The Policy Package Related to Essential Medical Service: The Key Is Elaboration and Solidification (필수의료 정책 패키지, 내실화가 관건이다)

  • Currently, the issue of poor accessibility to essential medical services has been brought to light as a social discontent. In order to strengthen the essential medical service system, the government has announced the "the policy package related to essential medical service" as a comprehensive solution and has vowed to invest more than 10 trillion won by 2028. As it contains crucial elements for changing the framework of the healthcare system, I would like to present several points to consider in policy implementation. Given that this package contains important elements for changing the framework of the healthcare system, there are a few issues to consider in policy implementation. First, a mechanism to prevent politicization should be established when designing the physician training system. Second, changing from a hospital centered on residents to one centered on specialists means that the society bears the cost of training residents, while paying a high price for specialist services. The willingness of society to pay for the costs incurred by such a change should be carefully considered, and an appropriate budget must be prepared. Third, as the operation of shared human resources and inter-organizational networking, among other detailed policy measures, are still at a level of conceptual discussion, various issues should be solidly reviewed and considered for in the mid to long term to suit the conditions of the domestic healthcare system.

The Effect of Hospital Internal Marketing on Nurses' Job Satisfaction and Customer Orientation (병원 내부마케팅이 간호사의 직무만족 및 고객지향성에 미치는 영향)

  • In this study, we analyzed the factors influencing internal marketing at hospitals on job satisfaction and customer orientation among nurses in order to provide basic data necessary for management of nursing personnel. From December 26, 2018 to January 4, 2019, the study surveyed 255 nurses at a general hospital in Gyeonggi Province. For data analysis, frequency analysis, factor analysis, correlation, and linear regression analysis were performed using the SPSS 21.0 program. First, the results of this study show that the effect of marketing inside the hospital on job satisfaction was statistically significant. Second, marketing inside hospitals was found to have a statistically significant effect on customer orientation. Thirdly, it was found that the effect of job satisfaction on customer orientation was statistically significant. Fourth, the effect of job satisfaction parameters on the relationship between marketing and customer orientation within the hospital 1st and 2nd stages appeared to have a statistically significant positive (+) impact, and three steps were found to be independent variables. The regression coefficients for organizational vision and future image, organizational attributes and systems, communication, and education/training system by sub-factors of internal marketing at hospitals significantly appeared.

A Correlational Study on ICU Nurses' Job Stress, the Way of Coping, and the Turnover Intention (중환자실 간호사의 직무스트레스와 대응방법, 이직의도에 대한 연구)

  • Purpose: This descriptive survey was aimed to investigate the ICU nurses' job stress, the way of coping, and the turnover intention and to identify the correlation among them. Methods: A 58 items-questionnaire composed of 7 sub-dimensions revised by Park, J. S. (2003) was used to measure the ICU nurses' job stress, and an questionnaire by Han, J. S. and Oh, G. S. (1990) which has 34 items from 6 sub-dimensions were used to measure the ICU nurses' way of coping. To figure out the ICU nurses' turnover intention, a 3 items-questionnaire used. A total of 456 nurses were participated in the study. Results: The participants' job stress was 2.86(4-point scale), and the way of stress coping was 2.27. Job stress had a positive correlation with the way of coping (r=.134, p=.004) and the turnover intention. The 54.1% of nurses addressed that they had some level of turnover intention. The turnover intention had a significant difference according to job stress(t=-2.041, p=.042), the type of hospital (${\chi}^2=8.052$, p=.005) and the total number of hospital beds (${\chi}^2=9.232$, p=.010). Conclusion: The findings of the study illustrated that the ICU nurses had at least moderate-high level of stress. The subjects' job stress showed a positive correlation with the way of coping and the turnover intention. These findings demonstrate necessity to develop an intervention for ICU nurses' stress management.

Relationship Between Job Satisfaction and Turnover Intention Among Nurses in General Hospitals (일부 종합병원 간호사들의 직무만족도와 이직의도와의 관련성)

  • This study was carried out to verify the relationship between the job satisfaction and turnover intention of nurses working in general hospitals The study included 312 nurses working in three general hospitals located in Gyeoungpook area. The data was collected by a survey using self-administered questionnaires structured from February 25 to March 16, 2013. The level of turnover intention was negatively correlated with job satisfaction and professional position, autonomy, administrative capacity, pay, task requirement. In logistic regression analysis, the adjusted odds ratio of the high risk turnover intention was increased significantly in the lower groups of job satisfaction, job autonomy, administrative capacity and pay than in the higher groups of these items. Hierarchical multiple regression analysis showed that the turnover intention correlated significantly with the education level, subjective sleep evaluation, job career, job position, monthly pay, satisfaction with work, fitness of the job, career choice motives, physical burden of work, and job satisfaction. In addition, there was a negative correlation between the job satisfaction and turnover intention in the studied nurses, which showed that the turnover intention increased with decreasing level of job satisfaction. The variations in the job related characteristics and job satisfaction have a significant influence on the turnover intention.

Competencies Necessary to Hospital Staffs and Managers in the Global Age (글로벌 시대에 병원행정 관리자가 갖추어야 할 역량)

  • Main purpose of this study is to provide some basic data necessary to research, education, and policy to cultivate professional talents, by investigating what competencies needed to hospital administrative staffs and managers in performing their jobs in the global age. A survey was conducted through self-administered questionnaires for hospital administrative staffs and managers including first-line hands-on workers who were working in 77 hospitals located in Busan, Keongnam, Ulsan areas in South Korea. Major results are as follows: (a) The analysis for the competencies needed to hospital administrative staffs and managers in order to perform successfully their jobs revealed that interpersonal competency, knowledge of management skills in healthcare field, understanding ability for healthcare, in that order of importance, had significant influence on performing their jobs. (b) Regarding the analysis for the detailed competencies necessary to hospital administrative staffs and managers in order to perform successfully their jobs revealed that problem-solving ability, sense of responsibility, positive attitude, leadership, in that order of importance, had significant influence on performing them. This study suggests that universities opened courses related to health care management should newly open a subject 'problem-solving ability in health care' to link with health care field and their curriculums are necessary to change over to practice-oriented curriculum system.

