Reliability and Concurrent Validity of the Balance Evaluation using Space Balance 3D and Tinetti Mobility Test in Subacute Stroke Patients (아급성 뇌졸중 환자에서 Space Balance 3D와 Tinetti Mobility Test를 이용한 균형 능력 평가의 신뢰도 및 동시타당도 연구)

  • The purpose of this study was to determine the test-retest reliability and the concurrent validity between tinetti mobility test (TMT), berg balance scale (BBS) and space balance 3D which is one of the computerized measurement and visual feedback balance assessment system in subacute stroke patients. Twenty three ambulatory acute stroke subjects were measured the TMT, BBS and space balance 3D. The test-retest reliability(intra-class correlation coefficient: ICC) indicated that the static and dynamic balance in space balance 3D considered moderate reliability and TMT, BBS were good reliability. In case of concurrent validity, there were moderate validity (p<.01) between static balance test with space balance 3D and each TMT, BBS. But there were only poor validity (p<.05) between center to forward-left, center to backward-left phase in dynamic balance test with space balance 3D and each TMT, BBS. These findings suggest that in subacute stroke patients the test-retest reliability and concurrent validity using the space balance 3D and TMT were valuable in balance test but there was limitation to evaluate dynamic balance test.

Correlation of the Berg Balance Scale and Smart Balance Master System for Chronic Hemiparetic Stroke (만성 편마비 뇌졸중 환자의 버그균형척도(Berg Balance Scale)와 Smart Balance Master System의 상관성)

  • The purpose of this study was to find correlation between the Berg Balance Scale which is most commonly used and the equilibrium scores from Smart Balance Master System which is used for the test of the balance ability for the patients with chronic hemiparetic stroke to predict their falling risk though examining the postural control. Twenty-two subjects who had first stroke participated in this study. Participants were measured the Berg Balance Scale and the Smart Balance Master System. The study found that there was a significant correlation between the Berg Balance Scale and the equilibrium scores from Smart Balance Master System. And there was no significant correlation at all "eye-closed and sway of surfaces" in the 6 conditions of Smart Balance Master System and there was a significant correlation with over median in the other 5 conditions. The study was uncovered that there was no meaningful correlation between the Berg Balance Scale and "eye-closed and sway of surfaces" because chronic hemiparetic stroke patients heavily relied on their vision to maintain their balance ability and the Berg Balance Scale did not properly exam it. Thus to predict their falling risk in clinical practice for the patients with chronic hemiparetic stroke, it would be more efficient ways for us to consider all the effects of sensory organs with more than one clinical evaluation method.

Relationship between Plantar Tactile Sensory Thresholds and Balance for Life-Care Increase in Patients with Stroke (뇌졸중 환자의 라이프케어 증진을 위한 발바닥 감각역치와 균형의 상관관계)

  • Sensory impairment is common following a stroke. Tactile afferent inputs from the plantar foot provide important information to the central nervous system to generate balance. The present study, which recruited 50 stroke patients, aims to determine the relationship between plantar tactile sensory threshold (PTST) and balance in patients with stroke. The PTST was evaluated at two sites (hallux and heel) using Semmes-Weinstein monofilaments. The balance abilities were measured by the degree of weight distribution in quiet standing (QS) and sitting-to-standing position (STS) using a MTD system and Berg balance scale (BBS). The hallux and heel sensory thresholds (ST) in the affected side were higher than the non-affected side (P < 0.05). The degree of weight distribution in the affected side was lower than the non-affected side. QS and STS showed P < 0.05, with BBS score of 36.00 ± 1.53. The hallux ST in the affected side correlated with BBS (r = -0.444, P < 0.05), QS (r = -0.332, P < 0.05), and STS (r = -0.390, P < 0.05), whereas the heel ST in the affected side correlated with BBS (r = -0.467, P < 0.05), QS (r = -0.532, P < 0.05), and STS (r = - 0.516, P < 0.05), suggesting that higher detected PTST might have an influence on the decreasing balance abilities. The study of sensory threshold in patients with stroke will help their rehabilitation and sensory evaluation.

Effect of Balance Ability and Walking in the Elderly by Taekwon-do Program (태권도 프로그램이 노인의 체중심 동요와 보행에 미치는 영향)

  • 연구목적 : 본 연구의 목적은 태권도 프로그램이 노인의 균형능력에 어떠한 영향을 미치는지에 대해 알아보는 것이다. 연구방법 : 대상자는 본 연구에 대해 설명을 들은 후 자원한자로서 65세 이상의 노인이었다. 실험군은 태권도 프로그램 그룹으로서 17명이었다. 태권도 프로그램을 주3회씩 6주간 시행하였고 모든 대상자의 균형능력을 평가하기 위하여 BPM, GAITrite, Berg 균형척도를 실험전후로 평가하여 자료를 수집하였다. 수집된 자료는 SPSS 14.0으로 대응표본 t-test를 사용하여 유의수준 ${\alpha}$=.05에서 통계 처리 하였다. 연구결과 : 태권도 프로그램을 시행한 결과 프로그램 실행 전에 비해 오른쪽 활보폭, 왼쪽 보폭, 왼쪽 활보폭, 버그 균형척도에서 통계적으로 유의한 증가를 나타내었다. 그리고 최대 동요속도, 동요거리, 오른쪽 기저 지지면, 오른쪽 양하지 지지기, 왼쪽 기저지지면, 왼쪽 양하지 지지기에서 통계적으로 유의한 감소가 있었다. 결론 : 태권도 프로그램을 시행한 결과 오른쪽 활보폭, 왼쪽 보폭, 왼쪽 활보폭, 버그 균형척도에서 유의한 증가와 최대 동요속도, 동요거리, 오른쪽 기저 지지면, 오른쪽 양하지 지지기, 왼쪽 기저지지면, 왼쪽 양하지 지지기에서 유의한 감소는 균형 제어에 긍정적인 영향을 미칠 것으로 판단된다. 따라서 이러한 균형 제어 능력의 향상은 노인의 보행에 있어서의 낙상 예방에 효과적일 것으로 사료된다.

