THE EFFECTS OF ATELO-COLLAGEN SPONGE INSERTION ON THE PERIODONTAL HEALING OF SECOND MOLARS AFTER IMPACTED MANDIBULAR THIRD MOLAR EXTRACTION (매복 하악 제3대구치 발치와에 Atelo-collagen Sponge 삽입이 제2대구치 예후에 미치는 영향)

  • Extracellular matrix(ECM) is known to function as a reservoir of endogenous growth factors, can be an effective delivery system of growth factor that easily lost bioactivity in solution. Fibrillar collagens like type I collagen, are the major constituent of the ECM and structural protein of bone. Also, it can be a scaffold for osteoblast migration. The purpose of this study was to compare the effects of absorbable Atelo-collagen Sponge($Teruplug^{(R)}$) insertion in tooth extraction sites on periodontal healing of the mandibular second molar after the extraction of the impacted third molar. The study population comprised 31 cases who had been scheduled for surgical removal of impacted mandibular third molars. All patients were in good general health and were not using any medication that would influence wound healing after surgery. In 15 cases control group, none was inserted into the tooth extraction site. In 16 cases experimental groups, $Teruplug^{(R)}$ was inserted into the tooth extraction site. We evaluated tooth mobility, pocket depth, gingival margin level preoperatively and 1 week, 2 weeks, 4 weeks, and 3 months postoperatively. The change was compared with two groups using Mann-Whitney test. The results were as follows. 1. There was no significant change of tooth mobility on both groups. 2. There was tendency of decreasing of previous pocket depth causing tooth extraction on both groups. 3. On gingival margin level, there was various change according to initial swelling and loss of attachment on both groups. 4. There was tendency of decreasing of gingival margin level on both groups because of removal of inflammation and decreasing of previous pocket depth. 5. There was large change of pocket depth on buccal middle, distal, lingual distal area because of tooth extraction and bone reduction. Compared with the control group and experimental group, we observed significant difference during some periods. The results of this study suggest that absorbable atelo-collagen sponge($Teruplug^{(R)}$) is relatively favorable bone void filler with prevention of tissue collapse, food packing and enhance periodontal healing.

  • Early eruption refers to an accelerated eruption of a tooth beyond the normal eruption period. The clinical findings of an early erupted tooth with little formation of crown and/or root include severe mobility, pain on chewing, hypocalcification of the enamel, and inclination, displacement, and rotation of the tooth. The radiographic findings include underdeveloped root and insufficient bone support. Early eruption of a permanent tooth can cause several complications such as chronic trauma, pain, edema, an increased rate of premolar impaction and tooth displacement and/or rotation. Therefore, when a permanent tooth erupts earlier than its normal eruption period with accompanying symptoms, appropriate treatments should be done as soon as possible. A female patient of age 7 without any systemic disease was referred from a local dental clinic with chief complaint of severe mobilities and pain in both upper first premolars. According to the clinical and radiographic examinations, the permanent teeth erupted earlier with barely formed roots, severe mobilities, edema, and pain. This case is to report the successful accomplishment of root formations and stabilization of teeth after applying intraoral fixed appliances using bands and spurs for 14 months.

  • 구순구개열은 선천성 악안면 기형중에서 발생률이 가장 높은 질환이며 특히 동양인에게 높은 빈도로 발생된다. 그리고 순구개열에서는 파열부 조직의 선천적 상실과 파열부 봉합 수술 후 형성된 반흔에 의한 상악골의 열성장과 상악궁의 협착, 코의 형태 이상, 등과 함께 치아의 수, 크기와 형태 및 맹출 이상도 높은 빈도로 동반된다. 선천성 치아 발육 이상이 구순열 또는 구개열 가진 환자에게서 종종 보고되고 있다. 이런 치아 이상은 과잉치, 결손치, 크기, 형태, 맹출시기, 법랑질 광화 등의 많은 특징을 포함한다. 이번 연구의 목적은 다음과 같다. 1. 구순열 또는 구개열을 가진 환자의 선천성 치아 결손 발병률을 결정하여 정상인과 비교하는 것이다. 2. 상하악에서 파열이 있는 부위와 없는 부위의 제2소구치 결손가능성을 비교 하는 것이다. 구강악안면외과에서 구순구개열로 진단한 환자 중 2005년 1월~2009년 8월까지 전북대학교 구강악안면외과에서 치조열부위 자가골이식수술을 받은 32명의 환자로 초진시의 교정 chart, 구강악안면외과 chart, x-ray(파노라마, 치근단사진, 교합사진(상악), 석고 모델, 구강내 외 임상사진을 사용하여 순구개열군 별로 매복치, 과잉치, 결손치, 왜소치의 유무와 위치를 조사하여 다음과 같은 결론을 얻었다. 1. 결손치는 비교적 높은 발생빈도(53.1%)를 보였으며, 치아별 발생빈도는 상악 측절치와 상악 제2소구치, 하악 제2소구치 순이었다. 구순구개열군이 구순치조열군에 비해 발생률이 높게 나타났다. 그리고 구순구개열군 내에서 양측성이 편측성에 비해 결손치의 발생률이 높게 나타났다. 2. 왜소치는 71.6%에서 발견되었으며, 치아별 발생빈도는 상악측절치에서 가장 많았다. 구순치조열군이 구순구개열군에 비해 발생률이 높게 나타났다. 3. 치조열을 가진 환자에 있어 상/하악간 제2소구치의 선천적 결손에 유의한 차이가 있었다. 4. 구순구개열이 인접한 상악측절치의 발생중인 미성숙 조직에 영향을 미치며 파열부위와 좀 더 떨어져 있는 상악 제2소구치에도 일정한 영향을 미침을 이번 연구를 통해 다시 확인할 수 있었다.

