had a positive effect by significantly increasing on problem-solving proactivity, clinical performance ability, and team efficacy. However, There are limitations in generalizing the study results. therefore based on a sufficient number of samples, the effects should be verified through randomized controlled pre-and post-test and, To determine whether the effects are consistent, it will be necessary to follow up in the long term.

  • 분만진행에 수직체위가 효과적이라는 것을 연구하는 연구자는 이러한 체위가 자궁수축력을 강화하고 분만시간을 단축하여 분만시 동통을 감소시킨다는 것을 파악했다. 분만과 출산 시 쪼그리고 앉는 방법이 역사적으로 오랜 기간 동안 적용되어 왔음에도 불구하고 연구자들은 여러가지 분만방법의 장점에 대한 연구가 부족한 실정이다. 임상 경험에 의해 쪼그리고 앉는 자세가 보다 효과적임을 알면서도 여러가지 위험을 우려하여 적용을 기피하고 있다. 그러나 연구자들은 앞으로 이러한 체위의 가치를 확인하고 특별히 다른 체위와 비교하여 쪼그리고 앉는 행위가 경관열상의 빈도, 분만 1,2기의 시간, 분만 중이나 분만 후의 뇨정체의 유무, 제대탈출의 빈도등이 어떠한지를 파악하여야 할 것이다. 쪼그리고 앉는 체위에 대한 가장 좋은 잇점을 얻기 위해 조산자는 이러한 체위를 언제 어떻게 할 것인가에 대한 지식을 가져야 한다. 또한 융통성있고 창조적인 접근방법을 시도하여 의학적수행과 분만시 사정을 용이하게 하고 산부와 의료요원이 보다 편안하도록 하여야 한다. 조산교육자나 조산자는 분만시 그들의 체위에 대한 교육을 포함하여 쪼그리고 앉은 방법의 효과적인 적용을 위해 노력해야 한다. 임부에게 안전, 편안함, 분만의 효과적인 진행, 모든 선택 가능한 체위들을 교육하고 그러한 체위 중 쪼그리고 앉는 방법을 선택하도록 격려해야 한다.

The needs and effects of convergence-based simulation practice for obstetrical nursing (분만간호에 대한 융복합 기반 시뮬레이션 실습 효과 및 요구도)

  • The purpose of this study was to evaluate the needs and effects of convergence-based simulation practice for obstetrical nursing. The study participants were 47 junior nursing students of a university in Chungnam province. Data were collected by a self-administered questionnaire from February 13 to 25, 2012. Convergence-based simulation practice for obstetrical nursing was applied 8 hours in a day for 2 days and consisted of four sessions. Self-efficacy in obstetrical nursing (t=-14.74, p<.001) and nursing skills related to obstetrical nursing (t=-17.51, p<.001) showed significant increase in the posttest and practice satisfaction was higher than practice stress. The needs of simulation practice was 7.96 out of 10 point and higher as practice satisfaction was higher. The needs of simulation practice before clinical practice was higher than simulation practice after clinical practice. These results indicate that simulation practice for obstetrical nursing can be used as the alternative for clinical practice. In addition to simulation practice should be done before clinical practice and strategies to increase the nursing student's cognition for the need of simulation practice are also needed.

Quality Improvement of Clinical Practice in Nursing Students : Focused on Delivery Room Clinical Practice (간호대학생의 분만실 임상실습 질 향상 방안)

  • Purpose of this study was to provide strategies to improve quality in clinical practice by identifying the nursing student contents of clinical practice, performance level and satisfaction in the delivery room setting and correlations between the performance level and satisfaction. As the result, Nursing students were mostly rely on observational experiences with very limited experiences of performance and assist. Nursing students' clinical practice satisfaction were relatively low with 69 percentile score. The women's health clinic group showed higher score on frequency of clinical observation, clinical practice satisfaction than the tertiary hospital group. Correlations among frequencies of observation, assist and satisfaction were identified. Simulation based clinical practice as alternative ways to enhance students' performance level, utilizing community based speciality healthcare facilities for clinical practice, increasing observational experiences in the clinical field were suggested as strategies to improve quality of nursing clinical practice and ultimately to improve quality of healthcare services.

