Real-Time, Simultaneous and Proportional Myoelectric Control for Robotic Rehabilitation Therapy of Stroke Survivors (뇌졸중 환자의 로봇 재활 치료를 위한 실시간, 동시 및 비례형 근전도 제어)

  • Objective : Conventional therapy approaches for stroke survivors have required considerable demands on therapist's effort and patient's expense. Thus, new robotics rehabilitation therapy technologies have been proposed but they have suffered from less than optimal control algorithms. This article presents a novel technical healthcare solution for the real-time, simultaneous and propositional myoelectric control for stroke survivors' upper limb robotic rehabilitation therapy. Methods : To implement an appropriate computational algorithm for controlling a portable rehabilitative robot, a linear regression model was employed, and a simple game experiment was conducted to identify its potential of clinical utilization. Results : The results suggest that the proposed device and computational algorithm can be used for stroke robot rehabilitation. Conclusion : Moreover, we believe that these techniques will be used as a prominent tool in making a device or finding new therapy approaches in robot-assisted rehabilitation for stroke survivors.

Arthroscopic Treatment for Localized Pigmented Villonodular Synovitis of the Knee (슬관절의 국소적 색소 융모 결절성 활액막염에 대한 관절경적 치료)

  • Diagnosis of localized pigmented villonodular synovitis (PVNS) is difficult because of its rarity and indistinctive symptoms. This study presented 11 cases of localized PVNS of the knees, which were diagnosed and treated by arthroscopic technique. There were 6 males and 5 females between the age of 15 and 59 years (mean 34.6 years). The interval from the onset of symptoms to treatment ranged from 2 months to 3 years (average 29.9 months). All patients complained knee pain and 7 patients complained palpable mass. Four of the 11 patients had trauma history. The most common involved site was anteromedial synovium near the anterior horn of medial meniscus (5 patients). The remaining cases were identified on anterior fat pad (2 cases), suprapatellar pouch, posteromedial compartment, medial gutter and the anterior horn of the lateral meniscus, respectively. Nine cases had 1 mass and the remaining cases each had 2 or 3 masses. There was no evidence of recurrence during the follow-up period (average 29.9 months). Arthroseopy is effective in the diagnosis of localized PVNS with minimal morbidity and in the definitive treatment for PVNS.

Eradication Effect of Penicillin Administration on the Asymptomatic Infections of Group A Streptococci (A군 연쇄구균 무증상 감염자에서 페니실린 투여 후 세균제거 효과에 대한 조사)

  • Purpose : Asymptomatic infections with positive throat culture for group A beta-hemolytic streptococci(GABHS) and high antistreptolysin O(ASO) concentration may lead to sequelae such as rheumatic fever or acute poststreptococcal glomerulonephritis. Children with asymptomatic infections were treated with oral penicillin V to evaluate the effectiveness of penicillin treatment on the asymptomatic infections. Methods : Throat culture and ASO concentration analysis were performed against healthy elementary school children. Thirty-six children with positive throat culture for GABHS and ASO concentrations of 400 IU/mL or more were divided into two groups. Twenty-two children were treated with oral penicillin V for 10 days, and the others were not treated. Eradication rate of GABHS and the change of ASO concentration between the two groups were compared after one month later. Results : Eradication rates of GABHS between treated and untreated children were 91%(20/22) and 50%(7/14) respectively(P<0.05). Children showing elevation of ASO levels more than 100IU/mL were 22%(4/18) in the treated group and 30%(3/10) in the untreated group, while children showing a decrease of more than 200IU/mL in the ASO level were 44%(8/18) and 40%(4/10) respectively. Conclusion : We confirmed the validity of penicillin treatment, because when we treated the asymptomatic children with penicillin V, the GABHS was eradicated effectively. But there was no significant difference of decrease in the ASO levels between the two groups due to long half-life of ASO or poor compliance. Treatment failure was 22% in terms of elevated ASO levels after penicillin treatment.

  • The prior purpose of this study is to introduce a optical biopsy and evaluate whether the optical biopsy, real-time, non-invasive technique, is a reliable tool to assess response to radiotherapy Four healthy volunteers, and four patients with inflammatory conditions of the oral cavity participated on the study. was obtained from each person enrolled in the study. Using FastEEM(Ercited Emission Matrix) as a optical biopsy tool, normal and tumor spectra are taken from the normal and the tumor regions. And then second optical biopsy are taken from the tumor regions in 4 patients with time delay at 7days.. Using a diagnostic algorithm, made by Gillenwater based on spectra excited at 337nm The Optical Biopsy turned out to be more suited for tumor diagnostic resulting in significant difference fluorescence spectra. The fluorescence intensity of cancerous tissue showed a higher position. The second fluorescence intensity of optical biopsy of cancerous oral tissue has more smaller than the first result. I conclude that optical biopsy, which technique don't need to remove tissue sample from body, and is a real time , and non-invasive measurement is a reliable tool to access to radiotherapy because FastEEM can do measure the variation of the tissue composition chemical, biological, and morphological after radiotherapy. Based on the fluorescence spectrum are taken from the optical biopsy in normal and tumor spectra as well as tumor spectra after 7days.

