Current Status of the Reimbursement for Pharmacist-provided Health Care Services in Japan, the United States, and the United Kingdom (국외 약사서비스 지불보상체계 현황 : 일본, 미국, 영국을 중심으로)

  • World-widely, there has never been a greater need for people to access high-quality expertise about the effective and safe use of medications. Therefore, the profession of pharmacy should meet these needs of the times, as the demographic shifts have led to a situation where older adults now outnumber children, and polypharmacy is also a commonplace. However, the reimbursement system covered by the National Health Insurance (NHI) in Korea is still limited to the traditional dispensing and compounding role of pharmacist. To provide a take-home message to Korean pharmacy reimbursement system, we aimed to review and analyze the international trends in pharmacy remuneration systems. This is a comparative study between Korea, Japan, the United Kingdom, and the United States. Comparison was conducted by reviewing each country's policy and enforcement programs, as well as the related literature. Japan, the UK and the US systems remunerate diverse patient-centered pharmaceutical care services. The Korean pharmacy service fee is, however, narrowly focused on the traditional product-oriented pharmacy services. This study discussed the future direction of improving pharmacist reimbursement systems in Korea, by expanding professional pharmacy service coverage and diversifying fee schedule.

Proposal of Mobilized Passive Earth Pressure to Allowable Wall Displacement and Movement Types in Sandy Soil (벽체 허용변위와 양상을 고려한 사질토지반에서 수동측토압 제안)

  • The evaluation of passive earth pressure plays a crucial role in the design of earth-retaining structures such as retaining walls and temporary earth-retaining walls to withstand horizontal earth pressure. In the earth pressure theory, active and passive earth pressures represent the earth pressures at the limit state, where the wall displacement reaches the maximum allowed displacement. In the design of earth-retaining structures, the passive earth pressure is considered as the resisting force. In this context, the limit displacement at which passive earth pressure occurs is significantly greater than that associated with the active earth pressure. Therefore, it is irrational to apply this displacement directly to the calculation of passive earth pressure. Instead, it is necessary to consider the mobilized passive earth pressure exerted at the allowable horizontal displacement to evaluate the structural stability. This study proposes an allowable wall displacement, denoted as 0.002 H (where H represents the excavation depth), based on a literature review that focuses on sandy soils. To calculate the mobilized passive earth pressure from the wall displacement, a semi-empirical equation is proposed. By analyzing the obtained data on mobilized passive earth pressure, a reduction factor applicable to Rankine's passive earth pressure is proposed for practical application in sandy soils under different wall movement types.

Taxonomic Review of Cottiusculus nihonkaiensis (Cottoidei: Cottidae) from Korea (한국산 점줄횟대 Cottiusculus nihonkaiensis(둑중개아목, 둑중개과)의 분류학적 재검토)

  • The genus Cottiusculus Jordan and Starks 1904 (Cottoidei: Cottidae) comprises four species worldwide, three species in Japan, and two species in Korea. Cottiusculus species are characterized by a compressed head and body without scales, except for lateral-line scales, a preopercle with four spines, a curved uppermost preopercular spine with small dorsal spines, and teeth on the upper jaws, vomer, and palatines. Two Cottiusculus species have been recorded in Korea: Cottiusculus gonez and Cottiusculus nihonkaiensis. Among these, "jeom-jul-hoet-dae" specimens were initially identified as Cottiusculus schmidti by many Korean Ichthyologists. However, since C. nihonkaiensis was reported by Kai and Nakabo (2009), most such specimens have been identified as C. nihonkaiensis. To clarify the taxonomic status of "jeom-jul-hoet-dae", we conducted morphological and molecular analyses collected from the coasts of South Korea, and compared our findings with previous reports. Japanese and Korean C. nihonkaiensis specimens mostly consistent in some proportional measurements, our molecular results indicate that they are the same species. C. nihonkaiensis, "jeom-jul-hoet-dae" distinguished from other congers by combination of following characters: simple nasal spines, two branched cirri at the upper jaw, cirri at the opercular, preopercular, and lateral lines, and a number of blotches below the lateral line.

