Patinets' Perception and Satisfaction with Nursing Care in Korea (일 종합병원의 간호행위에 대한 입원환자의 지각도와 만족도의 관계연구)

  • 목적: 의료수준의 발전과 더불어 대중들의 건강요구와 건강증진에 대한 기대 수준도 점점 높아지므로 간호사는 대상자의 요구를 이해하고 양질의 간호를 제공하여 환자의 건강상태를 바람직한 방향으로 변화시켜야 한다. 따라서 이 연구는 환자가 인지한 간호에 대한 경험과 대상자가 경험한 간호에 대한 만족도를 파악하고, 그 관계를 규명함으로써 환자 간호의 질향상과 간호 만족도 향상을 위한 기초 자료를 제공하고자 한다. 연구방법: 본 연구는 일 종합병원의 127명의 입원 환자가 간호 경험정도와 제공받은 간호에 대한 환자 만족도 정도를 규명하기 위한 서술적 상관관계 연구이다. 연구결과: 첫째, 입원환자의 간호 행위에 대한 간호 지각도의 정도는 각 항목별 비교 분석한 결과 신체적 간호행위에 대한 지각도가 평균평점은 2.74로 가장 높았으며, 또한 환자가 지각한 간호행위에 대한 만족도는 기술-전문적 간호행위(M= 3.55)의 만족도가 가장 높았다. 둘째, 일반적 제특성과 전체 간호행위의 지각도와 차이가 있는지 분석한 결과 연령(p< 0.1)에 따라 유의한 차가 있는 것으로 나타났다. 즉 연령이 높을수록 간호행위에 대한 지각도가 높게 나타났다. 그리고, 간호행위에 대 한 지각도의 하부영역 분류중 신체적간호행위에 대한 지각도는 입원과(p< 0.05)에 따라 유의한 차가 있는 것으로 보였다. 또한 일반적 제특성과 전체 간호행위의 만족도와의 관계에서는 입원과(p< 0.05)에 따라 유의한 차가 있었다. 일반외과 환자가 내과 환자보다 만족도가 높은 것으로 나타났다. 셋째, 입원환자의 간호 행위에 대한 환자의 지각도와 만족도간의 관계(r= 0.39, p= 0.00)는 서로 상관성이 있는 것으로 나타났다. 결론: 본 연구의 대상은 1개 종합병원에 국한된 것이므로 연구결과를 일반화 할 때는 신중을 기해야 한다. 연구결과에서 보여주듯이 정신적 간호행위가 가장낮은 지각도로 나타났듯이 간호사는 환자들의 정신적 지지의 간호가 잘 이루어 지도록 노력해야 할 것이며, 또한 교육적 간호행위의 만족도가 가장 낮게 나타났으므로 보다 더 환자와 간호사간의 정보교환이 잘 이루어지도록 해야 한다. 연구결과를 기초로 하여 전 진료과 병동으로 확대 반복 연구를 제언하며, 환자의 간호 만족도를 향상시키기 위해 전문적 지식 및 긍정적 의사표현등을 포괄하여 간호사의 적극적 간호행위가 수행되어야 할 필요가 있다.

Discrepancy in the scores of uncertainty perceived by patients and nurses′ interperson perception. (환자가 지각하는 불확실성 정도와 간호사의 대 환자(對 患者) 지각정도)

  • The purpose of this study was to assess and compare discrepancy in the scores of uncertainty perceived by patients and nurses' interperson perception. For this study, 124 hospitalized patients and the same numbered nurses assigned for direct care of each 124 patients were selected from general ward of C. University Hospital in Seoul during the time period from September to November 1987. Degree of uncertainty was measured by 27 items modified from Mishel Uncertainty in Illness Scale (MUIS), and was utilized by a Likert type scale The data were analysed by Mcnemar-test. Unpaired t-test. ANOVA, Scheffe - test and Stepwise multiple regression. The results are summarized as follows : 1. The discrepancy in the scores of uncertainty perceived by patients and nurses' interperson perception showed significant differences in 23 of 27 items : 11 of 23 items showed that the scores of patients' perception of uncertainty were higher than that of nurses' interperson perception of uncertainty. but 12 of 23 items were revealed reversely. 2. With regard to nurse's demographic variables, the discrepancy scores were the higest in the group under 22 years of age (F=3.20, p=.026) and in the group less than 1 year of nursing experience among 4 groups (F=4.41, p=.006). 3. The discrepancy scores had a tendency to be lowered in the higher age group(r= -.27. p=.0026) and in the longer experienced group (r=-.25, p=.0052). 4. The most important variable affecting the discrepancy scores was identified to be the nurses' age which acounted for 7.2% fo the total variances in the stepwise multiple regression analysis. This was followed by patient hospital days which accounted for an additional 4.5% of the total variances. To conclude, the discrepancy in the scores of uncertainty perceived by patients and nurses' interperson perception showed significant differences in 23 of 27 items. The discrepancy scores of uncertainty had a tendancy to be lowered in the higher age group and in the longer experienced group.

