The Effect of Knowledge about Foods on the Foods Purchasing (식품에 대한 지식이 식품선택에 미치는 영향에 관한 조사연구)

  • Family meals are very important for physical and mental health of family mebers. The purpose of this study is to contribute to change the habitual and unconscious cooking methods of housewives into more scientific moth(Ids. In order to achieve this purpose, this study tried to find out the degree to which housewives applied their knowledge of nutition and foods to actual behavior in purchasing. A special form of questionaire was prepared and distributed to 502 housewives in Seoul from Feb.8th to 22nd in 1989. The results were as follows: 1. Mean (score) of their nutritional knowledge was 14 7; if seems to be comparatively higher. 2. When they purchased food materials, their husbands' favor was the first consideration. Particularly, freshness was the first considered in purchasing meat, fishes, fruits, and vegetables. And Nutrition was so in case of seaweeds, oil, and fat. 3. For the most part, they cook three or four Subsidiary dishes for a meal. If they cooked one or two they chose to cook vegetables. If three or four they added meat and fishes. If more than four, they used various food materials.

  • 콜레스테롤은 성호르몬, 스테로이드 호르몬, 비타민 D, 담즙산 및 다른 체내화합물에 존재하는 생명에 필수적인 물질이며, 특히 세포막에는 이의 안정적인 기능수행을 위해 일정량의 콜레스테롤이 반드시 존재해야 한다. 이러한 중요성 때문에 우리의 몸안에는 완전히 음식물의 섭취에 의한 공급에 의존하지 않고서도 필요에 따라 체조직의 온전한 기능수행을 위해 즉시 이용될 수 있는 충분한 양의 콜레스테롤이 존재한다. 따라서 체내에서 자체적으로 합성되는 콜레스테롤의 양은 음식물을 통해 흡수되는 양보다 일반적으로 훨씬 높다. 매일 우리의 체내에서는 약 1,500mg 정도의 콜레스테롤이 합성되는데, 주로 간세포에서 만들어지며, 소장세포에서도 일부 합성이 이루어진다. 식단(食單)의 구성에 따라 물론 차이는 있으나, 우리가 매일 음식물을 통해 섭취하는 콜레스테롤의 양은 평균 300~700mg 정도이다 .이중 소장을 통해 흡수될 수 있는 양은 50% 정도에 불과하여 대부분 식사를 통해 흡수되는 콜레스테롤의 양은 실제적인 의미에서 그리 큰 비중을 차지하지 않는다. 대부분의 건강한 사람은 혈액내 콜레스테롤의 수준이 항상 정상적인 범위내에서 유지될 수 있도록 기능적인 체내 Feed-back 기작을 유지하고 있다. 음식물을 통한 식이(食餌)콜레스테롤의 섭취량이 많을 경우에는 이 물질의 장관(腸管)내 흡수율이 낮아지고, 체내 콜레스테롤의 합성량 역시 저하된다. 이러한 방법으로 대부분의 건강한 사람은 음식물을 통한 많은 양의 식이 콜레스테롤 섭취에 대해 효율적으로 보상하는 체내기작을 갖게 되어 혈중 콜레스테롤 수준이 필요 이상 높아지는 것을 막게 된다. 물로 s이러한 보상작용(補償作用)은 사람에 따라 일정하지는 않다. 