Preferences for Care Near the End of Life among Hospital Employees (일 대학병원종사자의 생애말기 치료 선호도)

  • Purpose: The purpose of this study was to investigate end-of-life care preferences of employees working in a university hospital. Methods: Of 650 eligible employees that were approached, 607 employees (386 nurses, 93 physicians, and 128 general staff) completed the Korean version of Preferences for Care Near the End of Life (PCEOL-K). Results: Among 5 dimensions of the PECOL-K, "Pain" was the most preferred care dimension and "Decision making by health care professional" was the least preferred care dimension. The item that received the highest mean score was "I want to let nature guide my dying and I do not want my life to be artificially prolonged in any way", and the lowest item was "I want health care providers to make all decisions about my care". As preferred care near the end of life, nurses gave lower scores to the life sustaining treatment and decision making by health care profession than physicians and general staff. Compared to physicians and nurses, general staff preferred the decision making by health care professional and by family. Conclusion: The results show that adequate pain relief is the most preferred care at the end of life among hospital employees and non-medical personnel preferred decision making by others.

An Analysis about Building-related Health Symptoms of Workers at Dental College Hospitals in Seoul (치과대학병원 종사자의 실내자각증상 관련요인 분석 -서울시내 소재 치과대학병원을 중심으로-)

  • In this study, recognized building-related health symptoms of various workers in dental college hospitals are surveyed and analyzed in order to improve indoor environment at dental college hospitals. We conducted a questionnaire survey for 682 workers in three dental college hospitals located in Seoul from Sep. 20 to Oct. 8, 2004. Most workers at dental hospitals complained about noise, poor ventilation, dusty environment, and air dryness. SBS symptoms score for subjects was 1.43${\pm}$0.74 (often feel) and major symptoms was throat, CNS, eye, and nose symptoms. Recognized Building-related health symptoms are significantly related to tasks, working experience, job-satisfaction, indoor environment recognition index, medical history, used to special material, and number of dental unit chair. Also working places and ventilation are significantly related to the symptom. Based on the results, it can be said that special dental treatment causes undesirable environment such as noise, dust, and bad odor. It is also related to recognized building related health symptoms reported by workers at dental hospitals. So managemental efforts are needed to improve indoor environment which is related by dental treatment.

A pilot study on job satisfaction of korean medical clinic workers (한의원 종사자들의 직무만족도에 관한 예비연구)

  • The purpose of this study is to find out the job satisfaction of the korean medical clinic workers, and understand the factors that affect the satisfaction thereby increasing the efficiency of human resource management and providing the basic data to establish the management strategy. We visited 50 korean medical clinics which were chosen randomly from the korean medical clinics in the city of Changwon, Gyungnam. First, we explained the purpose of the study and survey contents to the surveyees, then distributed the Questionnaire and collected them. The data analysis was done statistically using the SPSS WIN IBM 20.0 program. The results were as follows: Professional career, interpersonal relationship, work autonomy, and overall job satisfaction showed significant differences depending on ages. While satisfaction of interpersonal relationship showed significant difference depending on final education. Depending on marriage status, while married persons showed significantly high satisfaction on professional career, interpersonal relationship, work autonomy, administration, and overall job satisfaction, there is no significant difference on income satisfaction. The number of years worked in the current clinic showed significant difference on administration satisfaction. Also, daytime working hours showed significant difference on administration and overall job satisfaction. The work area showed significant difference on overall job satisfaction. The income level showed significant differences on professional career, administration, and overall job satisfaction. From this study, it was confirmed that demographic characteristics and work environment factors showed significant differences on overall job satisfaction of workers in korean medical clinics. Therefore, in order to facilitate efficient manpower utilization and organization efficiency maximization, and motivation of human resources, a detailed plan should be developed to improve working environment and take demographic characteristics into consideration.

The Effect of Organizational Justice on the Followership and Organizational Citizenship Behavior - the Moderating Effect of Leader-Member Exchange - (의료기관 사무직 종사자의 조직공정성이 Followership과 조직시민행동에 미치는 영향 - LMX 조절효과 -)

