Comprehension of 'gewuzhizhi' as the medical research methodology (의학연구 방법론으로서 '격물치지' 이해)

  • This study examined the structure that can explain the 'gewuzhizhi' as a medical research methodology and how to acquire knowledge from this structure. First, we derive a structure that implies the meaning of 'gewuzhizhi' shown in "Liji" and "daixue" in relation to "xueji" and "shaoyi". It is Xue- gewuzhizhi -'You(游)'. Then, we examined the purpose of the travelling Mt. Hua of Wang Lu, who was a landscape painter and medical researcher and physician and the implication of 'You' in Song Dynasty's painting theory of "Tuhua Jianwenzhi", "Linquan Gaozhi". In the course of the review, I explained the following. First, 'You' in the travelling Mt. Hua of Wang Lu and 'You' in the painting theories of Song Dynasty belongs to the structure of Xue- gewuzhizhi-'You'. Secondly, the meaning of 'gewuzhizhi' in the structure of Xue-gewuzhizhi-'You' was deepened in Song Dynasty. Third, The way in which the 'You' of the Song Dynasty's painting theory is to learn the reason of things is to observe things. Fourthly, the structure of the Xue-gewuzhizhi-'You' was practiced in the painting area in the Song Dynasty, and the field of practice was extended to the study of medicine by the landscape researcher who followed the painting theory of Song Dynasty. Fifth, through the assertion of Wang Lu's 'I take Mt. Hua as my teacher[師]', Wang Lu finds that the source of the recognition lies in nature(Mt. Hua). Through the above, 'gewuzhizhi' shown in "Liji" and "dai xue" is involved in the theory of cultivation and epistemology, and the structure of xue-gewuzhizhi- 'You' involved in epistemology and the method of recognition is found in observation. Through these investigations, I understood logically 'gewuzhizhi' as a method of medical research.

  • A growing interest in health has been leading to more interest in function of food rather than its taste and nutrition. Usually we think chemical-free or oriental food to be good for health. Yaksun is a food with herbal stuff, which reflects our desire for health and longevity and China's splendid food culture. It is based on the traditional medical thought of the Orient that both medicine and food have the same origin. Yaksun is a traditional functional or nourishing food with both nutritive and medicinal elements, which therefore provides such effects as epicurean pleasure, prevention of diseases and improvement of health. It is recorded that in China there was a dietitian in the royal court from the period of Seoju(B.C. 11∼7), who was responsible for supervising and controlling the health, nutrition and disease of an emperor. Therefore, herbal food has a very long history. Currently, there are many Yaksun stores in Japan and Taiwan as well as China, which are one of popular tourist destinations. Basically Yaksun follows the theory of the Oriental medicine. Yaksun is categorized into four(cold, cool, hot and warm) according to its temperature and into five basic tastes(bitter, sweet, pungent, salty and sour). Yaksun has the functions such as preventing diseases and aging improving internal organs, and healing diseases. In China many colleges of medicine have a department of Yaksun studies through which systematic researches have been being made since a long time ago. For Korea, the discipline of Yaksun studies is still at the beginning stage. To respond to a growing interest in health and prevent chronic adult diseases, it is required to develop a functional food by establishing a systematic theory of Yaksun and making more researches into it.

  • The common concerns of leadership approaches were focused on finding leadership factors or necessity of leadership change to proper leadership styles adapt to contingent change through observing various leaders' behaviors for improving organizational effectiveness. Basically, they assumed that the human could be adapt to any conditions regardless of his inherent characters though it is perceived generally that human beings have different attitudes and behaviors toward the same fact, condition, situation, etc. In other words, inherent factors that cause individual differences have been neglected but concerned only about postnatal environments which influence human characters in previous leadership studies. A number of studies concerning inherent human characters have been worked in Asia for the past thousands years, which enable answering to the questions; why the human beings have different characters and personalities, and how to manage them in the given life conditions. In this research, four type human theory(called Sasang Theory) which has already been widely used in medical field in Korea will be introduced and applied to leadership theory to indicate inherent causal factors of the human character that are expected to influence leadership styles. In process, two different methods, one is for leadership theory and the other is for Sasang theory, have to be considered to match the different ideas. The theory is so predictable that the result implies possibilities of further future researches in the organizational fields by presupposing human behavior.

