Medkum TAu Inversion Recover(MTIR) Sequence for White Matter Suppression in Brain Cortical Lesions (뇌피질 질환에서 뇌백질 신호 억제를 위한 중간시간 반전회복 영상 기법)

  • Purpose : The purpose of this study was to evaluate the image quality, contrast characteristics, and possible clinical utility of Medium Tau Inversion Recovery(MTIR) sequence with white matter suppression in patients with brain cortical lesion. Materials and methods : Two normal volunteers and twenty-one patients with cortical lesion were scanned with MTIR as well as other MR imaging sequences. Gray-white matter contrast was evaluated objectively using region-of-interest calculations, including percent contrast and contrast-to-noise ratio(CNR). MTIR sequence was visually compared with other sequences in 21 patients with cortical lesion including conspicuity and detection rate. Results : MTIR sequence had the highest present contrast and CNR between the gray matter and white matter. In twenty-one cases of cortical lesion including cortical dysplasia, MTIR sequence improved delineation and conspicuity of lesion, but MTIR sequence could not detect new lesions. Conclusion : The MTIR sequence well delineated the cortical lesions, particularly in including cortical dysplasia. It may be used as an adjunctive imaging sequence in case of poor gray and white matter differentiation with conventional T1-weighted sequences.

  • There has recently been increasing interest in the use of NMDA receptor antagonists as potential neuroprotective agents for the treatment of ischemic stroke. To evaluate the neuroprotective effect of the selective non-competitive NMDA receptor antagonist MK-801 in focal cerebral ischemia, local cerebral glucose utilization (ICGU) was examined in 15 neuroanatomically discrete regions of the conscious rat brain using the 2-deoxy-D[$^{14}C$] glucose quantitative autoradiographic technique 24 hr after left middle cerebral artery occlusion (MCAO). Animals received MK-801 (5 mg/kg i.v.) or saline vehicle before (20-30 min) or after (30 min) MCAO. Both pretreatment and posttreatment of MK-801 increased occluded/non-occluded ICGU ratio in 7 and 5 of the 15 regions measured, respectively (most notably in cortical structures). Following MK-801 pretreatment, there was evidence of widespread increases in ICGU not only in the non-occluded hemisphere (12 of the 15 areas studied) but also in the occluded hemisphere (13 of the 15 areas studied), while MK-801 postreatment did not significantly increase ICGU both in the normal and occluded hemispheres. These data indicate that MK-801 has a neuroprotective effect in focal cerebral ischemia and demonstrate that MK-801 provides widespread alterations of glucose utilization in conscious animals.

A study of Extraversion-introversion as a Dimension of Personality in Schizophrenics (정신분열증 환자에서 성격차원으로서의 외-내향성에 관한 연구)

  • The purpose of this study was to investigate the trait cortical arousal level, measured by extraversion-introversion, in schizophrenics. Subjects in this study were 231 (chronic schizophrenics=78, acute schizophrenics=62, and normal persons=91). All subjects were asked to respond to extraversion-introversion subtest scale in the korean EPQ(Korean Version Eysenck Personality Questionnaire). Collected data were analyzed by using the t-test and one-way analysis of variance. Results were as follows : 1) Trait cortical arousal level of chronic schizophrenics, which was measured by extraversion-introversion subtest scale in the Korean EPQ, was statistically higher than that of acute schizophrenics and normals. 2) Acute schizophrenics and normals were not significantly different in the trait cortical arousal level. However, the trait cortical arousal level of chronic schizophrenics was higher than that of acute schizophrenics. Therefore, it may state that the trait cortical arousal level of schizophrenics is getting higher when schizophrenic symptoms become more chronic.

  • Purpose : To investigate the difference of total activation in visual area, motor area, and cerebellum according to the stimulation paradigm. Materials and Methods : Functional MR imaging was performed in 5 healthy volunteers with visual and motor activity using EPI technique. LED and Checker-Board stimulation were performed for visual activity. Thumb motion and Finger Tapping were performed for motor and cerebellum activity. Stimulus timing was 60sec. off, 120sec. on, 60sec. off. Data processing was carried out by using the cross-correlation method for each pixel. Each pixel was then selected and assumed activated if the correlation coefficient was equal or larger than a threshold value. Time course data was obtained by calculating the total activation which was defined as the number of activated pixel x averaged pixel intensity. Results : In the case of visual activity with LED stimulation, we found increased total activity of more than 100% compared with Checker-Board stimulation. In the case of motor area and cerebellum with Finger tapping stimulation, we found increased total activity of more than 10% and 150%, respectively compared with Thumb motion stimulation.

