지역사회 거주노인의 안구건조증 유병률과 관련요인 (Dry Eyes and its Associated Factors in the elderly living in the community)

  • 본 연구의 목적은 지역사회 거주 노인의 안구건조증, 수면장애, 우울의 유병률과 안구건조증 관련 요인을 파악하여 노인의 안구건조, 수면장애, 우울의 관련성을 규명하기 위함이다. 연구대상은 만 65세 이상 재가 노인으로, 일반적 특성과 우울, 수면의 질, 안구건조증 측정도구가 포함된 질문지를 이용하여 설문조사를 하였다. 149명의 자료를 분석한 결과 노인의 안구건조증 발생은 연령(χ2=4.50, p=.034), 직업유무(χ2=7.14, p=.008), 만성질환(χ2=6.59, p=.010), 약물(χ2=4.86, p=.027), 눈수술 여부(χ2=6.02, p=.014), 수면장애(χ2=7.29, p=.007)에 따라 통계적으로 유의한 차이를 보였다. 다중로지스틱회귀분석에서는 수면장애가 안구건조에 통계적으로 유의한 영향을 미치는 것으로 규명되었다(OR=2.45, 95% CI=1.17-5.13). 따라서, 노인의 안구건조증 관리 및 예방을 위해 수면장애를 유발하는 원인을 찾고 수면의 질을 증진 시킬 수 있는 간호중재 및 전략을 수립하는 것이 필요하다.

요양병원 간호사의 임종간호경험 (End-of-life Care Experiences of Long-Term Care Hospital Nurses)

  • 현재 우리나라 요양병원은 노인인구 증가와 함께 지속적으로 증가하고 있다. 요양병원에 입원하는 환자는 주로 만성질환을 가진 노인들로서 장기요양을 목적으로 하기 때문에 요양병원에서 임종하는 경우가 많아 요양병원에서의 임종간호 중요성이 점점 커지고 있다. 이러한 특징에도 불구하고 요양병원 간호사를 대상으로 임종간호에 대한 연구로는 주로 양적연구들로서 임종간호 경험에 대해 심층적으로 파악하기에 제한이 있다. 따라서 본 연구의 목적은 현상학적 방법을 통해 요양병원 간호사의 임종환자 간호경험의 의미가 무엇인지 이해하고 그 현상의 의미를 심층적으로 기술하고 이해하고자 함이다. 본 연구의 대상자는 C 시에 위치한 요양병원에서 6개월 이상 근무하고 있는 간호사 7명을 선정하였다. 연구방법은 심층 인터뷰로 2021년 12월부터 2022년 3월까지 자료를 수집하였다. 인터뷰 자료는 Giorgi의 현상학적 방법으로 분석하였다. 연구 결과 '임종간호에 대한 간호사의 태도', '연명에 대한 양가 감정', '요양병원의 임종간호에 대한 현실', '삶과 죽음에 대한 성찰'으로 나타났다. 따라서 요양병원 간호사의 임종간호 태도를 변화시키고 대처능력을 향상시킬 수 있는 임종간호 보수교육 및 중재 교육프로그램이 개발되어야 할 것으로 사료된다.

저온투석이 만성신부전 환자의 소양증에 미치는 영향 (The Effects of Cool Dialysis on Pruritus of Chronic Renal Failure Patients)

  • Purpose: The purpose of the study is to identify the effects of cool dialysis on pruritus of chronic renal failure patients. Methods: The degree of pruritus in 30 patients treated at Hemodialysis Unit in D Medical Center was examined by the itching NRS tool. The data were collected from February 1 to February 28, 2009 and analyzed by the student t-test. The 30 patients were divided into two groups, 15 of experimental group and the same number of control group. Results: The first Hypothesis, that after two hours of hemodialysis the degree of pruritus for the experimental group would be lower than that of the control group is rejected (t=0.726, p=.474). The second Hypothesis, that right after receiving hemodialysis the degree of pruritus would be lower for the experimental group than the control group, is also rejected (t=1.321, p=.199). Conclusion: The research on searching for the effective method of nursing intervention to relieve pruitus for the chronic renal failure patients who receive hemodialysis should be continued.

