Kikuchi-Fujimoto Disease Mimicking Mesenteric Lymphadenitis in Children: A Case Report and Systematic Review

  • Kikuchi-Fujimoto disease (KFD) is an acute febrile disease that mainly involves histiocytic necrotizing lymphadenitis in children and young adults. Diagnosis of KFD is even more difficult if image-guided percutaneous biopsy is technically challenging. We present a case of clinically diagnosed KFD in an 11-year-old boy who presented with fever, abdominal pain, and mesenteric lymphadenopathy, resulting in a diagnostic challenge. Additionally, we conducted a systematic review, and our goal was to describe the spectrum of disease, therapy, and outcomes. We identified 15 cases of KFD with symptoms that mimicked mesenteric lymphadenitis. Reports from the Americas, Europe, and Asia were also included. Most patients were male, exhibited leukopenia and elevated inflammatory markers, and recovered without significant sequelae or complications. A high index of suspicion of KFD should be maintained in children presenting with prolonged fever and unusual manifestations, such as mesenteric lymphadenitis.

The Risk Factors for Infectious Complications after Elective Gastrectomy for Gastric Cancer (위암의 정규 위절제술 후 감염성 합병증의 위험인자)

  • Purpose: Postoperative Infectious complications are recognized as major complications that are associated with surgery. Although many studies have focused on the risk factors of postoperative complications, little is known about the risk factors of infectious complications after gastric cancer surgery, and especially after elective gastrectomy. There is now more and more interest in the risk factors of infectious complications in relation to controlling infection and as indicators of qualitatively assessing infectious complications. The aim of this study was to evaluate the risk factors related with infectious complications after performing elective gastrectomy for treating gastric cancer. Materials and Methods: We retrospectively reviewed a total of 788 patients who had undergone elective gastrectomy for gastric cancer between Jan. 2000 and Dec. 2007. The characteristics of the patients were divided according to the patients' factors and the operations' factors. Results: The patients' mean age was 58.9 (range: 24~91) years; 545 were male and 243 were female. The mean duration of the hospital stay was 20.3 days (range: 5~135 days), the mean operation time was 181.3 minutes (range: 65~440 minutes). The total complication rate was 17.1% (n=135) and the complication rate was 38.5% (n=52) among the 135 patients with infectious complications. The infectious complications were surgical site infection (59.7%), Pneumonia (19.3%), intra-abdominal abscess (11.5%), pseudomembranous colitis (5.7%), bacteremia (1.9%) and hepatic abscess (1.9%). On the univariate analysis, the significant risk factors were male gender, blood transfusion, smoking at the time of diagnosis, alcohol drinking, diabetes mellitus and previous cardiovascular disease (P<0.05 for all). On multivariate analysis that used a logistic regression model, the significant independent risk factors were smoking at the time of diagnosis (OR: 2.877. 95% CI: 1.449~5.713), blood transfusion (OR: 3.440, 95% CI: 1.241~9.534), diabetes mellitus (OR: 3.150, 95% CI: 1.518~6.538), and previous cardiovascular disease (OR: 2.784, 95% CI: 1.4731~5.2539). Conclusion: Pre- or post-operative blood transfusion and the patient's medical history such as previous cardiovascular disease, diabetes mellitus, smoking etc. are the risk factors for infectious complications after undergoing elective gastrectomy for gastric cancer. The patients that have these risk factors need to be treated with great care to prevent infectious disease after elective gastrectomy.

  • Experiments were performed in mice(Balb/C) to support the basic efficacy of the human immunoglobulin (IgG) preparation. The antibacterial activity of IgG purified from human sera was examined with or without the quinolone agent, ciprofloxacin(CPFX), against Pseudomonas aeruginosa isolated from clinical specimens. Results were as follows: Antibacterial activities in terms of the percentage of survivors, after administration of Ps. aeruginosa into mouse intraperitoneal cavity were in the following order, single IgG group, CPFX administration after IgG pretreatment group, IgG and CPFX combined administration group and CPFX alone group. The number of living bacteria was monitored in blood and liver tissue of mice infected with Ps. aeriginosa and treated by IgG administration. The increase of living bacteria in liver was more drastic than that in blood. Leukocytosis was observed in mice injected with IgG, excluding those only with ciprofloxacin, after 8 hours of administration to see a decrease to normal number of bacteria after 18 hours. No significant difference was noticed between pretreatment group and post treatment group. In vitro susceptibility test of IgG against Ps. aeruginosa, minimal inhibitory concentration(MIC) was $250{\mu}g/ml$, resistant to IgG, regardless of a combined administration with CPFX. In vitro test revealed that the IgG itself did not have anti-Ps. aeruginosa activity.

