Radionuclide Penogram in Diagnosis of Vasculogenic Impotence (혈관성 발기부전의 진단을 위한 방사성핵종 음경촬영술)

  • Yasculogenic impotence is produced by abnormalities of vascular blood supply or drainage, and is the most common cause of various organic impotences. An increasing awareness of vascular causes of impotence has resulted from development of various diagnostic tests, but precise measurement of penile blood flow is difficult. Radionuclide penogram has been introduced recently to diagnose vasculogenic impotence. Forty-one impotent patients and 12 normal men were studied with radionuclide erection penogram using Tc-99m pertechnetate and an intracavernous injection of papaverine. We defined arteriogenic impotence as arterial index less than 0.66, and venogenic impotence as venous index greater than 0.09. By this criteria, the false positive ratio in normal men was 17%, and the false negative ratio in radically cystectomized patients was 0%. Side effects were small purpura of the penile shaft and dull pain during injection of papaverine. The radionuclide erection penogram was noninvasive and gave a dynamic evaluation of the arterial supply, venous drainage, and blood flow in the corporeal bodies. This method should be considered as a valuable adjunct to evaluate patients with vasculogenic impotence.

Effects of Scolopendra on Ethanol-Induced Erectile Dysfunction in Rats (Ethanol로 발기부전을 유도한 흰쥐의 성기능 개선에 오공(蜈蚣)추출물이 미치는 영향)

  • Scolopendra was known to cure erectile dysfunction. This study was aimed to investigate the effects of Scolopendra on the nitric oxide synthase activity, nitrite level, cyclic-GMP and erectile responses in rat's corpus cavernosum penis. The crushed Scolopendra was extracted 3 times, each time with 3 volumes of methyl alcohol at $60^{\circ}C$ for 24 h. The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 14.2 g. Scolopendra extract was oral-administered 50 mg per 1 kg of body weight for 30 days. First, samples were treated with Scolopendra, and then ethanol-treated rats and L-N-Nitroarginine methyl ester (L-NAME) treated rats were fed with the samples. The level of urethral nitrite and nitric oxide synthase activity in the ethanol-Scolopendra double administered rats were as high as in the normal group, while the one in the ethanol-treated group was decreased. The level of urethral cyclic-GMP and guanylate cyclase actiyity in the ethanol-Scolopendra double administered rats were as high as in the normal group, while the one in the ethanol-treated group was decreased. The level of urethral phosphodiesterase activity in the ethanol-Scolopendra double administered rats was as low as in the normal group, while the one in the ethanol-treated group was increased. The erectile response to a cavernous nerve stimulation in L-NAME $(10^{-4})-treated$ rats was restored by the Scolopendra to the similar level seen in the normal group. The electile response to cavernous nerve stimulation in the ethanol-Scolopendra double administered rats were increased as high as in the normal group while the one in the ethanol-treated group was decreased. Scolopendra was effective in restoring the ethanol-induced or L-NAME-induced erectile dysfunction in rats.

Preparation and Evaluation of PGE1-ethyl Ester Intraurethral Solutions for Erectile Dysfunction (PGE1-ethyl Ester함유 발기부전 치료용 요도주입 액제의 제조 및 평가)

  • [ $PGE_1$ ]-ethyl ester intraurethral solutions were prepared in ethanol/propylene glycol mixture with penetration enhancer and viscosity-enhancing agent. The stability of $PGE_1$-ethyl ester in intraurethral solution was investigated at various temperature. Simultaneous determination of $PGE_1$-ethyl ester and $PGE_1$ was performed using a validated HPLC technique. In pentobarbital anesthetized cats, increase in intracavernous pressure(ICP), increase in penile length and duration of erectile response were determined after intraurethral application of $PGE_1$-ethyl ester solutions. $PGE_1$-ethyl ester solutions, when instilled into the eyes of rabbits, produces no noticeable irritation, or slight transient conjunctival irritation. From these results, ocular irritation of this solutions was judged as practically non-irritating. The stability study indicates that the therapeutically effective content in solution is well maintained for 46 weeks or longer when they are stored at $4^{\circ}C$. After intraurethral application of $PGE_1$-ethyl ester, ICP was increased and penile erection was induced. $PGE_1$-ethyl ester intraurethral solutions for erectile dysfunction could be developed and evaluated by employing feline erection model.

