  • Anaerobic degradation of petroleum hydrocarbons in a soil artificially contaminated with 10,000 mg/kg soil of diesel fuel was tested by adding an anaerobic sludge taken from a sludge digestion tank. Treatments of soil(50 g) with 15 mL/kg soil and 30 mL/kg soil of the digestion sludge(2,000 mg/L of vss(volatile suspended solids)) showed 37.2% and 58.0% of total petroleum hydrocarbons(TPH) removal during 90 days incubation, respectively. In evaluation of several anaerobic conditions including nitrate reducing, sulfate reducing, methanogenic, and mixed electron accepters condition, treatments with the digested sludge showed significant degradation of diesel fuel under all anaerobic conditions compare to a control treatment of soil without the sludge and a treatment of autoclaved soil treatment with autoclaved digestion sludge. The rate of diesel fuel degradation was the highest in the treatment with the sludge and mixed electron accepters (75% removal of TPH) for 120 days incubation followed in order by sulfate reducing, nitrate reducing, methanogenic condition as 67%, 53%, 43%, respectively. However, the removal rate of non-biodegradable isoprenoid was the highest in the sulfate reducing condition. These results suggest that anaerobic degradation of diesel fuel in soil with digested sludge is effective for practical remediation of soil contaminated with petroleum hydrocarbons.

  • Thirty eight patients with stage I and II primary gastrointestinal non-Hodgkin's lymphoma were treated in the Department of Therapeutic Radiology, Seoul National University Hospital between 1979 and 1984. There were 6 systemic disseminations during radiotherapy, and the overall failure rate were $31\%$ in the cases with tumor bulk less than 5cm in diameter before radiotherapy and $75\%$ in the cases with tumor bulk greater than 5cm in diameter (p <0,05). The overall 5 year survival rate were $69.2\%$ in 28 patients who completed radiotherpay and $72\%$ in 24 patients with tumor bulk less than 5cm in diameter (small or no tumor bulk). The 5 year disease free survival rate were $71\%$ in cases with tumor bulk less than 5cm in diameter and $25\%$ in cases with tumor bulk greater than 5cm in diameter (p<0.01). But the intitial stage was not related with treatment result in all cases or subgroups of cases. Thus the cases with small or no tumor bulk were shown to be curable with combined surgery and postoperative radiotherapy, but for the control of the cases with large tumor bulk that had a guarded prognosis combined radiotherapy and chemotherapy should be tried.

  • Fourty nine patients with squamous cell carcinoma of oral tongue were reviewed retrospectively for the evaluation of clinical manifestation and for the comparison between therapeutic modalites. The gross shape of the tumor was infiltrative in 22, ulcerative in 12, ad ulcer-oinfiltrative type in 10 patients. Direct extension of the tumor was most commonly to the floor of the mouth. The incidence of nodal metastasis generally increased with tumor stage. $55\%$ of the patients showed neck nodal metastasis at the time of diagnosis. Ipsilateral subdigastric node were most commonly involved, followed by submandibular nodes. The 5-year survival rate of patients treated with surgery and radiotherapy was $58.7\%$ in contrast to $21.6\%$ in radiation alone group. Overall 5-year survival rate was $31\%$ In radiation alone group, half of the patients in stage I, II were locally controlled. But the local control In stage III, IV was much inferior to early lesions. Especially, of 4 patients combined with implantation technique, 3 were completely controlled. 5-year survival rate of these implanted patients was $50\%,\;49.4\%$ of patients treated over 7,000cGy survived 5 years. This was significant in contrast to $6.4\%$ of the group treated below 7,000cGy. The most common sites of failures were primary sites. In early lesions primary radiotherapy with implantation would be an appropriate treatment in cancer of oral tongue, operation reserved for radiation failure. Operation and adjuvant radiotherapy is recommended in cases of advanced disease.

  • We investigated the usefulness of tumor-associated antigen (TA-4) by a radioimmunoasay method in estimating the extent of disease and tracking the clinical course of disease in 58 patients with cervical cancer. According to our results and those of other authors, the normal range of serum TA-4 was arbitrarily taken to be less than 2 ng/ml. The proportion of the pretreatment positive serum TA-4 level of 48 squamous cell carcinoma patients was $60\%.$ And $40\%$ in 5 adenocarcinoma patients. Advanced disease group showed higher incidence of positive serum TA-4 level; $40\%,\;72\%,\;63\%,\;and\;100\%$ in stage I to IV, respectively. And the absolute values of TA-4 were higher in advanced disease. In patients treated with radiation, elevated serum TA-4 level usually declined after 3000 cGy and further dropped to normal range in $44\%$ after 5000 cGy. The positive rate in primary cervical cancer was $59\%,$ (32/54) and $100\%$ (4/4) in recurrent conical cancer. And 15 patients with recurrent or persistent disease during follow-up revealed $80\%$ positive serum TA-4 level. In conclusion, it would be suggested that serial serum TA-4 measurements may be helpful in tracking the clinical course during and after treatment.

  • Purpose: Accurate assessment of the lesion after treatment of patients with bone lymphoma is difficult. In this patient who demonstrated complete remission after chemotherapy, the regions of fluorine-18 fluorodeoxyglucose $(^{18}FFDG)PET$ uptake diminished more rapidly fellowing therapy, indicating a complete response at much earlier stage than did Magnetic Resonance Imaging (MRI) or CT based findings. With the conventional methods, such as MRI and CT, It was difficult to assess whether the residual tumor tissue was viable or not. Decision to complete response is very important in patients with lymphoma to plan the further treatment. We experienced a patient with primary lymphoma of bone who revealed complete response to chemotherapy on $^{18}FFDGPET$ while CT showed persistent destructive bone lesion. Thus, $^{18}FFDGPET$ study after therapy may be superior to CT in the evaluation of response to treatment in primary lymphoma of bone.

