The Efficacy of Conjugated Linoleic Acid and Carprofen in Progression of Early Stage of Experimentally Induced Osteoarthritis in Dogs (개에서 Conjugated Linoleic Acid와 Carprofen이 실험적 골관절염 초기 진행에 미치는 효능)

  • This study was performed to compare early stage changes following the administrations of Conjugated Linoleic Acid (CLA), Carprofen and their combinations in the experimental canine cranial cruciate ligament rupture. Twenty five mongrel dogs were divided into five groups; Groups 1, 2, and 3 received a sectioning the cranial cruciate ligament (CCL) of the right stifle joint, and were administered CLA (250 mg/kg/day/orally), carprofen (4.4 mg/kg/day/orally), and their combinations for 4 weeks beginning 4 weeks postsurgery, respectively. Group 4 received sectioning CCL and no treatment. Group 5 was composed of unoperated normal dogs. The macroscopic observation of cartilage erosions on both the condyles and plateaus were evaluated, and the severity of the cartilage lesions and synovial inflammation was examined histologically at eight weeks after surgery. Histological examinations including hematoxylin and eosin stain, standard toluidine blue method, PAS technique and Masson trichrome technique, hematologic and radiographic evaluation were perfonned after experimental surgery. Slight yellowish discoloration of the surface was found in some of the experimental dogs. However macroscopic findings showed no significant differences among the groups. In radiographic findings, cranial displacement of the proximal tibia relative to the femoral condyles in all groups was observed but no significant differences among the groups was seen. This study showed that oral administration of CLA, carprofen and their combinations revealed no early stage change in the canine stifle joint following experimental rupture of the cranial cruciate ligament.

Comparison of Early Complications after Peritoneal Dialysis Catheter Implantation by Laparoscopic Surgery and Conventional Surgery in Children (소아에서 복막투석도관 삽입시 복강경을 이용한 방법과 기존의 수술법에 따른 초기 합병증 발생의 차이)

  • Purpose : To assess the early complication of laparoscopic peritoneal dialysis catheter implantation in children. Methods : Medical record review was carried out on 21 laparoscopic and 16 conventional peritoneal dialysis catheter implantations which were performed in 31 children under 18 years of age between 2002 and 2006. All medical records were retrospectively analyzed. The patients were followed until 2 months after catheter placement. Patient characteristics and catheterrelated complications, such as significant bleeding, leakage, obstruction, migration, insertion site infection and peritonitis during the first 60 days after implantation were recorded. Results : After conventional operation, dialysate leakage occurred in 2 of 16 cases and all cases improved after conservative management. In 1 case, significant bleeding occurred and re-operation was performed. Three cases of obstruction due to migration were reported, 2 cases underwent reoperation and 1 case improved without intervention. After laparoscopic surgery, outflow obstruction occurred in 1 out of 21 cases, which was caused by adhesion after several reinsertions of the catheter and recurrent peritonitis. No migration was noted after laparoscopic surgery. There was no significant difference in the complication rate between the two groups. Conclusion : Laparoscopic peritoneal dialysis catheter placement is feasible in children of all age groups, with at least equivalent functional results compared to conventional surgery. The additional advantage of laparoscopic catheter insertion is the option to identify and eliminate anatomical risk factors, such as intra-abdominal adhesions, and to perform partial omentectomy without additional incisions.

  • The aims of this study were analyze the amount of stress condition when the traction force was applied to the condyle head from the chin area of mandible and amount of distortion condition when intended 0.5 mm distraction distance from surface or one point of dissected midline of mandible. For this study, 3D finite element analysis were performed. The following results were obtained : 1. When traction force of 500 g was applied to the condyle head from the chin area, condylar neck area showed the greatest amount of stress and coronoid process was the least amount of stress area. For the amount of distortion condition, infra dental area showed the greatest. 2. When 0.5 mm of intended surface distortion was applied after dissection of mid-mandible area, base anterior area showed the greatest amount of stress but the least stress area was coronoid process. For the amount of distortion, infra dental, menton area showed the greatest amount. 3. One point distortion was applied after dissection of mid-mandible area, ramus posterior area showed the greatest amount of stress and menton area were the least stress condition. For the amount of distortion, menton area showed the greatest amount of distortion condition.
