Intraoperative neuromonitoring of thyroid surgery has gained universal validity to help in nerve identification, safe nerve dissection, and prediction of postoperative vocal cord function. In this article, standard intraoperative neuromonitoring procedure, interpretation about loss of signal, and the indications covered by health insurance will be described.

오트너 증후군은 중증 심혈관질환에 의해 대동맥궁과 폐동맥 사이에 왼쪽 되돌이후두신경이 압박되면서 마비가 발생하는 질환으로 쉰 목소리가 특징적 증상이다. 조절되지 않는 갑상선중독증은 폐동맥 고혈압을 유발할 수 있는데, 이때 확장된 폐동맥에 의하여 대동맥궁과 폐동맥 사이를 주행하는 왼쪽 되돌이후두신경이 물리적 영향을 받게 될 수 있다. 이로 인해 발생한 쉰 목소리는 갑상선중독증의 적절한 치료를 통해 완전히 회복될 수 있지만, 초기에 원인에 대한 이해가 없어 진단이 늦어질 경우 적절한 치료 시기를 놓치거나 불필요한 수술적 치료의 위험이 있어 영상의학적 판단이 중요하다. 이에 갑상선 중독증을 보인 청소년에서 발생한 오트너 증후군이 갑상선 중독증에 대한 약물 치료 후 호전된 증례를 보고하고자 한다.

The causes of vocal cord paralysis include iatrogenic injury during thyroid or cervical surgery, heart and chest surgery, and tumorous lesion such as laryngeal cancer and lung cancer. In addition to these common causes, rarely, cardiovascular disease can also cause vocal fold paralysis. A disease known as Cardiovocal syndrome, or Ortner's syndrome, causes left vocal fold paralysis when the left recurrent laryngeal nerve is compressed by the pulmonary artery and aorta, which is occurred by pulmonary hypertension from heart disease. We report for the first case in Korea the diagnosis of vocal fold paralysis caused by Ortner's syndrome.
