과도한 치아 마모와 과개교합을 보이는 환자에서 최소한의 수직 고경 증가를 동반한 전악 구강 회복 증례 (Full-mouth rehabilitation with increasing minimum vertical dimension in the patient with severely worn dentition and deep bite)

  • 심하게 마모된 치아를 가진 환자에서 수직교합고경 거상을 동반한 전악 보철수복이 계획 될 수 있다. 하지만 치아가 심하게 마모되더라도 치조돌기가 보상 성장하여 환자의 수직 고경은 변하지 않을 수 있다. 이런 경우 수복 공간 확보를 위해서 수직고경을 변화시켜야 한다면 심미적이고 기능적인 목표를 만족하는 최소한의 수직교합고경 거상을 선택해야 한다. 본 증례에서 환자는 전반적인 치아 마모로 저작 시 어려움을 호소하였고, 마모가 심한 하악 전치부의 심미적 개선을 원하였다. 수직고경 감소의 평가 및 적절한 수직고경 결정을 위해 안모계측, 임상적 치관 길이 평가, 교합안정공극 측정, 측방 두부계측방사선사진 분석 등을 시행하였고, 체계적인 진단과 평가를 거쳐 보철 수복 공간 확보를 위해 최소한의 수직고경 거상을 동반한 전악 구강회복술을 시행하였다. 중심위에서 안정적인 교합 접촉 및 조화로운 전방, 측방 유도 부여를 통해 환자의 기능적, 심미적 문제를 해결할 수 있었다.

  • The purpose of this investigation was to know correlation of mean values between centric occlusion and centric relation by the cephalogram in Angle's Class Ⅰ and Ⅲ malocclusion subjects. 22 adults with Angle's Class Ⅰ malocclusion (17 men and 5 women, 21 to 27 years of age) and 14 adults with Angle's Class Ⅲ malocclusion (10 men and 4 women, 21 to 27 years of age) were selected from the dental students in Yonsei University. Each subject was given two lateral cephalometric radiographies and cephalometric analysis was performed. All data from these analyses was recorded and statistically processed with CYBER computer system. 1. The results were obtained as follows: There was a strong positive correlation between centric occlusion and centric relation in all subjects with Angle's Class Ⅰ and Ⅲ malocclusion. 2. In Angle's Class Ⅰ malocclusion, measurements in lower facial height revealed significant difference between centric occlusion and centric relation (P<0.05). In Angle's Class Ⅲmalocclusion, measurements in facial axis angle, mandibular plane angle, convexity of A point, lower incisor protrusion, lower facial height revealed significant difference between centric occlusion and centric relation (P<0.05). 3. When the mandible was moved from centric occlusion to centric relation, the mean distance of mandibular movement was 1.27㎜ (0.2-2.8㎜) in Angle's Class Ⅰ malocclusion, 1.70㎜ (0.55-4.15㎜) in Angle's Class Ⅲ malocclusion, and 1.44㎜ (0.2-4.15㎜) in all subjects.

계수 영상화된 측모 두부방사선 규격사진의 계측오차 (ERRORS IN DIGITIZED LATERAL CEPHALOMETRIC RADIOGRAPHY)

  • The purpose of this study is to evaluate the cephalometric reproducibility in digitized cephalometric radiography by comparing the measurement errors between the remeasured and retaken conventional and digitized lateral cephalometric radiography. The mean of the differences and error percentage of each cephalometric measurement were obtained using 96 lateral cephalometric radiography from 48 patients between the age of 11 and 13. The results were as follows; 1. The repeated measurement group of conventional radiography showed least amount of mean differences while those of the retaken measurement group of digitized image showed largest amount of mean differences in every measurement. 2. The measurements which showed statistically significant difference between the conventional radiography and digitized image were S-Go, Ar-Go, S-Ar-Go and S-sN-sNT in repeated measurement group. 3. The measurements which showed statistically significant difference between the repeated and retaken measurement groups were N-S-Ar, MP-Ll and S-sN-sPog in conventional radiography and S-N, Go-Me and N-A in digitized Image. 4. Large amount of error percentage was shown at A-N-B and N-A in retaken measurement group of conventional radiography and N-S-Ba and A-N-B in retaken measurement group of digitized image. 5. The amount of error percentage at Ar-Go and N-S- Ar in retaken measurement group of conventional radiography and S-N, N-A, S-N-B and N-S-Ba in retaken measurement group of digitized image was more than double than those from remeasured groups.