Survey of the public's knowledge and opinions;the therapeutic effects of current orally administered drugs for periodontal diseases (시판중인 경구 치주질환치료제의 효과에 대한 일반인의 인식도 조사)

  • 목적 : 이번 연구에서는 일반인의 치주질환 치료제에 대한 인식도를 설문을 통하여 알아보았고 현재 국내에서 시판중인 치주질환, 치료제를 의약품 검색 사이트를 이용하여 조사하였다. 조사한 치료제의 효과를 해당 약품의 성분과 관련된 문헌을 고찰함으로써 알아보았다. 재료 및 방법 : 일반인의 치주질환 치료제에 대한 인지도를 조사하기 위하여 열 개의 문항으로 구성된 설문을 실시하였다. 설문은 한림대학교 성심병원 산업의학과에 정기 건강검진을 위하여 내원한 사람들을 대상으로 하였다. 보건복지부에 등록된 치주질환 치료제를 조사하기 위하여 킴스 온라인 홈페이지를 이용하였다. 의약품 종합 검색 페이지에서 '추가조건 입력하기'의 '복지부 분류'란에 '231. 치과-구강용약'의 조건을 주어 검색하였다. 검색 결과로부터 경구 치주질환 치료제를 구별하여 정리하였다. 치주질환 치료제와 관련된 문헌을 조사하기 위하여 미국 국립도서관의 검색 서비스인 Pubmed 홈페이지를 이용하였다. 검색어는 각 지료제의 성분명과 치은염, 치주염, 치주질환의 조합어를 사용하였다. 결과 : 설문에 참여한 사람은 100명이었다. 이 중 85%는 치주질환이라는 말을 들어본 적이 있었으며 72%는 자신의 치주건강상태가 좋지 않다고 생각하였다. 14%는 치주질환 치료제를 복용한 경험이 있었다. 61%는 치주질환 치료제에 관한 정보를 광고로부터 획득하였다. 향후 치주질환 치료제의 선택과 관련하여 35%는 치과의사에게, 24%는 의사에게 그리고 20%는 약사에게 문의하겠다고 대답하였다. 치주질환 치료제는 70개였다. 이 중 38개는 아스코르빈산 제제, 25개는 옥수수 불검화 추출물 제제, 5개는 저용량 독시싸이클린 제제였고 아보카도-콩 불검화물과 동양의학적 제제가 각각 한 개였다. 결론 : 치주질환 치료제에 대한 일반인의 인지도는 비교적 낮았으며 정보를 획득하는 주요 경로는 광고였다. 일반인들이 치주질환 치료제를 선택하는 데 치과의사는 큰 역할을 하지 못하였다. 시판중인 치주질환 치료제는 70개였다. 치료제는 다섯 개의 군으로 분류할 수 있었으며, 아스코르빈산 제제, 옥수수 불검화 추출물 및 저용량 독시싸이클린이 대부분이었고 아보카도-콩 불검화물과 동양의학적 제제는 각각 한 개였다. 옥수수 불검화 추출물과 저용량 독시싸이클린은 임상 연구에 의해 그 효과가 입증되었으나 아스코르빈산 제제는 임상 연구가 없었으며 따라서 이상적인 제제라고 할 수 있다. 기존의 문헌에 기초하여 볼 때 옥수수 불검화 추출물과 저용량 독시싸이클린만이 전통적인 치주질환 치료법(SRP)과 병행하여 치료제로 사용될 수 있다.