Health Behavior Motivators about Quit-Smoking in Some Outpatients of University Hospitals (일부 병원 내원 환자의 금연에 관한 건강행위 동기요소)

  • This study was aimed to investigate the motivators of smoking in 300 subjects using convenient extraction in one hospital. After informed consent, the survey that consisted of general characteristics, health behavior, stage of change, health behavior-related-perceived benefit, health behavior-related-perceived barrier, health behavior-related-family support, oral health behavior, subjective oral health and others was carried out. The smoking rate was higher in case of male (sex), high school graduates (education) and 2~3 million Korean won (KRW) (monthly income). The present smokers responded lower oral health behavior (p<0.001), lower health behavior-related-perceived benefit (p=0.021), higher health behavior- related-perceived barrier (p<0.001) and lower stage of change (p<0.001). The oral health related variables were not influenced by smoking. In logistic regression model, odds ratios in smoking were 0.378 in below average (health behavior-related-perceived barrier), 4.746 in below average (stage of change), 12.743 in male (sex) and 3.271 in 2~3 million KRW (monthly income/standard : over 3 million KRW). It was suggested that oral health related manpower should take account of not only smoking-related oral health but also motivators of smoking as to deliver effective no-smoking program.

The Perception and Emotional Experiences of Rare and Intractable Diseases in Caregivers and Pediatric Patients with Mitochondrial diseases (미토콘드리아 질환 소아 환자 보호자에서의 질환 인식 및 정서변화)

  • Purpose: This study aimed to investigate the perception and emotional experiences in rare and intractable diseases for caregivers of pediatric patients with mitochondrial diseases in order to provide therapeutic interventions for patients, caregivers, and families. Methods: A total of 83 caregivers of pediatric patients with mitochondrial diseases were recruited from the pediatric mitochondrial disease clinics of the Gangnam Severance Hospital in South Korea. Participants completed the survey about their perception of mitochondrial disease and emotional experiences after the diagnosis, and these clinical data were analyzed accordingly. Results: Surveys from a total of 83 caregivers of patients were analyzed, and the patients' age ranged from 6 to 12 years (33%), followed by ages 1 to 6 years (30%). Children with mitochondrial diseases were between 0 and 0.5 years of age at the time of first symptom onset (43%), and the duration of illness lasted more than 10 years in most cases (42%). Prior to diagnosis of mitochondrial diseases, the amount of awareness the caregivers had was 'Not at all' for both rare and intractable diseases and mitochondrial diseases in 44 cases and 68 cases, respectively. For the caregivers' emotional experiences, the most common initial responses were 'Discouraged/despair', 'Helpless/lethargic', and 'Disconcerted'. 'Anxious', 'Committed to treatment', and 'Responsibility as family members' were the most common emotional responses from the caregivers, followed by 'Disconcerted' and 'Helpless/lethargic'. Conclusion: It is important to consider the level of perception and emotional experiences of caregivers and patients with rare and intractable mitochondrial diseases for planning treatment programs.

Factors influencing health and quality of life among allergy and asthma patients: With specific focus on self-efficacy, social support and health management (건강과 삶의 질에 영향을 주는 요인에 대한 분석: 자기효능감, 사회적 지원 및 질병관리를 중심으로)

  • This article examines factors that influence health and quality of life. In addition to the symptomatology and physiological functioning, the influence of the psychological functioning and interpersonal relationship on the overall health and quality of life are also investigated. Using a case-study approach, a total of 70 patients suffering from allergy or asthma were interviewed using a semi-structured questionnaire developed by the present authors. It assessed the following six areas: Cause and onset of illness, psychological functioning, health management, trust, social support received and overall health and quality of life. Based on the transactional model (Bandura, 1997; Kim & Park, 2005), the results of the case studies have been integrated and divided into three aspects: (1) Cause and onset of illness that includes physiological and environment factors; (2) mediating influences that includes psychological functioning, health management, interpersonal relationship and social support received; and (3) outcome factor that includes symptomatology, health and quality of life. The psychological functioning includes self-efficacy (self-regulated efficacy, efficacy for enlisting social support, efficacy for managing the environment, and efficacy for overcoming difficulties), positive outlook, life goals, experience of stress, and proxy control. Interpersonal relationship includes trust of family members and the physician. Health management includes receiving proper health assessment, following the advice and prescription given by the physicians, control of the environment and maintaining a healthy lifestyle. The results indicate that physiological, psychological, relational and environment factors interact with each other and affect individual's overall health and quality of life. Self-efficacy, social support received from family members, trust of physicians, and the health care system are key factors promoting healthy lifestyle and quality of life. The results indicate the need for further interdisciplinary, indigenous and cultural psychological research.

