Prevalence and Management of Dyslipidemia, Hypertension, Diabetes Among Adults in Gangwon-do, Korea: the 2013-2014 KNHSP (강원도 성인의 이상지질혈증, 고혈압, 당뇨병의 유병률과 관리: 국가건강검진(2013-2014) 자료의 분석결과와 시사점)

  • Dyslipidemia, hypertension, and diabetes are well-established risk factors for cardio-cerebrovascular disease (CVD). Although the prevalence of dyslipidemia among Korean adults is very high, its management is known to be poor. The aim of this study was to assess the prevalence, awareness, treatment, and control rates of dyslipidemia among adults aged 30 years and older in Gangwon-do, Korea. Analysis included 58,121 adults (29,123 males and 28,998 females) participating in the 2013-2014 Korea National Health Screening Program (KNHSP). Dyslipidemia was defined according to the treatment criteria rather than the diagnostic criteria in Korea. Therefore, high-low-density lipoprotein cholesterol (LDL)-cholesterolemia was deemed present in individuals with LDL-cholesterol levels that exceeded the appropriate risk-based threshold. The age-standardized prevalence was highest in dyslipidemia (32.5%), followed by hypertension (25.1%), and diabetes (9.4%). The awareness rate was 76.7% for hypertension and 74.7% for diabetes, but only 10.6% for dyslipidemia. The lowest patient treatment was found for dyslipidemia (9.4%). The control rate among those undergoing treatment was highest for hypertension (75.8), followed by dyslipidemia (63.3%), and diabetes (43.9%). The higher CVD-risk categories showed lower control rates of hyper-LDL-cholesterolemia. The prevalence of dyslipidemia was higher than hypertension and diabetes, but awareness and treatment rates were lower. Our findings indicate there is a wide gap between the prevalence of dyslipidemia and subsequent treatment, which suggests that effective strategies are required to improve dyslipidemia management. It would be worthwhile to strengthen the follow-up management of patients with dyslipidemia in the KNHSP, especially for the high risk group of CVD.

  • 현재, 우리나라 당뇨병의 유병률은 빠른 속도로 증가하고 있으며 당뇨병의 유병기간이 길수록 각종 합병증이 발생하며 더욱 심각한 증세를 나타내기도 한다. 그 중, 비만 및 고혈당, 당대사장애로 인한 당뇨병성 혈관 합병증과 말초 혈관 경화증이 많이 발병하고 있는 실정이다. 본 연구에서는 광혈류량 측정법(Photoplethysmograaphy)으로 손가락에서 얻어진 맥파의 2차 미분 분석을 통해 당뇨병으로 확진된 36명의 당뇨병 환자에 대해 말초혈관 경화도를 연령별로 비교하고자 하였다. 이때 사용한 PPG 파형의 2차 미분 분석 평가 인자는 a와 d이고, d/a는 혈관의 경화도를 의미하며 경화도가 클수록 d/a의 값은 감소하게 되고, 연령이 증가할수록 경화도는 증가하게 된다. 50대 미만의 당뇨병 환자의 d/a값은 $-0.32{\pm}0.12$, 50대는 $-0.48{\pm}0.13$, 60대는 $-0.55{\pm}0.16$, 70대 이상에서는 $-0.59{\pm}0.19$로 연령이 높아질수록 d/a값이 감소하는 것을 확인하였다. 또한, SPSS 13.0 for Windows(SPSS Inc, USA)의 Oneway ANOVA 분석 결과, 연령과 경화도는 통계적으로 유의한 차이를 보였다(p<0.05). 하지만 70대 이상의 당뇨병 환자 그룹에서는 유의한 차이를 확인할 수 없었다. 본 연구에서는 PPG 파형의 2차 미분 분석을 통하여 연령별 당뇨병 환자의 말초혈판 경화도를 비교하였으며, 향후 보완 연구를 통해 비침습적이고 간단한 방법으로 당뇨병 환자의 말초혈관 경화도를 객관적으로 평가하고 진단할 수 있는 시스템 개발로 당뇨병 환자들의 심혈관계질환 사전예방과 치료효과 판정에 도움을 줄 것으로 기대한다.

