Critical Factor on Selection Indoor Air Quality improving alternatives for healthcare projects (병원의 실내 공기 질 솔루션 선택에 영향을 미치는 요인들)

  • Indoor Air Quality is crucial in hospital projects to ensure the health and safety of patients, staff, and visitors. The research methodology comprises an comprehensive literature review, then a comprehensive questionnaire survey conducted among stakeholders involved in Vietnamese hospital projects. 15 variables were identified and categorized into four distinct groups, elucidating their influence on the adoption of advanced IAQ-enhancing technology. This study uses factor analysis, a mean score method and hypothesis test to analyze the factor result from the survey. two-step process, including an in-depth literature review and questionaire survey. The study's findings culminated in the ranking, examination, and categorization of these 15 variables, which were clustered into four essential categories: economic factors, design elements, governance strategies, and technical requirements. Additionally, the research explored viable solutions to improve indoor air quality in Vietnam's unique environmental context, examining the factors that impact the selection of such solutions. The study's outcomes yield practical recommendations for architects, engineers, and hospital administrators in enhancing IAQ within healthcare facilities. Furthermore, it presents a framework attuned to local environmental factors and building materials, contributing significantly to the existing body of knowledge on IAQ within hospital projects, particularly in the Vietnamese context.

Factors Influencing on Selection of Specialty Hospital among Inpatients with Loyalty (전문병원 충성고객의 병원 선택에 영향을 미치는 요인)

  • Purpose : Customers with loyalty are very important to hospitals for sustainable growth in their medical market. Individuals with loyalty are likely to visit same hospital repeatedly when they need medical services. This study was to identify factors associated with selection of specialty hospitals among customers with loyalty. Methods : The subjects of this study were 735 inpatients in 22 specialty hospitals in 6 designated fields(joints, spine, colorectal-anal, obstetrics and gynecology, ophthalmology, otolaryngology). Customer types classified as customers with high loyalty, neutral customers, and customers with low loyalty according to net promoter score(NPS). Factor analysis was conducted to classify 22 hospital selection factors into some similar properties. Logistic regression analysis was conducted to confirm the selection factors related to loyal customers. Findings : Most of specialty hospitals received high NPS of 8 points or higher in all the designated fields. Five factors associated with selection of specialty hospital are (1) hospital facilities and convenience, (2) trust in doctor and hospital, (3) rapidness of treatment, (4) hospital awareness, and (5) accessibility. As a result of logistic regression analysis, selection factors related to loyal customers were 'hospital facilities and convenience', 'trust in doctor and hospital' and 'rapidness of treatment'. Differences in the degree of importance of three selection factors by customer types appeared for each designated field. Practical Implications : This study confirms the high level of patient experience among inpatients of specialty hospitals. Factors associated with selection of hospital among inpatients with loyalty are 'facilities and convenience of hospitals', 'trust of doctor and hospital' and 'rapidness of treatment'. This study will be meaningful as basic data to systematically enhance the roles and functions of the health care system and to provide securing competitiveness according to designated fields in the management aspect of specialty hospitals.

  • In the case of a patient with a small number of unilateral remaining teeth in the mandible, a restoration with conventional removable partial denture cannot provide sufficient retention and resistance. A harmful stress from the poor stability of denture on remaining teeth occurs due to alveolar bone resorption in edentulous area. Although implant fixed prosthesis can provide multiple benefits over conventional removable partial denture, there are limitations come from anatomic structures and costs. In this case, an implant-assisted removable partial denture (IARPD) with a couple of implants placed in strategic positions can be a treatment modality. The objective of this article is to present a case report of two patients showed satisfactory clinical outcomes during three-year follow-up period describing the fabrication and advantages of removable partial denture assisted by teeth and implants (IARPD) for a patient with a small number of unilateral remaining teeth in mandible using Locator as an implant attachment.

