A Case Report of Meralgia Paresthetica (Meralgia Paresthetica 치료(治療) 1례에(例) 대한 증례보고)

  • Objective : The purpose of this case is to report the improvement after treatment about a patient with meralgia paresthetica. Methods : We treated the patient with acupuncture therapy and Herbal medication from 11th October 2004 to 15th October 2004 by evaluating femoral function with VAS score and R.O.M of femur joint. Results : After 5 times of treatment, this patient achieved excellent outcome following the technique, showing that clinical symptom as like heating sense, edema and pain was disappeared, VAS changed from 10 to 1 and there was no limitation of ROM of femur. Conclusions : Meralgia paresthetica is a symptom complex that includes numbness, paresthesias, and pain in the anterolateral thigh, which may result from either an entrapment neuropathy or a neuroma of the lateral femoral cutaneous nerve(LFCN). Oriental medical treatment for meralgia paresthetica resulted in satisfactory results by diminishing the symptoms progressively during the five days of treatment. Differential diagnosis was based on careful physical examination. More research of meralgia paresthetica is needed.

Study of Mechanical Characteristics of Electric Cupping Apparatus in Korea for Suggestion of its Assessment Guideline (국내 평가 가이드 라인 제시를 위한 전동식 부항기의 특성 조사에 관한 연구)

  • Objectives : The mechanical properties of Korean electric cupping systems are studied via experimental measurements. The study aimed at establishing the fundamentals of industrialization and systemization of oriental medicine device industry, as well as improving the quality of life for many Koreans. Methods : We reviewed the studies on traditional cupping as well as modern one to fine necessary factors for electric cupping systems. To characterize the mechanical properties of Korean electric cupping systems, we measured the pressure characteristics of commercially available electric cupping system by using an automatic pressure acquisition system and a standard cup. The pumping capability was checked at 40 seconds, and the stability of the suction cup was checked at 600 seconds. We also acquired the noise level of each system in clinical setting. To check the portability of each system, we also measured its physical dimensions. We scrutinized system manuals provided by the system manufacturers. Results : It took less than 5 second to reach the pressure if the connection between the air hose and the vacuum valve of the cupping system was secure. Pressure diminished to no more than 10% for 600s for all systems. Noise levels were 55~70 dB. Increase in pressure was too fast to control for a designated vacuum level except for one product. Conclusions : The Pumping ability of the systems is impressive and reliable. Pressure retention ability of each cup is quite reliable and reproducible. Therefore, their mechanical performances were worthy of recommendation. Some of them had noise level higher than 60 dB and they were bothersome. It was also suggested that the control for low to middle pressure needed to be accomplished by the cupping system.

Clinical Effect of Cupping Therapy on Cerebral Infarction Patients with Hyperlipidemia (뇌경색환자의 고지혈증에 대한 부항요법의 임상적 효과)

  • Objectives : This study was aimed at examining significant curative effects of Cupping therapy on Cerebral infarction patients with hyperlipidemia. Methods : The subjects in this study were fifty-two patients visited or admitted to Dong-seo Oriental Hospital who broke out with Cerebral infarction from March 2007 through February 2008, with high total cholesterol(240mg/$d\ell$ above) or high triglyceride(200mg/$d\ell$ above) or low density lipoprotein(160mg/$d\ell$ above) Before prescription, they were tested for total cholesterol, triglyceride, low density lipoprotein with fasting and were then rescribed the cupping therapy for 4 weeks(take an average ten times), after which they were retested for total cholesterol, triglyceride, low density lipoprotein. Results : 1. In the high total cholesterol group(triglyceride and low density lipoprotein are normal), statistical significance of total cholesterol changes was recognized before and after the treatment. 2. In the high triglyceride group(total cholesterol and low density lipoprotein are normal), statistical significance of triglyceride changes was recognized before and after the treatment. 3. In the high low density lipoprotein group(total cholesterol and triglyceride are normal), statistical significance of triglyceride changes was recognized before and after the treatment. 4. In the high total cholesterol and high triglyceride group(low density lipoprotein is normal), statistical significance of total cholesterol changes was recognized before and after the treatment. But, triglyceride changes was not that. 5. In the high total cholesterol and high low density lipoprotein group(triglyceride is normal), statistical significance of total cholesterol and low density lipoprotein changes were recognized before and after the treatment. 6. In the high triglyceride and high low density lipoprotein group(total cholesterol is normal), statistical significance of triglyceride changes was recognized before and after the treatment. But, low density lipoprotein changes was not that. 7. In the high total cholesterol and high triglyceride and high low density lipoprotein group, statistical significance of total cholesterol changes and low density lipoprotein changes were recognized before and after the treatment. But, triglyceride changes was not that. Conclusions : Cupping therapy decreased total cholesterol in whole hyperlipidemia group including high total cholesterol.

