Estimated Exposure Dose and Usage of Radiological Examination of the National Health Screening (국가건강검진의 방사선검사 이용량 및 피폭선량 추정)

  • Korea conducts a national health screening program to improve and check-up on public health and in recent years, the screening usage has been increased. Given the increased screening usage for radiographic exams, this study predicts the frequency of using radiographic exams and the exposure dose. This study estimates the usage of radiographic exams by isolating radiographic exams from the 2011 analysis of the national health insurance corporation, and estimates the public exposure dose by applying each procedure's dose table from UNSCEAR 2008. As a result of the analysis, in the 2011 National Health Screening, the average exposure dose per person is assumed to be 0.57 mSv, and depending on the type of screening program from the radiographic exam, an examinee could be exposed to between 0.2 mSv and 11.081 mSv. The frequency of using radiographic exposure was found to be 16,005,914 and the exposure dose was 6,311.76 person-Sv. The most frequent exam is the Chest X-ray, which was performed 1,070,567 (69.17%), and the UGI has the highest exposure dose at 5,217.94 person-Sv (82.67%). The outcome is categorized based on gender and age, excluding those under 39 years old. In all age groups, the screening usage and exposure dose are higher in females than in males. In particular, females between 50 and 54 years old have the highest screening usage (1,674,787, 10.5%) and exposure dose (701.59 person-Sv, 11.1%). As UGI accounts for 82.76% of procedures, except when done for medical purposes, if the government supports a voluntary UGI exam (which includes the UGI exam in the National Screening Program) or abolishes it completely, as seen overseas, the cost-effectiveness and validity of the UGI exam, as well as the exposure dose from the National Screening Program will all decrease significantly.

Quantification of Experimentally Induced-Pleural Effusion in Beagle Dogs: Radiography versus CT and Ultrasonography (비글견에서 실험적으로 유발한 흉수의 정량 평가: 방사선, 컴퓨터단층촬영 및 초음파 검사 비교)

  • This study was performed to quantify the pleural effusion in radiography, ultrasonography and computed tomography(CT) and to evaluate and compare the usefulness of these methods. Normal saline of 10 ml/kg was infused into the pleural space until a final loading volume of 60 ml/kg body weight was reached in six Beagle dogs. The radiographic examination was performed for the detection and quantification of pleural effusion. On the ultrasonographic study, the maximum perpendicular distance was measured between the surface of the lung and the thoracic wall to evaluate pleural effusion. On the CT image, pleural effusion was evaluated as the perpendicular distance to the thoracic surface in the maximum pleural effusion volume on any transverse images with soft tissue window. Statistical analysis was performed using linear regression test. The volume of pleural effusion and measurements of radiography and ultrasonography had no statistical relationship. However, a significant correlation was identified between the volume of pleural effusion and the depth at right ($r^2=0.715$), left ($r^2=0.745$), and mean right and left depth ($r^2=0.844$) on the CT images. All of the thoracic radiographs, ultrasonography, and CT are useful in recognition of pleural effusion. In quantification of pleural effusion, the CT measurement method is superior to radiographic and ultrasonographic measurements.

A Study on the Application Ratio of ASIR in Abdominal CT (복부 CT 검사에서 ASIR의 적용율에 대한 연구)

  • The purpose of this study is to reduce the dose to the patient by different application of ASIR according to the Body Mass Index (BMI). In particular, we wanted to find out the application values of ASIR that are most appropriate for the standard weight. Studies have shown that increasing the application of ASIR to 50% in patients with a body mass index of 25 or less reduces CTDIvol 58.17% and DLP 60.49% compared to using only FBP. A comparative analysis of the noise by the BMI and the SNR values found that the noise increased as the BMI increased, but the application of ASIR resulted in less noise than the FBP alone. In addition, it was found that the more ASIR is applied, the more SNR is increased.