만성 간 질환자의 피로, 우울 및 수면장애 (A study of Fatigue, Depression and Sleep Disorders in Patients with Chronic Liver Disease)

  • The purpose of this study is to investigate the level of fatigue, depression and sleep disorders and to find the correlation between them in patients with chronic liver disease. Methods: The participants consisted of 130 patients with chronic liver disease who visited the C University Hospital in Daegu, Korea from July to August, 2011. Data were collected by a self-reporting questionnaire of the Revised Piper Fatigue Scale, Depression Inventory (BDI) and Sleep Scale. Collected data were analyzed by using PASW Statistics 18.0 program for descriptive statistics, T-test, one-way ANOVA and Correlation Analysis. Results: There were significant differences in fatigue scores (t=8.415, $p$=.004) and depression scores (t=10.08, $p$=.002) between subjects with symptoms of liver disease and those with no symptoms. There was no significant difference in sleep disorder scores. In addition, there is a significant correlation between fatigue and depression (r=.641), linking fatigue and sleep disorders (r=.578), and between depression and sleep disorders (r=.572). Conclusion: It is necessary to develop multidisciplinary intervention programs to relieve patientsuffering.

버가못 에센셜 오일 향흡입이 요추관협착증 환자의 수술 후 만성통증에 미치는 효과 (Effect of Bergamot Essential Oil-Inhalation on Chronic Pain after Surgery for Lumbar Spinal Stenosis)

  • Purpose: The purpose of this study was to examine the effects of a bergamot essential oil-inhalation on chronic pain after surgery for lumbar spinal stenosis. Methods: Fifty-two subjects were randomly assigned to the experimental (bergamot essential oil-inhalation) and control (almond oil-inhalation) group. All patients understood the purpose of this study and a written informed consent for the study was obtained. Results: Bergamot essential oil-inhalation was conducted during twenty minutes. Visual analog scale, blood pressure, heart rate, and respiratory rate were measured before and after treatment in the two groups. The visual analog scale score, heart rate, and respiratory rate in bergamot essential oil-inhalation group were significantly lower than those in almond oil-inhalation group. However, there was no significant difference in systolic and diastolic blood pressure between the two groups. Conclusion: Therefore, bergamot essential oil-inhalation is effective in alleviating pain, heart rate, and respiratory rate. These results suggest that bergamot essential oil-inhalation can be a useful method for the relief of chronic pain during the treatment after spinal surgery.

만성불면증 환자에게 적용한 단기 인지행동중재의 효과 (Effects of the Abbreviated Cognitive Behavioral Therapy for Chronic Insomniacs)

  • Purpose: This study was conducted to exam the effects of the Abbreviated Cognitive Behavioral Therapy(ACBT) on chronic insomnia. Methods: Study was one-group interrupted time series study that involved 13 adults(mean age=51.7, aged 25-77 years) with chronic primary insomnia who visited sleep disorder clinic of S Hospital from November 2004 to October 2005. The subjects received 2-session individual ACBT with 2 week-interval($1^{st}$: 1.5- 2hrs, $2^{nd}$: 20-30min). To measure the subjective insomnia severity and sleep patterns, 3 times of insomnia severity index and sleep logs were completed(before ACBT, after ACBT, and 3-month after ACBT). The main outcomes were subjective insomnia severity and sleep patterns(sleep onset latency, waking after sleep onset, and total sleep time, sleep efficiency). The data were analyzed with SPSS 10.0 version program by Friedman test, Wilcoxon signed rank test with Bonferroni correction. Results: There were statistically significant decrease in insomnia severity index, sleep onset latency, and waking after sleep onset, and increase total sleep time and sleep efficiency. Conclusion: ACBT was effective in reducing subjective insomnia severity and improving sleep patterns. Sleep improvement was better sustained over time with ACBT.