The Effect of Visual Feedback Training on Balance and ADL in Cerebellar Ataxia : Case Report (시각되먹임 훈련이 소뇌 실조증 환자의 균형과 일상생활 수행에 미치는 영향 : 사례보고)

  • Objective : The aim of this study was to identify the effect of visual feedback on balance and ADL in patient with cerebellar ataxia. Method : Between May of 2013, visual feedback applied to cerebellar ataxia patient. The visual feedback applied five times a week for two weeks to patient who are inpatients of Dae-jeon. To assess changes in balance, we performed the LOS, Romberg, BBS, We also assessed ADL using Canadian occupational performance measure(COPM). Result : For the patient of cerebellum ataxia, we can confirm the limitation of stability, and after the intervention, we can check out the changes which are maintained at Romberg test and Berg Balance Scale. In terms of daily activities, such as taking bath, moving to somewhere, using transportation, doing the laundry, and meeting activity, the level of performance and satisfaction has increased in all five fields. Conclusion : After the visual biofeedback training, patients with cerebellar Ataxia showed more increase in balance and ADL.

  • The purpose of this study was to evaluate the effect of lumbar mobilization combined with dual task training on balance ability in patients with chronic stroke. In this study, 30 adults who were diagnosed with a stroke were divided into 15 groups, each with a dual task training group, a dual task training group applying joint mobilization to the lumbar from July 1, 2019 to August 31, 2019, it was held for 30 minutes a day for two months, three times a week for a total of eight weeks. The clinical balance evaluation index was measured by K-BBS, and the surface area, step length and average speed were measured using Bioresque. In K-BBS, scores were increased in all groups, and the balance ability was increased in the surface area, step length and average speed using Bioresque. Therefore, the combination of dual task training for the lumbar was thought to have an effect on increasing the balance ability of chronic stroke patients.

  • This study to investigate the effects of proprioceptive motor program on proprioceptive and balance ability for patients with chronic stroke. Twenty nine subjects were recruited by means of a convenience sampling from Gyeonggi-do B hospital. The subjects were divided into two group: a proprioceptive motor program and general physical therapy(n=15), general physical therapy(n=14). The stimulation and exercises were conducted for 30 min per day, three days, per week for six weeks. All participants were assessed before and after taping training using proprioceptive, berg balance scale(BBS), timed up and go test(TUG), and balance ability was measured using a BioRescue. After training, the change values of the proprioception and balance ability in experimental group were significantly greater than control group(p<0.05). This findings show that progressive and proprioceptive motor program training convergence can be effective for stroke patients proprioception and balance ability. Continued development of convergence interventions for chronic stroke patients with proprioception and balance ability in the practice are suggested.

Effects of Mechanical Horseback Riding Training on Trunk Control and Balance function in Stroke patients (뇌졸중 환자의 체간조절, 균형능력에 기계적 승마치료가 미치는 영향)

  • The purpose of this study was to determine the effect of mechanical horseback riding training on trunk control, balance function in stroke patients. 15 stroke patients were recruited for this study. The subjects were randomly assigned to two groups: experimental(n=7) and control(n=8). All subjects received a routine physical therapy. The mechanical horseback riding training was for 30 min in the case of the EG subjects. This study was carried out approximately 8 weeks from March 4 to May 3. The assessment tools of this study involved the trunk control test, postural assessment scale for stroke, Berg balance scale and timed up and go test. The measurements were recorded before the intervention and after the intervention. In the cases of the EG subjects and CG subjects, the variables measured after the intervention significantly differed from trunk control test, postural assessment scale for stroke, Berg balance scale and timed up and go test before the intervention (p<.05). Also, there were significant differences in trunk control test, postural assessment scale for stroke, Berg balance scale and timed up and go test at post test between the 2 groups (p<.05). The findings indicate that mechanical horseback riding training exerts a positive effect on trunk control, balance function in stroke patients. Further studies are required to generalize the result of this study.

The Effect of Trunk Control Training Using Virtual Reality Game-based Training Program on Balance and Upper Extremity Function of Subacute Stroke Patients (가상현실 게임기반 훈련프로그램을 이용한 체간조절훈련이 아급성기 뇌졸중 환자의 균형 및 상지기능에 미치는 영향)

  • The purpose of this study was to investigate the effects of virtual reality game based training on balance and upper limb function in subacute stroke patients. Thirty patients with subacute stroke were randomly assigned to experimental groups(n=15) and control groups(n=15) applying virtual reality game-based training programs. Intervention is applied three times a week for 6 weeks, 30 minutes for 1 time. In the study group, there was a significant improvement in balance ability, upper extremity function, and trunk impairment scale(p<.05), and the difference between the two groups was significant in the BBS, TUG, TIS(p<.05). Based on these results, the virtual reality game based training program is clinically useful exercise program for subacute stroke patients.

  • The purpose of this study was to compare of the fall risk factors for elderly in the city. 62 people aged 65 years or older were classified as fallers and nonfallers based on experience of their falls in the previous year. By comparing the difference between the groups via evaluations of general characteristics, health related behavior and chronic disease, balance-related psychological (K-ABC) and physical measurement (BBS), depression (SGDS), and the correlations between the significant differences in variables were identified. According to the results, K-ABC, BBS, and SGDS are statistically significant differences between fallers and nonfallers (P<0.05). Also it has positive correlations between BBS and K-ABC (r=0.499) whereas negative correlation between K-ABC and SGDS(r=-0.472).
