A Utilization Strategy of Nursing Staff by Types of Medical Institutions - nurse staffing level of medium and small-sized hospitals (의료기관별 간호인력 활용방안-중소병원 간호사 확보를 중심으로)

  • This study analyzed the current situation of medium and small-sized hospital nursing staff and related policies, and identified the factors that affect staffing level to provide evidence for planning and adopting policy. By analyzing the statistical data published by public institutions such as the Ministry of Health and Welfare and Health Insurance Review and Assessment Service, the result was viewed based on the understanding of various internal and external health care environments. The number of active nurses was less than 50% of the number of licensed nurses and has decreased every year. This means that the cost-effectiveness of increases in nursing college enrollment should be reconsidered. Inpatient nursing fees by staffing grades has caused nurses to move from medium and small-sized hospitals, where there is a severe lack of staff, to more advanced general hospitals. As a result, the lack of nursing staff in medium and small-sized hospitals has worsened. In conclusion, reexamination is needed to improve effectiveness of inpatient nursing fees by staffing grades as a policy to secure the workforce of medium and small-sized hospitals. Furthermore, the tracking management system of licensed nurses must be able to solve the imbalance between demand and supply of nursing staff.

Effect of Nurse Practice Environment and Organizational Justice on Job Embeddedness in the Small and Medium Sized Hospital Nurses (지방 중소병원 간호사의 간호업무환경과 조직공정성이 직무배태성에 미치는 영향)

  • Purpose: This study was conducted to examine how nurse practice environment and organizational justice affected job embeddedness in the Small and Medium Sized Hospital Nurses. Methods: The data were collected from 233 nurses by means of self-reported questionnaires on August 23th to September 8th, 2017. Results: The model explained 49.7% of the total variance in job embeddedness scores. Hierarchical regression analysis showed that hospital nurses that had higher perceived nurse practice environment, higher organizational justice, and were older were more likely to have higher job embeddedness scores. Conclusion: To enhance nurses' job embeddedness, hospital and nurse leaders should improve nurse practice environment, particularly in relation to staffing and resource adequacy and collegial nurse-physician relations. Additionally, the nurse leaders can facilitate nurses' job embeddedness by creating and maintaining an organizational culture of fairness and justice.

  • This study aims to identify the nature and content of the desired nursing personality of clinical nurses and provide basic data for securing nursing competencies and providing quality nursing as clinical nurses. This is a qualitative study with a focused group analysis to comprehensively explore the desirable nurse personality expected in clinical nursing settings. From November 30, 2019 to Jan 20, 2020, 6 nurses with more than 5 years of clinical experiences and working in a regional university hospital participated in the four focus group interviews. The collected qualitative data was analyzed by the content analysis method. The personality required of clinical nurses is 'process of self-regulation and effort for patient care', 'components for human-centered nursing', 'influence to create healthy interpersonal relationships', and 'nursing'. Configure and operate the organization. It was analyzed into a total of five main themes and 17 sub-themes: "Influence to make changes" and "Influence to bring about organizational change and results." This study confirms the desirable elements of personality in nursing practice, and proposes the application and effect verification research of clinical nursing education programs for nurses in order to secure the nursing competence of professionals and provide high-quality nursing care in the future.

Nurse Staffing Level Relating Factors of the General Nursing Units, ICU, ER and OR in Acute General Hospitals (종합병원의 일반병동, 중환자실, 응급실, 수술장간호사 확보수준 관련 요인)

  • Purpose: To analyse hospital nurse staffing level of the general nursing unit, ICU, ER and OR in general hospitals. Method: The study sample was 105 acute general hospitals which had reported the bed size and number of nurses by the nursing units. Number of bed per nurse was analysed by the hospital characteristics and the staffing levels of the doctors and the nursing assistant personnels using t-test or ANOVA and Pearson's correlation. Results: Number of bed per nurse was 3.86 in general nursing units and 0.95 in ICU. Tertiary hospitals employed more nurses in general nursing units and ICU than general hospitals. Hospitals located in Seoul and public hospitals employed more ICU nurses. OR nurse staffing level was higher in academic hospitals. Hospital size was positively correlated with nurse staffing level of the general nursing unit, ICU, ER and OR respectively. Total nurse staffing level of the hospital was positively correlated with doctor and nursing assistant personnels staffing levels. Conclusion : Differentiated nursing fee schedule was needed to implement in ER or OR. Regulation policy should be needed for the hospitals which violated hospital nurse staffing level of the law.

Analysis of the factors that influence on the RN staffing level in the Specified general Hospitals (종합전문병원 간호사 확보수준에 영향을 미치는 요인)

  • This paper studied the factors that influence on RN staffing level in the 43 Specialized general hospitals. Market structure was analysed using the monopsony model. The degree of competition was estimated by Herfindal Index and market was defined as Great medical zone. As the result of the estimation, in the more competitive hospital market hospitals employed the higher level of RN staffing, so monopsony model was supported. Hospitals with above 1001bed employed more RN than hospitals with below 700bed did. Hospital type, hospital union and the number of medical doctor did not affect the level of RN staffing. There was positive correlation between the level of RN staffing and the number of nurse's aid. The structure of RN market in Specified general hospitals was proved monopsony market, it seems that government regulation will be needed to improve social efficiency and equity.

