Immunologic Aspect of Carcinogenesis (발암의 면역학적 관점)

  • 실험실에서 면역세포가 종양세포를 죽일 수 있다든가 혹은 드물지만 암 환자에서 암이 자연적으로 소실되었다는 임상적 보고 그리고 AIDS 환자와 같은 면역계의 장애를 가진 환자에서 Kaposi 육종, 특정 형태의 림프종이 더 잘 생긴다는 것 등으로 미루어 보면 동물이나 인간에 발생하는 암에 대해 어느 정도의 면역반응이 일어나는 것은 분명하다. 인체는 규칙적으로 암세포를 생산하지만 면역계에 의해 제거되는 것으로 추측된다. 그러나 아직도 면역계가 어떻게 암을 억제하는지 그리고 면역계가 암발생을 억제하는데 왜 실패하는지에 대해서는 정확히 모르고 있다. 이러한 의문점들은 앞으로 종양 면역학이 규명해야 할 과제이다.

The Morphological Changes of Cryopreserved Rat Trachea After Heterotopic Transplantation (쥐의 초냉동기관 이소 이식 후 형태학적 변화)

  • The best treatment of congenital or acquired tracheal stenosis is resection and end to end anastomosis. Various prosthetic material and tissue graft replacement can be considered when the stenotic segment is too long, but their uses are still limited due to many serious complications. The present study examined the effect of immunosuppression and cryopreserved allograft trachea after intraperitoneal omental implantation for evaluation of the possibility of tracheal transplantation. Thirty tracheal segments were harvested from fifteen donor Wistar rats. Among them eighteen segments were implanted immediately(group I, II, III) and twelve segments were used for cryopreservation(group IV, V). Heterotopical intraperitoneal implantation was performed in five groups of rats(n=6); Group I was Wistar syngeneic controls and received no immunosuppression. Group II and III were those of Sprague-Dawley recipients, the former receiving no immunosuppression and the latter receiving immunosuppression(Cyclosporin A 15mg/kg/day, Methylprednisolone 2mg/kg/day). Group IV and V were groups of Sprague-Dawley recipients, the former receiving immunosuppression and the latter receiving no Immunosuppression. After 28 days, rats were sacrificed and the tracheal segments were histologically evaluated. Epithelial thickness was significantly decreased in group II, IV. Epithelial regeneration score was also significantly decreased in II. All rats maintained well their round tracheal contour. In conclusion; I) trachea could be preserved for a long time with cryo method, 2) epithelium could regenerate fully with omentopexy in cryopreserved trachea, 3) immunosuppresion was not necessary with cryopreserved trachea.

  • 1979년 McCabe가 자가면역성 감음신경성 난청이라는 용어를 처음으로 사용한 이래. 많은 학자가 면역성질환에서 감음신경성 난청이 동반될 수 있다는 보고가 있어 왔다. 자가면역성 감음신경성 난청은 보통 양측성, 비대칭적으로 점진적인 난청이 수주 혹은 수개월에 걸쳐서 진행되는 것이 특징이다. 그간 내이의 면역학적인 측면에 관한 연구가 진행되어 왔던 바, 일반적인 치료에 듣지 않으며 면역억제요법으로 효과를 볼 수 있는 감음신경성 난청의 범주를 결정할 수 있었으며, 이는 치료가 가능한 감음신경성 난청이라는 점에서 이비인후과 영역에서 관심을 가져야 할 필요가 있다고 하겠다. 저자들은 최근 자가면역성 질환인 전신성 홍반성 낭창을 가진 27세 여자환자에서 감음신경성 난청을 관찰할 수 있었으며, 면역억제요법으로 청력의 호전을 경험하였기에 문헌적 고찰과 함께 보고하는 바이다.

  • 전염성 F낭병 즉, 감보로병은 닭의 면역기관의 하나인 F낭을 위축시켜 면역결핍 증상을 일으키는 질병이다. 면역체계에 이상이 생기므로 한가지의 질병만이 발생치 않고 여러 가지의 질병들이 한계군에서도 나타나게 된다. 이러한 질병에 대해 미국에서 발생한 몇 케이스를 살펴 보기로 하자.

The Morphologic Changes by Immunosuppression after Heterotopic Transplantation of the Murine Cryopreserved Trachea: An Animal Model for Obliterative Bronchiolitis (이소 이식된 쥐 기관의 면역억제 및 초냉동 보관에 의한 형태학적 변화: 폐색성 모세기관지염의 연구를 위한 동물 실험 모델)

