The Effects of Blood Flow on the Formation of Cerebral Aneurysms (혈류 유동이 뇌동맥류 형성에 미치는 영향)

  • 뇌동맥류는 뇌혈관의 일부가 풍선처럼 부풀어나는 혈관계 질환이며 뇌동맥류의 파열은 사망이나 심각한 후유 장애를 야기한다. 뇌동맥류의 다양한 발생 원인 중 혈관 내부의 혈류의 유동이 중요한 인자로 의심된다. 뇌동맥류의 형성에 미치는 혈류역학적 인자를 규명하기 위해 내경동맥에서 발생한 환자의 내경 동맥류 CT 사진을 이용하여 내경동맥류 모델을 제작하고, 모델 내부의 혈류유동장을 입자영상속도계를 이용하여 측정하였다. 동맥류가 발생한 내경동맥류 모델에서는 동맥류 원위부 목(distal neck)쪽과 반대쪽 내경동맥 벽에서 전단응력이 높게 나타났다. 동맥류 발생에 미치는 혈류역학적 인자를 규명하기 위해 동맥류를 제거한 내경모델을 제작하여 맥동유동에서 내부 유동장을 측정하였다. 심실수축기 동안 휘어진 내경동맥의 바깥쪽 벽에서 혈류의 혈관벽 부딪힘이 관찰되었으며 심실이완기 초반에도 이는 계속 유지되었다. 내경 동맥 내부의 부차적 유동특성을 연구하기 위해 동맥류 발생 위치에서 혈관 축과 수직인 평면의 유동장이 측정되었다. 혈관 단면에서는 휘어진 혈관의 바깥쪽에서 안쪽으로 시계방향의 와류가 형성되었으며, 이로 인해 혈관벽 바깥쪽과 시계방향으로 90도 정도 지역에서 전단응력이 높게 나타났다. 혈류 유동 특성과 동맥류 발생위치를 비교해 보면, 혈류의 혈관벽 부딪힘이 관찰되는 위치와 부차적 유동에 의해 전단응력이 크게 나타난 지역은 동맥류의 발생위치와 일치하였다. 따라서 혈류의 혈관벽 부딪힘과 부차적 유동에 의한 전단력이 동맥류 발생의 혈류역학적 요인으로 의심된다.

Detection of Radial Pulse Wave by Photoplethysmogram (광전용적맥파를 이용한 요골동맥 맥파 검출)

  • Photoplethysmogram is widely used to measure heart rate and arterial blood oxygen saturation in human. This paper describes radial pulse waves recorded by photoplethysmogram with 940 nm wavelength infra red light. Radial pulse waves were varied according to the recording site. When recorded on the skin over radial artery, the radial pulse wave was inverted, comparing to the photoplethysmogram at fingertip. The mechanism of inverted pulse wave seemed to be caused by the change of the blood volume in the subcutaneous tissue between radial artery and the skin, which was reduced during systolic period and increased during diastolic period of the cardiac cycle. These results suggest that radial arterial wall may reflect infra red ray.

Vasodilatation effect of Kirenol isolated from Sigesbeckia pubescens (털진득찰에서 분리한 Kirenol의 혈관 이완효과)

  • The purpose of this study is to investigate the vasodilatation effect of kirenol isolated from Sigesbeckia pubescens on the rabbit basilar artery. In this study, to determine the vasodilatation effect of kirenol on the rabbit basilar artery, arterial rings with intact or damaged endothelium were used for the experiment. And used an organ bath and force transducer were contracted by endothelin. Kirenol, major active constituents of S. pubescens, showed a moderate vasodilatation effect on the basilar arteries of rabbits. Therefore, treatment with kirenol may selectively accelerate cerebral blood flow through dilatation of the basilar artery. This result suggests a potential role of kirenol isolated from S. pubescens as a source of vasodilatation agent.

Evaluation of the Potential of Retrograde Flow Competition in the Right Gastroepiploic Artery Graft for Coronary Artery Bypass Grafting (우위대망동맥을 이용한 관상동맥우회술 후 역행성 혈류 발생가능성의 연구)

