The Comparison of Productivity Change Gap of Public Hospitals and Private Hospitals in Korea (공공병원과 민간병원의 생산성 격차 비교)

  • This study calculated meta Malmquist indices and their bootstraped estimates and then decomposed them into technical efficiency change(TEC), technology change(TC), pure technology catch up(PTCU), frontoer catch up(FCU), using annual data set of general hospitals from year 2007 to 2011 collected by Korean Hospital Association and then analyzed productivity change and technology gap of Korean general hospitals. The results and implications were as follows below. First, public general hospitals showed higher meta technical efficiencies than private general hospitals while exhibited lower technology gap ratio which meant a few large private general hospitals led the whole general hospitals. Second, group productivity of private general hospitals increased larger than public general hospitals due to the differences of PTCU rather than FCU. But, there was no statistically significant differences for technical efficiency, productivity change, technology gap. Thus, public general hospitals played the same role as the private general hospitals in terms of the number of patients treated. But, considering financial hardships of public general hospitals, public hospitals needed to share and learn medical and managerial skills of the best practice of private general hospitals.

  • 본 고는 성남병원 최염이사장의 연세대학교보건대학원 학위논문으로 정부에서 국민의료의 균점을 위해 추진중인 민간지역병원운영실태의 전반적인 특성과 근본적 문제점을 중심으로 병원운영의 제도적 발전방향을 제시하고 있다. 본 연구서는 51개 민간지역병원중 현재 개원한 38개병원을 대상으로 실태조사표, 설문서 및 자문을 통한 자료에 의한 것으로 결론적으로 정부가 국민에 평등한 의료혜택을 부개하고자 설립한 민간지역병원은 시행 2차년도를 맞는 현실에 있어 도출된 문제점에 의해 운영난에 봉착하고 있음을 나타내고 있다. 이에 따라 많은 투자를 통해 건립된 민간지역병원의 성공적인 정착과 국민의료의 균점시혜를 위해서는 본계획의 면밀한 재검토와 이에따른 제도적 보완이 뒤따라야 한다며 이에 관한 세세한 문제점을 제시하고 있는데 본 지는 병원운영 발전에 도움을 주고자 이를 전호에 이어 게재하고자 한다.

정부지원 민간지역병원 운영실태분석(상)

  • 전국적으로 균형있는 의료공급체계를 확립하기 위하여 정부가 건립 추진하여온 민간지역병원은 계획목표 년도가 2년이나 경과한 1984년 4월 현재 설립인가된 51개 병원중 38개 병원(74%)이 개원하였다. 이중 4개 병원은 심각한 재정난과 부실운영으로 이미 문을 닫은 실정이고 나머지 대부분의 병원들도 운영난에 봉착하였는바 이러한 민간지역 병원의 특성과 운영성과에 관련된 요인을 분석하기 위하여 각 병원별 실태조사표와 설문조사서 및 방문면담으로 개원중인 병원전체를 대상으로 자료를 수집,조사하였다. 각 병원의 운영성과에 미치는 요인중 가장 큰 것은 지역적 여건과 병원의 입지조건이었다. 병원운영이 다소 호전되는 병원일수록 의료수가나 진료미수금과 같은 병원외부요인에 관심을 갖는데 반해 악화되는 병원일수록 병원의 위치나 타병원과의 경쟁등 병원내부요인에서 더욱 많은 문제점을 느끼고 있었다. 또한 자력에 의한 운영자금 및 양질의 타인자본 조달능력이 중요한 것으로 지적되었다. 이러한 요인은 정부의 민간지역병원 건립계획에 대한 불충분한 검토와 병원들의 판단력부족 및 자금동원능력 부족과 관리운영미숙에도 문제가 있었다. 막대하게 소요된 시설자금 및 운영자금으로 인한 높은 부채비율로 인해 이를 감당할 만한 이자부담 능력도 없으며, 기존병원에 비하여 환자절대수마저 부족으로 정상운영을 기대할 수 없었다. 이와 같은 결과는 앞으로 병원신설을 할 때 기획단계부터 면밀한 검토가 필수적이라는 것을 시사해 준다.

