Objective : The purpose of this study was to verify the effectiveness of home-visit occupational therapy in stroke patients. Methods : Two patients with stroke who applied for home-based occupational therapy services at a health center in Seoul were enrolled. The home-visit occupational therapy program evaluates the subject's daily living, task performance, cognitive, and emotional functions, sets occupational therapy goals and plans interventions based on a client-centered approach. Occupational therapy programs consisted of 12 sessions based on the client's major problems. Results : COPM scores improved in both cases, there was an improvement in COPM scores, and in Case 2 there were improvements in MBI and K-MMSE scores. There was also an improvement in KGDS scores in Case 1. Conclusion : Home-visit occupational therapy was found to be effective in improving daily activities, cognition, and mental function of stroke patients. During home and community integration, continuous and continuous rehabilitation services need to be activated from institutional rehabilitation to community-based rehabilitation. Active home-visit occupational therapy is needed to promote physical, cognitive, mental and social access in stroke patients discharged from hospitals.

Objective : he aim of this study is to describe on the definition of the welfare state and scope of the social welfare and to analyze the development of the social welfare policy for the disabled in South Korea. Methods : We described on the definition of the welfare state and scope of the social welfare and analyzed the social welfare policy and welfare policy for the disabled among Korean Government through literature review. Finally, We also analyzed the welfare policy for the disabled in the Roh Moo-hyun Administration. Results : Welfare policy or welfare policy for the disabled was developed as economic growth from 'selective welfare' to 'universal welfare'. Scope of the social welfare was expanded from 'minimum guarantee' of the Lee seong-man administration to 'participatory welfare' of the Roh moo-hyun administration and that was similar to the welfare policy of administration for the disabled. Conclusion : We suggest that occupational therapist based community should find unmet health need and provide home-based rehabilitation services to the disabled and the elderly.

This study investigated the situations and satisfaction of home-based occupational therapy service for children under eighteen residing in Suncheon and Gwangyang. Arithmetic statistics were conducted by dividing the general facts of home-based occupational therapy service, the kinds of service used in home-based therapy, the satisfaction of home-based occupational therapy service, facts helpful for children, and facts helpful for parents or home. As a result, the most common source of home-based occupational therapy information was the association for children with disabilities and its satisfaction was high. This study will provide basic materials to build up systematic, stable, and continuous rehabilitation therapy support projects by the government.