The Relationship between Dysmenorrhea and Alternative Medicine among the Internet Age (인터넷시대에서의 생리통과 대체요법이용과의 관련성)

  • This study was investigated the subjects' satisfaction in the therapy among the internet age. The subjects of the study were 200 women who have menses in ages from 18 to 55 among visitors of five public health centers and attendants of A church in Gwangju. The period of the study ranged from September 6 to October 10, 2010. As a study instrument, this study used a questionnaire developed based on questions from studies published by An Seung-duk(2003). This study used SPSS 17.0, for a frequency analysis, a cross-tabulation and a chi-squared test. During the analysis, 43.4% of the subjects used the complementary alternative therapy. 56.3% visited used oriental medical clinics or herbal medicine shops as places to use the therapy, 42.5% received medicine, acupuncture, moxibustion and cupping at oriental medical clinics. As expenses of the therapy, 56.3% spent less than 500,000 won a year. After the therapy, 51% answered they were satisfied with the therapy. In conclusion, it was discovered that use rate of the therapy is increasing, the subjects decided to use the therapy for themselves or through recommendation from acquaintances, and they mainly depended on reliable oriental medical clinics or folk remedies against menstrual pain, and insurance benefits for the therapy should be considered except oriental medical hospitals or clinics.

Risk associated with Adverse Events of Folk Medicine Reported in the Internet News Articles (인터넷 신문기사로 본 민간요법 유해사례의 위험성)

  • Folk medicine is traditionally passed down to cure disease, and adverse events (AEs) of folk medicine are any unfavorable and unintended discomforts temporally associated with the use of folk medicine. The aim of this study was to analyze AEs types and risks of folk medicine through the internet news articles. Included in this analysis are all articles on the topic of folk medicine and AE reported in the top 3 online news websites (NAVER, DAUM and NATE) determined by InternetTrend$^{TM}$(www.internettrend.co.kr). It was searched in the last five years (between 1 January 2009 and 28 February 2014). In total, 18 AEs articles of 973 news articles met our inclusion criteria. A total of 27 people were experienced AEs associated with use of folk medicine. Age was from 4 months to 76 years old, and it was occurred in both men and women. Folk medicine that caused AEs in twice or more was therapy that patient taking the dictamnus or aconitum of toxic herbal medicines, vinegar therapy of external use to topical skin, and cupping or bee sting therapy by practitioners. Death as a kind of serious AEs was 11 people, and 10 people were died after treatment by unqualified practitioner. Folk medicine that is popular and widely used in Korea is actively interacted with information on the internet, so it apt to misuse and abuse without guidance of health professionals. Aspects of health care system, we point out that the need for government and medical society establish not only correct health information plan and promotion of risk but also system as reporting and monitoring of AEs by folk medicine.

Review of Dam-Reservoir water level considering Dam-downstream situation (직하류 하천환경을 고려한 댐 저수량 수위 확보 검토)

