  • It is a relatively common clinical experience to see an unerupted maxillary central incisor. This phenomenon is apparent at the dental age of almost eight years and over. Among the possible cause for failure of eruption, ectopic development of the tooth germ is mentioned. This is not fully understood but trauma or periapical imflammation of primary predecessors is accepted. The case with no history of trauma may be impacted by the periapical imflammation of primary predecessors. For bringing into the tooth eruption and the continued normal root developement by the Hertwig's epithelial root sheath, there are early considered of surgical invention and orthodontic traction with removable appliance. We reported successful treatment for inverted maxillary central incisor with proper eruption and normal root developement by forced eruption using removable appliance. But further observation will be required to evaluate the final root developement state and amount of keratinized attachment gingiva.

  • Odontomas generally appear as small, solitary, or multiple radio-opaque lesions found on routine radiographic examinations. It is a comparatively common odontogenic tumor, and may lead to interfere with the eruption of its associated tooth. In general, odontomas occur more often in permanent dentition and are very rarely associated with primary teeth. This report deals with five rare cases of primary teeth impaction associated with odontomas, with spontaneous eruption occurring in all five cases after simple surgical removal of the odontoma. Impacted primary teeth may be associated with defects in development and eruption of their permanent successors, and thus long-term observation is necessary until the permanent successors erupt.

  • Odontogenic myxoma is a rare tumor which occurs almost exclusively in the jaws, and seems to be of odontogenic origin. Clinically this tumor tends to appear in the second and third decades of life, and most of the lesions are located in the premolar - molar region. It is characterized grossly by mucoid or gelatinous gray-whitish tissue that replaces the cancellous bone and expands the cortex. It is invasive locally and has a high recurrence rate ranging from 10% to 33%. Radiographically, the appearance of this tumor is varied, but usually appears as a unilocular or multilocualr radioluscency of varying size. We experienced a rare case of odontogenic myxoma 12 years old patient related to mandibular bilateral impacted canines in the mandible, so we report this case with literature.

  • The maxillary canine is especially important as it has the longest root, provides guidance for lateral movement of the mandible and masticatory function and is a key in esthetics due to its position. Maxillary canine has the longest time to develop and a complex route from the place of formation to the site of eruption, and so it is prone to impaction more than other teeth. The clinician should consider the various treatment options : (a) No treatment and observation, (b) surgical exposure and orthodontic traction (c) autotransplantation (d) extraction. Surgical exposure of the canine and orthodontic treatment to bring the tooth into the line of occlusion is considered the most desirable approach. This case presents the results of treatment for impacted maxillary canine by surgical exposure and orthodontic tooth movement.

  • When many factors involved in the eruption of the teeth act as negative effects, they can cause eruption disturbance. Periodic observation, space acquirement, surgical exposure, orthodontic traction, orthodontic traction accompanied with surgical exposure, and surgical repositioning are considered as the treatment options of an impacted tooth, which is a form of eruption disturbance. In the first case, a male patient, age 9, visited Yonsei University Dental Hospital (YUDH) with a chief complaint of ectopic impaction of the upper left lateral incisor. We extracted the upper left primary lateral incisor and primary canine, and 5 months later, a window opening procedure was executed. Eight months later, the upper left lateral incisor partially erupted, and 18 months after the extraction, the axis of the tooth improved and the tooth erupted spontaneously. In the second case, a male patient, age 10, visited YUDH with a chief complaint of ectopic impaction of the upper right first premolar. We extracted the upper right first primary molar. Ten months later, the upper right first premolar erupted partially, and 19 months after the extraction, the upper right first premolar erupted spontaneously. We reported two cases in which improvement of eruption path and spontaneous eruption of an ectopic impacted tooth was achieved by extracting the deciduous tooth which interfered with the proper eruption of it.

  • This case report describes the treatment of an impacted tooth in relation to pediatric mandibular fracture. A 10-rear-old girl presented with au impacted mandibular canine related with a miniplate in the left mandibular body area. An ordinary surgical traction ended in failure. Good occlusion was established by removal of the miniplate. It would be considered important to make an exact diagnosis of the cause and remove the mechanical obstruction instantly for successful orthodontic treatment of an impacted tooth.