A study on learning needs about altered elimination of spinal cord injury patients : A comparison patients에 and nurses에 perceptions (척수손상환자의 배설장애에 대한 학습요구 : 환자 대 간호사 지각 비교)

  • The purpose of this study determine and compare spinal cord injured(SCI) patients’ and nurses’ perceptions of SCI patients’ learning needs about altered elimination and then provide nursing data for more effective SCI patients’ learning process. Data collection was done from September 3 to October 2, 1993. For this study, 36 SCI Patients(12 paraplegia, 24 quadriplegia) and 20 nurses who were working neurosurgery or rehabilitation unit in 3 general hospitals in C and I city were selected. Data collection was accomplished by Questionnaire method and the instrument developed by the investigator, elicited information about learning needs about altered elimination of SCI patients from SCI patient, nurse, rehabilitation doctor, nursing professor, nursing graduate students. The questionnaire composed altered urinary elimination part with 10 categories and altered bowel elimination part with 10 categories. The results of this study were as follows : 1. There was no significant difference in learning needs about altered urinary elimination of the paraplegic SCI patients’ perceptions and nurses’ perceptions of the paraplegic SCI patients. But there was significant difference in learning needs about altered urinary elimination of the quadriplegic SCI patients and nurses’ perceptions of the quadriplegic patients(t=2.48, p=.017). 2. There was no significant difference in learning needs about altered bowel elimination of the paraplegic SCI patients’ perceptions and nurses’ perceptions of the paraplegic SCI patients. But there was significant difference in learning needs about altered bowel elimination of the quadriplegic SCI patients and nurses’ perceptions of the quadriplegic patients(t=-3.00. p=.005). 3. Degree of paraplegic SCI patients’ perceived learning needs about altered urinary elimination was 2.4083 and quadriplegic SCI patients’ perceptions were 2.0750. Degree of paraplegic SCI patients’ perceived learning needs about altered bowel elimination was 2.3972 and quadriplegic SCI patients' perceptions were 2.181.

TRANSITIONAL TREATMENT OF AMLEOGENESIS IMPERFECTA IN MIXED DENTITION: A CASE REPORT (혼합치열기에 있는 법랑질형성부전증 환아의 이행적 치료)

  • Amelogenesis Imperfecta (AI) is a genetic disorder which retards the development of enamel and it can be classified into three types: hypoplastic, hypomaturation, hypocalcified type. This can occur both in deciduous and permanent dentition. A 8 year 8 month old patient with a chief complaints of delayed eruption on upper anteriors, calculus deposit on lower anteriors and anterior openbite visited the clinic. Anteriors had thin layer of enamel and were very narrow. Especially lower anteriors had rough surface and were in bad shape. Teeth were very hypersensitive to thermal changes. Upper and lower first molars showed severe attrition on the occlusal surface. Radiographs also verified hypoplastic enamel in the whole dentition including the teeth in the tooth bud. The patient was diagnosed as hypoplastic AI, and is being treated at the pediathc and prosthodontic department of the Kyunghee dental university hospital. To improve the function, esthetics, hypersensitivity of the AI patients, restorations on the posteriors and the anteriors with oral hygiene instruction are necessary, Constant follow-up check is needed until full growth and after full growth, cooperative care with the other department is needed.

A Study on the Stress Perception Response Following the Aftereffect in Poststroke Patients (중풍환자(中風患者)의 후유증(後遺症)에 따른 Stress 지각반응(知覺反應)에 대(對)한 조사연구(調査硏究))

  • The primary purpose of this study was to assess the relation of stress perception and poststroke. This study was done on 50 stroke patients in hospital. According to activities of functional impairment, they were classified into walking disturbance, motor weakness, dysphagia, or reattack etc. The stress perception test(GARS Scles) and stress response assessment shows the follwing results. 1. On the distribution in the stroke 50 patients, For the majority group were male in sex, sixty inage, middle towns people in residence. 2. On the comprehensive GARS scale scores, It marked the higest scores thirty at age, merchandise or salesman on jobs, a city in residence. There was no signigicant difference in mean GARS Scales scores between males and females group, the left hemiplegia and the Rt hemiplegia. 3. On the comprehensive GARS Scale tests, Overall glogal stressor(G8) and sickness stresor(G4) marked the highest scores of all GARS Scales. 4. On the comparative assessment of each group's stress reponse test scores, zung-bu(中腑) was showed higher scores than the other group in oriental diagnosis and hemorrage was showed higher scores than the other group in western medical diagnosis. 5. On the comprehensive assessment of each group's stress reponse test scores, aphsia(不語) marked the highest scores in another disphagia group's. It marked the highest scores in another group's that banshinbulsoo(半身不遂) as regards as hemiplegia generally. Standing disability is the higher scores than another group's walking alone or assist, and standing alone. 6. Secondary attack is the highest scores of all reattack stroke on the stress repones test. 7. Comparing and analyzing the GARS Scale total scores and GARS Scale subject fator at stroke, we found that sickness stressor and fininial stressor is showed the highest correlated to stress response fowlloing stroke. With those results, we can see that functional impairment following stroke is correlated to stress perception and reponse. In the furture studies using, we hope that the findings the study would have clinical relevance to the psychosocial adjustment and total rehabilation of stroke patients.
