A Retrospective Cross-sectional Study on Correlation between the Level of Malondialdehyde in Urinalysis and Cold Hypersensitivity (요중 malondialdehyde의 정도와 냉증의 연관성에 대한 후향적 단면연구)

  • Objectives: This study aims to research correlation of the malondialdehyde level in urinalysis with cold hypersensitivity. Methods: We studied 128 patients visiting ${\bigcirc}{\bigcirc}$ Korean Hospital from March, 2013 to May, 2013. The subjects were categorized into two groups: cold hypersensitivity group (n=46) and control group (n=79). Patients in cold hypersensitivity group had been stressed because of severe cold hypersensitivity. First, We investigated the difference of the level of malondialdehyde between two groups. In addition, temperature of the two points, ST32 and LR3, was measured by using Digital Infrared Thermal Imaging (DITI) to find out whether the extent of cold hypersensitivity is related to the level of malondialdehyde. Results: There is no different characteristics between two groups. The average of malondialdehyde in cold hypersensitivity group ($1613.15{\pm}1260.71$) is significantly higher than in control group ($1170.01{\pm}1015.66$)(p=0.04). But there is no correlation between level of malondialdehyde and the extent of cold hypersensitivity (r=-0.22, p=0.13). Conclusions: Although the level of malondialdehyde in cold hypersensitivity group is higher than in control group, there is no significant correlation between level of malondialdehyde and the extent of cold hypersensitivity.

A Clinical Research on the Health Condition and the Etiology in Oriental Medicine of the High School Girls Menstrual Disorders by Sasang Constitution (사상체질에 따른 월경장애 여고생의 건강도와 한의학적 원인에 대한 임상연구)

  • Objectives: The purpose of this study is to find out the health condition and the etiology in oriental medicine of the high school girls menstrual disorders by Sasang constitution. Methods: The data from the 795 participants were collected using a structured menstrual history questionnaire. Based on the survey responses, we had 97 adolescents with menstrual disorder as the test group and 97 adolescents without menstrual disorder as the control group. The clinical trials subjects were asked to respond to another questionnaire for identifying their constitutional types and respond to the DSOM and undergo Inbody. We identified significant variables for dysmenorrhea from comparison between the test and control group. Results: The DSOM test showed that the scores of qi stagnation, static blood, cold, dampness, phlegm, heart, kidney were significantly higher in dysmenorrhea, scant menstruation, menstruation at irregular intervals. For the Sasang constitution, there is a difference on the cause of the outbreak. Soyangin from yin deficiency, dryness and blood deficiency is associated with menstrual disorders. Taeumin from cold, dampness and qi deficiency is associated with menstrual disorders. Soeumin from heart, qi stagnation, static blood and spleen is associated with menstrual disorders. Conclusions: For the Sasang constitution, there is a difference on the cause of the outbreak. The result of a comparison of the test and control groups showed that there's no relevance to the body fat mass and body fat percentage with menstrual irregularities.

  • Objectives: This study aims to report the effect of Korean medicine treatments on a pelvic inflammatory disease (PID). Methods: The patient was treated with Qingbaozhuyu-tang, acupuncture at Jungwan (CV12), Gihae (CV6), Taechung (LR3), Hapgok (LI4), Sameumgyo (SP6), Sangryo (BL3), Charyou (BL32), Jungryo (BL33), Haryo (BL34), Cheonchu (ST25), Pungryung (ST40), Duyu (ST8), Taeyang (EX-HN5). And we evaluated treatment effects by visual analogue scale (VAS) and Urine analysis (UA) Finding. Results: After treatments, the symptoms such as lower abdominal pain, pelvic pain and fever were improved and there were no abnormal findings in urine analysis. Conclusions: This study suggests that Korean medicine treatments show effective on PID treatment.

The Analysis of Delivery Outcomes in Cases of Hyperemesis Gravidarum Patients Medicated with Pinelliae Rhizoma - to Establish a Guideline in Administration of Pinelliae Rhizoma (임신오조 치료에 있어 반하 사용지침 마련을 위한 반하 투약 사례의 분만결과 분석 연구)