Analysis on the Variability of Cerebral Cortex per Intellectual Category in Adolescents (청소년의 지능범주별 대뇌피질 변화성 분석 연구)

  • The brains of adolescents experience rapid changes, which has been studied to prove relatedness between neuroanatomical properties and IQ. But, most previous studies infer the relatedness from purely cross-sectional data. This study not only measured the thickness of the cerebral cortex once, but traced its variability and the relatedness between IQ and this variability, which was presumed to be 75. Healthy adolescents (M=16yr. and 4month) were divided into 5-stage categories based on their intellectual ability and MRI scan was made twice every 6 months to measure the variablity of their cerebral cortex. As a result, a big difference in the variability of the cerebral cortex was shown based on their IQ. Three groups with an IQ of more than 120 showed a decrease in the thickness of the cerebral cortex in 11 brain regions, while two groups with an IQ lower than 120 showed an increase in the cerebral cortex thickness in 5 to 8 regions. It is presumed that the lower the IQ, the slower the maturation of the cerebral cortex.

T2 Relaxation Times of the Cingulate Cortex, Amygdaloid Body, Hippocampal Body, and Insular Cortex: Comparison of 1.5 T and 3.0 T (대상회 피질, 편도체, 해마체, 도피질의 T2 이완시간: 1.5테슬러와 3.0테슬러 자기공명영상장치의 비교)

  • Purpose : To compare T2 relaxation times (T2) in the cingulate cortex, amygdaloid body, hippocampal body, and insular cortex between 1.5T and 3.0T MR imagers. Materials and Methods : Twelve healthy volunteers underwent FLAIR and CPMG imaging perpendicular to the hippocampal body at both 3.0T and 1.5T. T2 was measured in the cingulate cortex, amygdaloid body, hippocampal body, and insular cortex. The T2 relaxation time ratios of the cingulate cortex, insular cortex, and amygdaloid body to the hippocampal body were compared between 1.5T and 3.0T. Results : The mean T2 of the cingulate cortex, amygdaloid body, hippocampal body, and insular cortex at 1.5T were $109.5{\pm}3.1$, $117.0{\pm}7.1$, $114.7{\pm}2.4$, and $111.3{\pm}2.4$, respectively; $99.7{\pm}3.8$, $100.7{\pm}4.3$, $97.9{\pm}3.4$, and $96.2{\pm}2.0$, respectively, at 3.0T. Percentage changes of T2 in the cingulate cortex, insular cortex, amygdaloid body, and hippocampal body at 3.0T with respect to those at 1.5T were -8.9%, -13.5%, -14.6%, and -13.5%, respectively. The mean T2 ratios of the cingulate gyrus, insular cortex, and amygdaloid body to the hippocampal body at 1.5T and 3.0T were 0.96 and 1.02 (p = 0.003); 1.02 and 1.03 (p>0.05); 0.97 and 0.98 (p>0.05), respectively. Conclusion : T2 decrease in the cingulate cortex was less than the amygdaloid body, insular cortex, and hippocampal body at 3.0T. The mean T2 ratio of the cingulate gyrus to the hippocampal body was significantly different between 1.5T and 3.0T.

MRI Findings Suggestive of Metastatic Axillary Lymph Nodes in Patients with Invasive Breast Cancer (유방암 환자에서 액와부 림프절 전이를 시사하는 자기공명영상 소견)

  • Purpose This study aimed to investigate the diagnostic performance of features suggestive of nodal metastasis on preoperative MRI in patients with invasive breast cancer. Materials and Methods We retrospectively reviewed the preoperative breast MRI of 192 consecutive patients with surgically proven invasive breast cancer. We analyzed MRI findings of axillary lymph nodes with regard to the size, long/short ratio, cortical thickness, shape and margin of the cortex, loss of hilum, asymmetry, signal intensity (SI) on T2-weighted images (T2WI), degree of enhancement in the early phase, and enhancement kinetics. Receiver operating characteristic (ROC) analysis, chi-square test, t test, and McNemar's test were used for statistical analysis. Results Increased shorter diameter, uneven cortical shape, increased cortical thickness, loss of hilum, asymmetry, irregular cortical margin, and low SI on T2WI were significantly suggestive of metastasis. ROC analysis revealed the cutoff value for the shorter diameter and cortical thickness as 8.05 mm and 2.75 mm, respectively. Increased cortical thickness (> 2.75 mm) and uneven cortical shape showed significantly higher sensitivity than other findings in McNemar's test. Irregular cortical margins showed the highest specificity (100%). Conclusion Cortical thickness > 2.75 mm and uneven cortical shape are more sensitive parameters than other findings, and an irregular cortical margin is the most specific parameter for predicting axillary metastasis in patients with invasive breast cancer.

