Pelvic MRI Application to the Dosimetric Analysis in Brachytherapy of Uterine Cervix Carcinoma (자궁경부암의 강내조사치료에 있어서 흠수선량평가시 골반강 자기공명사진의 응용)

  • Purpose : Before we report the results of curative radiotherapy in cervix cancer patients, we review the significance and safety of our dose specification methods in the brachytherapy system to have the insight of the potential Predictive value of doses at specified points. Matersials and Methods : We analyze the 리5 cases of cervix cancer patients treated with intracavitary brachytherapy in the lateral simulation film we draw the isodose curve and observe the absorbed dose rate of point A, the reference point of bladder(SBD) and rectum(SRD). In the sagittal view of Pelvic MRI film we demarcate the tumor volume(TV) and determine whether the prescription dose curve of point A covers the tumor volume adequately by drawing the isodose curve as correctly as possible. Also we estimate the maximum Point dose of bladder(MBD) and rectum(MRD) and calculate the inclusion area where the absorbed dose rate is higher than that of point A in the bladder(HBV) and rectum(HRV), respectively. Results : Of forty-five cases, the isodose curve of point A seems to cover tumor volume optimally in only 24(53%). The optimal tumor coverage seems to be associated not with the stage of the disease but with the tumor volume. There is no statistically significant association between SBD/SRD and MBD/MRD, respectively. SRD has statistically marginally significant association with HRV, while TV has statistically significant association with HBV and HRV. Conclusion : Our current treatment calculation methods seem to have the defect in the aspects of the nonoptimal coverage of the bulky tumor and the inappropriate estimation of bladder dose. We therefore need to modify the applicator geometry to optimize the dose distribution at the position of lower tandem source. Also it appears that the position of the bladder in relation to the applicators needs to be defined individually to define 'hot spots'.

  • The pectoralis major myocutaneous flap represents a major contribution to head and neck cancer reconstruction. Its advantages are improved viability, one-stage reconstruction, and carotid protection. The oropharyngeal defect especially tonsillar area reveals valley shaped one with loss of a wide mucosal area. Using pectoralis major myocutaneous flap to this defect is sometimes difficult due to its natural figure of bulkiness. This article reviews our experience with patients undergoing 14 pectoralis major myocutaneous flap in carcinoma of the tonsillar area. Complications and their incidences were I total loss, 3 marginal loss, 2 minor seperation of suture, I wound infection and 2 hematoma. Most of the complications did not require a second procedure for reconstruction. Bulkiness of the flap and gravity force to the upper suture line were thought to be causes of the complications. Modification of the flap design with bilobular figure was useful to reduce its bulkiness at the folding area. More stable suture around hard palate was needed to overcome seperation of the suture.

The Change of Tumor Interstitial Fluid Pressure by Radiation Therapy in Patients with Metastatic Lymph Node in Head and Neck Area (방사선치료시 두경부 림파절 종양내 간질액 압력의 변화 양상)

  • Purpose : To determine if the tumor intersitial fluid pressure (TIFP) and/or its change in patients with metastatic Iymph node in head and neck area can predict radiotherapy outcome. Materials and Methods : In 26 biopsy Proven metastatic Iymph node Patients in head and neck area with accessible by direct inspection and palpation, and of sufficient thickness (>1 cm) to permit accurate needle placement, we measured TIFP at cervical Iymph node before and during radiotherapy. Tumor size was measured clinically and radiologically. Results : The mean preradiotherapy TIFP was 24.7 mmHg. Preradiotherapy TIFP had marginally significant relationship with tumor size (p=0.06). Preradiotherapy TIFP significantly decreased when tumor size decreased (p=0.009). Preradiotherapy TIFP was not different between complete response group and group with partial or less response (p=0.75). Radiotherapy outcome was not different between group with above and group with below than average TIFP (p=0.229). TIFP decreased 36mmHg in complete response group and 29.7 mmHg in group with partial or less response. Conclusion :The mean TIFP was elevated with 24.7 mmHg. Preradiotherapy TIFP had marginally significant relationship with tumor size (p=0.06). TIFP decreased 36 mmHg in complete response group and 29.7 mmHg in group with partial or less respone but there was no statistically significant relationship in two groups.

Radiotherapy of Metastatic Neck Nodes from an Unknown Primary Site (근원 불명 전이성 경부암의 방사선치료)

The 5YSRs of 21 patients of primary tumor extension to adjacent sites and the other 13 patients of tonsillar proper site were $28\%\;and\;38\%$, respectively but the difference was not significant statistically(p=0.52) There was a statistically significant difference in 5YSRs between the groups of the Patients who received radiotherapy in less than 61days vs more than 60days($60\%\;vs.\;18\%$, p=0.027). All living Patients without any tumor progression(n=11) had suffered from serious late sequelae such as xerostomia, edentia, dental caries and one patient had the osteoradionecrosis of mandible. On univariate analysis. the duration of radiotherapy and T-stage were the significant prognostic factors affecting 5YSR. On multivariate analysis, also the duration of radiotherapy was the only significant Prognostic factor(p=0.01). Conclusion : There was no survival difference between the radiotherapy alone and with neoadiuvant chemothe groups. Although it was a retrospective study, the role of conventional radiotherapy alone could be effective as the local treatment modality only for the early stage of tonsillar carcinomas. But for the purpose of more improved survivals and better quality of lives of living patients, other altered fractionation such as hyperfractionated radiotherapy with shorter treatment time and smaller fraction size rather than conventional radiotherapy might be beneficial and these prospective studies are needed.