Predictors of Institution Level of Smoking Cessation Rate and Job Satisfaction of Counselors in Smoking Cessation Clinics of Community Health Centers (보건소 금연클리닉의 금연 성공률과 금연상담사의 직무만족도 영향요인)

  • The purposes of this study were to identify factors of institution-level 6-month quit rates and job satisfaction of counselors in smoking cessation clinics of community health centers. The study was a mixed-method study including descriptive study and secondary data analysis and the data collection period was April 3, 2020, to October 31, 2020. Data were collected using a self-report survey with counselor representatives in 267 smoking cessation clinics in the nation. Part of the data regarding institutional and community characteristics was obtained from the national database. Most survey respondents were middle-aged 42.4±9.2 years old women (96%). The institutional level of 6-month quit rates was 35.3±0.1%, and the job satisfaction of survey respondents was 7.4±1.7/10 on average. Work competency was the only statistically significant contributor of both institution-level 6 month quit rate (aOR=3.57, 95% CI=1.65, 7.74, p=.001) and job satisfaction of counselors (aOR=4.64, 95% CI=2.01 10.74, p<.001). More studies are needed on the work competency of smoking cessation counselors and the development of interventions to improve their competency.

The Influence of Vocal Cords and Intensity by Hemodialysis in End Stage Renal Disease (말기 신부전 환자에서 혈액투석 치료가 성대 및 강도의 변화에 미치는 영향)

  • Kidney diseases rates in modern Korean society, which is entering an aging society, are growing. These diseases diagnosed at early stage can be cured with the drug or dietary treatment. If, however, at worsening stage, kidney dialysis or transplant is needed. In this context, we analyzed the relationship between kidney and voice by measuring and analyzing the changing rate of the vocal chord and intensity according to hemodialysis. For this, we collected voice samples before and after hemodialysis, and compared and analyzed them through application of vocal analytical element. Namely, based on oriental medical diagnosis theory that voice is quieted down when kidney is abnormal, we collected voice samples before and after hemodialysis, and applied trembling and intensity analysis of vocal chord.

A study on the medical thought of 'You-Yi(尤怡)' (우이(尤怡)의 의학사상(醫學思想)에 관(關)한 연구(硏究))