  • 본 첨단기기의 발전에 따라 의료분야에서도 사람들이 인지하고 있는 것보다 많은 위험요인이 존재한다고 알려지고 있다. 미국의 경우 병원환경의 안전성을 확보하기 위하여 정부와 의료계 모두 부단한 노력을 하고 있는 실정이다. 이에 본 연구는 4개 병원 350명의 의료인을 대상으로 환자안전에 대한 태도와 인식을 구조화된 설문을 통하여 진행하였다. 직종은 의사, 간호사, 약사, 의료기사로 구분하였다. 환자안전 개선을위해 적극적으로 노력하는 여부에 대하여 의사는 보통 또는 그렇지 않다고 39.5%가 응답한 반면, 보통이라고 간호사는 58.6%, 약사는 53.3%, 의료기사는 78.3%로 응답하였다(p=0.029). 환자안전 실수가 긍정적인 변화를 초래한다는 응답에 의사는 '매우 그렇지 않다'고 40.5%, 간호사는 '그렇지 않다'고 45.4%, 약사는 '보통'고 53.8%, 의료기사는 '그렇지 않다'고 54.5%가 응답해서 전반적으로 그렇지 않다고 생각하고 있는 것으로 나타났다(p=0.006). 병원내 환자안전에 대한 병원내 조직 문화를 살펴보고, 의료과오에 대한 논의와 의사소통의 현황을 파악하고, 의료인간의 환자안전에 대한 인식을 고취하여 보다 나은 환자안전 문화를 만들어야 할 것이라 생각된다.

The Relationship between COVID-19 related Post-traumatic Stress, Nursing Work Environments, and the Resilience of nurses in Tertiary General Hospitals (상급종합병원 간호사의 COVID-19 관련 외상 후 스트레스, 간호근무환경과 회복탄력성의 관계)

  • The purpose of this study was to determine the correlation between the COVID-19 related post-traumatic stress, nursing work environments, and the resilience among nurses in Tertiary General Hospitals. Data were collected from 155 nurses who directly cared for COVID-19 patients and were working at the K University D Hospital in D Metropolitan City using an online survey. The SPSS 28.0 program was used for data analysis. Based on the results of the survey, post-traumatic stress showed a significant negative correlation with the nursing work environments(r=-.17, p<.001), and the nursing work environments showed a significant positive correlation with the resilience of nurses(r=.20, p<.001). Through the results of this study, it is necessary to improve the nursing work environments for managing post-traumatic stress, to build an active support system by securing manpower from the government, and to develop and apply intervention programs for improving the resilience of nurses in the COVID-19 situation.

Nursing outcomes of inpatient on level of nursing staffing in long term care hospitals (요양병원 간호인력 확보수준에 따른 입원환자의 간호결과)

  • This study was conducted to explore the impact of nursing staffing on inpatient nursing outcomes in long term care hospitals. A secondary analysis was done of national data from the Health Insurance Review and Assessment Services including evaluation of long term care hospitals. Patients per RN was a significant indicator of foley catheter ratio in high risk group and low risk group. Patients per RN&NA was a significant indicator of decline in ADL for patients with dementia, non dementia, urinary incontinence and new pressure ulcer development in the high risk group. The average nursing outcome of inpatient in high grade was higher than that low grade in long care hospital. This higher level of nursing staffing and the higher the grade shown a positive effect on the nursing outcomes of the inpatient. We therefore recommend modifying the above nurse staffing policy so as to make it more effective in improving nursing outcomes.

The Impact of Nurse Staffing Level on In-hospital Death and Infection in Cancer Patients Who Received Surgery (간호사 확보수준이 수술한 암환자의 원내 사망 및 감염에 미치는 영향)

  • This study was conducted to examine the influence of the nurse staffing level on the level of in-hospital death and infection in cancer patients who received surgery. Secondary data were used and the subjects of this study were 24,510 patients who received surgery for six types of cancer with a high postoperative mortality rate in the first half of 2012 at 260 hospitals. Simple logistic and GEE multiple logistic regression analyses were used. After adjusting for the patient and hospital characteristics, a greater likelihood of dying was found in the nurse staffing level 2-3 group (odds ratio [OR], 1.46; 95% confidence interval [CI], 1.00-2.11) and in the level 6-7 group (OR, 3.28; 95% CI, 1.87-5.74) compared to the level 0-1 group. The likelihood of in-hospital infection increased with each additional bed per nurse, being 6.63 times higher (95% CI, 3.00-14.62) in the level 2-3 group, 5.79 times higher (95% CI, 1.88-17.78) in the level 4-5 group, and 8.4 times higher (95% CI, 1.82-38.84) in the level 6-7 group, as compared to the level 0-1 group. A lower nurse staffing level was associated with higher in-hospital death and infection levels. This shows that an appropriate nurse staffing level is associated with superior postoperative cancer patient outcomes. Policies for providing adequate nurse staffing should be maintained for the sake of ensuring improved care quality and patient safety.

Relationship between Verbal Abuse Experience, Emotional Labor and Health-Promoting Behavior of Nurses in Small and Medium-Sized Hospitals (중소병원 간호사의 언어폭력 경험, 감정노동 및 건강증진행위와의 관계)

  • The purpose of this study is to investigate the level of verbal abuse experiences, emotional labor, and health-promoting behaviors of nurses in small and medium hospitals and to identify the relationship between them. Participants were 138 nurses from 2 small-medium sized hospitals. Collected data were analyzed using t-test, ANOVA, and Pearson's correlation coefficients with the SPSS 20.0 for Windows Program. As a result of the study, 82.7% of them experienced verbal abuse, and there were differences in the degree of health- promoting behavior according to gender (p=.032) and department (p=.009). Besides, it was found that emotional labor was related to health-promoting behavior (p=.039), and spiritual growth (p=<.01), and stress management (p=.028) as sub-factors. Accordingly, this study will be used as basic data for developing strategies and customized programs to reduce emotional labor, increase health-promoting activities, and contribute to the stable securing of nursing personnel.