A Clinical Evaluation of Tuberculous Pleurisy -Report of 52 Cases (결핵성 흉막염 환자의 임상적 고찰)

  • A clinical analysis was performed on 52 cases of tuberculous pleurisy experienced in the department of thoracic and cardiovascular surgery and department of medicine, Chosun University Hospital during a period from Jan. 1994 to Dec. 1995. Among them, male was 39 cases, female was 13 cases, with age ranged from 7 to 73 years. The common symptoms were chest pain 75%, dyspnea 59.6%, cough 55.8%. The most common diagnostic tool was pleural biopsy. The protein levels in the tuberculous pleural effusion were 0.9∼6.5 gmojo, and ratios of effusion protein to serum protein were 0.48 ∼ 1.06. The glucose levels in the tuberculous pleural effusion were 37∼ 112 mg%. The LDH levels in the tuberculous pleural effusion were 80 ∼ 2440 unitlml, and ratios of tuberculous p eural effusion LDH to serum LDH were 0.48 ∼ 1.03. The ADA levels in the tuberculous pleural effusion were 24-63 lU/L. The common surgical methods of treatment in the tuberculous pleurisy were closed thoracostomy in 18 cases(66.7%), and thoracentesis in 5 cases(18.5%). This study compares the clinical results of group A and group B. There were no significant differences for age and sex, lag period from initial symptoms to admission, diagnostic method, and protein, pH, LDH, glucose, ADA levels in tuberculous pleural effusion. Authors noted that the discharge after admission on the tuberculous pleurisy was more faster in patients with surgical treatment than in patients with only medical treatment. Hepatic Resection in Patients with Liver Metastasis from Gastric Cancer (위암의 간전이에 대한 간 절제 수술)

  • The clinical significance of hepatic resection for gastric metastases is controversial, even though hepatic resection has been widely accepted as a modality for colorectal metastases. Very few patients with gastric hepatic metastases are good candidates for hepatic resection because of multiple bilateral metastases, extrahepatic disease, or advanced cancer progression, such as peritoneal dissemination or extensive lymph node metastases. Therefore, several authors have reported the clinical significance of hepatic resection for gastric metastases in a small number of patients. Considering the present results with previous reports. The number and distribution of tumors in hepatic metastases from gastric cancer was considered based on the present and previous reports. Several authors have reported significantly better survival in patients with metachronous metastasis than in those with synchronous disease. However, metachronous hepatic resection necessitates the dissection of adhesions between the pancreas, liver, and residual stomach to prepare for Pringle's maneuver. Patients with unilobar liver metastasis, and/or metastatic tumors <4 cm in diameter may be good candidates for hepatic resection. Synchronous metastasis is not a contraindication for hepatic resection. Most of the long-term survivors underwent anatomic hepatic resection with a sufficient resection margin. After hepatic resection, the most frequent site of recurrence was the remaining liver, which was associated with a high frequency of mortality within 2 years. A reasonable strategy for improvement in survival would be to prevent recurrence by means of adjuvant chemotherapy and careful follow-up studies.

Effects of Class-based Group Art Therapy Program on Child's Emotional Regulation and School Adjustment Improvement (학급단위 집단미술치료프로그램이 아동의 정서조절과 학교생활적응력 향상에 미치는 효과)

  • This study is aimed at verifying the effect of class-based group art therapy program on child's emotional regulation and school adjustment improvement. The target of this study consisted of the lowest-leveled 3rd graders among the students attending A Elementary School, located in 00, Daejeon City, who were tested for school adjustment. The class-based group art therapy program was implemented from October 15, 2014 to November 19, 2014, once a week, for 40 minutes for each session, a total of six rounds. For this study, the scale of emotional regulation and school adjustment was used by pre-post comparison. As an analysis method for verifying the effects of this study, ANCOVA was used in consideration of pre-test, in comparison of post-test results between groups, regarding the scale factors of emotional regulation and school adjustment. As a result of analysis, first, child's emotional regulation was significantly improved after the program performance. Second, child's school adjustment. was significantly improved after the program performance. In conclusion, class-based group art therapy program let children see through the process of emotional change, and properly regulate their emotions. At the same time, the program provided situations in which peers meet together casually, giving a positive change for school adjustment improvement based on the peer relationship. In addition, this study has a meaning to help enjoying school life, by constructing a proper program to be applied at school in a variety of ways.