  • Objectives: The aim of this study is to identify effects and safety of Korean traditional medicine, especially including Pinelliae Rhizoma, for hyperemesis gravidarum, and to suggest appropriate dosage and treatment duration of Pinelliae Rhizoma. Methods: The study was performed based on the medical records of 19 hyperemesis gravidarum patients treated with Korean traditional medicine including Pinelliae Rhizoma. Change of symptoms, continuance of pregnancy, malformation of infants, and delivery outcomes were analysed. Results: The symptoms of hyperemesis gravidarum were improved in 13 patients. With regards to pregnancy outcome, normal fullterm delivery was 16 case, preterm delivery was 1 case and abortion was 2 case, Infants malformation was not found in any case. The average daily dosage of Pinelliae Rhizoma was 6.53 g/day, and the average treatment duration of Pinelliae Rhizoma was 11.53 (day). Conclusions: In this study, using Korean traditional medicine including Pinelliae Rhizoma for hyperemesis gravidarum patients is considered safe if patients are treated with proper clinical dosage and duration of medication. More cases and studies in the future will be required for the appropriate use of Pinelliae Rhizoma for hyperemesis gravidarum patients.

The Effect of Guibi-tang Water Extract on Osteoclast Differentiation and Osteoblast Proliferation (귀비탕(歸脾湯)이 파골세포 분화와 조골세포 활성에 미치는 영향)

  • Objectives: This study was performed to evaluate the effect of Guibi-tang water extract (GB) on osteoporosis. Methods: We examined the effect of GB on osteoclast differentiation using murine pre-osteoclastic RAW 264.7 cells treated with receptor activator of nuclear factor kappa-B ligand (RANKL). The effect of GB on osteoclast was measured by counting TRAP (+) multinucleated cells and measuring TRAP activity. The mRNA expressions of osteoclastogenesis-related genes (Cathepsin K, MMP-9, TRAP, NFATc1, MITF, TNF-${\alpha}$, IL-6, COX-2) were measured by real-time PCR. We examined the effect of GB on osteoblast proliferation, ALP activity, bone matrix protein synthesis and collagen synthesis using murine calvarial cell. Results: GB decreased the number of TRAP (+) multinucleated cells and inhibited TRAP activity in RANKL-stimulated RAW 264.7 cell. GB decreased the expression of genes related osteoclastogenesis such as Cathepsin K, MMP-9, TRAP, NFATc1, MITF, COX-2 in RANKL-stimulated RAW 264.7 cell. But GB did not decrease the expression of iNOS and increased the expression of TNF-${\alpha}$, IL-6 in RANKL-stimulated RAW 264.7 cell. These genes (iNOS, TNF-${\alpha}$, IL-6) are thought to be related with the inflammatory bone destruction. GB increased cell proliferation of rat calvarial cell and also increased ALP activity in rat calvarial cell. GB did not increase bone matrix protein synthesis but increased collagen synthesis in rat calvarial cell. Conclusions: This study suggests that GB may be effective in treating osteoporosis by inhibiting osteoclast differentiation and its related gene expression and by increasing osteoblast proliferation.

  • Objectives: This study has been conducted on public health purposes for people with Menopausal disorders in rural areas. Methods: From February, 2017 to June, 2017, we performed the Korean Medicine in the "Menopausal Disorder Overcoming Program" conducted by the department of Health service, Imsil-Gun bogun Hospital. Menopausal women who participated were treated with acupuncture, moxibustion, Jaeumganghwa-tang for 20 weeks. Of the 34 patients, 16 patients were excluded due to absence of treatment at least once. And 18 treatment outcomes were analyzed. Results: The KI results showed that the average score decreased in 7 out of 11 questions after treatment. The results of BDI, the mean scores of the 17 questions except the 4 questions of the 21 questions after the treatment decreased. 12, 16, and BDI total score were statistically significantly reduced before and after treatment. Conclusions: The results of the Korean Medicine treatment seemed to be effective in alleviating facial flushing, insomnia, depressive symptoms and other symptoms in menopausal disorders.

Review on the Use of Oriental Herbal Medicine for Female Idiopathic Precocious Puberty (한약 치료를 이용한 여성 특발성 성조숙증의 임상연구에 대한 문헌고찰)

  • Objectives: This review aims to report the efficacy of Oriental herbal medicine for Female Idiopathic Precocious Puberty Methods: We searched clinical study about precocious puberty from Pubmed, OASIS, Korea Traditional Knowledge Portal and National Assembly Library up to July 2017. Results: Four case reports have been identified. Six cases in total were found. And in the four cases where estradiol (E2) levels were above the normal levels in pre-treatment, the figures dropped to the normal levels in post-treatment. In the three cases with breast development and breast pain in pre-treatment, symptoms improved after the treatment. And, the sustained growth of height was reported in the four cases with the treatment duration of 30 days or more. Conclusions: It has been identified that oriental medicine assisted improvements of hormone levels and breast pain and growth of height on patients with precocious puberty, yet more case reports and well-designed studies would be necessary to guide better oriental herbal medicine treatment of precocious puberty.

