Perceived Usefulness and Attitude toward Smart-glass for First-aid Remote Support among Coast Guards in Korea (응급처치 원격지도용 스마트글래스 사용에 대한 한국 해양경찰의 인지된 유용성 및 태도)

  • This study was to investigate the types of emergencies transported by the Southwestern Coast Guard, the need for telemedicine guidance, and the perception and attitude of smart glasses as a communication method targeting 31 coast guards. A relatively high frequency and training requirement were confirmed for bleeding, abrasion, and abdominal pain. The demand for telemedicine guidance on medication and triage was higher, and the perceived usefulness and attitude scores for the use of smart glasses were 3.76±0.61 and 3.64±0.45, respectively. A moderate correlation between perceived usefulness and attitude toward smart glasses was confirmed (r=.630, p<.01). With the development of technology, it is time to actively introduce new devices such as smart glasses.

A Case Study of Korean Medical Treatment for Indigestion and Abdominal Pain after Gastrectomy (위절제술 후 발생한 소화불량 및 복통에 대한 한방 치료 증례 보고)

  • Objective: This case study reports the effects of treating a gastric cancer patient who had undergone gastrectomy with Korean medicine. Methods: After gastrectomy, a 77-year-old female patient received acupuncture, electroacupuncture, herbal medicine (Banhabaekchulchunma-tang, Banhahubak-tang), and moxibustion for one month. Results: After treatment, changes were observed in the patient's numeric rating scale (from 7 to 2); visual analog scale (from 8.7 to 2.6); Short Form-36 Health Survey scores (physical from 20 to 48 and mental from 14 to 53); Functional Dyspepsia-Related Quality of Life results (from 84 to 41); and Korean Nepean Dyspepsia Index (from 63 to 32). Conclusion: Korean medical treatment for dyspepsia following gastrectomy is worthy of further research.

A Case Study of a Dumping Syndrome Patient Treated with Gamchobunmil-tang (조기 덤핑증후군 환자의 복통에 감초분밀탕을 사용한 1례)

  • The purpose of this study is to explore the effectiveness of Gamchobunmil-tang as a treatment for early dumping syndrome. A patient reported abdominal pain after eating, was not able to eat more than two spoons of thin rice gruel, and had lost 8 kg of his weight. The patient was treated with Gamchobunmil-tang three times a day for 12 days. The effects of the treatment were evaluated using the Visual Analogue Scale and the amount and form of the patient's intake. After treatment, the pain due to the dumping syndrome decreased. The Visual Analogue Scale score dropped significantly from 9 to 2, and increased amounts and novel forms of the food the patient consumed were observed. Despite the lack of a case, the results showed that Gamchobunmil-tang was effective in reducing the pain caused by the early dumping syndrome. Further research might prove that Gamchobunmil-tang could be an alternative treatment method for early dumping syndrome.

Classification of Infant Crying Audio based on 3D Feature-Vector through Audio Data Augmentation

  • Infants utilize crying as a non-verbal means of communication [1]. However, deciphering infant cries presents challenges. Extensive research has been conducted to interpret infant cry audios [2,3]. This paper proposes the classification of infant cries using 3D feature vectors augmented with various audio data techniques. A total of 5 classes (belly pain, burping, discomfort, hungry, tired) are employed in the study dataset. The data is augmented using 5 techniques (Pitch, Tempo, Shift, Mixup-noise, CutMix). Tempo, Shift, and CutMix augmentation techniques demonstrated improved performance. Ultimately, applying effective data augmentation techniques simultaneously resulted in a 17.75% performance enhancement compared to models using single feature vectors and original data.

  • Purpose: The purpose of this study was to report the effect of TKM (Traditional Korean Medicine) to abdominal pain after laparoscopic gynecological surgery(LGS). Methods: 10 patients who visited the department of gynecology in OO medical center from 1st August 2007 to 31st December 2008 with abdominal pains after laparoscopic operation. They complained abdominal pain and other pains such as back pain, shoulder pain and vaginal bleeding etc. We treated patients with herb medicine, acupuncture and moxibustion treatment. The progress of signs and symptoms was evaluated by checking the change of visual analog scale(VAS). Results: The mean age was 45.1 years(range 38-49), parity 2(0-3) and previous abdominal surgery case was 5. The mean of hospital admitting day was 20 days(range 9- 51) and taken for reducing VAS 10 to 3 were 10 days(range 4-24). After taking TKM, patient's signs and symptoms were alleviated or resolved and Hb, Hct were increased. Conclusion: After laparoscopic gynecological surgery, patients had taken pain such as abdominal pain, shoulder pain, back pain etc. TKM treatment is effective on the recovery after laparoscopic surgery.