  • 우리나라의 당뇨병 유병률은 경제발전과 식생활의 급속한 서구화로 빠른 속도로 증가하고 있다. 특히 당뇨병은 유병기간이 길수록 여러 합병증을 초래하는데 그 중 하나가 혈관경화증이다. 혈관 경화증은 신체 부위별로 그 진행 속도가 다르다. 광혈류측정법(PPG, photoplethysmogram)은 간편한 혈류량 측정방법으로 PPG 2차 미분 방법을 이용해서 혈관의 경화도를 진단할 수 있다. 이러한 PPG 2차 미분방법을 이용하여 당뇨병환자의 왼손과 오른손의 말초혈관 탄성도 차이를 정상인과 비교해 보았다. 실험결과 당뇨병환자와 정상인의 양 손가자 말초혈관 탄성도의 유의한 차이를 확인 할 수 있었다.

An Association of Periodontitis and Diabetes (치주질환과 당뇨병의 관련성)

  • This study was conducted to evaluate the association between periodontal disease and diabetes mellitus. The final analysis population of this study was composed of 4,830 adults with diabetes mellitus or periodontal disease and aged 19 years or older, based on the third-edition data of the 4th Korea National Health and Nutrition Examination Survey (KNHANES) (in 2009). Diabetic status and potential confounders were used in questionnaire materials and physical examination materials, and the presence of periodontal disease was used in the materials for oral health examination by a dentist. For diabetic status, fasting plasma glucose (FPG) levels <100 mg/dl were subcategorized into normal group and FPG levels ${\geq}100mg/dl$ into impaired fasting glucose group; glycosylated hemoglobin (HbA1c) levels <7% into normal group and HbA1c ${\geq}7%$ into diabetes group, on the basis of the American Diabetes Association. According to the 2009 Korea Health Statistics, the case where the pocket depth is 3 mm or more was defined as periodontal disease. The association between the two diseases was evaluated through $x^2$-test and logistic regression analysis using R-commander 2.14. In impaired fasting glucose group, community periodontal index (CPI) groups 3 to 4 had higher risks for periodontal disease 1.23 times (95% confidence interval, 1.07~1.42) than those of CPI groups CPI 0~2, even after adjustment for several confounders. In addition, periodontal disease and diabetes mellitus showed statistically significant differences depending on age, sex, income level, educational background, smoking status, alcohol consumption, and snack intake. The analyses of the third-edition data of the 4th KNHANES showed that there was a statistically significant association between periodontal disease and diabetes mellitus as examined by means of CPI in this study.

Prevalence and Risk Factors for Diabetes Mellitus and Impaired Fasting Glucose of Adults (30-69세 성인의 공복혈당장애와 당뇨병의 유병률과 위험인자 분석)

  • This study was conducted to draw out prevalence and the risk factors of diabetes mellitus and impaired fasting glucose for adults,(age 30-69). The subjects were 2096 adults, who had regular health examinations between January and December of 1999 at K Hospital in Seoul. The data was analyzed using chi-square test, unpaired t-test and logistic regression. Diabetes Mellitus and impaired fasting glucose were diagnosed by ADA (American Diabetes Association, 1997) criteria. The results were as follows: 1. Mens' prevalence of Diabetes Mellitus was 7.9% and womens' prevalence of Diabetes Mellitus was 3.8%. Mens' prevalence of impaired fasting glucose was 10.4% and womens' prevalence of impaired fasting glucose was 6.5%. Prevalences of Diabetes Mellitus and impaired fasting glucose increased with age. 2. Prevalence of Diabetes Mellitus and impaired fasting glucose of obese subjects (relative body weight>=162) was higher than that of overweight subjects (110<=relative body weight<=119) in men and women. 3. The diagnoses of Diabetes Mellitus and impaired fasting glucose increased with systolic blood pressure and triglyceride. 4. Significant factors associated with diabetes in the logistic regression best gut model were age, relative body weight, systolic blood pressure, triglyceride in men, and systolic blood pressure in women. In conclusion, as age, weight, systolic blood pressure and triglyceride get higher, Diabetes Mellitus and impaired fasting glucose prevalence also increases, porportionally.

  • Diabetes is a chronic disease that requires continuous medical care and patient-self management education to prevent acute complications and reduce the risk of long-term complications. The worldwide prevalence and incidence of diabetes mellitus are reached epidemic proportions in most populations. Early detection of diabetes could help to prevent its onset by taking appropriate preventive measures and managing lifestyle. The major objective of this research is to develop an automated decision support system for detection of diabetes using mixture of experts model. The performance of the classification algorithms was compared on the Pima Indians diabetes dataset. The result of this study demonstrated that the mixture of expert model achieved diagnostic accuracies were higher than the other automated diagnostic systems.

