Effect of MK-801 on the Prevention and Treatment of Tardive Dyskinesia (지연성 운동장애의 예방과 치료에 대한 MK-801의 효과)

  • Tardive dyskinesia(TD) is a serious side effect of long-term treatment with neuroleptic medications. To investigate if glutamatergic hyperfunction is associated with TD, effect of MK-801 on the prevention and treatment of TD was studied using a rat model, ie, vacuous chewing movements(VCM). When comparing VCM scores of GroupI(haldol decanoate+MK-801) with that of GroupII(haldol decanoate+phosphate buffer saline), late measured VCM scores of former were significantly lower than that of latter, meaning that MK-801 is effective in the prevention of VCM. Furthermore, when MK-801(0.1mg/kg, 0.3mg/kg) is administered to VCM(+) rats(VCM${\geq}$ 7/4min) of GroupII, VCM scores were significantly decreased, meaning that MK-801 is also effective in the treatment of VCM. From the above results, it is suggested that glutamatergic hyperfunction might be involved in the development of TD and MK-801 could be effective in the prevention and treatment of it.

The Phonatory Characteristics of the Profound Hearing-Impaired Adults' Voice: with Reference to F0, Intensity, and their Perturbations (심도 청각장애 성인의 발성 특성: 강도, 음도, 및 그 변동율을 중심으로)

  • This study investigates the differences in mean F0, intensity, jitter and shimmer across hearing aid, gender and vowels. For this study, 20 hearing-impaired adults and 20 normal hearing adults as a control group were asked to read 7 Korean vowels(/$\alpha$, $\Lambda$, o, u, ɯ, i, $\varepsilon$/). Subjects' readings were recorded by NasalView and analyzed by Praat. Results showed that the means of F0 were significantly higher in the hearing impaired group(HL) than in the normal hearing group(NH), in the female group than in male group, and in high vowels than in low vowels. Second, intensity was significantly higher in the normal hearing group(NH) than in the hearing impaired group(HL), in male group than in female group, and in low vowels than in high vowels. Third, jitter was significantly higher in the normal hearing group(NH) than in the hearing impaired group(HL), and in female group than in male group and in the back vowels than in front vowels. Finally, shimmer was significantly higher in the normal hearing group(NH) than in the hearing impaired group(HL), and in male group than in female group. In particular, the male group showed that front vowels tend to have higher shimmer than back vowels.

The Efficiency of Voice Therapy for the Patients with Mutational Falsetto (변성발성장애 환자에 대한 음성치료의 효과)

  • Mutational falsetto is a kind of voice disorders due to the failure to acquire proper low-pitched voice during the puberty. The patients with mutational falsetto can produce the normal low-pitched voice by the surgical treatment, like the type III-thyroplasty, or the voice therapy. The present study is, focusing on the latter treatment, to consider the efficiency of voice therapy for the mutational falsetto. The 7 patients who were diagnosed as mutational falsetto by the laryngologists, and treated by the voice therapist were selected as subjects. Their voices of pretherapy and posttherapy were analyzed on the aspects of acoustics and aerodynamics. Acoustic analysis was done by the MDVP(Multidimensional Voice Program) of CSL(Computerized Speech Lab, Kay Elemetrics, Co.), and aerodynamic analysis, by the Maximum Sustained Phonation of Aerophone II(Kay Elemetrics, Co.). By these measurements, we could find that fundamental frequency(F0) was significantly lowered, on the average, 65Hz. Maximum phonation time(MPT) was increased 4.57 second, and shimmer was decreased 1.644%, respectively, and each changes was statistically significant, too. On the average, jitter was decreased 0.499%, mean flow rate(MFR) was decreased 27.71ml/sec, and NHR was increased 0.023 which was the only parameter not showing improvement. But the changes of jitter, MFR and NHR were not statistically significant.