Factors Affecting Use Satisfaction with Hospital Information System of Hospital Administrator (병원행정직원의 병원정보시스템 이용만족도에 영향을 미치는 요인)

  • This study was designed to identify satisfaction with Hospital Information System(HIS) and the factors affecting them in hospital administrators. The data was collected for 24 days from Mar. 02 to 25, 2011. Among a total of 82 cases of the questionnaires, only 76 cases were used. From the survey, it was made known that there was statistically significant difference in speed, preciseness and stability of environmental factors depending on the type of hospitals while significant difference was found in stability by number of beds. Among the factors of system operation, the administrational method affected speed, preciseness, stability and convenience statistically significantly whereas, however, preciseness, stability and convenience became significantly different depending on the number of staff members. As for the factors that could exercise influence upon satisfaction with administrative service of the HIS, it was revealed that statistically considerable impact could be caused under management of own EDPS expert as well as under comanagement by the medical institution and professional business entity in case of hospital.

The Relationship between Organizational Justice, Organizational Trust, and Organizational Citizenship Behavior of Hospital Office Workers, Focusing on the Moderating Effect of Hospital Scales (병원행정직원의 조직공정성, 조직신뢰와 조직시민행동의 관계 -병원 규모의 조절효과)

  • Objectives : This study examines the voluntary participation of the members of an organization who maintain cordial and friendly relationships with their colleagues, so that the human resources department within a hospital can be used efficiently. Methods : A total of 600 copies of the questionnaire were distributed, and 524 copies were returned (88%). Of these, 518 copies were used to verify statistic results using IBM SPSS and AMOS Ver. 22.0, including factor analysis, path analysis, mediating effects, and moderating effects. Results : The effect of organizational trust on organizational citizenship behavior has shown that OCBI (Interpersonal) is highly affected at large hospitals. However, the effect of organizational trust on OCBO (Organizational) is highly affected at small hospitals. Conclusions : The results of a gap analysis indicated that among the different aspects of organizational justice, distributive justice and interactional justice have a positive effect on organizational trust in small hospitals. The results also revealed that organizational trust and organizational citizenship behavior in small hospitals are more strongly affected than they are in large hospitals. As a result, job efficiency is improved in smaller hospitals.

  • This study is aimed at searching for the ways to effectively cope with the conflicts found among the members of the hospitals by analyzing the conflict control practiced differently depending on the scale of the organization and characters of its members. The result of analysis into the types of conflict control by the members of the hospital comprising a multitude of manpower is presented as follows. 1. The hypothesis 1 which assumes that the sex will determine the type of conflict control is adopted in part at the level of p<0.05. 2. The hypothesis 2 which assumes that the working position will determine the type of conflict control is adopted in part at the level of P<0.05. 3. The hypothesis 3 which assumes that the age will determine the type of conflict control is adopted in part at the level of p<0.05. 4. The hypothesis 4 which assumes that the size of hospitals will determine the type of conflict control is adopted in part at the level of p<0.05. 5. The hypothesis 5 which assumes that the career will determine the type of conflict control is adopted in part at the level of p<0.05. 6. The hypothesis 2 which assumes that the educational background will determine the type of conflict control is adopted in part at the level of p<0.05. This study requires extended follow-up study to cover the cause of conflict and job satisfaction of the medical manpower.

Factors affecting intent to use of Respiratory management mobile application (호흡기질환 관리 모바일 애플리케이션 사용의향에 미치는 요인)

  • Purpose: To analyze the results of an online survey conducted to develop a user-friendly respiratory disease management mobile application. Methodology: The questionnaires were conducted from July 26, 2018 to October 23, 2018 for 90 days. A total of 267 respondents were used for the analysis. Chi-square test, t-test, and multivariate logistic regression were used for statistical analysis. Findings: As a result of the analysis, preference for functions related to medical services was high in all ages and the odds of positive intention to use respiratory disease management application was 4.76 times higher than 40 years old compared with less than 40 years old. The group with more than one effort was found to be significantly higher than those who did not. Practical Implication: It is expected that the functions derived from the main results in this study will be helpful for the effective health management of patients with chronic respiratory disease.

  • The present study examined factors for current conditions of dental office manager in Korean dental clinic. 108 randomly selected members of Korean dental office manager responded to a questionnaire. A survey was conducted for a month from 1, June 2008, and the chi-square test and ANOVA were used in the statistical analysis. The findings of the study were as follows: The number of members depending on the type of occupation dental hospital was the highest number of members, members of the dental hygienist at the dental hospital number significantly higher. The career of dental office manager followed by 1-5 year in dental hospital, 5-10 year in dental network, 1-5 year in dental clinic accounting for 48%, 52.9%, 69.4%. The difficulty during duty performance are high expectation of owner in dental hospital, learning of dental office manager duty in dental clinic. The duty of dental office manager followed by customer service, appointment scheduling, telephone etiquette, treatment plan. Henceforth, it needs to standardize the Korean name of dental office manager and duty. Also, it have to an official certification system.