Development of a Guideline for the Application of Diagnostic Devices for Menopausal Syndrome: Literature Review and Questionnaire Survey (갱년기장애 진단기기 사용에 대한 지침 개발 : 문헌검토 및 설문조사)

  • Objectives: This study was aimed to develop a guideline for the application of diagnostic devices for menopausal syndrome. Methods: We conducted a literature review and a questionnaire survey on diagnostic devices including Digital Infrared Thermal Imaging (DITI), pulse diagnosis device, Heart Rate Variability (HRV), body composition analyzer, Yangdorak. Results: We retrieved some clinical values for usage of devices from the articles. Especially, DITI was useful to identify the pattern of body temperature distribution. The respondents answered that they diagnosed menopausal syndrome by using body composition analyzer (62.3%), DITI (60%), HRV (60%), pulse diagnosis device (45.7%), Yangdorak (34.3%). The respondents answered that they don't use diagnostic devices when they diagnosed menopausal syndrome because of absence of device, cost, difficulty of interpretation, substitution of another diagnostic method. After experts survey, it was recommended to use DITI, HRV, body composition analyzer. There was no consensus on the use of pulse diagnosis device, Yangdorak in diagnosing menopausal syndrome. Conclusions: We developed a guideline for the application of diagnostic devices for menopausal syndrome.

The Effectiveness of Acupuncture and Moxibustion Treatment for Mastitis: A Systematic Review (유방염의 침구 치료에 대한 체계적 문헌고찰)

  • Objectives: The purpose of this study is to investigate the effectiveness of acupuncture and moxibustion for mastitis. Methods: We used ten databases including Embase, PubMed, and CNKI to investigate the literatures of mastitis using the keywords "mastitis" and "acupuncture", "breast inflammation" and "acupuncture", "mastitis" and "needle", "breast inflammation" and "needle", "mastitis" and "moxibustion". We excluded studies which included treatments that could affect the judgement of the effectiveness of acupuncture treatment, and the control group was limited to antibiotics. The quality of the paper was evaluated by Risk of Bias. A meta-analysis was performed using a "Review manager" to see the effectiveness of acupuncture and moxibustion compared to antibiotics. Results: Only six RCT were finally selected. Five studies use the total effective rate as a evaluating index, and it was significantly higher than that of the control group in two studies. In other three studies, acupuncture showed similar effectiveness to antibiotics. Meta-analysis was performed through three studies, there was no statistically significant difference in total effective rate between acupuncture and antibiotic treatment. Conclusions: Analysis of six RCT showed that acupuncture and moxibustion seem to have many possibilities as one of the treatments for mastitis. However the number of documents is not sufficient, more research should be done to obtain reliable information.

A Survey on Korean Medicine Doctors' Recognition and Treatment for Developing Korean Medicine Clinical Practice Guideline of Coldness of Hands and Feet (한의표준임상진료지침 개발을 위한 수족냉증에 대한 한의사의 인식과 치료현황)

  • Objectives: The purpose of this study were to researched a Korean medicine doctors' recognition about coldness of hands and feet, and developing of korean medicine clinical practice guidelines (CPG) for coldness of hands and feet. Methods: We conducted a questionnaire survey targeting 399 Korean medicine doctors belonging to the Association of Korean Medicine by e-mail and analyzed the answers. Results: 1. 86.86% of the respondents agreed about the necessity of CPG for coldness of hands and feet. 2. 84.2% of respondents wanted coding of Korean Standard Classification of Diseases (KCD) on coldness of hands and feet. 3. To diagnosis a coldness of hands and feet, the respondents used a Subjective symptoms (98.5%), Infrared thermographic imaging device (DITI) (26.32%) Heart rate variablity test (HRV) (17.04%), Thermometer (9.77%), Cold stress test (2.76%) 4. Causing of coldness of hands and feet, the respondents considered a constitution or heredity (84.71%), stress (73.66%), lack of exercise (64.91%), irregular eating habits (51.63%), Cold meals (32.83%), depression (31.33%), etc. 5. Treating coldness of hands and feet, the respondents used a herbal medicine (66.85%), acupuncture (70.7%) Pharmacopuncture (23.85%) and moxibustion (60.08%) for $10.91{\pm}8.03week$. Conclusions: We researched a Korean Medicine doctors' recognition of CPG, clinical diagnosis, treatment on a coldness of hands and feet, and policy they required.