  • This study was designed to compare the craniofacial structure of orthodontic patients with that of normal adult. For that purpose, 61 male and 64 female adults with normal occlusion were selected and utilizing the cephalogram that had taken under the natural head position and computerized graphic image, the special position of each anatomical structure against true horizontal and vertical reference plane was investigated. The following results were obtained. 1. The absolute special positions of each anatomic structure were calculated. 2. The inclination of Sella-Nasion plane to true horizontal plane was $8.3^{\circ}{\pm}3.9$ in male and $9.3^{\circ}{\pm}3.2^{\circ}$ in female. 3. The inclination of Frankfort horizontal plane to true horizontal plane was $1.3^{\circ}{\pm}3.0^{\circ}$ in male and $1.7^{\circ}{\pm}3.0^{\circ}$ in female. 4. The dimensions of Nasion-sella and Nasion-Anterior nasal spine to be used as the basic units of mesh diagram were $70.7mm{\pm}3.1mm$, $61.8mm{\pm}2.7mm$ in male and $67.8mm{\pm}3.3mm$, $57.0mm{\pm}2.6mm$ in female respectively. 5. The standard templates of craniofacial structure of male and female adult normal group were constructed. 6. The mesh diagrams of craniofacial structure of male and female adult normal group were constructed.

전치부 후방이동에 따른 연조직 측모 변화의 두부방사선 계측학적 연구 (A CEPHALOMETRIC STUDY ON THE SOFT-TISSUE PROFILE CHANCES FOLLOWING THE INCISOR RETRACTION)

  • This study was carried out in order to find out the soft tissue profile changes associated with maxillary incisor retraction in Angle's class I malocclusion patients. For this study fifty two female adult patients (Maximum Retraction Group 23, Minimum Retraction Group 29) who received orthodontic treatment were chosen. Following conclusions were obtained by analysing the changes of soft tissue and hard tissue before and after treatment. 1. When considering the mean changes of soft tissue and hard tissue, UP, LIP, Ls, Li (p<0.001), Point B, Si (p<0.01), Point A, Ss (p<0.05) were significant posterior movement in Maximum Retraction Group and UIP (p<0.001), LIP, Ls, Li (p<0.01), Point B, Si (p<0.05) were significant posterior movement in Minimum Retraction Group. 2. When considering the correlations between hard tissue and soft tissue changes, greater correlations were found in Minimum Retraction Group between UIP and Ls (p<0.01), Point A and Ss, UIP and Li, Point B and Si (p<0.05) than Minimum Retraction Group. 3. Correlations (p<0.01) were found between upper incisor retraction and posterior movement of the upper and lower lip in Thin Lip-Thickness Group, whereas no significant correlations were found in Thick Lip-Thickness Group. 4. Mean changes of the soft tissue thickness subsequent to incisor retraction were increased (p<0.01) in upper lip (Ls-Ls'), whereas no changes were found in lower lip.

상악 제1대구치의 이소맹출과 골격성 부정교합의 연관성 (Association between Ectopic Eruption of the Maxillary First Permanent Molar and Skeletal Malocclusion)

  • 본 연구는 5 - 10세 아동에서 상악 제1대구치의 이소맹출과 골격적 부정교합 유형 사이의 연관성을 평가하고자 하였다. 상악 제1대구치의 이소맹출이 있는 93명의 아동을 실험군으로, 이소맹출이 없는 693명의 아동을 대조군으로 분류하였다. 제1대구치가 맹출 전이거나 제2대구치가 맹출을 완료한 경우는 연구대상에서 제외하였다. 측모 두부 방사선 사진 계측을 통해 골격적 부정교합의 유형을 조사, 분석하였다. 실험군의 경우, 골격성 3급 부정교합이 57.0% 이었고, 대조군에 비해 골격성 3급 부정교합의 비율이 유의하게 높았다. SNA, ANB, A to N-perpendicular는 실험군에서 작은 경향을 보였고, A-B plane angle과 APDI는 더 크게 나타났다. 반면, SNB와 mandibular plane angle은 두 군 사이에서 유의한 차이가 없었다. 본 연구를 통해 상악의 열성장이 상악 제1대구치의 이소맹출에 영향을 미칠 수 있음을 확인하였다.

I급.II급.III급 부정교합환아에서의 설골의 위치 (HYOID BONE POSITION IN CLASS I, II AND III MALOCCLUSIONS)

  • 설골은 주위골 조직과는 직접적인 연결이 없이 근육과 인대를 통해 혀, 하악골, 두개골, 쇄골, 견갑골, 갑상연골, 인두 등에 연결된다. 이러한 특이한 해부학적 관계 때문에 설골의 위치는 하악의 위치, 머리의 위치, 기도상태에 따라 변하게 된다. 이에 저자는 설골과 부정교합과의 관계, 기도와의 연관성에 대해 알아보고자 경북대학교 소아치과에 부정교합을 주소로 내원한 환아 64명 (남자 : 39명, 여자 : 25명, I급 부정교합 : 25명, II급 부정교합 : 15명, III급 부정교합 : 24명)의 설골의 위치를 측면 두부방사선 규격사진상에서 계측하여 다음과 같은 결론을 얻었다. 1. I, II급 부정교합환아에 비해 III급 부정교합환아에서 설골은 더 전방에 위치하였고, 설골의 경사도도 더 완만하였다. 2. I, II, III급 부정교합환아에서 설골은 하악골과 척추골사이의 전후방적 위치는 일정하였다. 3. 설골은 하부기도의 전방경계를 이루는 골격구조임을 알 수 있었다.