  • Background: Due to the concern of flow competition or retrograde steal, it has been generally suggested that the right gastroepiploic artery(RGEA) pedicled graft should be used in critical coronary stenosis lesion. The study was designed to evaluate the potential of retrograde flow competition in the RGEA pedicled graft by measuring the native pressure differences(PD) between the normal coronary artery and celiac arterial pressure, which would be compared with trans-stenosis pressure gradients(TSPG) in coronary artery occlusive disease. Material and Method: Between July, 1998 and February, 1999, pressures of the right coronary artery and the right gastroepiploic artery(or the celiac artery) were measured in patients with the patent right coronary artery(n=12). The PD between the arteries was compared with the TSPG in the occlusive coronary arteries(n=32). Result: The pressures of the normal right coronary artery and celiac artery were 143$\pm$23 vs. 134$\pm$17mmHg in systole(p<0.005), 74$\pm$13 vs. 73$\pm$14mmHg in diastole(p=NS), and 100$\pm$16 vs. 97$\pm$15mmHg in mean (p<0.05). The PD between the arteries were -8~25mmHg in systole, -4~7mmHg in diastole, and -1~10mmHg in mean. The TSPG measured in the occlusive coronary arteries were -4~19(7$\pm$5.8)mmHg in the lesion less than 75% stenosis vs. 7~74(27$\pm$18.3)mmHg in the 75% or over stenosis lesion(p<0.005). The normally existing pressure difference between the coronary arteries and RGEA(15~20mmHg) was significantlyless than the TSPG in .the occlusive coronary artery with 75% or over stenosis(p<0.001). Conclusion: If the pressure gradient between the RGEA and the coronary artery distal to the stenosis is the main determinant of development of retrograde flow competiton in the RGEA pedicled graft, the above data suggests that there will be little chance of competition when It is used in the coronary lesion with 75% or over stenosis.

Vasorelaxing Mechanism of Crude Saponin of Korea Red Ginseng in the Resistance-sized Mesenteric Artery of Rat

  • It has been well known that Korea red ginseng has an antihypertensive effect. The antihypertensive effect may be due to its ability to change the peripheral resistance. Change of vascular tone in the resistance-sized artery contribute to the peripheral resistance, thereby regulate the blood pressure. Therefore, we investigated to clarify the vasorelaxing mechanism induced by crude saponin of Korea red ginseng in the resistance-sized mesenteric artery of rats. The resistance-sized mesenteric artery was isolated and cut into a ring. The ring segment was immersed in HEPES-buffered solution and its isometric tension was measured using myograph force-displacement transducer. Crude saponin of ginseng relaxed the mesenmetric arterial rings precontracted with norepinephrine (3$\mu$M) in dose-dependent manner (0.01 mg/㎖ -1 mg/㎖. The relaxation by crude saponin was smaller in endothelium-intact preparation than that in endothelium-denuded preparation. The contraction induced by A23187 or phorbol 12,13-dibutyrate was not affected by crude saponin of ginseng. The vasorelaxing effect of crude saponin of ginseng was significantly attenuated by the increase of the extracellular K$\^$+/ concentration. Crude saponin-induced vasorelaxation was not affected by tetraethylammonium (1 mM), glybenclamide (10$\mu$M), and 4-aminopyridine (0.1 mM) in these preparations. Ba$\^$2+/(10$\mu$M ∼100$\mu$M) markedly reduced the crude saponin-induced vasorelakation dose-dependently. From the above results, we suggest that crude saponin of ginseng may stimulate K$\^$+/ efflux and hyperpolarize the membrane, thereby cause the vasorelaxation in the resistance-sized mesenteric artery of rats.

Correlation Analysis of Cardiac Diastolic Function and Intima-Media Thickness in the Common Carotid Artery of Ultrasonography (초음파검사의 경동맥내중막두께와 심장이완기능의 상관관계 분석)

  • In this study, 230 subjects of medical examination were investigated to figure out the relationship with common carotid artery intima-media thickness and cardiac diastolic function. In addition, the change in the carotid artery intima-media thickness according to the presence or absence of metabolic syndrome was examined. As a result of the study the carotid artery intima-media thickness was thick as the age increased and there was a large difference in those in their 60s and over. There was no gender difference. As for metabolic syndrome the carotid artery intima-media thickness was thicker in the study subjects with high blood pressure diabetes and dyslipidemia. The correlation between the carotid artery intima-media thickness and diastolic function indexes was significant. As a result of hierarchical regression analysis the thicker the intima-media thickness in the carotid artery the lower cardiac diastolic function.

Ginsenoside $Rg_3$ Increases the ATP-sensitive $K^+$ Channel Activity in the Smooth Muscle of the Rabbit Coronary Artery

  • ATP-sensitive $K^+$ channels $(K_{ATP})$ are expressed in vascular smooth muscle cells, skeletal muscle cells, pancreatic ${\beta}$ cells, neurons and epithelial cells. $K_{ATP}$ contributes to regulate membrane potential to control vascular tone, to protect myocardial ischemia, and to regulate insulin secretion in pancreatic ${\beta}$ cells. We previously demonstrated that ginseng saponins and ginsenoside $Rg_3$ activated maxi $Ca^{2+}-activated\;K^+$ channel, and this might cause vasodilation. Because $K_{ATP}$ plays an important roles to regulate the resting membrane potential in vascular smooth muscle cells, we investigated whether ginsenoside $Rg_3$ produces vasodilation by activating $K_{ATP}$ We showed in this study that $K_{ATP}$ is expressed in rabbit coronary artery smooth muscle cells. $K_{ATP}$ was inwardly rectifying and was inhibited by intemal application of ATP. Micromolar minoxidil activated, but glyburide inhibited the activity of $K_{ATP}$ Ginsenoside $Rg_3$ relieved inactivaiton of whole-cell $K_{ATP}$ current without affecting the peak amplitude of $K_{ATP}$ currents presumably due to more opening of the channels.