  • Although reports of multiple primary malignant tumors have increased recently, cases of synchronous double primary tumors of lung and liver are rare. A 73-year-old man suffered from chronic cough. His chest x-ray showed segmental atelectasis of the right upper lobe. Bronchoscopy revealed a mass occluding the orifice of the anterior segmental bronchus of the right upper lobe, and a biopsy showed a squamous cell carcinoma. A synchronous hepatic mass was found by ultrasonography. However, F18-FDG-PET showed no evidence of a distant metastasis. The liver biopsy revealed a hepatocellular carcinoma. A right upper lobe lobectomy and a sleeve resection were performed for the lung cancer, and radiofrequency ablation was performed for the hepatocellular carcinoma.

  • A gas chromatography-mass spectrometric (GC-MS) assay method was developed for the rapid and sensitive determination of volatile- and semivolatile organic compounds in water. Two hundreds mL of water sample was extracted in a 250 mL separatory funnel with 1 ml of pentane at pH 6.5. Fluorobenzene and 1,2-dichlorobenzene-d4 as internal standards were added to water sample and the solution was mechanically shaken for 5 min and analyzed by GC-MS (selected ion monitoring) without more any concentration or purification steps. The peaks had good chromatographic properties and the extraction of these compounds from water also gave relatively high recoveries with small variations. The range of detection limits of the assay was 0.5-10 ng/L. Turnaround time for up to about 40 samples was one day. This method is simple, convenient, and can be learned easily by relatively inexperienced personnel. This method was used to analyze 15 volatile- and semivolatile organic compounds in water of a Lake, and raw and treated water from three Water Treatment Plants in Korea. As the analytical results, benzene, toluene, xylene, isopropylbenzene, 1,3,5-trimethylbenzene, 1,2,4-trimethylbenzene, naphthalene and 2,4,6-trichlorophenol were detected at concentrations of up to 0.4, 1.9, 1.3, 0.2, 1.8, 13.0, 1.7 and $1.1{\mu}g/L$, respectively. But chlorobenzene, trichloroethylene, tetrachloroethylene, ethylbenzene, n-butylbenzene and dibromochloropropane levels during that period were not significant. The removal effect of the compounds in three Water Treatment Plants was calculated. The compounds studied were generally removed during conventional water treatment, especially during the active carbon filtration.

  • Background: Lung cancer and chronic obstructive lung disease often coexist in the same person who are elderly and cigarette smoking. There are several reports that the presence of chronic obstructive pulmonary disease constitutes an independent risk factor for the development of lung cancer. Moreover, the association between mucus hypersecrtion and lung cacer has been reported. Method: In 72 cases with primary lung cancer which were confirmed histopathologically at Chonbuk University Hospital from August 1986 to July 1991, We evaluated the relationship between spirometry and lung cancer characteristics. Results: Six cases(8.3%) showed normal lung function, 16(22.2%) cases showed pure restrictive lung disease, 46(63.9%) cases showed moderated obstructive lung disease and 4(5.6%) cases showed severe obstructive lung disease. $FEV_1$(%) was lower in central type than in peripheral type, lower in advanced non-small cell cancer and lower in subjects with phlegm. $FEV_1$/FVC(%) was higher in small cell cancer than in squamous cell cancer and higher in patients without previous pulmonary disease than with previous pulmonary disease. But there was no statistically significant difference in lung function according to histologic types and smoking history. Lung cancers with $FEV_1$/FVC less than 75% consisted of 35 cases of squamous cell cancer, 7 of small cell cancer(14%), 5 of adenocarcinoma(10%), 2 of large-cell carcinoma and 1 of unclassified carcinoma. Squamous cell carcinoma occured more in patients with $FEV_1$/FVC<75% than with $FEV_1$/FVC$\geq$75%(p<0.05). Conclusion: It was suggest that low $FEV_1$/FVC, as reflection of obstructive lung disease, may be at greater risk for squamous cell carcinoma in cigarette smoker.

  • Background: Lung cancer is the leading cause of cancer over the world. P53 alteration is by far the most common genetic defect in lung cancer. The mutation of p53 protein involves the loss of inhibitory function of p53 related tumor suppressor gene and resultant oncogenesis. The analysis of p53 alterations consists of immunohistochemical stain, PCR based assay, or serologic ELISA (enzyme-linked immunosorbent assay). Methods : Serum levels of p53 mutant protein were measured in 69 cases of lung cancer (adenocarcinoma n=29, epidermoid n=16, small cell n=13, large cell n=1, undifferentiated n=1, undetermined n=9) and 42 controls of respiratory disorders using ELISA. Immunohistochemical stain in tissue was performed using monoclonal antibody of p53 in lung cancer subjects. Results: Both serum p53s in nonsmall cell cancer ($0.28{\pm}0.44ng/ml$) and in small cell cancer ($0.20{\pm}0.14ng/ml$) were not different from controls ($0.34{\pm}0.20ng/ml$). Also there was no significant difference in serum p53 according to tumor stages. P53 immunohistochemical stain showed 50% positivity overall in lung cancer. There were no close correlation between serologic level and positivity of immunohistochemical stain. Conclusion: The serologic determination of p53 mutant protein is thought to have no diagnostic role in lung cancer. Immunohistochemical stain in lung cancer specimen shows 50% positivity.