  • In a 43-year-old Korean man who underwent radical gastrectomy due to a malignant stromal tumor, was found to have an enlarged Iymph node at transverse mesocolon. The Iymph node exhibited histologically necrotizing eosinophilic granuloma formed around a track containing sections of a nematode larva. The well preserved nematode sections revealed polymyarian muscle cells, Y-shaped lateral cord, a large excretory gland cell, intestine and eosinophilic cuticle. The nematode sections were identified as a larva of Anisnkis species In Korea, thIns is the first case of elrtragas trio into stinal anisakiasis.

  • A massive mycetoma was observed in the abdominal cavity of a 3-year-old spayed female Labrador Retriever dog originated from France. The mycetoma lesion involved adhesion with one-half of the spleen, several portions of the small and large intestine, and the diaphragem. Histologically the distinct hypertrophy of the intestinal muscle layer, wide-spread deposition of the hemosiderin through the affected abdominal organs and thrombosis of splenic arterioles with atheromatous change were recognized as persistant, debilitating changes of the affected organs. From the histological appearance of the fungal colonies it was presumed that the causative fungus to be identified as Allescheria boydii.

Experimental life history of Spirometra erinacei (우리 나라에 분포하는 스파르가눔의 실험실 내 생활사)

  • The complete life cycle of Spirometra erinacei has been experimentally maintained in the laboratory. The cyclops were reared as the first intermediate host, and the tadpoles of Rana nigromaculata as the second intermediate host. ICR mice were used as another second host. The experimental definitive hosts were dogs and cats. Maturation and hatching of the eggs took 8 to 14 days by incubation at 29℃. The coracidium measured 43.8×36.9㎛. Mesocyclops leuckarti and Eucyclops serrulatus were susceptible to the coracidial infection. The procercoids older than 5 days in the cyclops had minute spines at the anterior end, calcium corpuscles in the body parenchyme and the cercomer at the posterior end. Procercoids 10 to 20 days old were infective to tadpoles, and 15 or 21 day old worms could infect the mice. The plerocercoids from the tadpoles at 15 days after experimental infection were pear-shaped and shorter than 1 mm in the length and were infective to mice. Fifteen to 18 days after experiMental inoculation of plerocercoids to dogs or cats, the adult worms began to produce eggs. One life cycle from egg to egg needed 48 to 67 days in the laboratory. The morphology of larval or adult worms was compatible with the description of Spirometra erinacei.

Antiretroviral Effects of 2',3'-Dideoxycytidine and Recombinant $Interferon-{\alpha}-A$ on the Infection of Anemia-inducing Murine Friend Virus (Anemia-inducing Murine Friend Virus 감염에 대한 2',3'-dideoxycytidine 및 $Interferon-{\alpha}-A$의 항retrovirus효과)

  • The anemia-inducing strain of Friend virus (FVA) is a murine retrovirus which stimulates the proliferation of erythroid progenitor cells. The progenitor cells synthesized by FVA-stimulation are unable to proceed with differentiation and accumulate in the spleen resulting in splenomegaly in infected mice. Using FVA-inoculated mice as a model, we have investigated the antiretroviral effects of 2',3'-dideoxycytidine (ddC) and recombinant $interferon-{\alpha}-A\;(rIFN-{\alpha}-A)$ on FVA infection. The extent of the infection was determined by measuring the weights of the spleens. Daily intraperitoneal injection of ddC (100 mg/kg body weight), $rIFN-{\alpha}-A$ (10 KU/mose) and the combination of both drugs to FVA inoculated mice for 18 days resulted in suppression of the growth of spleens by 15.1%, 52.7% and 61.6%, respectively. When ddC was dissolved in drinking water (0.1 mg/ml) and administered to a group of FVA inoculated mice ad libitum, and $rIFN-{\alpha}-A$ (10 KU/mouse) was intraperitoneally injected daily to another group of ddC (0.1 mg/ml) drinking mice for 18days, the growth of spleens was suppressed by 38.4% and 83.2%, respectively. These results indicate that administration of ddC via drinking water is more effective in suppressing FVA infection than the daily injection of ddC, and that the combined effects ddC and $rIFN-{\alpha}-A$ are not synergistic but additive. In order to determine whether ddC treatment alters the characteristic of the progenitor cells with respect to $Ca^{++}$ uptake, $Ca^{++}$ uptake in erythroid cells and the effect of cyclohexyladenosine (CHA) on the $Ca^{++}$ uptake were studied. $Ca^{++}$ uptake in the erythroid progenitor cells was about 20-fold greater than in mouse erythrocytes and the inhibition of $Ca^{++}$ uptake by CHA was the greatest in the progenitor cells from FVA infected mice which were treated with ddC. The inhibition was obviated by theophylline. Results of CHA binding studies showed that the erythroid progenitor cells contain both high and low affinity CHA binding sites, whereas mose erythrocytes contain only the low affinity CHA binding sites.