Effect of Lepidium spp. (Maca) Extract on the Improvement of Sexual Function in Rats Induced Erectile Dysfunction with Ethanol (에탄올로 발기부전을 유도한 흰쥐의 성기능 개선에 마카추출물이 미치는 영향)

  • The purpose of this study was to examine the effects of Maca water and/or ethanol extract on the nitric oxide (NO) production in human umbilical vein endothelial cells HUVAC and on erectile dysfunction in rats. Maca was extracted due to both solutions, which are water and ethanol. Each Maca extract was applied to HUVAC, and NO production was checked. Additionally, three different dosages (250, 500 and 1,000 mg/kg) of Maca ethanol extract was administered to Sprague-Dawley rats for 4 weeks. All rats were sacrificed and each sample was collected for analysis. The control rats received only the saline vehicle. The NO production of HUVAC was significantly increased by domestic and homemade Maca water extracted at $60^{\circ}C$ group. Both NO generation and testosterone release were not influenced due to the oral administration of Maca. In the EtOH group rats, the number of sperm was reduced compared to that of the control group. All Maca groups had a high number of sperm and each sperm count had increased as a result of the Maca extract dose. The results of this research suggest that Maca has a positive effect on male erectile dysfunction, which need to be examined further in future studies.

Effectiveness of Korea Red Ginseng in Erectile Dysfunction-Multi-National Approach (발기부전 환자에서의 홍삼의 효능에 관한 연구 -동남아시아의 다국적 연구)

  • Ginseng has been used in maintaining physical vitality throughout the far-eastern countries and recently its metabolism and actions on neurologic, cardiovascular, and endocrinologic systems are studied. Korean red ginseng (KRG) has been used in various ailments, and to prove its efficacy for erectile dysfunction an international study on Asians other than Korean was performed. Patients with borderline organic and psychogenic erectile dysfunction were included. KRG were given daily, and placebo were given as controls. Treatment lasted a total of 3 months. Surveys including libido, erection, ejaculation, sexual activity, and sexual satisfaction were given. Serum testosterone and erectile function study were taken. Among the 64 patients, 37 patients were followed with KRG. Five had diabetes, 5 hypertension, 5 hypercholesterolemia, 6 low testosterone, 6 psychogenic, and 11 idiopathic. The improvement after KRG administration was $70.2\%$ on objective questionnaire and $75.7\%$ on subjective analysis. When KRG were given, all parameters surveyed have shown improvements compared to the placebo. The effects of KRG in Chinese and Singapores were similar to the Koreans. Serum testosterone levels were nonnalized in 6 patients with KRG, who's serum testosterone levels were reduced from pre-study. Two patient reported constipation, and 2 gastric upsets in the KRG group. In conclusion, KRG has beneficiary action on male erectile capabilities with little side effects. KRG is effective in Koreans and also Asians. The exact action mechanism and the active ingredients in KRG need to be studied.

The Relaxation Effects of Cynomorii herba in Isolated Corpus Cavernosum Tissues (발기부전 처방에 사용되는 쇄양(鎖陽)이 음경해면체 이완에 미치는 영향)

  • Objective: This study aimed to examine the relaxation effects and underlying mechanisms of Cynomorii herba (CH) extract in isolated rabbit corpus cavernous tissues.Methods: We experimented with CH extract (0.01-3.0 mg/mL). Nω-nitro-L-arginine (L-NNA) was experimented before the CH extract to contracted strips induced by phenylephrine (PE, 1 μM)and compared with nonexperimented. In addition, we experimented with calcium chloride (Ca2+, 1 mM) after pretreatment of the CH extract in Ca2+-free Krebs-Ringer solution to contracted strips induced by PE. The cell viability and nitric oxide (NO) concentration of human umbilical vein endothelial cells (HUVECs) were measured by an methylthiazol-2-yl-2, 5-diphenyl tetrazoliumbromide (MTT) assay and Griess reagent system. The ratio of smooth muscles to collagen fibers, in addition to eNOS- and PDE-5-positive reactions, was examined by histochemical and immunohistochemical staining.Results: The CH extract significantly induced the relaxation of the cavernous strips, and the pretreatment with L-NNA inhibited CH extract-induced relaxation. The L-NNA pretreatment reduced the increased contraction induced by the addition of Ca2+in Ca2+-free solution. Furthermore, the NO concentration of the HUVECs increased. When the CH extract was applied to the corpus cavernosum of the penis (CCP) of Sprague Dawley rats, the ratio of smooth muscles to collagen fibers by PE and the formation of eNOS around the helicine artery increased. However, the CH extract treatment decreased PDE-5 positive reactions.Conclusions: These results show that the relaxation effects induced by the CH extract are associated with the suppression of the influx of extracellular Ca2+ via the production of NO and eNOS and inhibition of PDE-5.

Effects of Mantidis Vagina Ovorum on the Cimetidine-Induced Erectile Dysfunction in Rats (상표소(桑螵蛸)가 Cimetidine으로 발기부전을 유도한 흰쥐의 성기능 개선에 미치는 영향)

