Improvement of colitis preventive effects of Gochujang by addition of Lactobacillus plantarum on C57BL/6 mice (Lactobacillus plantarum 첨가 고추장의 C57BL/6 마우스에서 대장염 예방 증진효과)

  • Gochujang, a traditional Korean food, is fermented by mixing red pepper powder, various grain, meju and salt. Changes in the kind of ingredients and fermentation method could increase health functionalities. In this study, in vivo anti-colitis effects of gochujang prepared with mixed grains, bamboo salt baked 3 times and meju starters on DSS-induced colitis in C57BL/6 mice were studied. We prepared gochujang prepared with mixed grains (MG), bamboo salt, and Aspergillus oryzae (A) and Baccillus subtilis (B) mixed starters (MG-AB) and gochujang prepared with MG, bamboo salt and A, B and Lactobacillus plantarum (L) mixed starters (MG-ABL). MG-AB and MG-ABL significantly increased body weight and colon length compared to the control (p<0.05). MG-ABL showed significantly decreased interleukin-6 (IL-6) expression in serum compared to the control and MG-AB group (p<0.05). MG-ABL also regulated mRNA and protein levels of pro-apoptotic Bcl-2-associated X protein (Bax) and anti-apoptotic B-cell lymphoma-2 (Bcl-2) in the mice colon tissue (p<0.05). Therefore, MG-ABL exhibited the increased anticolitis effects by inhibiting damage of colon tissue, probably by regulating a pro-inflammatory cytokine of IL-6 and regulated apoptosis related genes. These results indicated that gochujang changed with good ingredients and starters had colitis preventive effects and might be due to active compounds in mixed grain and bamboo salt, and produced by L during the fermentation of gochujang.

Anti-inflammatory Activity of Perilla frutescens Britton Seed in RAW 264.7 Macrophages and an Ulcerative Colitis Mouse Model (RAW 264.7 대식세포와 궤양성 대장염 마우스 모델에서의 들깨의 항염증 효과)

  • This study aimed to investigate the anti-inflammatory activities of raw (P) and roasted (RP) Perilla frutescens Britton (perilla) seeds in RAW 264.7 macrophages and an ulcerative colitis mouse model. In lipopolysaccharide-treated RAW 264.7 cells, treatment with ethanol extract of P at the concentrations of 75 and $150{\mu}g/mL$ decreased nitric oxide (NO), interleukin-6 (IL-6), and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) levels to 48-85% of the control (p<0.01). Treatment with RP extract exhibited similar effects on NO, IL-6, and TNF-${\alpha}$, decreasing those levels to 51-84% of the control (p<0.01). In dextran sulfate sodium-treated ulcerative colitis mice, dietary treatment with 1% RP for 7 days decreased the colonic levels of prostaglandin $E_2$ and leukotriene $B_4$ to 34% and 58% of the control, respectively (p<0.05). Dietary P treatment, however, did not decrease those levels significantly. These results indicate that roasted perilla seed exerts anti-inflammatory activity both in vitro and in vivo.

  • Since the discovery that Mycobacterium aviumsubsp. paratuberculosis (Map) is the causative agent of Johne's disease (JD) in cattle at the end of the nineteenth century, movement of livestock latently infected with Map has led to the spread of JD throughout the world. A new form of enteritis with clinical features of JD in cattle appeared in humans concurrent with the appearance of Map as a disease problem in livestock. The demonstration that Map is a zoonotic pathogen and the causative agent of the new form of enteritis in humans, however, wasn't recognized until late in the twentieth century when methods were developed to detect the presence Map in tissues from patients with the new form of clinical enteritis. The objective of this short review is to provide a brief history explaining how Map has become a major disease problem in livestock and humans and then provide a review of the progress that has been made in treating patients with an enteritis caused by Map and the strategies underway to develop a vaccine to control infection in livestock.

Imaging Techniques and Differential Diagnosis for Inflammatory Bowel Disease (염증성 장질환의 영상기법 및 감별진단)

  • The two main types of inflammatory bowel disease (IBD) are Crohn's disease and ulcerative colitis. Currently, when IBD is suspected, CT enterography is widely used as an initial imaging test because it can evaluate both the bowel wall and the outside of the bowel, helping to differentiate IBD from other diseases. When IBD is suspected, it is necessary to distinguish between Crohn's disease and ulcerative colitis. In most cases this is not difficult; however, in some cases, it is difficult and such cases are called IBD-unclassified. CT findings are often non-specific for ulcerative colitis, making it difficult to differentiate it from other diseases using imaging alone. In contrast, characteristic CT findings for Crohn's disease are often helpful in diagnosis, although diseases, such as tuberculous enteritis can mimic Crohn's disease. Recently, mutations in the gene encoding a prostaglandin transporter called SLCO2A1 have been discovered as the cause of the disease in some patients with multiple ulcers and strictures, similar to Crohn's disease. Therefore, genetic testing is being used to make a differential diagnosis.

  • Chronic diarrhea in children is a common problem with numerous causes. Although most of these causes are benign, critical illness may present as chronic diarrhea. In a patient of chronic diarrhea, gastrointestinal infections are the most common causes in children of all ages and antibiotics may cause chronic diarrhea by altering intestinal microflora, which can result in the emergence of bacterial overgrowth. Overgrowth of Clostridium difficile may cause pseudomembranous colitis. We experienced 25-month-old boy who suffered from chronic diarrhea and partially treated with antibiotics irregularly. Colonoscopic findings of this child showed multiple plaques with white to yellowish exudate which adhere to the mucosal surface of a variable length of rectum. Histollogically, each plaque comprised a pseudomembrane of mucous debris, inflammatory cells, and exudate overlying groups of partially disrupted glands. A latex agglutination test on patient's stool was positive to toxin A of Clostridium difficile. He was recovered after stopping the antibiotics he has been prescribed, and being given vancomycin for 2 weeks. We report this case with brief review of literature.