  • Kikuchi-Fujimoto disease (KFD) is an acute febrile disease that mainly involves histiocytic necrotizing lymphadenitis in children and young adults. Diagnosis of KFD is even more difficult if image-guided percutaneous biopsy is technically challenging. We present a case of clinically diagnosed KFD in an 11-year-old boy who presented with fever, abdominal pain, and mesenteric lymphadenopathy, resulting in a diagnostic challenge. Additionally, we conducted a systematic review, and our goal was to describe the spectrum of disease, therapy, and outcomes. We identified 15 cases of KFD with symptoms that mimicked mesenteric lymphadenitis. Reports from the Americas, Europe, and Asia were also included. Most patients were male, exhibited leukopenia and elevated inflammatory markers, and recovered without significant sequelae or complications. A high index of suspicion of KFD should be maintained in children presenting with prolonged fever and unusual manifestations, such as mesenteric lymphadenitis.

A Case of the Large Colon Impaction in an Adult Donkey (성숙한 당나귀의 큰잘록창자 폐색 증례)

  • The purpose of this report is to describe the diagnosed case of a large colonic impaction in an adult donkey. An adult female donkey (Equus africanus asinus; 6-years-old) was admitted to the Equine Hospital of Jeju Race Park with signs of anorexia, which had started 1 day previously. Clinical examination revealed: weak gut sounds, dehydration, and severe abdominal distension, but normal respiratory and heart rates and no signs of pain. Transrectal palpation identified an impaction in the colon. The patient was treated with nasogastric siphonage, fluid therapy, and nonsteroidal anti-inflammatory drugs. Over the course of a day, her condition worsened; she started trembling, could not stand, and 2 hours later she died. Upon necropsy, a vast amount of ingesta was found in the large colon, particularly in the left ventral colon. The cecum and small colon were normal. Eleven days prior to presentation, the stable management and diet of the donkey had changed. We suspect that the change of feed, reduced water consumption, and lack of exercise precipitated the illness. However, the exact cause of the sudden death of the donkey requires further investigation.

  • Elderly patients with a history of chronic alcoholism presented to our hospital with episodes of melena, abdominal pain, and anemia. During admission, hemorrhagic cystic lesion at the pancreas was observed on abdominal CT. Transcatheter angiography confirmed active bleeding foci and arterial embolization was performed. After the procedure, the bleeding was resolved. The authors report two cases of hemosuccus pancreaticus and pancreaticocolic fistula associated with pancreatitis, a rare cause of gastrointestinal bleeding, treated with vascular intervention.

  • Undifferentiated pleomorphic sarcoma (UPS) is a high-grade soft tissue sarcoma that arises from mesenchymal tissue. Primary UPS of the small intestine is extremely rare, and only a few cases have been reported in the literature. Its presentation is usually nonspecific; however, it may occasionally present as intussusception with intermittent abdominal pain. It is a highly aggressive tumor with a propensity for early distant metastasis to the peritoneum, lymph nodes, other abdominal organs, lungs and brain. To our knowledge, there are no reported cases of endobronchial metastasis from small intestine UPS. We report a rare case of UPS of the small intestine with endobronchial metastasis presenting as intussusception.

  • Malrotation and midgut volvulus are surgical emergencies that commonly occur within the first month of life. The classic symptom is acute bilious vomiting, while nonspecific symptoms such as recurrent abdominal pain may be present in older children. Malrotation can be associated with duodenal obstruction caused by an abnormal peritoneal fibrous band or congenital anomalies, such as an annular pancreas or a preduodenal portal vein. Volvulus can lead to bowel ischemia and a life-threatening condition, thus prompt and accurate diagnosis is crucial. Diagnosis can be made through upper gastrointestinal series, ultrasonography, and CT, with ultrasonography being preferred as a screening tool due to its rapid and accurate diagnosis, without radiation exposure, in children. This pictorial essay discusses the key imaging findings and diagnostic approaches for malrotation and midgut volvulus, as well as diagnostic pitfalls based on actual cases.