최근의 연구결과에 의하면, 건강한 사람들의 약 60%정도는 하루에 3개 정도의 달걀에 포함되어 있는 양 정도의 콜레스테롤을 추가적으로 보상시킬 수 있다고 하는데, 이는 달걀 이외의 음식물에서도 섭취되는 콜레스테롤 양을 감안할 때 하루 총 1,000~1,500mg에 해당하는 양이다. 뿐만 아니라 이중 일부 사람들은 일반적인 식단 하에서 6개의 달걀을 추가하여 섭취하여도 혈중 콜레스테롤 수준이 높아지지 않는다고 한다. 여러 나라에서 아직도 일률적으로 권장되는, 음식물을 통한 일당 콜레스테롤 섭취량을 최고 300mg으로 제한해야 한다는 것은 건강인에 있어서는 앞에서 언급한 바와 같은 생리적인 피이드-백 기작으로 말미암아 혈중 콜레스테롤 수준을 저하시키지 못하거나 미미한 정도에 불과하다. 혈중 콜레스테롤 수준이 정상적인 범위인 180~240 mg/dl에 해당하는 대부분의 건강인에게도 콜레스테롤이 많이 함유된 달걀이나 기타 축산물을 이용한 식품의 지속적인 섭취를 적극 피하도록 권장하는 것은 국민보건상 별로 큰 위미가 없다고 생각한다. 왜냐하면 이로 말미암아 국민 건강상 문제점이 크게 향상되었다는 연구보고는 아직 발견되지 않고 있기 때문이다. 더욱이 동물성 콜레스테롤 다량 함유식품인 달걀, 우유, 유제품 및 육류 등의 섭취를 꺼리게 되면 이들 식품들이 영양생리학상 매우 중요한 양질의 영양소를 많이 함유하고 있기 때문에 여러 중요한 필수 영양소의 공급상태를 뚜렷히 감소시키게 된다. 병적으로 혈중 콜레스테롤의 수준이 높은 사람은 관상성 심장병의 발병 내지는 심장경색에 의한 사망에 대한 통계학적 위험성이 증가된다. 고콜레스테롤 혈증(청, 중년층의 경우 260mg /dl 이상) 환자중 많은 사람들은 대부분 음식물을 통한 다량의 콜레스테롤 섭취에 의해 혈중 콜레스테롤 수준이 높아진 것이 아니고, 주로 유전적인 콜레스테롤 대사 결함에 그 원인이 있다고 한다. 이런 환자들의 경우에는 콜레스테롤 및 지방함량이 낮은 음식물만 계속적으로 섭취한다고해서 혈중 콜레스테롤의 수준을 만족할만한 정도까지 낮출 수 있는 것은 아니기 때문에 주로 의학적 약물투여에 의한 치료법을 적용한다. 식이성 고콜레스테롤 증상을 보이는 사람들에 있어서는 음식물의 섭취방법을 조절해 줄 필요가 있는데, 가장 효과적인 식이요법은 체중을 정상적인 수준으로 조절하는데 있다. 이러한 환자의 경우는 지방을 통한 열량 섭취량을 총열량의 30% 정도 수준까지 감소시키는 것이 좋으며, 팔미틴산의 함량이 높은 지방의 섭취는 가능하면 삼가는 것이 좋다. 단순 및 고도 불포화지방산의 함량이 높은 지방의 섭취는 혈중 콜레스테롤의 수준을 한 포인트 정도 낮추는데 추가적으로 기여할 수 있다. 최근의 연구에 의하면, 단순 불포화 지방산인 올레인산을 많이 함유하고 있는 올리브 기름이 특히 혈중 콜레스테롤 수준에 대한 유리한 효과를 나타낸다고 하는데, 이는 오로지 혈액내 LDL-Cholesterol을 감소시키는 작용에 기인한다. 이에 반해 고도 불포화지방산인 리놀산은 저밀도지단백질 부분 외에도 심장 보호성(心臟 保護性) HDL-부분까지도 저하시키게 된다. 따라서 특수하게 리놀산을 강화시켜 제조한 마아가린이나 이와 유사한 기름을 이용한 리놀산 강화식품의 섭취는 이러한 이유 외에도 일정한 부작용이 알려져 있어 더 이상 강하게 추천되지 않고 있다. 만일 이러한 식이조절에도 불구하고 혈중 콜레스테롤 수준이 원하는 정도까지 떨어지지 않을 경우, 물론 콜레스테롤의 섭취를 최저로 제한할 수 있는 방법을 꾸준히 시도해 봐야 하나 일반적으로 미미한 정도의 효과만을 가져온다. 유전 및 식이와 관계되는 사항 외에도 추가적으로 고려해야 할 점은, 혈중 콜레스테롤 수준은 나이, 성별, 스포츠, 흡연, 스트레스 및 음주 등과 같은 요인에도 결정적인 영향을 받는다는 사실이다. 물론 현재까지 과도하게 증가된 혈중 콜레스테롤 수준을 저하시킴으로써 전체 국민의 사망률을 현저하게 낮추고 평균수명을 연장시켰다는 증거는 발견되지 않고 있다. 왜냐하면 단독 식이 요법을 통한 해당 연구들이 현재까지 뚜렷한 긍정적인 결과를 제시하지 못하고 있기 때문이다. 의약품의 투여로 심장경색 및 이로 인한 사망을 감소시킬 수 있었다는 보고는 여러 연구결과들에서 보여주고 있으나, 그대신 다른 질병 및 암에 의한 사망률 역시 동시에 증가되었다고 한다. 이러한 이유로 말미암아 콜레스테롤 저하요법에 있어서 본래의 치료목적 이외의 위험성에도 항상 주의해야할 필요성이 존재한다는 사실을 주지해야 한다.

