The opinions of health-unrelated department university students on medical personnel of dental hygienists (비보건계열 대학생의 치과위생사 의료인화에 대한 견해)

  • 비보건계열 대학생들을 대상으로 치과위생사 의료인화 정책에 대한 찬반의견을 조사하였다. 비보건계열 대학생들은 의료인, 의료기사의 구분에 대한 인식 수준, 치과위생사 업무에 대한 인식 수준이 낮았고, 40%의 응답자가 치과위생사 의료인화 정책에 찬성하였다. 치과위생사를 단순히 의료인으로 편입시키는 정책보다는 치과의료와 관련된 법률을 제정한다거나 치과위생사 직업의 단독법을 제정하는 등 국민구강건강을 향상시키는 데 바람직한 체계가 무엇인지 다양한 시각으로 모색할 필요가 있다.

The Conflict over the Separation of Prescribing and Dispensing Practice (SPDP) in Korea: A Bargaining Perspective (의약분업을 둘러싼 갈등 : 협상론의 관점에서)

  • We report and analyze the Korean physicians' recent general strike over the implementation of the Separation of Prescribing and Dispensing Practice (SPDP) in which more than 18,000 private clinics and 280 hospitals participated. Utilizing game-theoretic models of bargaining we explain why the Korean physicians were so successful in organizing intense collective action against the government and securing very favorable policy outcomes. In particular, we highlight the role of distributional conflict among social actors in shaping the details of institutional reform. The introduction of the SPDP was a necessary first step in the overall reform of health care system in Korea. However, the SPDP was perceived to be a serious threat to the economic viability of their profession by the vast majority of Korean physicians who had long been relied on the profits from selling medicines to compensate for the loss of income due to the low service fee under the previous health care system. The strong political coalition among heterogeneous physicians enabled them to organize an intense form of collective action, the general strike. Thus, physicians were successful not only in dragging the government to a bargaining table, but also winning in the bargaining and securing an outcome vastly favorable to them. On the other hand, the lack of an overall reform plan in the health care policy area, especially the finance of the National Health Insurance and the need for maintaining an image as a successful reform initiator, motivated the government to reach a quick resolution with the striking physicians.

  • 보건교육은 건강사업 전달 체제의 중대한 부분이다. 질병 예방과 건강증진이라는 인간의 기본욕구를 만족시키는 것이 보건교육의 새로운 촛점이 되고 있다. 모든 사람들, 어른, 청소년, 심지어는 어린아이들까지도 자신들의 건강을 관리하는데 있어서 참여자(participant)가 되는 것이 중요하다. 의료인들에게 자신의 건강관리를 맡기고 지시하는 대로 따르는 것이 아니라 능동적인 참여를 하여 건강관리 계획을 같이 세우고 실천해 나가는 것이다.

Extending the Scope of License Restrictions for Medical Personnel and Limiting Fundamental Rights - Focusing on the Revision of the Medical Law - (의료인의 면허제한 범위 확대와 기본권 제한 -의료법 개정안을 중심으로-)

  • Reasons for disqualification to restrict a medical person's license should be considered in functional and moral terms. In this sense, the grounds for disqualification of medical personnel should be expanded to include all crimes that have been declared to be "imprisonment without labor or greater punishment" by a court. Because a sentence of "imprisonment without labor or greater punishment" means that it is highly reprehensible and undermines the trust of the state as well as the trust in medical personnel. Therefore, the expansion of the scope of license restrictions for medical personnel cannot be regarded as a violation of "the Less Restrictive Alternative" rule. However, it is necessary to reflect the specificity of medical services in the license restrictions of medical personnel. This is because not all diseases can be treated with current medical services. In addition, unpredictable situations can occur at any time during medical practice. Consequently, the negligence that occurs during medical practice should be carefully examined from a functional perspective of the medical personnel. And it should be treated differently from ordinary crimes. To this end, an independent license review organization should be established to establish expertise in license management.

A Study on the Legislation of Telemedicine in France (프랑스 원격의료 법제에 관한 고찰)

  • Article 34 of 'Medical Service Act' of Korea provides telemedicine service between medical personnel. Telemedicine between medical personnel and patients, therefor, in principle, is not recongnized. Increasing demand for telemedicine fueled by COVID-19 pandemic and accumulation of telemedicine experience lead a change in stubborn opposition of the medical community, tenuous though it may be. This article focuses on the telemedicine legislation in France, which can be used as a reference for the telemedicine legislation premised on telemedicine between medical personnel and patients. The legislation stipulates the concept, types, and conditions of telemedicine performance through 'Code de la santé publique'. The principle that telemedicine shall be performed alternately with direct medical treatment to a patient and details relating to such telemedicine performance as telemedicine costs, medical fees, and telemedicine equipment support are stipulated through an agreement between the medical community and health insurance organizations. From this point, the implications for our legal system were presented.

The Effects of Supplementary Education Awareness on Interpersonal Communication for Health Care Providers (종합병원 의료인의 교육훈련 인식이 의료인 상호간 커뮤니케이션에 미치는 영향)

  • This study was conducted to identify the effects of interpersonal communication between health care providers after receiving supplementary education. The participants of this study were 433 health care providers who work at 29 general hospitals in Gwangju Metropolitan City and Jeollanamdo Province. Data were collected from June 8 to June 25, 2018 and evaluated by t-tests, dispersion analysis, correlation analysis and stepwise regression. The results were produced by investigating interpersonal communications according to socio-demographic and health-related characteristics including age, education level, bed size of the hospital at which the participant worked, job satisfaction, hospital location, personal health status, experience with health care management and experience with depression. There were significant differences in communication observed according to supplemental education awareness regarding age, bed size of hospital, occupation, wage, type of medical institution of employment, job satisfaction, work location, health status, health care education experience and chronic disease. There were positive correlations between supplemental education awareness in health workers and their interpersonal communication. The factors that had positive effects on interpersonal communication were level of education and health-related education experience, while age, hospital bed size and job dissatisfaction had negative effects. Finally, support environment, learning transfer and results were identified as sub-factors of supplemental education. Based on the results above, it was proposed that educational training to enhance results, provide a supportive environment and foster learning transfer be developed to increase communication between health workers and provide a safe health service for patients.

Related Factors of Musculoskelectal Symptoms in University Hospital Workers (대학병원 보건의료인의 근골격계 증상 관련요인)

  • This study is to identify the related factors of musculoskeletal symptoms in university hospital workers. The subjects were 313 workers who were working at a university hospital. The data was analyzed by descriptive statistics, logistic regretion using the SPSS/PC 21.0 program. The results showed that 201 respondents(64.2%) had musculoskelectal pain. According to a survey of musculoskeletal pain in body parts, the shoulder was the most common region; 37% of the health-workers(116 respondents). Musculoskeletal pain are more common in health-workers under 35 years old or work more than 6 years, or have more physical burden. In conclusion, health-workers need more active health care such as planning a preventive activity and stretching during work in order to prevent the deterioration of musculoskeletal symptoms into diseases.