A Literature Review on Unmet Needs of High-Prevalence Cancer Survivors: Focus on Breast Cancer, Thyroid Cancer, Colorectal Cancer, and Lung Cancer (호발암 생존자의 미충족 수요에 대한 문헌 고찰: 유방암, 갑상선암, 대장암, 폐암을 중심으로)

  • Objectives : This study aimed to identify unmet needs and influencing factors for patients who have breast cancer, colorectal cancer, lung cancer, and thyroid cancer. Methods : We reviewed the SCIE publications on unmet need of four prevalent cancer patients published after 2010 through a web search. Results : The measurement tools primarily used were Cancer Survivors' Unmet Needs and Supportive Care Needs Survey questionnaire. Lung cancer patients reported a relatively higher rate of unmet needs. Breast cancer patients frequently reported unmet needs in the healthcare system and information, while thyroid cancer patients in post-treatment management and psychological issues. Colorectal cancer patients reported unmet needs in psychological and comprehensive care domain, and lung cancer patients reported unmet needs in physical and daily life management. Younger age, a shorter time since diagnosis or treatment, and higher levels of anxiety, depression, distress, and reduced quality of life were associated with more significant unmet needs. Conclusions : Unmet needs and influencing factors vary by cancer type. Considering the characteristics of each patient group and unmet needs can help in development of more effective treatment and support programs.

Psychosocial Pre-Transplant Assessment of Living Kidney Donors (생체 신장 이식 공여자에 대한 정신사회적 평가)

  • In Korea, the dependence on living donations is high due to the shortage of organs available for donation compared to the number of people waiting for transplants and the number of living organ donations continues to increase. In particular, the number of living-donor transplantations is high worldwide, highlighting the importance of pre-transplant psychosocial evaluation of living kidney donors. According to previous studies, when evaluating living organ donors before transplantation, it is crucial to determine whether the donor can give informed consent and be aware of the risks after surgery. Pre-transplant evaluation tests such as ELPAT living organ donor Psychological Assessment Tool (EPAT), Live Donor Assessment Tool (LDAT), Living Donation Expectancies Questionnaire (LDEQ), Minnesota Multiphasic Personality Inventory-2 questionnaire (MMPI-2) and Temperament and Character Inventory (TCI) are conducted for donors. After reviewing the literature on these pre-transplant psychosocial assessment tools, we will also look at legal considerations for living kidney donors in Korea and suggest an effective and essential pre-transplant screening evaluation method for living kidney transplant donors.

Study on Establishment of a Monitoring System for Long-term Behavior of Caisson Quay Wall (케이슨 안벽의 장기 거동 모니터링 시스템 구축 연구 )

  • In this paper, a sensor-based monitoring system was established to analyze the long-term behavioral characteristics of the caisson quay wall, a representative structural type in port facilities. Data was collected over a period of approximately 10 months. Based on existing literature, anomalous behaviors of port facilities were classified, and a measurement system was selected to detect them. Monitoring systems were installed on-site to periodically collect data. The collected data was transmitted and stored on a server through LTE network. Considering the site conditions, inclinometers for measuring slope and crack meters for measuring spacing and settlement were installed. They were attached to two caissons for comparison between different caissons. The correlation among measured data, temperature, and tidal level was examined. The temperature dominated the spacing and settlement data. When the temperature changed by approximately 50 degrees, the spacing changed by 10 mm, the settlement by 2 mm, and the slope by 0.1 degrees. On the other hand, there was no clear relationship with tidal level, indicating a need for more in-depth analysis in the future. Based on the characteristics of these collected database, it will be possible to develop algorithms for detecting abnormal states in gravity-type quay walls. The acquisition and analysis of long-term data enable to evaluate the safety and usability of structures in the event of disasters and emergencies.

  • The Personal Information Protection Commission was launched in August 2020 as an integrated control tower for personal information protection, but several problems have been pointed out in the personal information protection operation system. First, despite the fact that Korea's personal information protection system has an integrated legal system that regulates both the public and private sectors, it has been pointed out that it is difficult to carry out smooth personal information protection functions due to incomplete integration of protection functions, such as the Financial Services Commission being in charge of personal credit information protection and the Korea Communications Commission being in charge of personal location information protection. Next, despite the increasing number of public sector personal information leakage incidents, there is a lack of personnel with expertise and specialized support organizations to efficiently investigate them, and there is a concern that the lack of an efficient response system to personal information infringement by global IT companies in Korea in the era of digital commerce may weaken the protection of citizens' personal information. In order to solve these problems, I reviewed overseas cases and literature and proposed the following measures. First, it is necessary to centralize the personal information protection supervision function for credit information and location information to the Personal Information Protection Commission. Second, it is necessary to secure expertise by securing specialized personnel and establishing specialized institutions to respond to public sector personal information leakage incidents. Third, it is necessary to revitalize the domestic agency designation system and establish an international cooperation system to protect people's personal information in the digital commerce era. I believe that these measures to develop the personal information protection system will lead to more systematic personal information protection.