  • This study public hospital productivity and private hospitals, comparative analysis was divided into capital productivity, labor productivity, value-added productivity, based on the findings public hospitals are able to ensure the public interest to provide stable and profitable health services of good quality.   • 오늘날의 병원들은 환자가 의료기관을 선택하는 구매자 시장으로 변하게 되었고, 군 병원도 2004년부터 현역병의 의료보험적용이 가능해지면서 민간병원처럼 선택 받아야 하는 위치에 놓이게 되었다. 그러므로 군병원도 민간병원의 흐름과 함께 병원 이용고객들이 인식하는 의료서비스 품질, 고객만족도 및 고개충성도에 대한 관심이 필요하다. 따라서 정과 중심의 SERVPERF모델을 이용하여 지각된 군 병원의료 서비스 품질과, 고객만족 및 충성도에 관한 연구로 빠르게 변하고 있는 의료서비스 환경에서의 국민으로부터 신뢰를 받으며 군 의료서비스품질의 발전 방안을 모색하기 위해 본 연구를 시도하였다.

  • This study, using questionnaires to target Servicemen were investigating. Study period of 2009 October to 24 was 20 days. All 600 questionnaires distributed affair, but 565 cases were collected. The purpose of the this study was serviceman investigate the actual conditions of utilization of private hospitals and awareness for the level of military hospital medical service as well as to the selection of future for the private hospitals is to finding. Results of the study are as follows: Outpatient utilization rate of each other the military hospital and private hospitals was 60.7% and 18.9%.(p<.000) The results of frequency study of the factors which affect the private hospital is satisfaction of medical workforce(58.9%), medical equipment and facilities(49.6%), etc. The choice of hospital were inpatient and outpatient both the selected by private hospitals. Had to prefer a private hospital were statistically significant (p<.005). In conclusion, to increase the preference of the military hospital is level of private hospital health care level of should be to developed. More support to financial should provide for medical workforce and setting of cutting edge medical equipment in the military hospital. Military hospital is should be competition with private hospital. Also should be improve the quality of military medical service.

Analysis of Relationship between Emotional Labor and Organizational Commitment in Hospital Workers (병원 근로자의 감정노동과 조직몰입간의 관계 분석: 국·공립과 민간종합병원의 비교를 중심으로)

  • This study examined the perceptions of emotional labor among the National General Hospital and Private General Hospital in Chungcheong area. The purpose of this study is to examine the perceptions difference of public and private general hospitals. In addition, we tried to analyze the effect of emotional labor on organizational commitment by distinguishing between workers in National and Private General Hospitals. The purpose of this study is to provide the theoretical base data for mitigating emotional labor of hospital workers in the future. The results of the analysis are as follows: First, In the case of employees working at the National General Hospital, the influence of the emotional labor of the hospital workers on the organizational commitment of the hospital workers increased as the frequency of emotional expression increased. However, it was found that as attentiveness of emotional expression and emotive dissonance deepened, the degree of organizational commitment of hospital workers decreased. On the other hand, the influence of emotional labor of the hospital workers on the organizational commitment of the hospital workers in the Private General Hospital was found to be better in the organizational commitment as they pay more attention to emotional expression. However, in the case of emotive dissonance, The more deeply emotive dissonance, the less the organizational commitment of Private General Hospital workers. Second, the frequency of emotional expression in the hospital workers' perception difference of emotional labor was higher than that of the National General Hospitals. However, in the variety of emotional expression, it was confirmed that the employees of the Private General Hospitals had more varied expressions of emotions than those of the National General hospitals. In addition, it was found that the difference of recognition of the organizational commitment of the hospital workers according to the type of hospital work was higher in the organizational commitment of the employees of the National General Hospital than those of the Private General Hospitals.