  • Purposes: Based on samples of government officials who are working at public health centers, following research examines the Effect of Organizational Justice on the relationships between Followership and Organizational Citizenship Behavior and analyzes the moderating effect of Leader-Member Exchange. Methodology: For the methods, Sphericity verification, Exploratory factor analysis, Correlation analysis, Causal relation, Mediating effect of followership and Moderating Effect of The Leader-Member Exchange have been analyzed with SPSS 22.0 & AMOS 22.0 Ver. Findings: As a result of Correlation analysis, Predictive validity and Construct validity have meaningful effect. Also, In case of Path analysis, distributive justice and interactional justice have meaningful effect but procedural justice has no such effect on Followership. As a result of Mediated effect of followership analysis, distributive and interactional justice have positive impact on (Organizational and Interpersonal). As to the mediation effect of followership analysis, It is shown that distributive and Followership have meaningful effect on In-group, not Out-group. Lastly, It is shown that procedural justice and Followership have no meaningful effect on both In-group and Out-group. Practical Implications: For the implications, distributive justice and interactional justice have positive impact on Followership, while procedural justice has no such impact on Followership. Also, It also possibly has same result to the moderating effect of Leader-Member Exchange. Consequently, It is recognized that a fair process without a fair result cannot be valued. also, We can know that reward for procedural justice is evaluated to kind of negative.

The Influence of Employees' Perception and Performance Expectations on Intention to Use and IT Technology Introduction in the Fourth Industrial Revolution (4차 산업혁명의 IT기술에 대한 의료기관 종사자의 인식과 성과기대가 IT기술도입 및 사용의도에 미치는 영향)

  • Purpose: The purpose of this study is to determine the influence of hospital employees' perceptions and performance expectations on IT technologies introduction and intention to use in the Fourth Industrial Revolution. Methodology: A survey was conducted on 524 employees at 42 hospitals. Pearson's correlation analysis, regression analysis, Sobel test, and structural equation were used for the analysis. Findings: The factors of technical situation (complexity, compatibility, technical readiness, and security concerns), those of environmental situation (competitive pressure and regulatory support), and organizational situation (top management support and costs) have influenced on the recognition of employees in the hospital and the performance expectations. IT technology introduction and use intention have been influenced by technical situation, organizational situation, and environmental situation in order. But security concerns and cost factors have not statistically affected. In addition, due to the perception and performance expectations of hospital employee who have a role in mediating the introduction and use intention of IT technology, the greater the perception and performance expectations of hospital employees, the higher the intention of introduction and usage of IT technology. Practical Implications: Rather than considering the position of external customers and the position of competitive hospitals, more attention is required for the perception and performance expectations of the internal members.

A Study on Dental Hospital Staff's Acceptance toward the Dental Institutions Accreditation System - A Study of Infection Management Part - (치과의료기관 종사자의 치과의료기관 평가제도에 대한 수용에 관한 연구 - 감염관리 부문을 중심으로 -)

  • The Government is going to enforce assessment of dental institutions in addition to assessment of medical institutions so that it can provide good-quality medical service to people having much interest in quality of medical service. But the empirical research on the assessment of dental institutions which is still in a model assessment stage, is insufficient. Accordingly, the present research aims to help dental institutions preparing the main assessment by researching an acceptance level of workers of dental institutions in a process performing assessment of dental institutions, based on the preceding researches that the perceived utility has influence on implementation intention. The present research proved the influence that job relevance, result demonstration, usability and education & training of workers and manager's leadership of an infection management part affects perceived utility and implementation intention. As a research result, all the job relevance, result demonstration, usability have a positive(+) influence on perceived utility and implementation intention, and the influence of job relevance most affects especially. The leadership and education & training have influence on what workers get to have implementation intention after perception of utility of infection management according to the order. According to the above results, it can be understood that medical institutions preparing for assessment of dental institutions take charge of the assessment part having high relevance with work of workers and need to make a manager ordering performance of its assessment be able to foster a leadership for improving effectiveness of assessment performance.

A Comparison of the Perception of and Adherence to the COVID-19 Social Distancing Behavior Guidelines among Health Care Workers, Patients, and General Public (의료종사자, 환자, 일반인의 코로나19 사회적 거리두기 행동 지침에 대한 인식과 수행 비교)

  • Purpose: To identify the differences in perception of and adherence to the COVID-19 social distancing behavior guidelines among health care workers, patients, and the general public and to use them to prevent the spread of COVID-19. Methods: From October 16 2020 to November 30, 2020, a survey was conducted among 85 health care workers, 85 patients, and 82 general public regarding the perception of and adherence to the COVID-19 social distancing behavior guidelines. Results: Patients scored significantly higher than the general public in the adherence to the COVID-19 social distancing behavior guideline, and there was no difference between health care workers and general public, and patients and health care workers. In the multivariate analysis, the factors that influence the adherence of the COVID-19 social distancing behavior guide were found in women and the perception of the COVID-19 social distancing behavior guideline. Conclusion: In order to promote the implementation of the COVID-19 social distancing behavior guideline, it is necessary to increase the perception of the COVID-19 social distancing behavior guideline and provide additional education in men. It is necessary to investigate the reason behind why men have low adherence to the COVID-19 social distancing behavior guideline.