A Survey on the Perception and Usage Status of Dietitians in Food Service Business for Meat Products (단체급식소 영양사의 육가공품에 대한 인식 및 이용실태 조사)

  • This study attempted to investigate the perception and usage status for meat products of dietitians in the food service business in Seoul and Gangwon province. 32.8% of dietitians decided the menu reflecting the taste of students. Most of the dietitians (89.2%) checked the manufacture date of the processed meat products when they were supplied. To address the question as to whether they could distinguish the difference among ham, pressed ham, and sausage, most of them answered; 'can distinguish a little' (47.5%) or 'can't distinguish' (36.7%). The most frequently provided processed meat products were ham (33.9%), followed by sausage (21.7%). However, it is assumed that the dietitians might have mistaken the pressed ham for the classic ham. The most common frequency of the serving processed meat product in the food services was once a month (41.6%). Most desired aspect of quality improvement for the processed meat products was answered as 'hygiene/safety' (31.5%), and followed by 'higher quality' (26%). In conclusion, the improvement of the product quality and the strengthening of public information would be prerequisites for expanding the use of processed meat products in the food services and business-to-business (B2B) market in the future.

A Study of Job Satisfaction and Related Factors of the National Hospital Nurses (국립병원 간호사의 직무만족과 관련요인 연구)

  • By this time, a few of previous studies of factors related to separation from their jobs and job satisfaction only have dealt with the separation rate. the cause of separation and related factors that induce job satisfaction and incentive factors, the actualities of morale some suggestions for reduction of the separation rate. This study is attempted to determine factors that have effect on job satisfaction of national hospital nurses. and to proide information and materials for the development of the administration of nursing through the appreciation of factors influencing on job satisfaction between isolated ward nurses and general ward nurses working at national hospitals. 185 nurses of national hospitals responsed th this study, and were divided into two groups. Group 1: 57 nurses working at isolated wards for tuberculosis patients and Group 2 : 128 nurses at general wards. Relevant data were collected from August, 5, 1992 through August 20, 1992. The questionnaire consisted of 8 genalized items and 4 items concerning job satisfaction. The collected data were processed with SPSS, and the relationship among vaviables was analyzed by means of $X^2-test$, Pearson Correlation, Multiple Regression. The results of this study are as follows: 1. There is no significant difference between two groups in terms of generalized items. Age distributions show $44.3\%$ under the category of less than 34. and $55.7\%$ under the category more than 35, $19.3%$ was single and $74.6\%$ was married respectively. 2. $79.4\%$ of the nurses have the desire to have in-service education under the satisfactory physical environments such as welfare system, accommodating structures and facilities, instruments or management systems of the hospital, but under the category of unsatisfactory circumstances, $60.3\%$ have the intention of having in- service education. The concern in terms of in-service education shows statistically significant difference between two groups $(X^2=8.85,\;p<0. 05)$. This result accepts the hypothesis that good physical environments could intensify interests in service education. 3. The extent of satisfaction related to psychological environments is heightend according to good physical environments. In result, the hypothesis that the extent of satisfaction in terms of physical environments could raise satisfaction about psychological environment is accepted. 4. In the light of the extent of satisfaction about physical environments, $33.3\%$ of isolated ward nurses are satisfied with physical environments, but only $11.7\%$ of general ward nurses are satisfied. $(X^2=10.88,\; p<0.01)$. This result shows that the satisfaction degree about phusicalenvironments of isolated war nurses was higher than that of general ward nurses in spite of high physical and psychological risks due to exposure to infection. Consequently. the hypothesis was rejected that the satisfaction degree about physical environments would be lower in isolated ward nurses than in general ward nurses. 5. The fact that $87.7%$ of isolated ward nurses took interest in service education and $53.19\%$ of general ward nurses took interest in service education demonstrats that isolated ward nurse have more interest in service education than gerneral ward nurses. The result shows that the hypothesis is accepted that isolated ward nurses would have mor interests in service education than general ward nurses. 6. In the extent of satisfaction about physical environments such as morale human relationship promotion, there is no significant difference between two groups in terms of statistics. The hypothesis is rejected that satisfaction about psychologic environments would be high in isolated ward nurses than in general ward nurses. In conclusion. factors influencing on job satisfaction are considered to have effect on. another, and also interdisciplinary amelioration of factors accompanied with systematic inter cooperative investigation is necessary.