Analysis of changes National Health Insurance Policy and Claim Data of PET (양전자단층촬영 건강보험 적용 정책 및 이용량 변화에 관한 연구)

  • In this study, the health insurance application of PET and the change in the pay standard were examined, and the amount of health insurance use over the past 10 years was analyzed. Positron tomography was applied as health insurance in 2006, and after 18F-FDG was first applied as health insurance, positron tomography tests using various radioactive isotopes have been applied as health insurance. As of 2019, the number of positron emission tomography tests was 198,651 cases, and the treatment amount was about 88.3 billion won, and the number of tests according to general characteristics was higher in men than in women, and by age, the number of tests was the highest in 60s. The number of outpatient examinations was higher than that of inpatient examinations, and the number of examinations in tertiary hospitals(68.2%) was significantly higher than that of general hospitals and hospitals. As for the test site, torso test was the most common at 86.6%, and radioisotope was the most at 93.6% using 18F-FDG. The change in the use of PET for 10 years increased steadily from 2010 to 2014, but the amount of use decreased sharply afterwards as the recognition of asymptomatic long-term follow-up tests was deleted due to the government's change in health insurance application standards in 2014. As changes in health insurance standards have a great influence on changes in health insurance usage, continuous monitoring will be required in the future.

A Study on the Distribution of Scatter Ray in Chest Radiography of a Health Examination Bus (건강검진 차량 내 흉부 방사선검사 시 공간산란선 분포 연구)

  • The purpose of this study was to evaluate the distribution of spatial scatter ray on the chest radiographs of patients on health examination bus. In this paper, we propose a method for minimize unnecessary exposure by measuring the scattered dose after exposure the actual subject and comparing the body mass index (BMI) with the tube current amount mAs. The results of this study showed that the mean BMI of the subjects was $23.31{\pm}3.12$. The mean mAs value was $2.92{\pm}1.19$, which males was higher than females. The mean value of the scatter ray at position 1 in the radiography room was $771.81{\pm}151.15{\mu}Sv/hr$. The mean value of the scatter rays at the position 2 outside the entrance of the radiography room was measured as $53.86{\pm}25.66{\mu}Sv/hr$. As the BMI and mAs was increase the spatial scatter dose was increased at position 1 and position 2 in the photographing room. In order to minimize the exposure dose of scatter ray, radiation workers should shoot the radiation as low as possible within the range that does not impair the quality of the image. It will be necessary to make efforts to not wait for a waiting person near the entrance door of the photographing room.

Radiological Perspectives for Diagnosis of Vasospastic Angina with Coronary Angiography (이형성 협심증 진단 조영 검사의 방사선학적 관점)

  • If complete coronary artery occlusion occurs due to severer coronary spasm, malignant arrhythmias can lead to death. Therefore, early screening for coronary artery spasm angina is essential. Among the test methods, the drug injection test through coronary angiography is generally performed. Therefore, the purpose of this study was to evaluate the advantages of ergonovine drug test for vasospasitc angina examination during coronary angiography, such as the relationship between the procedure time, contrast medium usage, and radiation exposure effects of coronary angiography. Follow-up data of 142 patients who underwent coronary angiography and variant angina examination from september 2021 to february 2023 were used. As a result of analyzing contrast usage dose and dose area product and air kerma dose and number of imaging series and procedure time, variant angina examination was statistically significantly higher than coronary angiography. (p<0.001) In conclusion, variant angina examination other than coronary artery angiography are radiologically negative. Therefore, we think it is better to avoid excessive inspection. Nevertheless, in the case of the provocation test, the longer the examination time, the higher the fluoroscopy time and the amount of contrast medium used, so it is better to conduct the test as quickly as possible or shorten it.

A Study of Applied Pressure According to Knee variable During Lachman Test for Anterior Cruciate Ligament using Telos Device (Telos Device를 이용한 전방십자인대 Lachman 검사 시 무릎두께와 활성도에 따른 인가압력에 대한 고찰)

  • The purpose of study is to investigate the difference of the daN force applied during the examination using Telos Device and the degree of fluctuation of anterior cruciate ligament according to the individual differences of the muscles involved in knee stability. The examination was done by the Lachman test using Telos Device and the changes of anterior cruciate ligament were measured by varying the force of 0. 15, 30 daN on each right and left side and the force of 30 daN after the bruce protocol. Computed tomography (CT) was used to measure muscle mass. As a result of measuring the degree of fluctuation of the anterior cruciate ligament according to the change of the force applied to the Telos Device, there was a statistically significant difference in the knee fluctuation when 15 daN and 30 daN were applied on both right and left. Also, it is analyzed that the degree of fluctuation of anterior cruciate ligament varies according to the difference of individual's muscle mass. Therefore, it is considered that the force applied to the Telos Device should be changed according to the difference of individual's muscle mass.