Reconstruction of Hypopharynx and Cervical Esophagus : Choice of Flap (하인두 및 경부식도 결손의 재건 : 재건술의 선택)

  • Background and Objectives: Various flaps are using for reconstruction of hypopharyngeal and esophageal defect. However, complication and indication of each flap are not fully analyzed. Patient and Methods: Records of 52 hypopharyngeal cancer patients who had surgical treatment and 13 other head and neck cancer patients who underwent hypopharyngeal and/or esophageal reconstruction with flap were retrospectively analyzed. Eighty three percent(54 cases) of patients needed reconstruction other than primary pharyngeal closure. Five split thickness skin graft, 1 pectoralis major myocutaneous flap, 20 forearm free flap, 13 jejunal free flap, 15 gastric pull up were used. Result: Flap failure was noted in 2 cases who had subsequent gastric transposition. Wound dehiscence and fistula were most common problem of forearm free flap. Most fistulas were developed in patients with conduit type reconstruction of forearm flap while there wasn't any fistula in patient with patch type reconstruction. Stenosis of lower anastomosis was the frequent problem of jejunal transfer. Gastric pull-up has frequent com-plication of stomal stenosis. All but three patients had reached oral feeding postoperatively. Conclusion: Based on this study, forearm flap is effective in partial hypopharyngeal defect while jejunum is the choice for circumferential defect. Gastric pull-up is for combined esophageal defect.

  • Pilomatricoma is a benign skin tumor that develops from hair matrix cells. It most commonly occurs in the head and neck, followed by the upper extremities. Accuracy of preoperative diagnosis was low in previous studies and excisional biopsy was even performed frequently without imaging studies. We report a case of pilomatricoma of the shoulder that was easily diagnosed by ultrasound including color Doppler, which is a more useful imaging modality than computed tomography or magnetic resonance imaging scans not only because of its cost effectiveness but also because of the precise information obtained from mass contents.

  • Three cases of hyperparathyroidism are presented from our experience at Presbyterian Medical Center in Chonju in the hope that this will alert us all to the variegated patterns of clinical presentation. In the first case the principal symptom was muscular weakness. In the second a bone cyst (which was part of the syndrome of osteitis fibrosa cystica); and in the third case rib cage tenderness, backache, and persistent epigastric pain. All three had adenomas, but in Case 2 the adenomas were multiple. All three responded to surgical resection and remain well.

  • A 42 year old male patient complained of increasing pain and mass around the right ankle with 4 months duration. Simple Roentgenogram and CT showed ill-defined osteolysis and cortical perforation in the neck of the right talus. The pathologic findings showed high grade osteoblastic osteosarcoma. The tumor mass of the ankle increased and pulmonary metastasis was found in spite of administrating of two cycles of preoperative chemotherapy. Below-knee amputation and open wedge pulmonary resection were performed for primary lesion and multiple pulmoanry metastasis, respectively. However, the patient died with multiple pulmonary metastasis in spite of three cycles of postoperative chemotherapy at ten months after the operation.

Solitary Extramedullary Plasmacytoma of the Head and Neck

  • The details of 5 patients with exramedullary plasmacytoma of the head and neck were reviewed for the period from 1970 to 1984. All patients were presented with localized disease at the time of diagnosis. Out of 5 patients the one treated with surgery alone developed local recurrence and disseminated disease. He died at 3 years and 6 months. Rest of the four were alive with no evidence of the disease. Two patients were treated with radiation therapy alone while other two were treated with surgery and postoperative radiation therapy. Curative radiation therapy is recommanded after the diagnosis is established by biopsy examination.

An Microscopic Study of the Irradiation Effect on the Submandibular Glands of the White Rat (방사선조사가 흰쥐의 악하선에 미치는 영향에 관한 현미경적 연구)

  • This study was undertaken to observe the histopathologic changes in submandibular glands of the white rats when exposed to megavoltage fractionated dose of CLINAC 2100 C-D 6 MV X-RAY irradiation and 42 female white rats, weighing approximately 100gm, were divided into control and 2 experimental groups. At sacrifice, submandibular glands were excised and examined microscopically and electromicroscopically. The results were as follows : 1. The acinar cells of submandibular gland showed damage varied with dose, 12 Gy resulted in very mild injury while 24 Gy caused extensive injury. 2. The acinar cells of sumandibular gland showed similar ultrastructual alterations, appeared as pleomorphic nucleus, decreased numbers and pleomorpgism of secretory granules, distention of rough endoplasmic reticulum, expansion and pallor appearance of mitochondria, and hypertrophy of Golgi complex. 3. A serous cells were the most sensitive components, displaying morphological alterations of radiation damage as early as 3 hours, followed by submandibular seromucinous cells and secretory tubular cells. 4. The mucous cells, as well as the whole ductal lining cells, displayed no significant alterations. 5. No evidence of microvascular injury through whole experimental groups indicated that microvascular impairment dose not contribute to early. salivary gland injury.