Effects of Doula-type-delivery Nursing Care on Plasma $\beta$-Endorphin, Serum Cortisol related to Delivery Stress during Labor, and Postpartum Status Anxiety of Primipara - Clinical trial - (Doula식 분만간호가 초산부의 분만중 분만스트레스로 인한 혈장 베타엔돌핀, 혈청 코티졸 및 분만후 상태불안에 미치는 효과 - 임상 실험연구 -)

  • This study was designed to verify the effect of Doula-type-delivery nursing care on plasma $\beta$-endorphin, serum cortisol, related to delivery stress during labor, and postpartum anxiety of primipara by a quasi experiment(nonequivalent control group pretest-posttest design), from December, 1999 to August, 2000. The subjects of this experiment consisted of sixty eight primipara, with single gestation, full term, uncomplicated pregnancies, thirty three for the experimental group and thirty five for the control group. Their mean age was 26.1 years for the experimental group and 25.5 years for the control group. Their mean gestation period was 39.7 weeks for the experimental group and 40.1 weeks for the control group. As treatment, Doula-type-delivery nursing care was given for the experimental group. Data assessed plasma $\beta$-endorphin, serum cortisol during labor, and anxiety during postpartum. Plasma $\beta$-endorphin, serum cortisol were measured in the latent phase before treatment(pre-test) and the transition phase after treatment(posttest). Also, anxiety was measured in the latent phase before treatment(pre-test) and 24 hours postpartum after treatment(posttest). Data was analyzed by t-test, $x^2$-test, Repeated measures ANOVA with SAS Program. The results of this study were as follows; 1. Plasma $\beta$-endorphin was significantly elevated in the experimental group who were cared for with Doula-type-delivery nursing care during labor(P=.0463). 2. No significant group effects were found, but significant time effects were found for serum cortisol. 3. The postpartum anxiety of the experimental group was significantly lower than the control group(P=.0110). In conclusion, these findings indicate that Doula-type-delivery nursing care during labor could be effective in increasing maternal plasma $\beta$-endorphin and decreasing postpartum anxiety. Doula-type-delivery nursing care during labor could be applied as an effective nursing treatment for primipara.

Effects of Doula Support in LDR (Labor-Delivery-Recovery) on Anxiety, Labor Pain, and Perceived Childbirth Experience of Primiparas (일개대학병원 가족분만실에서 듈라(Doula)식 분만지지간호가 초산부의 불안, 분만통증 및 분만경험지각에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effects of Doula support during labor on anxiety, labor pain, and perceived childbirth experience of primiparas. Method: Of 65 primiparas who were hospitalized in LDR from March 1 to September 30, 2007. 32 women were placed in the Doula group and 33 in the control group. VAS was used to measure the degree of labor pain and anxiety in the latent, active, and transitional phases. Perceived childbirth experience was measured within 2 hours after birth. Results: The Doula group had a significantly lower anxiety level than the control group in the active phase (t=-2.13, p=.04) and the transitional phase (t=-3.99, p=.000). The degree of labor pain of the Doula group was significantly lower than that of the control group for the active phase (t=-3.10, p=.003) and the transitional phase (t=-7.24, p=.000). Also, There was no significant difference in perceived childbirth experience between the two groups (t=.19, p=.85). Conclusion: The results of this study show Doula support in LDR decreases not only anxiety of primiparas but also labor pain in the active and transitional phases. Therefore Doula support by nurses in LDR can be a useful intervention during childbirth.

Effects of Clinical Practice and Simulation-Based Practice for Obstetrical Nursing (분만 간호에 대한 시뮬레이션 실습교육과 병원 분만실 실습교육의 효과)

  • Purpose: The purpose of this study was to evaluate the effects of clinical practice and simulation-based practice for obstetrical nursing in terms of self-efficacy, practice satisfaction, and practice stress. Methods: A non-equivalent control group post test design was used. The participants were 171 junior nursing students, 103 in the experimental group, and 68 in the control group. Simulation-based practice consisted of nursing assessment during labor, nursing assessment and intervention for normal and high risk mother, nursing education for mother and family during labor, and open lab. The experimental group participated in simulation-based practice for two days and the control group participated in clinical practice at delivery room for two weeks. Results: The experimental group showed higher level of self-efficacy (p= .043), practice satisfaction (p<.001) and practice stress (p=.003) compared to the control group. Conclusion: Simulation-based practice is an effective learning method for obstetrical nursing and can be used as the alternative for clinical practice. However, stress management strategies are needed for simulation-based practice.