  • The soft tissue covering of the face plays an important role in facial esthetics, speech and other physiologic functions. Thus, it is recognised by all clinical orthodontists that success of orthodontic treatment is closely related to the changes in soft tissues of the face. The purpose of this study was to evaluate the changes of bony and soft tissues in prepost treatment of Angle's Class III malocclusion. The sample consisted of 18 males and 37 females, pretreatment age of 9 years to 11 years. For this study 11 landmarks were plotted, 14 linear length, 4 soft tissue thickness and 2 angles were measured. The obtained results were as follows; 1. In the linear measurements of bony and soft tissue changes, A, Is, Ss, Ls and Li were located more anteriorly in both sexes. However Si and B showed more remarkable anterior movement in female. 2. In the comparison of the changes of the soft tissue thickness, Ss and Li in male subjects and Ss in female subjects increased. 3. In the degree of correlation between changes in the soft tissue profile and changes in the skeletal profile, Is: Ls, Il: Li and B: Si in both sexes had significant correlations. However A:Ss had remarkable significant correlation in female. 4. There were significant correlations between change in ${\angle}A$ and change in ${\angle}B$ in all sexes. 5. There were little correlation between changes in distance difference of Is and Ii and Change in distance difference of Ls and Li in all sexes.

  • The purpose of orthodontic treatment is to produce functional occlusion and to create or maintain facial esthetic harmony. The soft-tissue covering of the face also plays an important role in facial esthetics, speech and other physiologic functions. The study of the soft-tissue profile is important for the planning of orthodontic treatment. The author studied cephalometric X-ray films on 49 patients (23boys, 26 girls) with Angle's class II division 1 malocclusion, ranged from 9 to 13 years of age. Roentgenocephalmetric X-ray films were taken pre and post orthodontic care. Tracings were made in usual manner. The obtained results were as follow. 1. There was no significant sexual difference on mean changes. 2. In the comparison of the soft-tissue thickness changes, Ls-Ls' and Si-Si' in male subjects were remarkable. 9. There were significant correlations between osseous (Ss') change and soft-tissue (Ss) chang, of maxilla in male and female subjects subsequent to orthodontic treatment. 4. The ratios between the protraction of the Ss' and that of the Ss were 1:1.5 in all sexes, the ratios between the Si' and that of the Si were 1:1.4 in male and 1:1.2 in female. 5. There were significant correlations between maxillary central incise. angulation change $({\angle}A)$ and upper lip inclination change $({\angle}B)$ in all sexes. 6. There were little correlations between change in distance difference of Is and Ii and change in distance difference of Ls and Li in all sexes in all sexes.

상악전돌에 관한 방사선 두부계측학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON MAXILLARY PROTRUSION)

  • This study was undertaken to compare the craniofacial morphology of Class II, Division 1 malocclusion with that of normal occlusion in children, and to investigate the incidence of various Class II, Division 1 craniofacial skeletal patterns. The subjects consist of thirty seven boys and fifty three girls with Class II, Division 1 malocclusion, and forty six boys and eighty one girls 10-15 years with normal occlusion. Measurements were recorded, tabulated and analyzed on the lateral cephalograms by the degree of SNA, SNB and ANB. The following characteristics of the Class II, Division 1 skeletal pattern were observed. 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 1 was very similar to that of normal occlusion. 2, Mandible of the Class II, Division 1 malocclusion was in the posterior position in relation to the cranial anatomy when compared to normal. 3. The chin point as measured by SN Pog and NS Gn showed distal positioning in relation to normal occlusion. 4. SN to mandibular plane angle was large in Class II, Division 1 malocclusion. 5. Mandibular incisor inclination was not significantly different between Class II, Division 1 malocclusion and normal occlusion, but maxillary incisors inclined and positioned labially and consequently overjet was large in Class II, Division 1 malocclusion. 6. Class II, Division 1 malocclusion was divided into four types of craniofacial skeletal pattern. The most common Class II, Division 1 pattern was found to be type C in which SN-Mand. Pl. was above mean range of normal occlusion. The next frequent pattern was found to be type A in which maxilla and mandible were within normal range of protrusion while upper incisors were severly labially inclined.