A CLINICAL AND RADIOGRAPHIC STUDY OF DENTIGEROUS CYSTS (함치성 낭의 임상 방사선학적 연구)

  • The purpose of this study was to obtain information on the clinical and radiographic features of the dentigerous cysts in the jaws. For this study, the authors examined and analysed the clinical records and radiographs of 233 patients who had lesions of dentigerous cyst diagnosed by clinical and radiographic or histopathological examinations. And the obtained results were as follows: 1. Dentigerous cysts occurred the most frequently in the 2nd decade(38.2%) and occurred more frequently in males(67.4%) than in females(32.6%). 2. The most common clinical symptom was swelling of the jaw(33.9%), and the lesions were treated by the method of surgical removal. 3. The type of lesions was mainly observed as central type(72.5%), and size of the lesion was most frequently observed 2 - 2.9cm in the widest length. 4. The lesions were most frequently observed well-defined outline with hyperostotic border(49.8%), and smooth margin(73.4%), and homogeneous lesional radiolucency(79.4%). 5. Cortical thinning and expansion of the lesions(82.0%) were observed, and their direction were most frequently observed toward buccal side(64.0%). 6. The effect on the causative tooth were observed as tooth displacement(41.2%) and delayed root development(l9.3%), and the distance between cemento-enamel junction and lesional wall attachment of the causative tooth was mainly observed as below 2mm(79.6%). 7. The effect on the adjacent tooth were observed as loss of lamina dura(66.8%), root resorption(33.9%), and tooth displacement(31.5%). 8. The effects on the adjacent anatomic structures were observed as displacement of the mandibular canal(46.5%) and maxillary sinus or nasal cavity(72.2%).

Diagnostic imaging analysis of the impacted mesiodens (매복 정중치의 진단영상분석)

  • Purpose : The research was performed to predict the three dimensional relationship between the impacted mesiodens and the maxillary central incisors and the proximity with the anatomic structures by comparing their panoramic images with the CT images. Materials and Methods : Among the patients visiting Seoul National University Dental Hospital from April 2003 to July 2007, those with mesiodens were selected (154 mesiodens of 120 patients). The numbers, shapes, orientation and positional relationship of mesiodens with maxillary central incisors were investigated in the panoramic images. The proximity with the anatomical structures and complications were investigated in the CT images as well. Results : The sex ratio (M : F) was 2.28 : 1 and the mean number of mesiodens per one patient was 1.28. Conical shape was 84.4% and inverted orientation was 51.9%. There were more cases of anatomical structures encroachment, especially on the nasal floor and nasopalatine duct, when the mesiodens was not superimposed with the central incisor. There were, however, many cases of the nasopalatine duct encroachment when the mesiodens was superimpoised with the apical 1/3 of central incisor (52.6%). Delayed eruption (55.6%), crown rotation (66.7%) and crown resorption (100%) were observed when the mesiodens was superimposed with the crown of the central incisor. Conclusion : It is possible to predict three dimensional relationship between the impacted mesiodens and the maxillary central incisors in the panoramic images, but more details should be confirmed by the CT images when necessary.

  • Supernumerary teeth are teeth which result from the continued budding of the enamel organ of the preceding tooth or from excessive proliferation of cells. They are most often found in the maxillary anterior region. And they can be responsible for a variey of irregularities in the primary and transitional dentition. There are two morphological types of supernumerary teeth, supplemental and rudimentary. Supplemental teeth have normal shape and size. In contrast, rudimental teeth have abnormal shape and smaller size. Supplemental supernumerary teeth are most common in permanent lateral incisor area. Its extraction must be decided more carefully with differential diagnosis between normal teeth, because it has normal shape and size. We reports 3 cases of the normal incisor shaped teeth in the maxillary anterior region. In all cases, we used the 3D Dental-CT as well as the conventional plain film such as periapical, occlusal, and panoramic radiograph. Consequently, 3B Dental-CT was valualble to figure out the exact position and morphology of supernumerary teeth, to do more conservative surgery and to reduce surgery stress and time.

  • Most supernumerary primary teeth erupt in the primary lateral incisor region and are reported to occur in 0.03 to 1.9% in the primary dentition. Most parents fail to recognize them because of their normal shape, eruption and exfoliation. Several theories have been proposed for the etiology of hyperdontia and the dental lamina hyperactivity theory is most widely accepted. When a supernumerary primary tooth develops, it is reported that there also exists a supplemental or rudimentary form of a succedaneous supernumerary tooth. Supernumerary primary teeth do not need specific treatment as 75% of them erupt and exfoliate normally Only periodic radiographic exams are necessary to find out whether there is any problem with the eruption of the succedaneous tooth. In the following two cases, a supplemental supernumerary primary tooth with a loss of the anterior developmental space was observed at the maxillary lateral incisor region. Radiographic examination revealed an impacted succedaneous supernumerary tooth at the palatal side of the supernumerary primary tooth.

