Analysis of factors related to teeth needing extraction (발거가 필요한 치아와 관련된 요인분석)

  • This study used raw data from the 2013-2015 National Health and Nutrition Survey, and conducted a multivariate logistic regression analysis to identify factors related to teeth extraction from Korean elderly aged 65 and older (4.037). As a result, the risk of tooth extraction was 1.84 times higher in males, 1.52 times in age 75 or older, 2.36 times higher in income, 2.04 times higher in income, and 1.44 times higher in smokers. The results were statistically significant as 1.49 times without oral examination and 2.30 times without dental treatment for 1 year. Through this, the current health behavior and oral health behavior can reinforce the basis of risk factors related to the need for tooth extraction. At the national level, we need to look for diversification.

  • This study was conducted on 1,865 drunk elderly people who had teeth that were over 65 years old. As a result of analysis of factors related to tooth extraction, the higher the income, the lower the income, the higher the risk of having to remove teeth from smokers, poorer oral health status, and no oral hygiene products. The difference was statistically significant. In order to identify the risk factors of tooth extraction for the elderly and prevent tooth loss early and to extend the life of residual teeth as much as possible, it is necessary to improve the drinking culture and seek oral health care strategies in the previous life cycle, not the present, Oral health policy should be pursued.

  • We decided among extraction, orthodontic traction and autotransplantation such as direction and position of unerupted tooth, degree of developing root apex, eruption space, being of supernumerary tooth or odontoma or cyst when tooth impacted. Autotransplantation is considered when orthodontic traction is unrealistic or when tooth movement can absorb root of neighbor tooth. The prognosis for successful autotransplantation is dependent on a number of factors such as root development, surgical technique, patient's age, endodontic treatment, time and type of splinting, preservation of periodontal ligament and storage medium. Especially when severe osseous defect is being, bone graft considered for reducing of mobility and for assisting recovery. In all cases, chief complaint is unerupted tooth and various causing factors were supernumerary, odontoma, ectopia and so on. Before autotransplantation, space regaining was done if needed and demineralized freezed dried bone and autogenous bone graft was done when there is severe osseous defect by extraction of supernumerary tooth or odontoma. Splinting was removed after 2-3weeks At 3-4weeks after autotransplantation, endodontic treatment was decided. At follow up check, normal recovery was done and there was no inflammatory or replacement root resorption in periapical radiograph.

  • Recently, the dental information systems were rapidly developed in order to store and process the data of patients. But, these systems should serve a doctor a good quality information against disease for diagnostic and surgery purpose so as to success in this field. This function of the system it important to persuade patients to undergo proper surgical operation they needed. Hence, 3D teeth model capable of simulating the dental surgery and treatment is necessary Teeth manipulation of dentistry is performed on individual tooth in dental clinic. io, 3D teeth reconstruction system should have the techniques of segmentation and 3D reconstruction adequate for individual tooth. In this paper, we propose the techniques of adaptive optimal segmentation to segment the individual area of tooth, and reconstruction method of tooth based on contour-based method. Each tooth can be segmented from neighboring teeth and alveolar bone in CT images using adaptive optimal threshold computed differently on tooth. Reconstruction of individual tooth using results of segmentation can be manipulated according to user's input and make the simulation of dental surgery and treatment possible.

  • The infraocclusion usually occurs in the mixed-dentition stage, and it is commonly accepted that the cause of the infraocclusion is ankylosis. The treatment options for patient with infraocclusion of primary molars are observation, restoration or surgical removal of the affected teeth. If the successors are present, most of the infraoccluded and ankylosed primary molars may occur normally. However, when the permanent successors are absent and the progression of infraocclusion is found, affected teeth may need to be extracted. In the case of infraocclusion which can cause vertical alveolar defect due to ankylosis, extraction before growth spurt should be performed for the future prosthetic treatment. A six-year-old female had the ankylosis and infraocclusion of multiple primary molars and congenital missing of premolars. The affected primary molars were extracted before growth spurt to avoid a significant vertical ridge defect and to promote the vertical development of alveolar bone, and the result was observed for many years. The purpose of this report is to report the management of multiple infraoccluded primary molars without permanent successors in a young patient.