Analysis of cognitive factors affecting stroke patient's activity of daily living performance -Using the computerized neurocognitive function test- (뇌졸중 환자의 일상 활동 수행에 영향을 미치는 인지요인 분석 -전산화 신경인지기능검사 중심으로-)

  • This research analyzed the cognitive factors affecting stroke patient's activity of daily living performance and suggests the approaches which can contribute to the effective activity of daily living performance in the rehabilitation treatment of stroke patients. In this study, Seoul Computerized NeuroCognitive Function Test (SCNT) and MBI have been performed for 21 patients under extensive rehabilitation treatments, hospitalized in the rehabilitation clinic of A hospital after being diagnosed as a stroke caused by cerebrovascular disorders. To assess the effectiveness of the cognitive factors which affect the stroke patient's activity of daily living performance, activity of daily living performance values were set as dependent variables and 10 cognitive factors were included in the model to carry out analysis of the multiple regression analysis. The results show that stroke patient's activity of daily living performance have statistically significant correlations with divided attention, motor control and selectivir attention. In addition, cognitive factors explained 69.8% of the stroke patient's activity of daily living performance. Consequently, if divided attention and motor control are considered as a focal point of training in the rehabilitation treatment of stroke patients, we can effectively promote the improvement of the activity of daily living performanceroutine activities.

Hospital Selection Factors and Degree of Satisfaction of Aesthetic Plastic Surgery Patients (미용성형 환자의 병원 선택 요인과 만족도)

  • Recently hospitals are trying to meet requests of patients by reflecting regular survey results of patients into hospital's management in order to increase patients' satisfaction and the rate of return visit. In this paper, degree of patients' satisfaction and the rate of return visit were analyzed using surveys of hospital selection factors and the characteristics of aesthetic plastic surgery. This analysis results can be used as data for setting up efficient management direction of aesthetic plastic surgery hospitals. In the main results, facilities and medical trust factor was the primary cause for aesthetic plastic surgery patients to select hospitals and the results showed that hospitals have to consider carefully not only aesthetic plastic surgery patients but also guardians such as parents or friends because patients are acquiring surgery information from them. The results also showed approachableness to hospitals was not important factor to select a hospital.

  • 건유기에서 비유기로의 이행기는 젖소에서 위험률이 높은 시기이고 이 시기에 가장 큰 문제가 될 수 있는 것이 유방염이다. 환경성 세균들이 분만 전후기에 가장 문제가 될 수 있는 병원체이다. 대장균군에 속하는 세균들은 경산우에서 심각한 감염을 일으킬 수 있고, 처녀우의 경우는 coagulase-음성 포도상구균에 감염될 가능성이 높다. 분만 전후기 중에는 호르몬이나 기타 요인들로 인해 젖소들의 면역상태가 다소 억압된다. 성공적인 유방염 방제 프로그램은 건유우, 분만우, 및 처녀우의 관리에 초점을 맞춰야 한다. 청결하고 안락한 환경, 적절한 사료공급, 비타민과 미량 원소가 함유된 적절한 사료의 보충이 유방 건강을 유지하는데 필수적이다. 건유 시작할 때 유두관의 폐쇄를 강화하고 케라틴 마개가 형성될 때까지 유두 끝을 세균으로부터 보호해 줄 전략들을 사용해서 분만 후의 유방염 감염 위험을 감소시켜야 한다. 건유기 치료는 상당히 성공적으로 이용되어 왔지만 전체적으로 실시하기 보다는 선택적으로 치료하는 접근법이 권장된다. 환경성 유방염에 감염될 위험이 높은 우군에서는 항생제 이외의 접근법이 건유기 중의 신감염 예방을 위한 유용한 도구가 될 수 있다.

Factors influencing the intent to return to practice (work) of inactive RNs (유휴간호사 재취업 의향에 영향을 미치는 요인)

  • The purpose of this study is to examine factors affecting the intent of re-employment of inactive registered nurses. This study presents a secondary analysis of data collected in 'Nurse Turnover On-line Survey' by Korean Nurses Association and Korea Institute for Health and Social Affairs in 2014. The analysis shows that 70.9% of inactive RNs has an intent to return to practice, and most of them preferred 'flexible working options' (47.8%) or 'fixed day shifts' (43.3%) as a work pattern. Main reasons for resigning from their last job have been found to be 'high work intensity' (18.8%) and 'difficulties of night shifts' (16.7%). Inactive married RNs who have working histories in a general hospital or a long-term care hospital or have preferences for traditional shift works showed a stronger intent to return to practice than their reference group. Our study shows that, for inactive RNs to return to practice, it is recommendable to adopt various non-traditional working patterns, to make a staffing distribution considering the labor intensity and to develop education programs designed to increase RNs' professional satisfaction.