Study of Sulfur Dioxide Contents in Various Fresh Vegetables During the Drying Process (건조에 따른 채소류 중의 이산화황함량 분석)

  • This study was planned to monitor the change in the content of sulfur dioxide during the drying process of fresh vegetables. The analysis of sulfur dioxide was conducted by the Optimized Monier-Williams Method based on the Korea Food Code. The samples were kinds of vegetables which consisted of naturally-originated sulfur compounds (green onion, onion, cabbage, garlic, radish leaves, radish). Fresh vegetables (n = 182) and dried vegetables (n = 41) purchased from different local areas were investigated for the content of sulfur dioxide. The fresh vegetables were dried at 50~$60^{\circ}C$ using hot-air dryer. The moisture contents of dried samples were adjusted to keep 10 percents. The contents of sulfur dioxide in self-dried vegetables were 104.6 mg/kg in green onion, 75.4 mg/kg in onion, 129.1 mg/kg in cabbage, 197.6 mg/kg in garlic, 23.0 mg/kg in radish leaves and 52.5 mg/kg in radish, respectively. The increase of sulfur dioxide content according to the moisture content reduction was different from the expected. It means that the contents of sulfur dioxide can be altered by other factors except moisture contents. This results can be utilized as materials for the safe management of sulfites of dried vegetables.

The Introduction of the Concept of "Original Form" to the Heritage Conservation and Management and the Establishment and Development of the Principle of "Maintaining the Original Form" (한국의 문화재 보존·관리에 있어서 원형개념의 유입과 원형유지원칙의 성립, 그리고 발달과정)

  • The concept of "original form" and the principle of "maintaining the original form" take center stage in conservation, management, and promotion of the domestic heritage. Introduced in the 20th century, there were little discussion or deliberation about the concept of "original form" therefore it remains a vague and somewhat abstract notion subject to individual interpretation. Without a specified practical meaning, "maintaining the original form" became the fundamental principle for heritage conservation and management in the 1999 version of the Cultural Heritage Protection Act, engendering difficulties in applying the principle in practice. Conceived as an important first step toward resolving the issues stemming from the indistinct concept of "original form," this paper explores the process through which the concept was introduced to Korea and then established and developed as a legal principle for heritage conservation, management, and promotion. While the examination of the related documents and various cases shows that the development of the concept of "original form" has centered on specific periods and architectural styles, this essay explicates that the notion "original form" is commonly used as a term referring to the form at the earliest possible temporality. It also explains that this view emanates from perceiving heritages not as multivalent objects, but as a material object that exclusively carries aesthetic and, more importantly, historical value, and that comes from the history awareness of the times. This essay suggests that the concept "original form" should be reestablished with full consideration of the diverse values of heritage and diverse forms through which heritage can be expressed. After reviewing the feasibility and practicality of the concept a set of concrete guidelines should be presented for application in practice.

A Study on the Influence of Affct Based Trust and Cognition Based Trust on Word-of-Mouth Behaviors -Focusing on Friendship Network and Advice Network- (정서기반신뢰와 인지기반신뢰가 구전행동에 미치는 영향 연구 -친교네트워크와 조언네트워크를 중심으로-)

  • As developed IT, Word-of-Mouth(WOM) used varied terms as buzz marketing and viral marketing, and impressed that importance. Despite introduced new marketing tool on managers and professionals, online word-of-mouth including SNS lack of study on social network what based viral in marketing. In social network, patterns of relationship between individuals influence each other individual behaviors. Therefore this research grouped friendship-network and advice-network by characteristics, studied on trust of information source that antecedents of word-of-mouth in network. This study examined that affect- and cognition based trust affect WOM acceptance as WOM behaviors and examined effect of type of product as moderating variable. Additional this literature studied that WOM acceptance affect WOM recommend. To find the Influence of Trust on Word-of-Mouth Behaviors, a survey has done 206 samples(undergraduate students). The results of this study are as following : First, type of trust different friendship network and advice network. Affect-based trust is outstanding in friendship network than in advice network, while cognition-based trust stands out in advice network than another. Second, affect- and cognition based trust positive affect WOM acceptance. Contrary to expectations, what is preconceived trust in network have a similar effect for WOM acceptance regardless of type of trust. Third, WOM acceptance positive affect WOM recommend. Fourth, affect based trust affect WOM acceptance of hedonic product rather than utilitarian product. Upon especially in friendship network terms, affect-based trust has a more effect on WOM acceptance than cognition-based trust. This study has many implications. First, it is important that trust what have an influence WOM acceptance grouped affect- and cognition based trust. Second, it confirmed that trust is antecedents of positive WOM. Third, it is important that network grouped friendship network and advice-network by trust. Fourth, it gave managerial implications that they have to supply WOM through which network by type of product. We This study classified network and trust based on previous study. Then it examined relations between WOM behaviors. Further research could do enrich various things for example various age group, valence of message, quality of information.