만성질환을 가진 아동과 가족의 극복력 (Rresilience in Children with Chronic Illness and in Their Family)

  • Purpose: The purpose of this study were to describe of resilience in children with chronic illness and family resilience, and to identify their correlations. Method: Data was collected from 108 children and their families, being treated by hospitals C and K in Seoul, who are diagnosed with nephrotic syndrome, IgA nephritis, diabetes, asthma at least six months ago. Descriptive, t-test, One-way ANOVA, and Pearson's Correlation were done. Result: The mean score of resilience in children was 101.31 and family resilience was 60.14. The variable which showed a statistically meaningful difference in the resilience in children according to a general characteristic was the gender of a child (t=6.209, p<.05), diagnosis (F= 6.315, p<.01), age of a mother (t=2.237, p<.05), and school grade (F=12.838, P<.01). In terms of the family resilience according to a general characteristic, the variable showing a statistically meaningful difference was the order of sibling (F=13.468, p<.01). There was significant positive resilience in children between family resilience (r=.356, p<.01). Conclusions: In this study we proposed the implement programs for the increase of the resilience in children with chronic illness. In order to give a positive effect on the resilience in children, implement for the increase of the family resilience is also proposed.

만성질환아동 부모의 부모되기 경험 (Parenting Experience of Parents with Chronically Ill Children)

  • Purpose: The purpose of the study was to describe the parenting experience of parents of children with chronic illness in Korea. Methods: A conventional contents analysis was used for the study. Twelve mothers of chronically ill children participated in the study. Qualitative data were analyzed using the Morse and Field method. Results: Four categories, 10 subcategories and 42 codes emerged from the data on the parenting experience of parents of children with chronic illness. The four categories were 'Sacrifice and full-engagement within self-mortification', 'Re-normalization of collapsed daily life', 'Paving a new way for independence' and 'Growing together of myself and the family'. Conclusion: Parents of children with chronic illness experienced not only negative aspects such as a confusion but also re-normalization and growing together. Based on the results, health professionals need to develop effective nursing interventions toward positive parenting for these parents and their children with chronic illnesses.

투석 전 만성 신부전 환자의 사구체 여과율에 따른 삶의 질 (Quality of Life in Pre-dialysis patients with Chronic Kidney Disease at Glomerular Filtration Rates)

  • Purpose: To examine the quality of life in pre-dialysis patients with chronic kidney disease. Methods: The subjects were 91 patients who visited the nephrology outpatient department of a tertiary hospital located in Seoul. Data on demographic and clinical characteristics, and quality of life (QOL) were collected between July 19 and 23, 2010. The relationship between QOL and various Stages of dependence on glomerular filtration rates (GFRs) and factors related to QOL were investigated. Collected data was analyzed with the SPSS WIN 12.0 program. Results: There was a significant difference in the quality of life of the subjects in different stages (F=18.12, p<.001). The Scheffe post hoc test confirmed that patients at higher stages had a lower level quality of life. In addition, GFRs, uremic symptoms, gender and age predicted value accounted for 38.5% of the variance on QOL (F=25.09, p<.001). Conclusion: Strategies to develop a systematic management program for improving QOL of pre-dialysis patients are urgently needed.

복합만성질환자의 의료이용 과정에 관한 근거이론적 탐색 (An Exploratory Study of Healthcare Utilization Process among Patients with Multimorbidity)

  • Purpose: This study examined: 1) what it meant for patients to have multimorbidity 2) how they manage their illness using health care facilities, and 3) what contextual backgrounds exist regarding the health care system. Methods: This qualitative research obtained individual in-depth interviews from 22 patients. Participants were selected by purposive sampling among those who saw physicians more often than the national average from the 2013 medical-claim data. The transcribed data were analyzed using Ground theory. Results: Multimorbidity patients suffered from "chain-like serial symptoms" and "pain without a breakthrough". They expressed themselves as "indeed patients among patients", "my body is a general hospital", and "an incompletely normal person". There was a demand to resolve the anxiety arising from the situation in which the body keeps getting sicker without showing any signs of recovery. In a state of reduced social support and a lack of trust in the doctor, their desire to be healed led them to receive a temporary relief through dependence on medical institutions. Conclusion: This research recognized the desire of multimorbidity patients to seek temporary relief through dependence on medical institutions. A more patient-centered directed care plan, specific to the needs of the patients with multimorbidity is required.