Analysis on the Characteristics of Ventilation and Cooling for Greenhouses Constructed in Reclaimed Lands (간척지 온실의 환기 및 냉방 특성 분석)

  • The purpose of this study was to provide basic data for development of environmental design technology for greenhouses constructed in reclaimed lands. The climatic conditions around seven major reclaimed land areas with a plan to install advanced horticultural complexes in Korea were analyzed. The characteristics of natural ventilation and temperature rise through the thermal environment measurement of the greenhouse in Saemangeum were analyzed. The part to be applied to the environmental design of the greenhouses in reclaimed lands were reviewed. Results of comparing the ventilation rate of the greenhouse according to the presence or absence of plants showed the greenhouse with plants had the lower ventilation rate, but the smaller rise of indoor temperature due to the evapotranspiration of plants. In the greenhouse with plants, the number of air changes was in the range of 0.3 to 0.9 volumes/min and the average was 0.7 volumes/min. The rise of indoor temperature relative to outdoor temperature was in the range of 1 to $5^{\circ}C$ and the average $2.5^{\circ}C$. The natural ventilation performance of the experimental greenhouse constructed in the reclaimed land almost satisfied the recommended ventilation rate in summer and the rise of indoor temperature relative to outdoor temperature did not deviate considerably from the cultivation environment of plants. Therefore, it was determined that the greenhouse cultivation in Saemangeum reclaimed land is possible with only natural ventilation systems without cooling facilities. As the reclaimed land is located in the seaside, the wind is stronger than the inland area, and the fog is frequent. This strong wind speed increases the ventilation rate of greenhouses, which is considered to be a factor for reducing the cooling load. In addition, since the fog duration is remarkably longer than that of inland area, the seasonal cooling load is expected to decrease, which is considered to be advantageous in terms of the operation cost of cooling facilities.

  • Cyberknife with small field size is more difficult and complex for dosimetry compared with conventional radiotherapy due to electronic disequilibrium, steep dose gradients and spectrum change of photons and electrons. The purpose of this study demonstrate the usefulness of Geant4 as verification tool of measurement dose for delivering accurate dose by comparing measurement data using the diode detector with results by Geant4 simulation. The development of Monte Carlo Model for Cyberknife was done through the two-step process. In the first step, the treatment head was simulated and Bremsstrahlung spectrum was calculated. Secondly, percent depth dose (PDD) was calculated for six cones with different size, i.e., 5 mm, 10 mm, 20 mm, 30 mm, 50 mm and 60 mm in the model of water phantom. The relative output factor was calculated about 12 fields from 5 mm to 60 mm and then it compared with measurement data by the diode detector. The beam profiles and depth profiles were calculated about different six cones and about each depth of 1.5 cm, 10 cm and 20 cm, respectively. The results about PDD were shown the error the less than 2% which means acceptable in clinical setting. For comparison of relative output factors, the difference was less than 3% in the cones lager than 7.5 mm. However, there was the difference of 6.91% in the 5 mm cone. Although beam profiles were shown the difference less than 2% in the cones larger than 20 mm, there was the error less than 3.5% in the cones smaller than 20 mm. From results, we could demonstrate the usefulness of Geant4 as dose verification tool.

Optimization for the Physical Properties of Steamed Foam Cakes Prepared with Single-stage Method by Response Surface Methodology (반응표면 분석법에 의한 단단계법 거품형 찜 케이크의 물리적 특성의 최적화)

  • In preparation of steamed foam-cakes, effects of whipping time, amount of wheat flour, and amounts of emulsifier on physical properties of the steamed foam cakes were investigated using RSM (response surface methodology). The three independent variables selected for the RSM experiment were whipping time $(X_1)$, amount of wheat flour $(X_2)$, and amounts of wheat flour $(X_2)$, and concentration of emulsifier $(X_3)$ were set for single-stage mixing, respectively. A rotatable central composite design was used for treatment arrangement. The responses from the product for loaf volume, color values and textural properties were analysed. In the analysis of variance for the foam cakes prepared by single-stage method, significant interactions were observed between independent variables (experimental factors) and physical property like loaf volume (p<0.05); textural properties like hardness, gumminess, and chewiness (p<0.05). Among independent variables, concentration of emulsifier had the most effects on physical properties while whipping time. The ordinary points in surface response showed maximal points with physical property like colorimetric b value while other properties revealed saddle points. The 3-dimensional response surface graphs of the predicted regression models displayed decreasing loaf volumes with increasing whipping times and emulsifier concentrations beyond optimum levels. The optimum conditions for best loaf volume and textural property (hardness, gummimess and chewiness) of the products selected by extracting intersectional areas of the contour maps that commonly overlapped all characteristics were; $11\~13$ min whipping time, $470\~486\;g$ amount of wheat flour, and $19\~20\;g$ emulsifier concentration, in case of single-stage method. The median values extracted from the RSM experimental results for optimum manufacturing conditions for single-stage method, i.e., 12 min whipping time, 478 g amount of wheat flour, and 20 g emulsifier concentration were empirically proven to fit the predicted levels of physical properties from the final foam cakes.