Usefulness of Direct Cortical Stimulation During Intraoperative Monitoring in Patients with Brain Tumor Near Motor Cortex: Case Report (운동피질영역 주변의 뇌종양 환자에서 수술 중 직접피질자극 검사의 유용성)

  • The aim of this study was to preserve the corticospinal tract during surgery and assess more accurately the motor performance in brain tumor patients around the motor cortex. TceMEP is not entirely reliable, even though there has been no change in waveforms due to a mixture of false positive and false negative signals. For a more detailed examination, DCS was employed to selectively stimulate the motor cortex. In both cases, the indications could find the region to which the cortex was responsible, and constantly check and examine the changes in amplitude, thereby preserving the motor pathway and performing surgery. On the other hand, patients who did not implement the DCS but did implement the TceMEP experienced a decrease in their postoperative motor performance. DCS is a very useful examination and it is a method that can reduce the post-surgery disorder that may occur in patients with the TceMEP in brain tumor surgery.

THE LIMITATION OF ALVEOLAR BONE REMODELING DURING RETRACTION OF THE UPPER ANTERIOR TEETH (상악 전치부 견인 시 치아이동에 따른 전방 치조골개조량의 변화에 관한 연구)

  • In many cases of orthodontic treatment the upper anterior teeth are retracted. Periodontal problems may arise during incisor retraction, if the amount of tooth movement and the amount of remodeling in the anterior cortical bone are not the same. Therefore in this study, to find out the relationship between the amount of tooth movement and the amount of bone remodeling during retraction of the upper anterior teeth, lateral cephalograms of 56 female patients over 18-year-old were taken before and after treatment. Among the 56 patients, two groups were divided according to the type of root movement during retraction. 26 patients mainly moved by tipping and 30 by bodily movement. The cephalograms taken before and after treatment were superimposed upon the true horizontal plane. In the Tip-Group, the horizontal bone remodeling/tooth movement ratio was 1:1.63, and in the Torque-Group it was 1:1.66. Because the amount of tooth movement and the amount of bone remodeling were not the same in both groups, in the Tip-Group the root apex moved away from the palatal cortical plate and closer to the labial cortical plate, whereas in the Torque-Group the root moved away from the labial cortical plate and closet to the palatal cortical plate. Therefore, there are limitations in the amount of incisor retraction in patients with a very thin anterior cortical plate in the maxilla, and in patients with severe skeletal discrepancies orthognathic surgery should be considered and when orthodontic camouflage treatment is the only possible method, the orthodontist must be aware of the limitations of treatment.

Three-Dimensional Brain Surface Rendering Imaging of Cortical Dysplasia (뇌피질 이형성증의 3차원 뇌표면 연출영상)

  • Purpose : The study was to evaluate the localization of the abnormal gyral and sulcal patterns obtained by means of brain surface rendering imaging. Materials and Methods : Nineteen patients with cortical dysplasia who underwent brain surface rendering MR imaging were included in this study. We acquired MP-RAGE sequence and created the 3-D surface rendering MR images by using $VoxelPlus^{(R)}$. Anatomical locations and configurations of abnormal gyri and sulci were reviewed. Results : Abnormal gyral and sulcal patterns were seen 18 in 19 patients. The configuration and orientation of affected gyri and sulci were clearly evaluated in the brain surface rendering images. In a lissencephaly, the a cortex was not delineated and showed markedly thick and smooth gyral pattern. In a schizencephaly, there were wheel shaped broad gyral pattern around the cleft. In a hemimegalencephaly, an affected hemisphere were enlarged and displayed thick and wide gyral pattern. In CBPS, the insular cortex was exposed and the gyri of the lesion were thickened. In focal cortical dysplasia, there were irregular serrated or thick and enlarged gyri. Conclusion : Brain surface rendering MR imaging is useful for the evaluation of a detailed gyral pattern and accurate involvement site of abnormal gyri.