Actual Condition of Quality Control of X-ray Imaging System in Primary Care Institution: focused on Gwangju Metropolitan City (1차 의료기관의 엑스선 발생장치 정도관리에 관한 현황조사:광주광역시 지역을 중심으로)

  • With the expanded use of radiation in modern medical practices, the most important issue in regards to efforts to reduce individual exposure dose is quality assurance. Therefore in order to study the present condition of quality assurance, the Gwangju Metropolitan City area was divided into five districts each containing ten hospitals. Four experiments were conducted: a reproducibility experiment for kVp, mA, and examination time (sec) intensity of illumination; half-value layer (HVL) measurement; and beam perpendicularity test matching experiment. The tube voltage reproducibility experiment for all fifty hospitals resulted in a 95.33% passing rate and mA and examination time both resulted in a 77.0% passing rate. The passing rate for intensity of illumination was 86.0% and 52.0% for HVL, which was the lowest passing rate of all four factors. For the beam perpendicularity test matching experiment, generally the central flux is matched to within $1.5^{\circ}$. Of all fifty hospitals 30.0% were beyond $3^{\circ}$. The results of the survey showed that 58% responded that they knew about quality assurance cycle. All fifty respondents stated that they have not received any training in regards to quality assurance at their current place of employment. Although quality assurance is making relative progress, the most urgent issue is awareness of the importance of quality assurance. Therefore, the implementation of professional training focusing on safety management and accurate quality assurance of radiation will reduce the exposure to radiation for radiologists and patients and higher quality imaging using less dosage will also be possible.

A Study on the Necessity Verification of Convex Probe Disinfection (Convex Probe 소독 필요성 검증에 관한 연구)

  • The study was conducted surveying ultrasound room workers on hospital infection awareness in Daejeon and Choong-chunng region. The contamination of ultrasonic probes used in clinical trials was measured using ATP, and the results were verified after using 70% alcohol sterilization. It was measured on the group's general characteristics and the specific categories such as academic background, job type, having professional certificate and infection education. After the examination, the gel removal and method, disinfection status of the probe and variable correlation analysis were performed to analyze the recognition of the ultrasonic probe disinfection. After examination in ultrasound room, it was found that towels were used the most for cleaning, and the gel container was not replaced for more than three months. After 70% alcohol disinfection, ATP contamination was reduced from $1055.4{\pm}944.2$ to $133.5{\pm}93.2$ and the result was analyzed to be statistically significant.(${\rho}<0.01$) The found bacteria were CNS, Gram positive bacillus, and Micrococcus specs. In order to solve this problem, 70% alcohol sterilization was applied and the bacteria were not detected after the treatment. The research shows that regular training on infection control and efforts to prevent infection are necessary, and that 70% alcohol is effective in disinfect the bacteria. Therefore, the medical institution should provide active hospital infection control education to improve the awareness of hospital infection among workers and contribute to the prevention of patient infection. It is also understood that proper use of the results of this study will help prevent infection by means of ultrasonic probes.

A Performance Evaluation of Diagnostic X-ray Unit Depends on the Hospitals Size (병원 규모별 진단용 X선 발생장치의 성능 평가)

  • The purpose of this study is to measure the tube voltage, the tube current/volume, exposure time and exposure dose of diagnostic X-ray unit in each doctor offices, hospitals and general hospitals for evaluating the performance of such device, to learn the method and technology of its measurement and to suggest its importance. Research subjects were total 30 X-ray units and divided into groups of 10 X-ray units each. The tube voltage, the tube current/volume, exposure time and exposure dose were measured using percentage average error, and then reproducibility of exposure dose was measured through calculating coefficient of variation. The results are like followings; The tube voltage correctness examination showed that incongruent devices among total 30 X-ray units were 5 devices (16.7%). The tube current correctness examination showed that incongruent X-ray units were 3 devices (10.0%). The tube current volume correctness examination showed that incongruent X-ray units were 4 devices (13.3%). Finally, according to exposure time correctness examination, incongruent X-ray units were 5 devices (16.7%) and according to reproducibility examination of exposure dose, incongruent X-ray units were 7 devices (23.3%). Above results showed serious problem in performance management based on management regulation of diagnostic X-ray unit; it means that regular checkout and safety management are required, and as doing so, patients will be able to receive good quality of medical service by the reduction of radiation exposure time, image quality administration, unnecessary retake and etc. Therefore, this study suggests that the performance of diagnostic X-ray units should be checked regularly.