A study on healthcare institution selection of healthcare consumers using theory of consumption values : Focusing on relations among clinics or small sized hospitals, general hospitals, and large-sized hospitals (소비가치 이론을 이용한 의료소비자의 의료기관 선택 요인 분석 : 중소병원, 종합병원, 대형종합병원 비교 중심으로)

  • The healthcare environment today is changing rapidly with factors of healthcare consumers in selecting medical institutions also altering at a fast pace under the circumstances. In this study, the theory of consumption values established by Sheth in 1991 is adopted in order to examine particular value affecting consumer selection of healthcare institutions. For the purpose of this study, healthcare consumers were surveyed using questionnaires developed based on the five values of Sheth supplemented by value of effort to acquire hospital information and value in health. Consequently, 24 consumption values affecting selection process were confirmed through discriminant analysis. As a result of regression analysis on factors affecting consumer selection of healthcare institution, effort to acquire hospital information and age among demographic characteristics of respondents are determined important predictors for consumer selection of general hospitals over clinics or small-sized hospitals. Further, service, reputation scale of healthcare institution among functional values and importance of health and effort to acquire hospital information among value in health are identified as significant predictors for consumer selection of large-sized general hospitals over clinic or small-sized hospitals. This study suggests not only vital implications for marketing strategy of healthcare institutions, but also methods to promote positive image for healthcare providers. In addition, this study closely examines the cause of the leaning phenomenon of healthcare comsumers toward large-sized general hospitals.

Improving Hospital Referral System based on Perception of Delivery of Healthcare by Enrollees (국민건강보험 이용자의 의료전달체계 인식에 따른 진료의뢰 개선 방안)

  • This study was carried out to suggest hospital referral system improvements based on health insurance subscriber perceptions of the delivery of healthcare. Health insurance subscriber outpatients (n = 207) referred from a stage 1 medical institution to the S university hospital located in Seoul responded to the study survey. The study's item reliability is reliable as the Cronbach's alpha coefficient was greater than 0.7. This study results showed that 5.9% of patients were referred from a higher stage hospital to a same stage hospital. The main factor attracting patients to S university hospital were physician reputation and confidence. The highest factor ($4.40{\pm}.92$) was xxxx. In addition, survey respondents reported that the concentration of patients in extra-large hospitals in Seoul ($4.24{\pm}.97$) was an important issue, and the issue with the highest priority for improvement ($4.05{\pm}1.02$). A positive correlation was detected between the recognition and improvement of delivery of healthcare (p < 0.01). Based on the results, we suggest that improvements in the delivery of healthcare should focus on patients rather than suppliers of national health insurance or other insurers. Hospital attributes considered by patients with spinal diseases in choosing speciality or general hospitals (척추질환환자의 전문병원과 일반병원의 병원선택요인)

  • The present study was conducted to assess relative importance of hospital attributes considered by patients with spinal diseases in choosing specialty or general hospitals. A total of 230 patients hospitalized with spinal diseases in sampled study sites, including 2 specialty and 2 general hospitals, participated in a self-administered questionnaire survey from April 26 through May 8 2007. Patients were asked to rate the degree of agreement on each of the 15 attributes on a 5-point Likert-type scale (1 : strongly disagree, 5 : strongly agree) for which they chose the hospital because of that specific attribute. Based on a Factor analysis, the attributes were grouped into 4 : facility and environment, accessibility, interpersonal factor, and credibility. Both patient groups from specialty (mean scale score ; 3.75) and general hospitals (3.62) commonly considered 'credibility' to be the most important attribute, followed by 'facility and environment (3.05 and 3.21).' Logistic regression analysis showed that men(Odds ratio(OR)=0.333) and those with monthly income of $\geq$ 4 million won (OR=0.298) were less likely to choose specialty hospitals. Age groups of 30 to 39 years old (OR=5.140) and $\geq$ 60 years old(OR=4.761), and professionals (OR=5.207) tended to choose specialty hospitals. Patients expressing more importance on 'facility and environment' attribute were less likely to use specialty hospitals (OR=0.571), whereas those emphasizing 'accessibility' were more likely to use specialty hospitals(OR=1.487). The findings of significant difference in patient's demographic characteristics and consideration in hospital attributes would contribute to have a better understanding on patient's choice behaviour and to develop strategy to improve patient's satisfaction.