Growth and development of Fibricola seoulensis metacercariae in tadpoles (Fibricola seoulensis 피낭유충의 실험감염 올챙이내 성장 및 발육)

  • In order to observe the growth and development of Fibricola seoulensis metacercariae, the tadpoles of Rana nigromaculata were experimentally infected with the cercariae. The meta cercariae of various developmental stages were recovered from the tadpoles after 2 to 65 days of infection. They were prepared for morphological observation, and were given orally to mice to observe their infectivity. The following results were obtained. 1. All of the tadpoles exposed to the cercariae were observed to harbour the larvae in their abdominal cavity. 2. The young metacercariae of 2 days after infection were $121.1{\mu}m$ long and $63.3{\mu}m$ wide. They grew linearly for the first 14 days to be $262.0{\mu}m$ long and $166.4{\mu}m$ wide. Thereafter, no more growth recognized until 65 days. 3. The larvae of 2 days old were similar with cercarial body and had 2 suckers, a pharynx, 2 ceca and a primordium of germ cells but no tribocytic organ. On the 8th day, they had tribocytic organ, and their morphology resembled that of mature metacercariae. 4. The metacercariae younger than 10 days could not infect the mice. Only the metacercariae older than 14 days had infectivity. The recovery rates increased by the age of metacercariae from 19.0% in 14 days old to 70.0% in 40 days old. Above findings indicate that the tadpole is indispensable for metacercarial development and it needs at least 2 weeks for maturation. The tadpole is a pivotal host in the life cycle of F. seoulensis for connection between the snail and the frog.

Pathological changes of the heart of olive flounder Paralichthys olivaceus in experimental Streptococcus parauberis infection (Streptococcus parauberis 인위 감염에 의한 넙치, Paralichthys olivaceus 심장의 병리학적 변화)

  • We conducted bacteriological and histopathological analysis for olive flounder Paralichthys olivaceus after experimental infection with Streptococcus parauberis (FP2284) isolated from diseased olive flounder under different stress conditions. Experimental challenge was performed in healthy flounder (40.4 g in average body weight) by intraperitoneal (i.p.) injection with $2{\times}10^{8}$ CFU/fish under normal (no stress) or netting (for 2 min, twice/day) stress condition. The cumulative mortalities of no-stress and netting stress group were 70% and 95%, respectively. The most prevalent changes observed in experimentally infected flounder were darkness of skin and inflammation of the heart. Severe pericarditis, myocarditis and fibrosis were observed in the heart of the affected flounder. The results of viable counts showed the number of bacteria of the heart tissue was maintained over the $10^{4}$ CFU $g^{-1}$ heart for 3 weeks after inoculation. Histological lesions of the heart was more extensive and gradual decrease in bacterial numbers of heart tissue was delayed under stress condition.

  • Herlyn-Werner-Wunderlich syndrome(HWWs) is a rare variant of Mullerian ductal anomalies characterized by the presence of a hemivaginal septum, a didelphic uterus, and ipsilateral renal agenesis. It usually presents after menarche with progressive pelvic pain, and palpable mass due to hemihematocolpos. If a cystic mass is detected behind the urinary bladder in children, in association with the absence of a kidney, the diagnosis of uterus didelphys with imperforate vagina and hydrocolpos should be considered. When renal agenesis is found in asymptomatic children, the small size and the tubular shape of the uterus makes it almost impossible to evaluate uterine anomalies, so follow-up should be performed until the end of puberty. Appropriate preoperative diagnosis and treatment will prevent unnecessary procedures and offer relief of symptoms. We report one case of didelphic uterus with blind hemivagina and ipsilateral renal agenesis with biopsy- proven thin glomerular basement membrane disease which is not related to the above syndrome.
