Primary Undifferentiated Sarcoma of Left Atrium (좌심방에 발생한 원발성 미분화 육종-1례 보고-)

  • Primar99y cardiac tumors are rare and about 25% of primary cardiac tumors are malignant. A primary undifferentiated cardiac sarcoma, which very rare, is presented as follows: A 28-year old woman at the 32th week of pregnancy was admitted to the hospital because of dyspnea. A large intracardiac (left atrium) tumor was found with cardiac echocardiography and MRI. Emergency operation was performed under the diagnosis of left atrial myxoma. After Cesarean section, LA-tomy was done under the cardiopulmonary bypass. Tumor removal including endocardium of left atrium was done and final pathologic diagnosis was primary undifferentiated laft atrial sarcoma. After adjuvant radiotherapy, she has been followed up in out patient bases without problems up to now.

The Characteristics of the Vocalization of the Female News Anchors (여성 뉴스 앵커의 발성 특성 분석)

  • This paper covers the studies on common voice parameters through the voice analysis of female main news anchors on weekday evening by the station, and differences of relative voices and sounds among stations. To examine voice characteristics, 6 voice parameters were analyzed and it showed anchors of each station had distinctive characteristics of voices and phonations over all fields except the speech rate, and there were also differences in sound systems. As major analysis parameters, basic pitch, tone of the 1st formant and pitch ratio, level of closeness by pitch bandwidth, type of sentence closing through average pitch position within pitch bandwidth, average speech rate, and acoustic tone analysis by energy distribution by frequency band were used. Analyzed values and results could be referred to and utilized in the criteria of phonation characteristics for domestic female news anchors.

Clinical Characteristics of Functional Dysphonia (기능성 발성장애의 임상적 특성)

  • Background and Objectives : Functional dysphonia is a voice disturbance in the absence of structural or neurologic laryngeal pathology characterized by voluntary misuse of laryngeal muscles. The present report reviews clinical characteristics of 25 patients with functional dysphonia. Materials and Method : We analyzed medical records, perceptual and acoustic analysis of voice samples, aerodynamic studies and laryngoscopy. Results : There was no sex or age predilection. Eighty four percent of patients presented sudden onset of symptoms and 76% had specific events at the onset. Most patients showed breathy or strained voice and various degree of vocal fold insufficiency with supraglottic compensatory contractions. Acoustic analysis revealed non-diagnostic, but mean flow rate was lower than normal in all cases. All patients responded to voice therapy except for 4 patients who were tort to follow up. Mean number of voice therapy sessions required to get responses is 1.9 sessions. Conclusion : We concluded that patients with functional dysphonia responded very well to short-term voice therapy and should be included in differential diagnosis in patients with dysphonia cannot be explained by structural or neurologic etiology.

Treatment of Snapping Hip Caused by a Tight lliotibial Tract (장경대 구축에 의한 탄발성 고관절의 치료)

  • Purpose: The purpose of this study was to analyze the treatment results of snapping hip caused by a tight iliotibial tract using Z-plasty . Materials 8nd Methods: Five patients (seven hips) were treated by Z-plasty using Brignall and Stainsby's method from March, 1995 to May,2002 There were three males and two females and their average age at operation was 24.4 years. All patients complained of snapping, pain, and difficulty of squatting with attaching both knees . The average of follow-up was one year and four months. The local anesthesia was done in three patients (3 hips) and epidural anesthesia in two patients (4 hips). Results: Snapping was absent, squatting with attaching both knees was possible, and pain relief was excellent in all patients. One patient (one hip) experienced occasional aching associated with exercise but it is not serious. Conclusion: This Z-plasty of iliotibial band was an excellent surgical technique for the symptomatic snapping hip.

  • Primary malignant melanoma occurring in the lung is very rare and only few cases have been reported in the literatures. We have experienced one case of primary malignant melanoma of the right middle lobe. The patient was a 65-year-old male who had cough with blood tinged sputum for one month. Chest computed tomography showed about 4.5$\times$3 cm sized mass at the right middle lobe encasing the bronchus. Percutaneous needle biopsy was performed and confirmed to malignant melanoma. Physical examination and additional clinical history showed that the mass had not metastasized to other possible primary sites. The patient underwent thoracotomy with right middle lobe lobectomy. Histologically, the tumor showed sheets of tumor cells that have nuclear atypia and melanin pigment Postoperative course was uneventful and the patient has been followed up for 24 months with no recurrence.