  • Incidence of tooth impaction varies from 5.6 to 18.8% of the population. Failure of eruption of the first and second permanent molars is rare; the prevalence in the normal population is 0.01% in case of the first permanent molar, and 0.06% in case of the second permanent molar. Permanent molars are particularly important for providing sufficient occlusal support and co-ordinating facial growth. Failure of eruption of permanent molars may result in various complications such as decrease in vertical dimension, posterior open bite, extrusion of antagonistic teeth, resorption and inclination of adjacent teeth, formation of cyst and so on. Treatment options of impacted teeth are periodic observation, surgical exposure, surgical exposure with subluxation, orthodontic relocation, and surgical extraction before prosthetic treatment. Early diagnosis and treatment are important, because delayed treatment induces various problems such as decreased spontaneous eruptive force, decreased successful percentage, increased treatment period, increased various complications. Prevalence of the failure of mandibular first molars is rare but eruptive guidance before extraction of impacted teeth is necessary due to importance of permanent molars. We reported two cases of surgical exposure of impacted mandibular first molar. In these cases, we could observe different result of the impacted mandibular first molar after surgical exposure.

  • Tooth impaction is a frequently observed eruption anomaly in pediatric dental practice. Young patients with impacted or unerupted teeth have more prediction for dentigerous cyst formation. Dentigerous cyst presents radiographic features, unilocular or multilocular radioluscency. Cysts occur most frequently in the premolar region except third molar. Dentigerous cysts can grow to a considerable size, and large cysts may be associated with a painless expansion of the bone in the involved area. Extensive lesions may result in facial asymmetry, osseous destruction, root resorption of proximal teeth and displacement of associated tooth. The nature of the causative tooth influences the type of surgical treatment required for the dentigerous cyst. If the cyst is associated with a supernumerary or wisdom tooth, complete enucleation of the cyst along with extraction of tooth may be the first treatment choice. Otherwise, preservation of the associated teeth should be considered to prevent a young patient from psychological and mental trauma because of the loss of tooth. We should consider the degree of tooth displacement, osseous destruction and growth pattern of oromaxillofacial area when planning treatment. Thus a proper and logical treatment planning can help a proper growth and development of oromaxillofacial area and can save the patient from a psychological and mental trauma. This report describes 4 cases of the management of impacted premolars and molars associated with dentigerous cysts in children.

Factors that influence treatment duration for patients with palatally impacted maxillary canines (구개측으로 매복된 상악견치의 교정치료기간에 영향을 미치는 요소)

  • Objective: The aim of this study was to determine the relationship between the status of a unilateral palatally impacted maxillary canine (as seen on the panoramic radiograph) and the orthodontic treatment duration. Methods: A total of 36 subjects were chosen (8 males and 28 females, $13.7{\pm}2.5$ years). All patients had undergone orthodontic traction of the impacted canine after a closed flap surgery. The position of impacted canine on the panoramic radiograph was traced and calculated with regard to the treatment duration. Results: The canine overlap over the lateral incisor (COGr) had significant statistical difference between the short-term and the long-term treatment duration groups (divided by average treatment time of 21 months) (p<0.05). Multiple regression indicated that the variables of significance (with treatment duration as the dependent variable) were the canine vertical height from the occlusal plane $(HCV,\;r^2=0.115;\;p<0.05)$ and the mandibular anterior width $(MnDW,\;r^2=0.142;\;p<0.05)$. The treatment duration, the canine angle to the incisor midline (CA), and HCV decreased from primary dentition (under 12 yrs) to permanent dentition (15 yrs). Howerer, these increased again at the adolescent ages (over 16 yrs). Conclusion: These findings suggested that orthodontic treatment of a palatally impacted canine would show good prognosis at an age of early permanent dentition with the canine showing smaller CA and HCV.

SURGICAL REPOSITIONING OF THE DISPLACED IMPACTED MAXILLARY CENTRAL INCISOR WITH DILACERATED ROOT : CASE REPORT (비정상 맹출로와 치근 만곡을 지닌 매복 상악영구전치의 자가치아이식 치험례)

  • In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. If there are genetic or general factors to effect the eruption of tooth, the clinician treats these first and then takes the early treatment for eruption guidance. If there are physical factors to intercept eruption, the clinician put them off first. However, if there are no factors to effect eruption of tooth and enough space for eruption, the clinician can consider extraction of deciduous teeth, forced eruption and surgical reposition. In case of surgical repositioning, proper time for root development, proper socket formation, and minimal trauma are important for success. This case presents displaced impacted maxillary central incisor with dilacerated root. The development of root is Nolla's stage 7, and the tooth was treated by surgical repositioning. We can observe no root resorption and good healing pattern.