This study was aimed to investigate the effects of Mantidis Vagina Ovorum on the nitric oxide synthase (NOS) activity, nitrite level, antioxidation and erectile responses in rat's corpus cavernosum penis. The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 16.6 g. Mantidis Vagina Ovorum extract oral-administered 75 mg per 1 kg of body weight for 30 days. First, samples were treated with Mantidis Vagina Ovorum, and then cimetidine-treated rats and L-N-Nitroarginine methyl ester (L-NAME) treated rats were put with the samples. The level of urethral lipid peroxide in the cimetidine-Mantidis Vagina Ovorum double administered rats was decreased as low as in the normal group, while the one in the cimetidine-treated group was increased. The urethral NOS activity, the level of urethral nitrite, the level of testosterone and the electile response to cavernous nerve stimulation in the cimetidine-Mantidis Vagina Ovorum double administered rats were increased as high as in the normal group while the one in the cimetidine-treated group was decreased. The electile response to cavernous nerve stimulation and the level of nitrite in L-NAME ($10^{-4}$)-treated rats was restored by the administration of Mantidis Vagina Ovorum as high as in the normal group. Mantidis Vagina Ovorum was effective in restoring the cimetidine-induced or L-NAME-induced erectile dysfunction in rats.

Effects of Lycii fructus Extracts on the Erectile Dysfunction by Chronic Ethanol Consumption in Rats (에탄올 음용으로 유도된 발기부전 동물모델에 대한 구기자 추출물의 개선효과)

  • Erectile dysfunction (ED) is a highly prevalent disorder that affects millions of men worldwide. ED is now considered an early manifestation of atherosclerosis, and consequently, a precursor of systemic vascular disease. Lycii fructus extracts (LFE) were administered for 4 weeks to assess the improving effects on ED. Animals were divided into one normal group and four LFE-treated groups (0, 0.3, 0.6, and 1.2 g/kg). We induced ED in the study animals by oral administration of 20% ethanol instead of water everyday for 4 weeks. This study was designed to investigate the effects of LFE on the mRNA levels of inducible nitric oxide synthase (iNOS) and endothelial NOS (eNOS) expression; NO levels of nitric oxide (NO) and cyclic guanosine monophosphate (cGMP); blood profile; and erectile response of the corpus cavernosum of the rat penis. The libido of the LFE-administered male rats was higher than that of the ethanol control group. The erectile response of the corpus cavernosum was restored after LFE administration, to a level similar to the normal group. In addition, the iNOS in the corpus cavernosum of the male rats administered LFE decreased. In contrast, compared to the control group, LFE-administered male rats showed increased eNOS, NO and cGMP levels in the corpus cavernosum. These results indicate that LFE effectively restored ethanol-induced ED in male rats.

A Survey of Sexual Dysfunction by Antidepressants (항우울제와 연관된 성기능 장애에 대한 조사)

  • Objectives: Antidepressants are frequently associated with sexual dysfunction. Especially, there were few report of sexual dysfunction related with venlafaxine and mirtazapine in Korea. The purpose of this survey was to evaluate the frequency and nature of sexual dysfunction related with selective serotonin reuptake inhibitors(SSRIs), venlafaxine and mirtazapine by the use of specific questionaires. Methods: In one hundred twenty two patients, sexual dysfunction was investigated cross-sectionally by using The questionaires for sexual dysfunction, which includes questions about decreased libido, delayed orgasm or ejaculation, premature ejaculation or orgasm, absence of orgasm or ejaculation, erectile dysfunction in men/vaginal lubrication dysfunction in women, sexual pain and patient's tolerance of the sexual dysfunction. BDI was also measured. Sexual dysfunction was analyzed in association with the duration and the dose of medications and the severity of depression. Results: The incidence of sexual dysfunction during antidepressants use in our survey was 37.7%. There were no difference of incidence in sexes(p=.746). In comparison of paroxetine, venlafaxine and mirtazapine, there were no significant differences of the incidence(p=.065) and the severity of any type of sexual dysfunction. Conclusion: Although there were no significant differences of sexual dysfunction in our survey, sexual dysfunction may be closely associated with antidepressants. Further prospective study of sexual dysfunction should be needed.

Comparison of the Anti-inflammatory Effects of the Extracts from Rubus coreanus and Rubus occidentalis (토종 복분자와 외래종 복분자 추출물의 항염증효과 비교)

  • The dried fruit of the Rubus coreanus, which is well-known in Korea and referred to as 'Bokbunja,' has been employed as a traditional medicine for centuries. This crude drug has been utilized in Korea for the management of impotence, spermatorrhea, enuresis, asthma, and allergic diseases. Our previous study demonstrated that the ethanol extracts of R. coreanus have anti-inflammatory effects. The principal objective of the present study was to conduct a comparison of the anti-inflammatory effects of the ethanol extracts of R. coreanus and R. occidentalis; here, we tested the unripe (URCE), half-ripened (HRCE), and ripened fruits (RCE) of R. coreanus, and the unripe (UROE), half-ripened (HROE), and ripened fruits (ROE) of R. occidentalis. We found that URCE, UROE, HRCE, and HROE reduced the production of nitric oxide and prostaglandin $E_{2}$ as well as pro-inflammatory cytokines in lipopolysaccharide-stimulated RAW264.7 murine macrophages. Interestingly, the R. coreanus extracts showed stronger inhibitory effects on the production of these inflammatory mediators than the R. occidentalis extracts.