Dietitians' Customer Orientation at Contract Foodservice Management Company (위탁급식전문업체 영양사의 고객지향성 분석)

  • The purposes of this study were to measure customer orientation of dietitians engaged in contract foodservice management companies, and to examine the impacts of personal characteristics and organizational characteristics on the customer orientation of dietitians. The questionnaires developed for this study were distributed to 230 dietitians of contract foodservice management companies, including 4 major companies, in Seoul and Gyeonggi areas. A total of 187 (81.3%) questionnaires were used for analysis, and statistical analyses were conducted using SPSS (Win 11.5) for descriptive analysis, correlation analysis, t-test and ANOVA. The average customer orientation score of the dietitians was 3.77 (SD=0.40) and it seemed that most respondents evaluated themselves as customer-oriented. Further analyses revealed significant differences in customer orientation among different position (p<0.05) and payroll (p<0.01) groups.'Type of menu' also affected the customer orientation (p<0.01). Overall, it is needed to design efficient training programs that improve service skills and customer orientation of dietitians, and the understandings on variables affecting customer orientation would help it.

Percutaneous Mini-open Reduction for Mason II or III Radial Head and Neck Fracture (요골 두 및 경부 골절의 경피적 도수 정복술)

  • Purpose: We wanted to evaluate the result of percutaneous, mini-open reduction for the treatment of Mason II or III radial head and neck fractures. Materials and Methods: 13 patients (8 male, 5 female) with Mason II or III radial head and neck fractures were treated by 1 cm percutaneous mini-open reduction under fluoroscopy. The average age of our subjects was 29 years. Follow up duration was 18 months. Results: Union was noted in all cases. Mean radial neck angulation was decreased from 33.2 degrees to 7.8 degrees. The mean change in angulation between the immediate post-operative and last follow-up was 0.7 degrees. The mean range of motion at the elbow joint was at last follow up, 133.1 degrees in flexion, 7.3 degrees in extension, 80 degrees in pronation and 84.3 degrees in supination. Postoperatively, mean Mayo Elbow Performance Index, American Shoulder and Elbow Surgeons elbow score, and Disabilities of the Arm, Shoulder and Hand score were 96.2, 97 and 1.2. Temporary posterior interosseous nerve palsy (1 case) and minimal cubitus valgus deformity (1 case) were noted. Conclusion: Selected Mason II or III radial head and neck fractures can be treated satisfactorily with percutaneous mini-open reduction.

Video Assisted Thoracoscopic Sympathetic Ramus Clipping in Essential Hyperhidrosis -Cadaver Fitting Test and Clinical Application (다한증 환자에서 클립을 이용한 교감신경 교통가지 차단술 -사체 연구 및 임상적용-)

  • Background: It has been known that the most effective treatment method of hyperhidrosis is video-assisted thoracoscopic sympathetic nerve block. Postoperative compensatory hyperhidrosis and anhidrosis are major factors that decrease the postoperative satisfaction. Although sympathetic rami have been selectively blocked to decrease the complications, technical difficulties and excessive bleeding have prevented the universal application. Material and Method: Three pre-fixative cadavers were dissected before clinical application. Bilateral sympathetic chains were exposed in supine position after the whole anterior chest wall was removed. Second and third sympathetic rami were blocked using clips. After the sympathetic chains including ganglia were removed, we evaluated the extents of rami block. Twenty-five patients were subjected to the clinical application. Surgeries were performed in semi-fowlers position under general anesthesia and bilateral ventilation. 2 mm thoracoscopy and 5 mm trocar were intro-duced through third and fourth intercostal space, respectively. Second and third sympathetic rami were blocked using thoracoscopic clips. The postoperative complications, satisfaction, and compensatory hyperhidrosis rate were evaluated retrospectively. Result: Sympathetic rami were completely blocked in cadaver dissection study Hyper-hidrosis symptom was improved in all patients without operative complication. Operative time was shorter than that of traditional ramicotomy. All patients, except four, were satisfied with postoperative palmar hyperhidrosis. Com-pensatory hyperhidrosis was more severely happened in fifteen patients (60%). The remaining six patients had no complaint. Two patients had a minimal degree of gustatory hyperhidrosis. Conclusion: This operative method had shorter operative time and less complication rate, compared with traditional ramicotomy Operative success rate was similar to the traditional syrnpathicotorny; lower extent and occurrence rate of compensatory hyperhidrosis. The thoracic sympathetic rami clipping was suggested as an alternative method for treatment of palmar hyperhidrosis.