$K^{+}$ 통로 개방제의 혈관근 이완작용에 대한 연구 : sarcoplasmic Reticulum에서의 $Ca^{++}$ 유리에 대한 효과

  • 1.목적 $K^{+}$ 통로 개방제인 cromakalim과 levcromakalim이 원형질막의 $K^{+}$ 통로를 개방시켜 막의 과분극을 일으킴으로서 강력한 혈관 이완작용을 일으킨다는 것은 주지하는 바다. 본 연구진들은 지난 2년간의 신약개발 연구사업을 통하여 cromakalim과 pinacidil 등 여러종의 K+통로개방제가 건전한 평활근 세포에서 phenylephrine뿐만 아니라 saponin처리에 의한 투과성 근세포 (permeabilized cells)에서 inositol 1,4,5-triphosphate (IP$_3$)에 의한 수축도 억제함을 보고 하였다. 본 연구에서는 이러한 수축작용에 대한 SPEX fluolog-2 spectrophotometer를 사용하여 돼지의 관상동맥 혈관근의 세포질내 $Ca^{++}$의 농도 ([$Ca^{++}$])의 변동을 관찰하였다. 정상 관상동맥 혈관근 조직에서 acetylcholine (0.1-1$\mu$M)에 의한 [Ca$^{++}$]농도의 증가와 b-escin 처리에 의한 skinned strip에서의 IP3 (1-5$\mu$M)에 의한 [Ca$^{2+}$]의 증가는 cromakalim과 levcromakalim의 전처치에 의하여 현저히 억제되었다. Skinned strip에서 이러한 K+ 통로 개방제에 의한 $IP_3$-요도 ($Ca^{2+}$)i 증가의 억제가 apamin (1-5 mM)과 glibenclamide (1$\mu$M)에 의하여 봉쇄되었으나, chrybdotoxin (0.1$\mu$M)에 의하여는 영향을 받지 아니하였다 한편 skinned strip에서 cromakalim은 GTP${\gamma}$s에 의한 [$Ca^{2+}$]i의 증가는 봉쇄하였으나 caffeine에 의한 [$Ca^{2+}$]i의 증가는 영향을 미치지 아니하였다. 이러한 연구결과로 보아 cromakalim과 levcromakalim과 같은 $K^{+}$ 통로 개방제가 세포막의 $K^{+}$ 통로를 개방하는 작용 뿐만 아니라 적어도 sarcoplasmic membrane에서 $Ca^{2+}$의 유리를 봉쇄함으로써 강력한 혈관 평활근 이완 작용을 나타내는 것으로 시사된다.

Differential Function of EDRF in Systemic Arterial and Pulmonary Arterial System of Renal Hypertensive Rats (신성 고혈압쥐의 전신성 동맥계와 폐동맥계에 대한 EDRF 기능의 차이)

  • To investigate the endothelium dependent vascular reactivity of the systemic arterial and the pulmonary arterial system in acute renal hypertensive rats of 2-kidney, 1-ligation type (RHRs), acetylcholine (ACh)-induced vasodilation and depressor effects were evaluated in isolated arteries and in vivo, respectively, in the presence and absence of functional endothelium. ACh $(10^{-5}\;M)$ relaxed the intact thoracic aortas from RHRs and normotensive rats (NRs), but the effect was significantly smaller for those from RHRs (34 and 86%, respectively, p<0.01). ACh-induced vasodilation was completely abolished after removal of endothelial cell or pretreatment with EDRF inhibitors, L-NAME and MB, indicative of its dependence on intact endothelial or EDRF function. ACh also induced vasorelaxation of the intact pulmonary arteries from RHRs and NRs; however, unlike the effects on the thorcic aorta, no significant difference in amplitude was noted between two groups. ACh $(0.1{\sim}10\;{\mu}g/kg,\;i.v.)$ reduced mean systemic arterial pressure in anesthetized RHRs and in NRs to the similar magnitude (% change: 39 and 46% at $10\;{\mu}g/kg$, respectively) and these hypotensive effects were significantly decreased after pretreatment with L-NAME (30 mg/kg, i.v.). Deprssor effects of ACh on mean pulmonary arterial pressure were similar in RHRs and NRs with and without pretreatment of L-NAME. However, in both NRs and RHRs, the depressor effects of ACh on mean pulmonary arterial pressure were significantly reduced compared with those for mean systemic arterial pressure, and the increment of mean pulmonary arterial pressure noted after L-NAME $(0.1{\mu}100\;mg/kg,\;i.v.)$ was significantly smaller than that for mean systemic arterial pressure. These results indicate that in RHRs the endothelial cell function was impaired, at least in part, in systemic arterial system, but not in pulmonary arterial system, and both ACh-evoked and basal release of EDRF was less in the pulmonary arterial system than in systemic arterial system of both NRs and RHRs.