  • The estimated relative incidence of primary pulmonary sarcoma to lung cancer is 0.4%. Furthermore, osteogenic sarcoma of the pulmonary artery is extremely rare. We report a case of a 63-year-old woman who visited our emergency room with the chief complaints of chest pain, dyspnea and dizziness. On echocardiography, right heart failure due to acute pulmonary artery embolism was diagnosed and we performed emergency operation. After opening the main pulmonary artery trunk, we found a mass attached to the arterial wall and massive thrombi around the mass. The mass was diagnosed as primary pulmonary artery osteosarcoma through postoperative evaluation. The patient received chemotherapy and radiotherapy. The patient is alive without specific symptoms 16 months postoperatively.

Development of Vocal Signals in the Pipistrellus abramus(Chiroptera: Vespertilionidae) (집박쥐의 발성 시그널 발달에 관한 연구)

  • This study reports developmental changes in the vocal signals produced by infant or young Pipistrellus abramus. In contrast to adult bats, infant bats of the P. abramus emitted calls characterized by multi harmonics and variable signal patterns. Infants at two day of age emitted a irregular signal and showed gentle FM signals between 10 and 20 days. After about 40 days of age, the young bats emitted similar signals to the echolocation calls of adults. As the infant bats growing up, vocal signals trend showen a decrease in pulse duration(p<0.001), pulse interval(p<0.001) and number of harmonic(p<0.001). And the peak frequency(p<0.001), starting frequency(p<0.001) and ending frequency(p<0.001) in infant bats increased with the age. The largest change in pulse interval occurred in 5 days. And the pulse duration, peak frequency, starting frequency and ending frequency of the bat's sounds changed most dramatically in 25 days of age.

Primary Hypoparathyroidism in a Miniature Schnauzer Dog (미니어처 슈나우저에서 발생한 원발성 부갑상선기능저하증)

  • A 7-year-old intact female, Miniature Schnauzer dog was presented with an acute seizure episode and tremors. Fever and panting were noted when presented. Physical examination revealed apparent forelimb muscle rigidity, fasciculations and stiff gait. Characteristic laboratory findings in this dog were severe hypocalcemia with hyperphosphatemia. The low serum concentration of intact parathyroid hormone and ionized calcium levels were consistent with the diagnosis of primary hypoparathyroidism. In addition, there were increased urinary excretion of calcium and decreased urinary excretion of phosphorus in this case. Urgent treatment was initiated with 10% calcium gluconate. The dog remained healthy with vitamin D analogue and oral calcium supplementation. A transient hyperphosphatemia was controlled well with sevelamer hydrochloride. To the author's knowledge, this is the first case report describing the clinical and laboratory characteristic features of canine primary hypoparathyroidism and its clinical outcome in Korea.

  • Primary retroperitoneal mucinous cystadenocarcinoma is a very rare malignancy. To date, 51 cases have been reported, including 3 in pregnant women. Herein, we report magnetic resonance findings of a 31-year-old Korean woman (15 weeks and 3 days pregnant) with primary retroperitoneal mucinous cystadenocarcinoma. On abdominal magnetic resonance imaging (MRI), a mass was identified in the retroperitoneal area with a nodular lesion showing heterogeneous signal intensity and focal wall thickening on T1- and T2-weighted images. Exploratory laparotomy and tumor excision were performed. Histological examination revealed primary retroperitoneal mucinous cystadenocarcinoma. The patient subsequently underwent total hysterectomy, bilateral salpingo-oophorectomy, and omentectomy for metastatic mucinous cystadenocarcinoma of both ovaries 15 months after her initial surgery.

Expression Pattern of RB and p53 Proteins and its Correlation with Prognosis in Primary Lung Cancer (원발성 폐암에서 종양억제유전자 RB와 p53 단백질 발현양상과 예후와의 상관관계)

  • Immunohistochemical stains for RB and p53 tumor suppressor gene products were performed on 72 cases of resected primary lung cancer tissues to study the correlation between their expressions and the histologic types, the clinical stage, and the survival rate. The results were as follows. 1. The RB protein was altered or absent in 38 cases (52.8%), and the mutant p53 protein was detected in 35 cases (48.6%). 2. The incidences of RB and p53 protein expression were significantly different among the histologic types (p<0.05) but were not correlated with the clinical stages of lung cancer (p>0.05). 3. The two year survival rate of patients with alteration of both RB and p53 genes (RB-/p53+) was 22. 4%, and that with no alteration of both genes (RB+/p53-) was 63.1%. This difference was statistically significant (p=0.01). 4. It was shown that alteration of RB protein greatly affects the prognosis of lung carcinoma by multivariate analysis of prognostic factors. The presence or absence of RB and mutant p53 protein in tumor cells is closely related to the survival of primary lung cancer patients, and it is suggested that RB gene expression is an independent prognostic factor of primary lung cancer.
