Mock Circulatory Robot with Artificial Aorta for Reproduction of Blood Pressure Waveform (혈압 파형 재현을 위한 인공 대동맥 기반 모의 순환계 로봇)

  • As the importance of cardiovascular health is highlighted, research on its correlation with blood pressure, the most important indicator, is being actively conducted. Therefore, extensive clinical data is essential, but the measurement of the central arterial blood pressure waveform must be performed invasively within the artery, so the quantity and quality are limited. This study suggested a mock circulatory robot and artificial aorta to reproduce the blood pressure waveform generated by the overlap of forward and reflected waves. The artificial aorta was fabricated with biomimetic silicone to mimic the physiological structure and vascular stiffness of the human. A pressurizing chamber was implemented to prevent distortion of the blood pressure waveform due to the strain-softening of biomimetic silicone. The reproduced central arterial blood pressure waveforms have similar magnitude, shape, and propagation characteristics to humans. In addition, changes in blood pressure waveform due to aging were also reproduced by replacing an artificial aorta with various stiffness. It can be expanded to construct a biosignal database and health sensor testing platform, a core technology for cardiovascular health-related research.

An R-wave Detection method in ECG Signal Using Refractory Period (ECG 신호에서 불응기를 이용한 R-파 검출 방법)

  • The accurate detection of R-wave is important for other feature extraction in ECG, and R-wave has a lot of medical information about heart. Numerous R-wave detection algorithms have been studied on the ECG signal shape analysis, but it was difficult to find accurate R-wave when the shape of R-wave is similar to the shape of P-wave. This paper presents an R-wave detection method based on the refractory period that is the period of depolarization and repolarization of the cell membrane after excitation. And we also use the shape of kurtosis in the refractory period. The proposed method is validated using the ECG records of the MIT-BIH arrhythmia database. Experimental results show that the proposed method significantly outperforms other method in case of 105 and 108 record that have R-wave similar to P-wave, as well as other records.

Responses of Health Physical Fitness and Arterial Stiffness through Cigarette Smoking (흡연습관이 성인 남성의 건강관련체력 및 동맥경직도에 미치는 영향)

  • This study was to compare arterial stiffness and hemodynamic responses between male smokers and non-smokers. This study also investigates the influences of smoking before exercise on arterial stiffness and hemodynamic responses. 24 male subjects of age 20-29 without history of cardiorespiratory disease were divided into smokers and non-smokers. Smokers had more than 5 years of smoking experience. In order to evaluate the effects of pre-exercise smoking, smokers were tested twice, once with a cigarette before the exercise and the other once without one. Data was collected from bio-impedance analysis, SphygmoCor XCEL, graded exercise test, and fitness test. Main results of this study are as follows: First, there are differences between smokers and non-smokers in cardiorespiratory and hemodynamic response functions, as shown by maximal oxygen consumption, exercise duration, and heart rate. Second, the although the arterial stiffness between smokers and non-smokers showed statistically significant differences in the speed of the pulse wave velocity and augmentation index, smoker had a faster rate. It shows that smoking behavior has a negative impact on the cardiovascular system. Third, pre-exercise smoking behavior does have an impact on cardiorespiratory and hemodynamic response functions, as shown by exercise duration and heart rate. Lastly, arterial stiffness between smokers and non-smokers showed statistically not significant in the speed of the pulse wave velocity and augmentation index. However, the difference was not statistically significant. Brachial systolic pressure, a component of pulse wave analysis, on the other hand, was significantly dependent on pre-exercise smoking behavior. Subjects who participated in this study are college students in early 20s. Given their relatively short history of smoking, it is possible that their smoking habits are not severe enough to develop into cardiorespiratory or cardiovascular diseases. But Smokers showed lower levels of cardiopulmonary functions, as maximal oxygen consumption and exercise duration than nonsmokers.