  • 완전히 발육된 하악 제3대구치가 매복되어 있거나 또는 부분적으로 맹출된 상태일때는 임상증상을 일으키기 전에 발치하는 것이 치관주위염이나 이로 인한 합병증 및 치주질환 등을 예방할 수 있느느 가장 좋은 방법이며 이미 임상 증상을 유발한 매복 하악 제 3대구치는 대부분 재발방지를 위하여 발거하게 된다. 성공적인 발치의 기준을 1) 치아의 완전한 발치 2) 인접조직에 대한 최소한의 외과적 손상 3) 술후 합병증의 극소화 4) 가능한한 시술 시간의 단축 등이라 할때 매복 하악제 3대구치의 성공적인 발거를 위하여는 발치의 기본 원칙을 숙지하여야 함은 물론이며 더욱 면밀한 술전 평가와 외과적 발치술 시 필요한 기구의 완비 등이 필수적이라 할 것이다.

  • Impaction is defined as the cessation of the eruption of the tooth caused by a clinically or radiographically detectable physical barrier in the eruption path or by the ectopic position of tooth germ. Besides the third molars and the maxillary canines, the most common impacted tooth is the second premolar. The overall frequency of premolar impaction has been reported to be 0.5%. In some cases, orthodontic traction and surgical repositioning may be indicated. When impacted second premolar is involved with prolonged retained second primary molar, extraction of primary molar and space maintenance lead to eruption of second premolar. In these cases, all patients visited to department of pediatric dentistry of Kyungpook National University Hospital for the chief complaint of unerupted second premolar. Extraction of prolonged retained second primary molar and space management are tried for spontaneous eruption of impacted second premolar tooth. The results were as follows: 1. When impacted second premolar is involved with prolonged retained second primary molar, minimal treatment via elimination of primary molar leads to successful results. 2. Proper space management and periodic radiographic examination are required before eruption of second premolar. 3. Sufficient time must be allowed for confirm of tooth movement before orthodontic traction or surgical repositioning. 4. The result is more successful in incomplete root development.

  • In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. If there are genetic or general factors to effect the eruption of tooth, the clinician treats these first and then takes the early treatment for eruption guidance. If there are physical factors to intercept eruption, the clinician put them off first. However, if there are no factors to effect eruption of tooth and enough space for eruption, the clinician can consider extraction of deciduous teeth, forced eruption and surgical reposition. In case of surgical repositioning, proper time for root development, proper socket formation, and minimal trauma are important for success. This case presents displaced impacted maxillary central incisor with dilacerated root. The development of root is Nolla's stage 7, and the tooth was treated by surgical repositioning. We can observe no root resorption and good healing pattern.

  • Odontomas generally appear as small, solitary, or multiple radio-opaque lesions found on routine radiographic examinations. It is a comparatively common odontogenic tumor, and may lead to interfere with the eruption of its associated tooth. In general, odontomas occur more often in permanent dentition and are very rarely associated with primary teeth. This report deals with five rare cases of primary teeth impaction associated with odontomas, with spontaneous eruption occurring in all five cases after simple surgical removal of the odontoma. Impacted primary teeth may be associated with defects in development and eruption of their permanent successors, and thus long-term observation is necessary until the permanent successors erupt.

  • Congenital tooth agenesis is the most common developmental dental anomaly, of which oligodontia is defined as the absence of six or more permanent teeth, except the third molars. Tooth agenesis causes malocclusion, alveolar atrophy, aesthetic and psychosocial problems. This clinical report describes a multidisciplinary treatment for a patient diagnosed as oligodontia, who exhibited absence of 14 permanent teeth, atrophy of maxillary alveolar bone, and mandibular protrusion. Restoration space was secured and tooth axis was improved by the extraction of deciduous teeth and orthodontic treatment. However, edge-to-edge bite of posterior teeth and arch dimension discrepancy due to atrophic maxilla was remained. To restore the aesthetics and functionality, implant retained prosthesis was planned. Considering minimal bone grafts, location and number of dental implants and prostheses design were determined. Through the gradual adjustment of provisional restoration, the appropriate centric and eccentric occlusion was reflected into a definitive prosthesis. Currently, stable functional results were attained, however, regular follow up and maintenance care over lifetimes should be performed.