This study was aimed to provide the basic data for the development of effective educational program by reflecting the opinions of the visiting health service workers. The subjects were 144 visiting health service workers in Gwangju and Jeollanam-do area who responded the mail questionnaire. The data were collected from June to July, 2001 using questionnaire composed of the education need, knowledge by subjective appraisal, and experience of education. The major findings of this study were as follows: 1. The number of respondents who received at least one education within recent three years were 43(29.9%) at the central level, 57(39.6%) at the provincial level and 53(36.8%) at the district level. The satisfaction with education was higher at the central level than at the provincial and district level. 2. Knowledges by subjective appraisal on the 'chronic degenerative diseases management' and 'health promotion' was relatively high. while that of 'rehabilitation' was low. 3. The knowledge by subjective appraisal of visiting health service was related with experience of education and license status. The knowledge was higher in registered nurses than in nurse aids. The curricula related to increased level of knowledge of visting health service workers were 'elderly health care', 'rehabilitation' and 'psychiatric-mental health nursing' educations at the central level 'continuing education for the community health practioners' and 'psychiatric-mental health education' at the provincial level and 'elderly health care', 'rehabilitation' 'psychiatric-mental health' and 'acute diseases control' educations at the district level. 4. The respondents preferred elderly health management as the contents of education, officer group education as the method of education, province(30.4%) as the main body of education, exercise and practice as the form of education, 2-3 times per year as the frequency of education, and 3-5days as the period of education. The findings of this study could suggest that future education program should be planned to increae the knowledge level of visiting health service workers by reflecting their educational need.
The objective of this study is to suggest measures to support the community-based rehabilitation sports service for health of people with disabilities, which can be applicable to South Korea, by analyzing the rehabilitation sports service support system of Germany and Japan. Through literature review and direct visiting to Japan, the researcher explored such factors as, background of initiating the rehabilitation sports service, concept, range, and subject of theservice, service delivery procedure, and institutions, and finance of the service. The institutional implications are as follows: First, the concept and range of rehabilitation sports are gradually expanding. Initially, the rehabilitation sports started targeting for veterans with disabilities for the purpose of medical rehabilitation in Germany and Japan. As time passes, however, the importance of rehabilitation sports has emphasized, and the subjects have gradually expanded for the purpose of preventing disability or secondary diseases caused by disability. Second, in order to perform rehabilitation sports service, it is essential to have manpower with expertise in medical treatment, welfare, disability, and exercise prescription, likewise Germany and Japan do. Third, Germany and Japan emphasize the assignment of rehabilitation sports instructors in institutions, rather designating specific institutions, which is moving toward the manpower-based rehabilitation sports service, rather place-based service. Fourth, rather having simple medical programs or sports-oriented programs, it would be necessary to have the systematized disease-oriented programs for each type of disability. Lastly, the clients-oriented service delivery process should be established.
This study of the elderly in the future expansion of long-term care insurance as a priority landing at the introduction of physical therapy services aim to provide baseline data and long-term care insurance for him, the elderly that provides physical therapy services under the elderly property of a physical therapist recognition system for the investigation was conducted. As a result, the perception of the elderly long-term care insurance was higher by 88.1% of the higher needs, but also the absolute need for more than 40 years of age the response was higher with 60.3 percent. In addition, the number of visits that care should be priced higher relative to the 59.7 percent was the highest opinion. The purpose of this study was to expand the home-based rehabilitation services in the long term care insurance. This study was reviewed on the home-based physical therapy in the OECD. The way of this study was literature search and review. This was to reviewed Proquest, Sciencedirect, SpingerLink, Pubmed. Kew word is "home-based therapy". This was collected 71 articles and final analyzed article was 49. The results were as follows; The article number is seven in Canada, fifteen in USA, five in UK, six in Australia, three in Sweden, five in New Zealand, two in Netherlands, one in Turkey, South Korea, Japan, France, Spain, Italy respectively. Randomized controlled trial is fifty, case-control study is eleven, interview is three, literature review is two, and case report, cost-analysis, cost-effectiveness, pilot study, single blinded parallel design, demonstrative study, survey is one respectively. Physical therapist played an important role in home-based rehabilitation among OECD than nurse. Therefore, we must introduce home-based physical therapy in long term care insurance in South Korea.
This study aims to investigate the awareness and needs of patients as consumers and therapists as providers on the introduction of home physical therapy in the long-term care insurance. The survey participants were 96 patients and 132 physical therapists to a questionnaires, we analyzed for frequency of each questionnaire about awareness and necessity. As a result of the test, they considered that physical activity support services provided by nurse and nurses aid ware not specialized enough(pt 60,4%, PT 75,4%) and quality therapy should be available through a home physical therapy(pt 47.9%, PT 59.2%). Also, patients responded that the priorities to improve long-term care insurance were given to lowering expenses(35,4%) and expanding coverage of subjects(32.3%) while the physical therapists responded that professionals should be expanded(73.8%) and diverse rehabilitation services(20.2%) and a rehabilitation team should be introduced. They responded that the reasons of home physical therapy were needed that it was hard for patients to visit medical centers(PT 30.0%) themselves and it would be improvable for them to get physical function in elderly(pt 47.1%) through the service. In conclusion, as most patients as well as physical therapists responded that home visit therapy services should be introduced and their awareness of it was very high, it is suggested that the service should be quickly introduced as soon as possible.
Objective : The purpose of this study was to examine the roles and practices of occupational therapists in public health centers based on their working experience of occupational therapists who are working in these facilities. Methods : We used Giorgi's descriptive phenomenological research method. Six occupational therapists were interviewed, and their recorded data were analyzed using Claizzi's thematic analysis. Results : As a result, 4 categories, 16 themes, and 24 elements were elicited. The major roles of occupational therapists in public health centers were the evaluation of clients, home-visit occupational therapy, management of dementia programs and community connection projects. Interventions that are needed to create a healthy community include case and program management, compensatory approaches, and healthy lifestyle interventions. The difficulties experienced by occupational therapists include their limited number job insecurity, performance-oriented administrative policy, and the lack of communication among professionals. Occupational therapists require education in health management and practice in public health centers to build their capacities. Conclusion : The results of this study could be used for guidelines for occupational therapists who work in community healthcare centers, as well as basic training for competent occupational therapists in the community.
The study was performed to identify the functional status change of beneficiaries of Long-Term Care Insurance and its related factors. We conducted the logistic regression with 17,652 beneficiaries during August and September in 2008. As a result, activities of daily living(ADL), behavioral changes, rehab, instrumental activities of daily living(IADL) and cognitive function, followed by nursing care area were improved in a greater degree. For the institutional service, level-1 beneficiaries was significantly improved in rehab area and level-2 beneficiaries was improved in ADL. For the home-visit care service of in-home services, level-1 beneficiaries was improved in ADL, level-2 beneficiaries was improved in ADL and rehab area, level-3 beneficiaries was improved in ADL, cognitive function and behavioral changes. For the day-and-night care service, level-1 beneficiaries was improved in ADL, IADL, behavioral changes and rehab area, level-2 beneficiaries was improved in behavioral changes, level-3 beneficiaries was improved in cognitive function and behavioral changes. For the short-stay service, level-3 beneficiaries was improved in behavioral changes. By the above results, there was a difference in a functional improvement by level and used services. Therefore, government need to provide the personalized service system based on the objective and comprehensive understanding for health and functional status of beneficiaries.