  • Normally, as a result of increased laryngeal growth, the male voice drops about one octave in pitch level during adolescence. Failure of the voice to drop in pitch is consider to be a clinically significant voice disorder - 'mutational dysphonia'. The aim of this article is to evaluate the changes brought about by voice therapy, using the analysis of the EGG measure from Lx Speech Studio program(Laryngograph Ltd, UK) as well as acoustic, and aerodynamic studies in 18-year-old mutational dysphonia patient. The results from the Lx Speech Studio program demonstrated bimodal distribution of DFx(Hz), DQx(%), QxFx and diplophonic characteristic. After voice therapy combined with manual compression method, the distribution of DFx, DQx, QxFx was changed uniform with a dramatic reduction of higher pitch level. In addition, this finding suggests the EGG measure helps to choice treatment options, monitor the efficacy of therapy, and estimate the prognosis of diseases.

Comparison of the Surgical Results in Mutational Dysphonia between Unilateral Shortening of Thyroid Cartilage Method and Bilateral Shortening of Thyroid Cartilage Method in Type III Thyroplasty (변성발성장애의 제3형 갑상연골성형술시 갑상연골익의 편측절제술과 양측절제술과의 치료성적 비교)

  • Failure to change from the higher pitched voice of preadolescence to the lower pitched voice of adolescence and adulthood is called "mutational dysphonia" The voice is weak, thin, breathy, hoarse, and mono-pitched. If the voice theraphy was failed, surgery to lower vocal pitch which is refered to thyroplasty type III, is indicated. We compared the post-op acoustic parameters with pre-op data in unilateral antero-posterior shortening of the thyroid cartilage method and bilateral antero-posterior shortening of the thyroid cartilage method each other. Bilateral antero-posterior shortening of the thyroid cartilage method shows significant drop of fundamental frequency and speaking fundamental frequency statistically than unilateral shortening method. There was no significant differences in Jitter, Shimmer, SNR, MFR and other psychoacoustic analysiss parameters between two groups. These data shows that unequal tension of the vocal cord in uilateral antero-posterior shortening of the thyroid cartilage method does not control the pitch effectively so bilatreal shortening method in Type III thyroplasty is recommandable procedure in surgery of the mutational dysphonia.

The Aerodynamic & Respiratory Muscle Pressure Aspects of Patients with Adductor Spasmodic Dysphonia (내전형 경련성발성장애의 호흡압력과 공기역학적 특징)

  • This study was conducted to investigate the respiratory and aerodynamic function of adductor spasmodic dysphonia (ADSD) patients. Participants were (1) 18 females SD patients with non- Botulinum toxin injection (2) 14 females SD patients who had taken treatment of Botulinum toxin injection. (3) 14 age- and sex- matched normal female controls. Spirometer and phonatory function analyzer were used for respiratory muscle pressure (MIP: Maximum inspiratory pressure), MEP: Maximum expiratory pressure)& MPT(Maximum phonation time) and aerodynamic(F0:Fundamental frequency, intensity, MFR: Mean flow late, Psub: Subglottal pressure) measurement. The results were as follows: (1) Normal group was significantly higher in MIP, MEP, MPT than two SD groups (p < .05); (2) MPT was significantly lower in SD with non-Botulinum toxin injection group than SD with the treatment experience of Botulinum toxin injection (p < .05); (3) All aerodynamic parameters, F0, intensity, MFR, Psub, were not significantly different among three groups(p > .05).The reason of short MPT in ADSD may use lower respiratory pressure than normal group as strategy to decrease their tremulous voice quality. Moreover respiratory muscle pressure was lower than normal group regardless of botulinum toxin injection treatment.

A Case of the Laryngeal Papilloma Observed for the Past 13 Years (13년간 경과를 관찰한 후두유두종의 1예)

  • It is well known that laryngeal papilloma is histopathological benign, but clinically one of the most troublesome disease in otolaryngologic department, usually seen in child age. Since Cleesmann reported papilloma of the vocal cord, 1817 first, the etiology had not been known exactly. The symptoms in children are dyspnea, dysphonia due to recurrence, and papilloma in adult can change into neoplasm. The papilloma is thought as real neoplastic lesion and there is not effective treatment, using now repeated removal and combined therapy. This report is based on the typical findings and progress of one case of laryngeal papilloma observed for the past 13 years with repeated removal and topical application of 5-FU at our department. Authors have experienced this subject is interesting that the change of the tumor region during the course than to any new therapeutic methods employed, and so report with some reviews of the literatures.