  • 다목적댐은 비어있는 저수지 용량을 활용, 상류에서 유입되는 홍수를 저류하여 하류의 피해를 경감시키고, 저류된 저수량을 활용하여 갈수시 생 공 유지용수 등을 목적에 맞게 하류로의 공급에 주요 목적이 있다. 이를 위해 댐 계획에는 상류에서 유입되는 목표 빈도 계획홍수량을 산정하고, 이에 따른 여수로의 계획방류량 등이 포함된 홍수 조절 방안이 수립된다. 다목적댐은 그 중요도 때문에 많은 사회적 재화(財貨)가 투입되어 하천에서 가장 높은 빈도인 200년 빈도 홍수량에 대비하도록 설계되어 있다. 그러나 대부분의 댐 하류 하천에서는 긴 연장에 따른 투자비용 등의 한계로 상류 댐에 미치지 못하는 100년 빈도 이하로 설정되어있다. 이런 상 하류의 홍수계획간 괴리로 댐의 계획 방류량 수준의 홍수 조절시에는 하류 하천에 재난상황으로 발생할 우려가 있다. 기상예보의 고도화로 단기 예보의 정밀도가 높아져 단기간 댐 운영에는 효과적으로 활용되고 있다. 하지만 5일 이후를 예보하는 중 장기예보는 아직까지도 불확실성이 높은 실정이다. 여기에 기후변화 등 기상의 불확실성을 고려하여 홍수기 초 장마와 태풍 등의 영향에 대비하기 위해 무작정 댐을 저(低)수위로 운영하면 이후 여름 가뭄에 취약해 지고, 반대로 저수량 확보의 고(高)수위 운영을 할 경우 연이은 태풍 등 홍수상황에 취약해 지게 된다. 최근 '14~'15년도 낙동강 수계에 여름가뭄으로 안동 임하댐이 가뭄단계에 돌입하는 등 홍수기초 적정 저수량의 확보는 안정적 수자원시설 운영에 중요성이 커지고 있다. 본 연구에서는 이러한 불확실한 기상상황에 대비하고 댐의 이치수 목적 달성을 위해, 홍수기초 가능한 저수량을 확보하되 계획홍수량이 들어와도 하류피해는 경감시킬 수 있는 저수지 수위를 찾고자하였다. 이를 위해 낙동강 수계 9개 다목적댐(안동, 임하, 성덕, 보현산, 군위, 김천부항, 합천, 남강, 밀양댐)을 대상으로 댐 설계 및 직하류 하천 상황을 고려한 "홍수기초 댐 저수량 확보 수위 검토"를 실시하였다. 댐 직하류 하천의 홍수대비 상황 및 홍수조절시 제약사항 등을 조사해 주요지점(Critical Point)을 선정하고 그 지점의 홍수량을 파악한다. 이어서 저류함수 모형을 활용하여 댐별 저수지 모의 운영을 실시하였다. 대상 댐 저수지에 계획홍수량을 유입시키고 수위를 점차 조정하면서 하류 지점의 홍수량 내의 규모로 방류를 하는 저수지 모의 운영을 실시하였다. 이때 저수지의 계획홍수위를 초과하지 않는 등 안정적 홍수조절이 가능한 특정 수위를 찾아 하류 하천 환경을 고려한 댐 운영 수위로 선정하였다. 최근 홍수기총에는 강수량이 계속 줄어들고 홍수기 이후 더 많은 강수가 내리는 등 기후변화로 댐 운영은 날로 어려워지고 있다. 본 연구의 분석 결과는 홍수기초 가뭄 및 장마 또는 태풍 등에 대비하여 댐 운영의 이 치수 두 가지 목적을 달성하는데 참고가 될 수 있을 것이다.

One Case of Otomycosis with Chronic Suppurative Otitis Media Treated with Korean Medical Treatment (만성 화농성 중이염에 동반된 이진균증 환자 한방 치료 1례)

  • Objectives : Otomycosis is superficial fungal infection of external auditory canal accompanied with itching, pain, discharge, hearing loss, tinnitus and ear fullness. And chronic inflammation like chronic suppurative otitis media exacerbates otomycosis. The purpose of this study is to report one case of Korean medical treatment for otomycosis with chronic suppurative otitis media. Methods : This study was conducted with one case of otomycosis with chronic suppurative otitis media patient hospitalized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean medical Hospital. This patient was treated with Korean medical treatment including herbal medicine, acupuncture, pharmacopuncture and venesection. After the treatment, we took picture of external auditory canal by otoscope and evaluated subjective symptoms. Results : After the treatment, this patient had improved condition of external auditory canal and subjective symptoms. Conclusions : This study suggests that Korean medical treatment may be effective for otomycosis with chronic suppurative otitis media.

  • This case study showed improvement both in pain and gait disturbance caused by SCH. KM can be a positive treatment for reducing pain or improving gait disturbance caused by SCH, and it can be considered a treatment option for SCH. Future studies with a larger number of cases and longer period of follow-up in SCH with gait disturbance and rigidity are needed.