A Review of Recent Domestic Research on Herbal Medicine Treatments for Ulcerative Colitis: A Focus on OASIS Platform Research (궤양성 대장염의 한약치료에 대한 국내 연구동향 분석 - 오아시스 검색을 중심으로)

  • Objectives: This study analyzes domestic research trends on herbal medicine treatments for ulcerative colitis (UC) by examining 28 selected domestic papers using the Oriental Medicine Advanced Searching System (OASIS) as the main search source. Methods: A total of 28 domestic papers were selected on the OASIS platform as the main search source using the keywords "(궤양성 대장염) or (궤양성 대장병)" and "(한약) or (한의학)". Results: There were 11 case report studies and 17 case-control studies published from 2002 to 2022. All the case-control studies only dealt with mice and did not include human subjects. The most common pathologies were damp heat and Changpung. Hwangryunhaedok-tang was the most frequently prescribed herbal formula, and Coptidis Rhizoma was the most commonly used herb across all studies. The results showed that many studies demonstrated the significant effects of herbal treatment. Conclusion: Herbal medicine may exhibit therapeutic effects on various aspects of UC. However, evidence-based studies, such as clinical trials involving human subjects, are needed.

  • The environmental factors and the distribution of coliform group were investigated in Suyeong bay on the summer, 1989. The ranges of environmental factors were as follows : Dissolved oxygen $0.54{\sim}6.82m{\ell}/{\ell}$, COD(Chemical Oxygen Demand) 0.28~9.84ppm, Salinity 14.7~33.5%, Nitrite $0.13{\sim}20.49{\mu}g-at/{\ell}$, Nitrate $0.89{\sim}62.77{\mu}g-at/{\ell}$, Ammonia $3.81{\sim}246.98{\mu}g-at/{\ell}$, Silicate $6.12{\sim}177.5{\mu}g-at/{\ell}$. The coliform group ranged $10^4{\sim}10^5/100m\ell$ in the mouth and it was decreased in the open sea. Among the composition of coliform group, E. coli I type was dominent at distance area. The correlation between coliform group and ammonia was r=0.629. There were negative correlation between salinity, dissolved oxygen and coliform group during study period.

  • The management of failure of colonic graft or corrosive stricture with impaired colonic circulation continues to be a challenge to surgeons. We present four cases of complicated corrosive esophageal stricture, which were successfully treated using jejunal free graft. As the initial surgical treatment, three patients underwent colonic interposition, and one underwent Whipple's procedure due to panperitonitis. Ore patient underwent transposition of jejunal free patch graft, two underwent interposition of jejunal free graft, and one underwent esophagojejunocolojejunostomy using jejunal free graft. All patients did not show dysphagia after long term follow-up. Complex esophageal reconstruction with jejunal free graft was performed successfully and long term functional outcome was good.

  • 장염의 원인은 매우 다양한데, 식중독과 같은 각종 바이러스성 감염과 세균 감염 등으로 인한 감염성 장염이 흔하며, 크론씨병, 궤양성 대장염과 같은 만성 질환도 있다. 주로 나타나는 증상은 복통과 설사이며 열이 동반되는 경우도 있다. 복통은 정확히 위치를 알 수 없는 묵직한 통증이 가장 흔하며, 묵직한 통증으로 시작하여 뒤틀리는 듯이 심하게 아픈 통증으로 진행하는 경우가 흔하다. 설사는 적게는 하루에 2~3회에서, 많게는 20여 회가 넘는 경우까지 그 빈도가 다양하다. 대장이 감염된 경우 변에 코와 같은 점액이 많이 섞여 나오고, 소장이 감염된 경우에는 점액이 별로 섞여 나오지 않는다. 주로 6개월~24개월 사이의 아이에게 잘 발생하며, 춥고 건조한 늦가을부터 겨울철(10월~1월)에 가장 많이 발생하는데, 우리나라에서는 시기에 관계없이 발생하기도 한다.

Factors related to Quality of Life of Patients with Ulcerative Colitis (궤양성 대장염 환자의 삶의 질 영향요인)

  • Purpose: The purpose of this study was to identify factors related to the quality of life of patients with ulcerative colitis. Methods: A total 116 patients with ulcerative colitis were asked on general characteristics, illness-related characteristics, depression, and quality of life. The collected data were analyzed by t-test, ANOVA, and stepwise multiple regression. Results: As for depression the 81.03% were normal, the 8.62% were mild depression, the 6.90% were moderated depression, and the 3.45% were severe. As for quality of life, the social functions was highest, followed by intestine-related symptoms, systemic symptoms, and emotional functions. Quality of life was positively correlated to subjective health status, age, and body mass index, and negatively to depression. Main factors affecting the quality of life included depression, subjective health status, physician's global assessment, age, days of loss in social life, and present abdominal pain. Conclusion: Physical and psychosocial health problems related to ulcerative colitis affected the quality of life of the patients. Further research is warranted for developing educational programs and psychosocial strategies in order to efficiently handle the illness.