Effects of vowel types and sentence positions in standard passage on auditory and cepstral and spectral measures in patients with voice disorders (모음 유형과 표준문단의 문장 위치가 음성장애 환자의 청지각적 및 켑스트럼 및 스펙트럼 분석에 미치는 효과)

  • Auditory perceptual assessment and acoustic analysis are commonly used in clinical practice for voice evaluation. This study aims to explore the effects of speech task context on auditory perceptual assessment and acoustic measures in patients with voice disorders. Sustained vowel phonations (/a/, /e/, /i/, /o/, /u/, /ɯ/, /ʌ/) and connected speech (a standardized paragraph 'kaeul' and nine sub-sentences) were obtained from a total of 22 patients with voice disorders. GRBAS ('G', 'R', 'B', 'A', 'S') and CAPE-V ('OS', 'R', 'B', 'S', 'P', 'L') auditory-perceptual assessment were evaluated by two certified speech language pathologists specializing in voice disorders using blind and random voice samples. Additionally, spectral and cepstral measures were analyzed using the analysis of dysphonia in speech and voice model (ADSV).When assessing voice quality with the GRBAS scale, it was not significantly affected by the vowel type except for 'B', while the 'OS', 'R' and 'B' in CAPE-V were affected by the vowel type (p<.05). In addition, measurements of CPP and L/H ratio were influenced by vowel types and sentence positions. CPP values in the standard paragraph showed significant negative correlations with all vowels, with the highest correlation observed for /e/ vowel (r=-.739). The CPP of the second sentence had the strongest correlation with all vowels. Depending on the speech stimulus, CAPE-V may have a greater impact on auditory-perceptual assessment than GRBAS, vowel types and sentence position with consonants influenced the 'B' scale, CPP, and L/H ratio. When using vowels in the voice assessment of patients with voice disorders, it would be beneficial to use not only /a/, but also the vowel /i/, which is acoustically highly correlated with 'breathy'. In addition, the /e/ vowel was highly correlated acoustically with the standardized passage and sub-sentences. Furthermore, given that most dysphonic signals are aperiodic, 2nd sentence of the 'kaeul' passage, which is the most acoustically correlated with all vowels, can be used with CPP. These results provide clinical evidence of the impact of speech tasks on auditory perceptual and acoustic measures, which may help to provide guidelines for voice evaluation in patients with voice disorders.

Effects of vocal aerobic treatment on voice improvement in patients with voice disorders (성대에어로빅치료법이 음성장애환자의 음성개선에 미치는 효과)

  • This study aimed to investigate the effects of vocal aerobic treatment (VAT) on the improvement of voice in patients with voice disorders. Twenty patients (13 males, 7 females) were diagnosed with voice disorders on the basis of videostroboscopy and voice evaluations. Acoustic evaluation was performed with the Multidimensional voice program (MDVP) and Voice Range Profile (VRP) of Computerized Speech Lab (CSL), and aerodynamic evaluation with PAS (Phonatory Aerodynamic System). The changes in F0, Jitter, Shimmer, and NHR before and after treatment were measured by MDVP. F0 range and Energy range were measured with VRP before and after treatment, and the changes in Expiratory Volume (FVC), Phonation Time (PHOT), Mean Expiratory Airflow (MEAF), Mean Peak Air Pressure (MPAP), and Aerodynamic Efficiency (AEFF) with PAS. Videostroboscopy was performed to evaluate the regularity, symmetry, mucosal wave, and amplitude changes of both vocal cords before and after treatment. Voice therapy was performed once a week for each patient using the VAT program in a holistic voice therapy approach. The average number of treatments per patient was 6.5. In the MDVP, Jitter, Shimmer, and NHR showed statistically significant decreases (p < .001, p < .01, p < .05). VRP results showed that Hz and semitones in the frequency range improved significantly after treatment (p < .01, p < .05), as did PAS, FVC, and PHOT (p < .01, p < .001). The results for videostroboscopy, functional voice disorder, laryngopharyngeal reflux, and benign vocal fold lesions were normal. Thus, the VAT program was found to be effective in improving the acoustic and aerodynamic aspects of the voice of patients with voice disorders. In future studies, the effect of VAT on the same group of voice disorders should be studied. It is also necessary to investigate subjective voice improvement and objective voice improvement. Furthermore, it is necessary to examine the effects of VAT in professional voice users.