Study on Reliability of Interpretation and Reproducibility of a Pulse Analyser (맥진기 판독의 신뢰도 및 파형의 재현성 연구)

  • Objectives : This study was performed to evaluate inter-rater and intra-rater reliability of interpretation and reproducibility of a pulse analyser (MAXMAC27-Plus). Methods : 38 of 40 volunteers completed the pulse analysis consecutively. Three Korean medical doctors who had at least 2 years of clinical experience interpreted the pulse waves for 3 aspects of size, depth and shape, then inter-rater reliability and crude agreement was obtained. Reinterpretation was done 2 weeks later and intra-rater reliability and crude agreement was obtained. Intra-rater reliability and crude agreement between 1st and 2nd measurement was calculated. Cohen's weighted kappa for size, Cohen's kappa for depth and shape were used as statistical analysis. Results : Inter-rater reliability of size, depth and shape among 3 raters was 0.598, 0.604, and 0.312, respectively, showing moderate to substantial agreement. Average intra-rater reliability between 1st and 2nd interpretation of size, depth and shape was 0.806, 0.705, and 0.638, respectively, showing substantial to almost perfect agreement. However, intra-rater reliability between consecutive measurements of size, depth and shape was 0.221, 0.121, and 0.194, respectively, which showed only poor to fair agreement. Conclusions : Intra-rater and inter-rater reliability of one pulse wave showed relatively high concordance. Training by a clinical expert may effect better concordance among raters. Test-retest reliability showed poor agreement. Improvement of measurement technique and device performance will be needed.

Cardiovascular response to surprise stimulus (놀람 자극에 대한 심혈관 반응)

  • Basic emotions such as happiness, sadness, anger, fear, and disgust have been widely used to investigate emotion-specific autonomic nervous system activity in many studies. On the contrary, surprise emotion, Suggested also as one of the basic emotions suggested by Ekman et al. (1983), has been least investigated. The purpose of this study was to provide a description of cardiovascular responses on surprise stimulus using electrocardiograph (ECG) and photoplethysmograph (PPG). ECG and PPG were recorded from 76 undergraduate students, as they were exposed to a visuo-acoustic surprise stimulus. Heart rate (HR), standard deviation of R-R interval (SD-RR), root mean square of successive R-R interval difference (RMSSD-RR), respiratory sinus arrhythmia (RSA), finger blood volume pulse amplitude (FBVPA), and finger pulse transit time (FPTT) were calculated before and after the stimulus presentation. Results show significant increase in HR, SD-RR, and RMSSD-RR, decreased FBVPA, and shortened FPTT. Evidence suggests that surprise emotion can be characterized by vasoconstriction and accelerated heart rate, sympathetic activation, and increased heart rate variability, parasympathetic activation. These results can be useful in developing an emotion theory, or profiling surprise-specific physiological responses, as well as establishing the basis for emotion recognition system in human-computer interaction.

  • In this paper, with the reference of the pulse wave acquired by the pulse-checking device, it is measured the impedance on the key measuring points of the 12 kyungmaks of the human body's left and right by using 24-channels Yangdorak machine. Then, based on the Fuzzy theory, this study diagnosed the each meridian's strength and weakness. After that, both the strengthening and weakening stimulus of magnetic fields are applied to the dominant direction to find out how the degree of strength and weakness of the meridian changed. Ultimately, the magnetic therapy that can stimulate the magnetic field at the time of diagnosis and thereby balancing the interactive of five-system(O-hang) have been materialized. For the stimulation of magnetic fields, a stimulating device which can change the direction and time on a specific part of the key measuring points of the limbs of 24 kyungmaks have been developed and used. The therapeutic methods are as follows. First, the strength and weakness of the meridian have been determined. Second, both the extremely weak meridian of Yin(Shade) and Yang(Shine), and the extremely strong meridian of Yin and Yang were adjusted by applying appropriate ascending and descending stimuli respectively. All these adjusting processes can now be carried out automatically on a personal computer(PC).