  • The present writer studied the medical thought of 'You-Yi(尤怡)', the medical expert of the early 'Qing(淸)' Dynasty, which can be summarized as following. 1. 'You-Yi' writed many essays and treaties about 'Shang-Han-Lun(傷寒論)' and 'Jin-Gui-Yao-Lue(金?要略)', both of which are the writings of 'Zhang-Zhong-Jing(張仲景)', the medical saint of 'Han(漢)' Dynasty, and those writings include 'Shang-Han-Guan-Zhu-Ji(傷寒貫珠集)', 'Jin-Gui-Yao-Lue-Xin-Dian(金?要略心典)', 'Yi-Xue-Dou-Shu-Ji(醫學讀書記)', 'Jin-Gui-Yi(金?翼)', and 'Jing-Xiang-Lou-Yi-An(靜香樓醫案)'. 2. A theory of 'Tai-Yang(太陽)' syndrome, including 'San-Gang-Ding-Li(三綱鼎立)', was proposed by 'Fang-You-Zhi'(方有執)' and 'Yu-Chang(喩昌)' and it came to be one of the leading theory of the 'Shang-Han-Lun', 'You-Yi' rejected their opinions in which they insisted that the 'Feng(風)' hurts the 'Wei'(衛)' and the 'Han(寒)' hurts the 'Ying(營)', and he advocated his particular idea that the 'Xie-Qi'(邪氣)' can directly hit skin, flesh, 'Ying-Wei(營衛)', the Five Organs and the Six Viscera(五臟六腑), regardless of 'Zhong-Feng(中風)' or 'Shang-Han(傷寒)', and he insisted that there is need to be anxious about Whether the 'Ying-Wei' is strong or weak. 3. In 'Shang-Han-Guan-Zhu-Ji', 'You-Yi' invented the eight classifications in treating the 'Shang-Han' and he did not follow the old theories hitherto. He divided the book into six parts via the clinical experiences, and the Classification and Logics in Treatment(辨證論治), and he put several titles on 'Jin-Gui-Yao-Lue. And he newly edited the original text of 'Shang-Han-Lun' and arranged 'Zhong-Jing(仲景)'s Six Meridian(六經) categories. Each syndrome in 'Tai-Yang' 'Yang-Ming(陽明)', 'Shao-Yang(少陽)', 'Tai-Yin'(太陰)', 'Shao-Yin(少陰)' and 'Jue-Yin(厥陰)' has its own categories in treatment. 4. In explaining the Six Meridian(六經) and the Organs and Viscera(臟腑), 'You-Yi' legislated the syndrome in 'Three Yang(三陽)' into Meridian(經) and Viscera(腑) ; the syndrome in 'Three Yin(三陰)'into Meridian(經) and Organs(腑). He also concluded that 'Shang-Han-Lun' not only discussed 'Wai-Gan(外感)' but also included the Internal Hurt(內傷) and Miscellaneous Diseases(雜病). 5. In his academic research, 'You-Yi' followed 'Zhong-Jing' in classifications and prescriptions and succeeded the theory of 'Ma-Chu(마숙)' and 'Li-Zhong-Zi(李中梓)', but was not tied to any stereotyped former practices. He put emphasis on the Spleen(脾) and the Kidney(腎) as he harmonized the Middle Energizer(中焦焦) with the taste of 'Gan(甘)' and 'Wen(溫)' and enjoyed strengthening 'Yang' by benefitting the 'Qi(氣)'. 6. He discussed in detail the causes, mechanism and symptoms on 'Tan-Yin(痰飮)' and proposed the seven categorizes in treatment. 7. He sorted the causes of 'Zhong-Feng' into two kinds of the 'Feng' via 'Wai-Gan' and Internal Hurt, and recognized these two 'Feng's correlate as mutual sources of diseases. He insisted that the origin of the 'Feng' exists in the 'Liver(肝). He also established the eight categories in treating the 'Feng'. 8. 'You-Yi' belong to the classifications and Logical Treatment School. And he, considering he respected and followed 'Zhong-Jing' and 'Li-Zhong-Zi', belongs to the successor to the 'Wen-Bu(溫補)' School. 9. His writings, especially the 'Shang-Han-Guan-Zhu-Ji' and the 'Jin-Gui-Yao-Lue-Xin-Dian', are excellent commentary works on the originals, so they served as significant guide books for many junior scholars. For penetrating the 'Shang-Han-Lun' with his unique way of classification of treatment, he has been highly appreciated by later generations including scholars like 'Tang-Li-San(唐立三)' in the 'Qing' Dynasty, and so many more ones thenceforth. In conclusion, on the base of this study, it is hopefully proposed that the still more profound research on the medical thought of 'You-Yi', as one of the most distinguished scholars, an expert clinician and an earnest writer as well.

A STUDY ON 4 TYPE CONSTITUTION AND SIFE CHARACTER OF OBESE PATIENTS (비만인의 생활특성과 사상체질에 관한 연구)