ACCURACY OF CONE-BEAM COMPUTED TOMOGRAPHY IN PREDICTING THE DIAMETER OF UNERUPTED TEETH (Cone-beam computed tomography를 이용한 미맹출 영구치의 계측)

  • The purpose of this study was to evaluate the accuracy and reproducibility of measuring the size of unerupted permanent tooth via cone beam computed tomography(CBCT). Ten children were scanned with dental CBCT, and 3-dimensional reconstruction of the dentitions were generated CBCT. Mesio-distal dimension and buccolingual dimension of the teeth were made directly on the model with a high-precision digitalcaliper and on the CBCT by using three-dimensional dental imaging software. Reliability and accuracy were assessed by using intraclass correlation and paired $t$-tests. ($p$ <0.05) The results were as follows : 1. Intraclass correlations were above 0.9 for Both the CBCT and the model measurements, showinghigh reliability. 2. Although there were high correlation values(r=0.91) between CBCT and model messurement methods, comparisons between the CBCT and model messurement methods showed a statistically significant difference($p$ <0.05). 3. The CBCT measurements tended to slightly underestimate by 0.2 mm. But, the systematic difference of CBCT measurements were clinically acceptable Therefore, CBCT measurement method can be used to measure the size of unerupted teeth in a sufficiently accurate way.

THE EFFICACY OF ANTIBIOTIC ADMINISTRATION IN THE IMPACTED MANDIBULAR THIRD MOLAR EXTRACTION (매복된 하악 제 3 대구치 발치시 항생제 투여의 효용성)

  • To evaluate the efficacy of antibiotic administration in the impacted mandibular third molar extraction the author investigated 107 patients requiring extraction of the lower third molar who visited at the Department of oral and maxillofacial Surgery, Chosun Dental Hospital and were healthy without medical problems and had no signs and symptoms of infection around the lower third molar. The patients were divided into 4 groups according to the method of antibiotics administration: 13 patients without antibiotics administration(group 1), 30 patients with only intravenous injection of $Cefazoline^{(R)}$ 2.0g 30 minutes before the procedure(group 2), 39 patients with intravenous injection of $Cefazoline^{(R)}$ 2.0g 30 minutes before the procedure and oral administration of follow-up dosages of $Augmentin^{(R)}$ for 1 day(group 3), and 25 patients with intravenous injection of $Cefazoline^{(R)}$ 2.0g 30 minutes before the procedure and oral administration of follow-up dosages of $Augmentin^{(R)}$ for 3 day(group 4). Infection rates after extraction were calculated and compared according to gender, the age of the patients, the level of impaction, and also compared between four groups. The results were as follows. 1. The overall infection rate was 8.4%. 2. The infection rate was higher in male(11.9%) than in female(4.2%), but there were statistically no significant differences between them. 3. Infection rate was lower in patients under age 30(7.2%) than in patients over age 30(12.5%), but there were statistically no significant between them. 4. Infection rate was higher in patients with complete bony impacted tooth(11.1%) than in patients with partial bony impacted tooth(6.5%), but there were statistically no significant differences between them. 5. Infection rate was 7.7% in group 1, 10.0% in group 2, 10.3% in group 3, 4.0% in group 4 and there were statistically no significant differences between groups. In summary, it is more important to extract the impacted lower third molar under aseptic condition and to minimize the injury of tissue if possible than to administer the preventive antibiotics in reducing the infection rate in healthy patients without medical problems who had no signs and symptoms of infection around the lower third molar.

  • The primary cause of missing teeth vary depending on the region. The loss of posterior teeth is mainly due to dental caries, whereas that of the anterior teeth occur primarily due to trauma at the age of standing and walking and secondarily from the rampant dental caries. Particularly, reduction of the arch length in the cases of premature loss of primary teeth may compromise the eruption of succedaneous permanent teeth. This may result in crowding and impaction of the permanent teeth, and asymmetry of arch, thus a careful consideration for space maintenance should be made in such cases. Space maintainer is required in the case of premature loss of primary posterior teeth, because space loss result from the approximate and centrifugal movement of the neighboring teeth. Generally, in the case of primary incisor, space loss occurs when 1) tooth contact is relevant, 2) crowding in primary dentition is present, and 3) a primary incisor is lost before the eruption of primary canine. Contrarily, in the case of primary dentition with interdental space, space loss will not be observed, mostly when a primary incisor is lost after the eruption of primary canine. Thus, using a space maintainer in cases of premature loss of primary incisor has been introduced primarily not for the purpose of space maintaining but for an aesthetic purpose, prevention of parafunctional oral habits such as tongue thrust, and of pronunciation. Additionally, few case studies have been reported of space loss in cases of premature loss of primary incisor. This study is to report cases of the space loss following the premature loss of primary incisors observed in children.