A Study of EEG and Melatonin in Plasma According to Exercise Type in Elderly with Sleep Disorder (수면장애 노인의 운동유형별 뇌파와 혈 중 멜라토닌 농도 비교)

  • Objective : Sleep disorder caused by stress or disease to elderly currently. We tried to make clear constant exercise according to exercise intensity would have effect on sleep disorder in elderly using EEG and melatonin. Methods : Thirty subjects were over 65 years old who lived a senior facility in some parts. They were able to communicate and understand the purpose of the study. They also expressed their intension to participate actively in experiment. They should have alert consciousness and orientation about time, place, and people. Sleep disorder was assessed via below 6 hr total sleep time and Pittsburge Sleep Quality Index, which was satisfied with both of criterion at the same time. Exercises composed of low intensity walking, moderate aerobic exercise, and high intensity resistance strength. We used QEEG 8-System (LAXTHA Inc. KOREA) to check wave type and Polysomnograpy (Compumedics, Australia) to test quality of sleep. and Histologic features were observed by TTC (triphenyltetrazolium chloride) staining and H & E (Hematoxylin & Eosin) staining. Results : There was a significant sleep index and change of melatonin after aerobic exercise. There was a significant SOL, S1, S2, and SWS in aerobic exercise, but there was a significant SWS in walking and resistance strength. There was a significant change of delta wave especially in frontal and temporal region between exercises. Conclusion : They had different effects according to type of exercise, when elderly who have chemical, behavioral change of circadian rhythms did exercise consistently. Aerobic exercise had more effect on sleep disorder than other exercise. Therefore, we may supply proper exercise to elderly and high quality of life.

A Systematic Review of Nonpharmacological Interventions on Activities of Daily Living in Dementia (치매 환자의 일상생활 수행능력 향상을 위한 비약물 중재에 관한 체계적 고찰)

  • The purpose of this study is to provide a systematic review of the nonpharmacological interventions of ADL improvement for dementia published in foreign journals for the past 10 years, and to provide general information on nonpharmacological interventions and effective intervention methods to dementia expert in clinical practice. We searched published studies in the Medline Complete, PubMed Central, and Scopus databases from January 2007 to November 2016. The main keywords used "Dementia AND (ADL OR Activities of daily living) AND Nonpharmacological interventions" and total of 9 studies were selected for analysis from 974 searched articles. The level of evidence were belonged to 3 each studies in I~III level. The mostly provided nonpharmacological interventions were exercise, the study field was variety of areas including rehabilitation(occupational therapy, physical therapy), nursing, and psychology. The Barthel Index(BI) was most frequently used assessment tools to evaluate basic activities of daily living, Cognition was most dependent variables measured with the ADL. Through this study, we provided evidence for the understanding of nonpharmacological interventions of ADL improvement for dementia. In the future, based on this study, helpful intervention needed for ADL training.

A Ukulele Playing Intervention for Improving the Hand Function of Patients With Central Nervous System Damage: A TIMP Case Study (중추신경계 손상 성인 대상 손 기능 향상을 위한 우쿨렐레 활용 치료적 악기연주(TIMP) 사례)

  • The effects of therapeutic instrumental music performance (TIMP) using a ukulele were examined in adults with central nervous system damage and impaired hand functions. The participants were three adults with neurological damage who participated in 30-min sessions twice a week over 6 weeks. Changes in hand function was measured by the Box and Block Test (BBT), the 9-Hole Peg Test (9-HPT), and the Jebsen-Taylor Hand Function Test (JTHFT). Following the intervention, all three participants showed increases in the BBT and 9-HPT scores, indicating positive changes in fine motor coordination and dexterity. In terms of the JTHFT, all three participants showed increases in the "writing" and "card flipping" subtask scores, indicating that the intervention was effective in improving more coordinated finger movements. All participants reported the satisfaction with the intervention. They also pointed out that they were motivated to play the ukulele and that following the intervention used their affected hand more frequently in daily activities. These findings suggest that TIMP with a ukulele for patients with central nervous system damage can have positive effects on their functional hand movements and motivate these patients to practice their rehabilitation exercises.
