A Study on the Practice Model for Practical Education for Health and Medical Information Management (보건의료정보관리 실습교육을 위한 실습모델 연구)

  • In this study, a practical model for health information management education using the EMR education system at universities for nurturing health care information managers was studied. Currently, there is no practical training course for health care information management in the standards for evaluation and certification of health care information management education introduced to strengthen the job competency of health care information managers. Accordingly, the program was constructed so that the practice program suggested as an educational environment in the Health and Medical Information Management Education Evaluation and Certification Manual can be practiced in the EMR education system. In addition, a practical model that can be performed according to the on-site practice guidelines for health and medical information management for each program was studied. Using the health care information management education EMR system, master data management, patient registration, doctor prescription, medical cost calculation, health insurance claim management, form management, discharge registration, cancer registration, unrecorded management, health care data management, health care statistics, A practice model was studied so that practice on information protection/security management can be performed. It will be possible to play a role as a health care information management expert by raising the quality level of health care information management education through systematic and standardized health care information management practice courses at universities. Accordingly, it is necessary to cultivate health care information management experts who develop and manage medical services based on medical data analysis through practical training of health care information managers.

Factors Associated with the Exercise of Right to Self-determination about non-benefit Medical Services (의료소비자의 비급여 진료에 대한 자기결정권 행사와 관련 요인)

  • Purposes: This study was to investigate intention to exercise the patient's right of self-determination on adopting the non-benefit medical services and was to identify factors associated with intention to self-determined decision. Methodology: A total of 1,000 adult respondents aged 20 to 65 years were recruited using stratified random sampling and surveyed by online. Multivariate logistic regression analysis was performed to identify factors associated with intention to self-determined decision using SAS 9.4(SAS Institute Inc. Cary, NC, USA). Findings: 61.9% of total participants(n=592) had intention to exercise patient's right of self-determination on adopting the non-benefit medical services. Significant differences were observed in the exercise of self-determination in relation to prior explanation and opportunity for self-determination. Practical Implications: This study suggested that explanation duty of provider might influence on increasing intention to exercise the patient's right of self-determination. Considering appropriate use of non-benefit services, it is important to enhance explanation duty of provider.

Relationship between the Level of Local Extinction and Total Medical Service Uses (지역소멸수준과 지역의 총 의료이용 간의 관계)

  • Background: The purpose of this study was to explore the relationship between the local extinction index and total medical service utilization. Methods: A fixed effects model in panel analysis was performed for the 228 administrative districts in Korea. The statistical yearbook on the usage of medical services by region and Korean Statistical Information Service data were used from 2010 to 2019 for analysis. Medical service utilization was represented by the number of visits day, the number of inpatient days, and medical charges. Control variables were selected by using an Anderson model. The local extinction index was calculated using resident registration population data. Results: Descriptive statistics showed that the number of areas at risk of extinction increased from 61 to 95 for the study years. In addition, the number of visits, the number of inpatient days, and medical charges all increased during the study years. After controlling for variables affecting medical service utilization and doing a panel fixed effects model, the result suggested that a one-step increase in the local extinction index was significantly associated with a 12.29% decrease in medical charges of inpatients, a 7.33% decrease in medical charge of outpatient, a 5.21% decrease in the number of inpatient day, and a 5.54% decrease in the number of visits day. Conclusion: This study showed that the higher the region's extinction risks, the higher the region's total medical service utilization. The results of this study suggested that there was a disparity in medical service utilization between areas at risk of extinction and areas not at risk of extinction, so measures should be taken to address this disparity.

The Influence Factors on the Performance of Regional Public Hospitals (지방의료원의 성과에 영향을 미치는 요인)

  • Background: This study is designed to estimate the factors that affect the level of three different performance (publicity, efficiency, profitability) among regional public hospitals. Methods: The units of analysis are the regional 30 hospitals, which have the operating data during 22 years (from 1933 to 2014). The research method is used by fixed panel analysis. The publicity is measured by medicaid outpatient proportion and medicaid inpatient proportion. The efficiency is measured by two types of efficient score by DEA (data envelopment analysis). The profitability is measured by medical income to medical revenue and ROA (return on total asset). Results: At first, the increase of bed gives negative affect to the publicity but give positive effect to the efficiency and profitability. Because it means the increase of the region population, it gives more profitability compare to hospital with small number of beds. The more the operating period is the higher effect to the publicity and efficiency because of it's refutation. The debt ratio gives negative effect to publicity, but positive effect to profitability. It is the normal belief that there is inverse relationship between publicity and profitability. The turnover rate of bed gives the negative affect to the publicity, but positive affect to the efficiency and profitability. That give us the implication that type of the inpatient make different effect the hospital performance. The ratio of labor cost give negative effect to all kind of performance. That means that the higher labor cost don't mean the higher publicity and labor cost control is very important factors to hospital performance. So the region hospital have to focus the labor factors more to make higher performance. Conclusion: As the conclusion, the independent variables give similar effect to the efficiency and the profitability, but give inverse effect to the publicity. That means that if an region hospital want to make the more publicity, it loss the higher efficiency and profitability. Specially publicity is higher negative relation with the profitability.