  • Disease depends on the three factors, agent, host and environment. According to history of disease, by early 1900s the case of deaths is infectious disease, in late 1900s care of infectious diseases and tremendous scale of chronic disease, i.e., heart disease, diabetes, cancers and etc, makes care of chronic diseases be a most important theme. Now, life-style of diet is being westernized and in high industry-oriented society, obesity makes attack fate remarkably increase and life-expectancy become short, so that it causes severe problem of health. Chronic disease, such as obesity, is not affected by specific agent, but depends of interaction between host and environmental factors. There is the theory of constitutional medicine in Korean Medicine. According to it, all the people have constitutional specificity and disease. Because obesity is a kind of disease, there is the corresponding constituent being apt to be fat. Oriental Medicine utilizes herb-medication, acupuncture, and massage-therapy in treating obesity. Therefore study on relationship between constituent and obesity for OPD patients of Sangji-Oriental Medicine Hospital is carried out. The results are summarized as followings. 1. 70.2% of obesity patients are Taeumin(太陰人), 26.9% of those are Soyangin(少陽人), 2.9% of thoese are Soeumin(少陰人). 2. Most cases, high value of Free Fat Acid and Triglyceride not that of Total Cholesterol and Low Density Lipoprptein is meaningful in obesity patient blood. The corelationship between lipid test and Constitution is meaningful in Triglyceride and Free Fatty Acid. 3. Obesity is not related with gene. 4. Obesity is not related with Boyak(Herb-Med : 補藥). 5. Obesity mostly happens after delivery, contraception and operation. 6. Obese Patients are apt to eat between meals, especially food of wheat flour such as a snack. 7. The aim of treating obesity is not persuit of beauty but of keeping healthy. 8. 2.2Kg of body weight is lost after 4 week-treatment. 9. Common cause of obesity is overeating of carbohydrate and lipid than meat.

  • Since new continental was discovered, potatoes and sweet potatoes originated from America have been known worldwide. Through literary consideration about potatoes and sweet potatoes, several conclusons were obtained as enumerated below. 1. Based on work done by taxonomist in Orient, both of sweet potatoes and potatoes which were classified to the families of Convovulaceae and Solansceae repectively were included in Seo-rue(薯類). In contrast, both plants' identities were differed systematically. 2. Because potatoes fulfilled with like-cold keep warm, potatoes help So-um-in(少陰人) achieve Yang-nan-ji-gi(陽煖之氣). Meanwhile, the characteristic of sweet potatoes typically in wild living style assists Tae-um-in(太陰人) to own Ho-san-ji-gi(呼散之氣). That sweet potatoes affect on bodies as much as Mar(山藥=마) does makes this pant be defined as good food. 3. The inside contents of Nam-bang-bon-cho-sang(南方本草狀) cited from sweet potatoes chapter in the book of Bon-cho-gang-mok(本草綱目) indicated that the word of sweet potatoes was not found, but called as a kind of Mar(山藥=마) which lived wildly in a mountain Unfortunately, due to that medical scientist cited contents of Nam-bang-bon-cho-sang(南方本草狀) without pursuing carefully, sweet potatoes have been misunderstood. 4. According to the book of Sasang Constitutional medicine, sweet potatoes are good for Tae-um-in(太陰人) who has constipation, and potatoes are good for So-um-in(少陰人) who suffers form stomach illnesses like ulcer and hotburn. Therefore two plants have been used for curing those illness. ln addition, the taking oriental medicine with those plants are recommenced for dietary treatment according to the Constitution.

Studies on Nutritional Analysis and Antioxidant activity of Oriental Medicines with Bloodstream Improvement (혈류개선 효능을 지닌 한약재의 영양성분 분석 및 항산화 활성에 관한 연구)

  • This study investigated applicable values and developments of 5 different kinds of oriental medicine materials as functional supplements or medicinal uses, known to improve blood circulation by means of analyzing nutritional compositions and antioxidant activities. Among general nutritional compositions, protein, iron and copper contents in Polygini Multiflori Radix (20.28%, $132.22{\mu}g$/100 g, $26.54{\mu}g$/100 g), Angelica Gigantis Radix (16.54%, $98.20{\mu}g$/100 g, $32.15{\mu}g$/100 g) and Paeoniae Radix(18.67%, $62.10{\mu}g$/100 g, $20.32{\mu}g$/100 g) were significantly higher than other materials. Angelica Gigantis Radix had the highest of total polyphenols and flavonoid contents, Polygini Multiflori Radix (middle), Paeoniae Radix (the lowest). Antioxidant activities by using DPPH radicals removal showed the highest in Polygini Multiflori Radix, Angelica Gigantis Radix (middle), Paeoniae Radix (the lowest). This study tried to use integrated approaches, including Western and Oriental medicine to improve blood circulation and to set up base to develop products originated from natural compositions.