  • Background: The replacement of the narrowed long-segment trachea with various prosthetic materials or tissue grafts remains a difficult and unsolved surgical problem. Homologous cryopreserved tracheal transplantation has been considered to treat the irreversibly-damaged organs, such as in the lung or heart transplantation and also to overcome the limited supply of donor organs. We examined the morphological changes and the immunosuppressive effects of the cryopreserved trachea after the heterotopic transplantation in the rats. Material and Method: Sixty tracheal segments harvested from 30 donor Wistar rats were heterotopically implanted into the peritoneal cavity of 20 recipient Wistar rats and 40 Sprague Dawley rats. The 60 recipient rats were divided into 6 groups(10 rats/ group). In groups I, II, and III, 30 tracheal segments were implanted immediately after the harvesting and in groups IV, V, and VI, the segments were implanted 28 days after the cryopreservation. Groups I and IV were Wistar syngeneic controls. Groups II and V were Sprague Dawley recipients receiving no immunosuppression and Groups III and VI, were Sprague Dawley recipients receiving immunosuppressive agents. At 28 days all rats were sacrificed and the tracheal segments were evaluated grossly and histologically. Result: Immunosuppression of the tracheal segments had a significant influence on the changes of the tracheal lumen and tracheal epithelial cells, irrespective of the cryopreservation of the trachea(p<0.001). In groups III and VI receiving immunosuppressive agents, the tracheal lumen was patent and the normal epithelial cells were observed, however in the other groups not receiving the immunosuppressive agents, there were almost luminal obliteration by the proliferation of the fibrous tissues and a loss of the epithelial cells, the findings were similar to those in the case of obliterative bronchiolitis after a lung and a heart-lung transplantation. Conclusion: With the appropriate immunosuppressive agents, the lumen and the respiratory epithelium of the transplanted tracheal segment were well preserved, even after the cryopreservation of the tracheal segment, which shows the possibility of the long-term preservation and homologous transplantation of the trachea. But fibroproliferative obliteration of the tracheal lumen and the loss of the normal respiratory epithelial cells, characteristic findings of obliterative bronchiolitis, were observed in the groups without the immunosuppression. This experiment using the rat trachea may be useful in studying the pathogenesis, treatment, and prevention of obliterative bronchiolitis after a lung and a heart-lung transplantation.

  • The authors experienced a case of a difficult decannulation in a 4 years old boy, in which. case decannulation was possible 2 years later a tracheostomy. Some immunoglogical phenomena could be considered in this case as though cancer can remiss spontaneously. Such complications of tracheostomy as subglottic edema and perichondritis could become relatively less troublesome to breathing during his aging process. And another reason for the easy decannulation is considered as the patient could be accustomed to breathing through natural air pathway by continual cannula corking training.

Effects of Dietary ${\omega}$-3 and ${\omega}$-6 Polyunsaturated Fatty Acids on Fatty Acid Composition of Immune Organs in Young Chicks (${\omega}$-3 및 ${\omega}$-6계 지방산 첨가 사료의 급여가 어린 병아리에서 면역기관 내 지방산 조성에 미치는 영향)

  • Effects of various combinations of corn oil (CO) and perilla oil (PO) as respective dietary sources of ${\omega}$-6 and ${\omega}$-3 polyunsaturated fatty acids on fatty acid profiles of immune organs were studied in young chicks. Seventy-five 1-day-old male (ISA Brown) chicks were assigned to five treatments with three replications. Semi-purified-type diets containing glucose and soybean meal as major ingredients were added with 8% CO, 6% CO+2% PO, 4% CO+4% PO, 2% CO+6% PO and 8% PO and fed for 7 weeks. There were no significant differences in body weight gain, feed intake and relative weights of liver and immune organs (g/100g weight) among dietary groups. Dietary fatty acid patterns were generally reflected in the fatty acid compositions of all immune organs such as spleen, thymus and bursa of Fabricius. The levels of a-linolenic acid(LNA), eicosapentaenoic acid (EPA) and docosahexaenoic acid in various immune organs increased with increasing levels of perilla oil in the diets, whilet the levels of linoleic acid (LA) and arachidonic acid (AA) decreased. Thymus appeared to have capacity to retain remarkably higher (P<0.05) levels of LA and LNA up to 37 and 22%, respectively, compared to the other organs. Thymic tissue contained ${\omega}$-3 fatty acid and ${\omega}$-6 fatty acid 10~36 times and 3~5 times higher than the other organs, respectively. Spleen tissue was specifically higher (P<0.05) in the levels of AA and EPA and the ratios of AA/LA and EPA/LNA, compared to the other organs, suggesting that the tissue might have high desaturase activity to convert LA or LNA to AA or EPA, respectively. BSA antibody production tended to increase by 18 ~ 32% with higher levels of perilla oil in diet, although the increase was not statistically significant. In conclusion, fatty acid compositions of immune organs very depending on the lipid composition of the diets and each organ appears to respond differently for its fatty acid profile to dietary lipids. Considering AA and EPA are precursors of many important eicosanoids, further studies are required to clarify the responses of the immune organs to the dietary fatty acids.

Development of Secretory Element in Murine Trachea and Larynx (백서 기관 및 후두에서 분비조직의 발생)

  • Mucosal surface of the trachea and larynx is protected by several complex defense mechanism, namely mucociliary clearance, immunoglobulin, antibacterial secretory enzyme, which have also been demonstrated in the middle ear mucosa and eustachian tube. The morphologgy of secretory glands and cells of the trachea and larynx is well-known, but knowledge concerning their development related to secretory activity is still sketchy. The secretory element of the murine trachea and larynx, aging from gestational day 16 to postnatal day 21, was studied using hematoxylin & eosin and alcian blue/periodic acid-Schiff staining including lysozyme immunohistochemistry to investigate the development of secretory element of the murine trachea and larynx and to provide with basis of the future study for developmental morphology of the trachea and larynx. The results of this study suggest that the secretory activity starts to be established immediately after birth with the aeration of the lung.