  • New ways of work-shift and learning programs, which were based on the concept of 'performance improvement through people', have been introduced to healthcare organizations. I analyzed the performance of the changes and the performance differences. Data were collected through interview and survey. I discussed that modification of management practices which were developed in manufacturing organizations is important for successful implementation in healthcare organizations.

  • Demographic variables have a great deal of impact on the utilization of health services. In this paper, the use of segmented polinomials is shown to be superior to the simple use of dummy variables and simple polinomials in explaining differences in health care utilization with respect to sex and age differences.

The Effect of inpatients' Experience on Patients' Satisfaction and Willingness to Recommend Hospital (입원경험이 환자의 만족도와 병원추천의향에 미치는 영향)

  • This study examined to identify the factors influencing on inpatients' satisfaction with hospitalization and willingness to recommend hospital to others. Data from the 2018 National Patient Experience Survey were used for the analysis. Of the 593 patients experiencing inpatient services, multivariate linear regression analysis was conducted to explore the factors such as physician care, nursing care, facility and administrative support, and healthcare system on inpatients' satisfaction after controlling for their socio-demographic and health related factors. The study found that physician care, nursing care, administrative support and healthcare system were related to inpatients' satisfaction and willingness to recommend hospital. To improve inpatients' satisfaction, it is necessary to improve healthcare professionals' overall patient-centerdness attitude, user friendly hospital facilities and administrative support services and efforts to trust and satisfaction on healthcare system from the patients' perspectives.

  • Background & Objectives: The purpose of this study is to present infectious diseases control and crisis response plans for International students by investigating the management of International students and prevention of infectious diseases targeting International students infectious disease control managers at universities in Busan in a situation where the risk of infectious diseases increases due to the spread of COVID-19. Methods: The data were collected from 14 International students infectious disease control managers at universities in Busan. Statistical analysis was done by using SPSS 22.0. Results: There were differences in the importance and performance of COVID-19 infectious disease control and support work by university. Conclusions: It is necessary to integrate management of support and roles of infectious diseases control for foreign students. Furthermore, strengthening the competence of experts and communication tools with international students are needed.

A study on perceptions of university students about the COVID-19 vaccine (코로나19 백신에 대한 대학생의 인식 조사)

  • This study conducted a survey using Google Survey targeting 415 college students over the age of 20 to investigate college students' perceptions of the COVID-19 vaccine. As a result, the average result of 'I think the COVID-19 vaccination is necessary' for herd immunity was 3.90, and 65.8% of the 'necessity of vaccination' recognized the 'necessity of vaccination', but 35.4% negatively evaluated 'the safety of the vaccine'. showed. As for the intention to vaccinate against COVID-19, 34.7% said 'I will vaccinate as soon as the order arrives'. This showed that the current COVID-19 vaccination is not positive. As the reasons for not wanting to be vaccinated, 65.3% answered 'adverse reaction to the vaccine' and 25.8% 'distrust of the vaccine itself'. In addition, they perceived the vaccine supply between developed and underdeveloped countries as unequal, and the average was 3.94, indicating that they were afraid of adverse reactions to the COVID-19 vaccine. Therefore, in order to more effectively acquire information about the COVID-19 vaccine, research, platform development, and education on publicity methods through the media frequently used by college students are required.

Relationship between Local Extinction Index and Medical Service Uses of Chronic Diseases (지역 소멸위험지수와 지역의 만성질환 의료이용의 관계)

  • Background: This study purposed to analyze the relationship between the local extinction index and medical service uses of chronic diseases. The local extinction index is an indicator of the demographic structure and population aging of the region. Methods: The 2014-2018 statistics of National Health Insurance Corporation and Korean Statistical Information Service data were used for the analysis. First, descriptive statistics were used to analyze the general status of research variables. Second, a panel analysis was performed to analyze the relationship between the local extinction index and medical service uses of chronic diseases (hypertension, diabetes mellitus, periodontal disease, arthritis, mental health, epidemic disease, liver disease). Medical service uses were measured by the number of visits/inpatient days and medical charges of seven chronic diseases. Results: Panel analysis results showed that higher local extinction risks (meaning lower local extinction index) had a positive relationship with the number of visits/inpatient days and medical charges of chronic diseases. But the relationships were varied when the seven chronic diseases were analyzed separately. Conclusion: This study showed a significant relationship between the local demographic structure and medical service uses of chronic disease. Analyzing the local demographic structure will be an essential prerequisite step for implementing appropriate regional health care policies.