A Study on the Prevalence Rate of Hypertension and Diabetes among Adults in the Urban Communities (도시지역 지하철을 이용하는 성인의 고혈압 및 당뇨병 유병률)

  • The purpose of this study is to identify the prevalence rate of hypertension and diabetes a in the urban communities and to provide the basic data for development of health promot The subjects of this study were 526 people over the age of 20, living in Seoul. Data for this collected from June 19, 2001 to September 25, 2001. The results of this study are as follows: 1. The subject group is comprised of males, $57.4\%$ and females, $42.6\%$. Their ages range from 20 to 89, and the weights from 40 to 94 kilograms. 2. In the systolic blood pressure, $36.1\%$ showed high, $56.5\%$ normal. and $7.4\%$ low. In the diastolic blood pressure, $50.5\%$ showed high, $48.9\%$ normal, and $1.7\%$. low. In the blood glucose, $70.3\%$ showed normal. $27.9\%$ high, and $1.7\%$. low. 3. There was a significant difference in the mean(SD) of the systolic blood pressure by age(p=.017) and weight(p=.005). Another significant difference was found in the mean (SD) of the diastolic blood pressure by age(p=.006) and weight(p=.007). There was a significant difference in the mean(SD) of the blood glucose by sex and age(p=.001). 4. There were significant correlations between the blood pressure and the blood glucose and the sex, age and weight. 5. The multiple regression analysis showed that the age and weight explained $9.9\%$ of the systolic blood pressure, that the weight and age explained $7.1\%$ of the diastolic blood pressure, and that the age and systolic blood pressure explained $7.0\%$ of blood glucose. The results were useful in developing health promotion programs. This study suggests that a further study be needed.

Health and nutritional status of Korean underweight women in their twenties: Based on the 2010~2012 Korea National Health and Nutrition Examination Survey (20대 저체중 한국여성의 건강 및 영양 상태: 2010~2012년 국민건강영양조사 자료를 이용하여)

  • Purpose: The current study was conducted in order to identify health and nutritional status of Korean underweight women in their twenties by comparing with normal, overweight, and obese women. Methods: Data from the 2010~2012 Korea National Health and Nutrition Examination Survey were used in this study. Subjects were 897 women aged 20~29 years. Subjects were classified according to underweight, normal weight, overweight, and obese groups based on body mass index. Health status was assessed using data from the health questionnaire and examination. Nutritional status was assessed using data from 24-hour dietary recall and a food frequency questionnaire. Results: The underweight group had higher education and exercised less than the obese group. Means of blood chemistry and blood pressure of the underweight group did not differ from those of the normal weight group. Means of fasting glucose, insulin, lipid profiles, blood cell counts, and blood pressure in the underweight group were different from those of the overweight or obese group. Proportions of delivery, breast feeding, and irregular menstruation in the underweight group were lower compared with the other groups. Except retinol intake, means of nutrient intake did not differ among four groups. There were significant differences among four groups in the intakes of cereals and eggs and frequency of consumption of dairy products. Conclusion: The current findings showed that health and nutritional status of underweight women in their twenties did not differ from those of normal weight women. In particular, health status of underweight women in their twenties was better than that of obese women of the same ages. Further study on health and nutritional status of extremely underweight women is necessary.

Clinical Features of Enuresis in Children with Diabetes Mellitus (소아 당뇨병 환아에서 야뇨증의 임상적 특징)

  • Purpose : Diabetes mellitus (DM) is known as one of the common causes of secondary enuresis in children. However, enuresis in diabetic children is overlooked only as a symptom of polyuria due to hyperglycemia. We evaluated the prevalence of nocturnal enuresis in children with diabetes mellitus in this paper. Methods : Among children with diabetes in three hospitals in Daegu area, 117 agreed to 'Tele research by means of a questionnaire'. Results : Diabetic patients were divided into two groups: Nocturnal enuresis and nonnocturnal enuresis group. thirty-two of 117 (27.0%) patients were in enuresis group, with more daytime urination than non-nocturnal enuresis group ($4.2{\pm}1.6/3.6{\pm}1.2$ times, P=0.016). HbA1c at diagnosis was $12.0{\pm}2.3%/12.0{\pm}2.5%$, and at follow-up $9.3{\pm}2.3%/8.3{\pm}2.3%$ (P =0.042). Insulin was administered at $1.1{\pm}0.5/1.1{\pm}0.4$ units/kg/day. Ten children of enuresis (31.2%) group were monosymptomatic (MNE) and 22 (68.8%) children were non-monosymptomatic enuresis (non-MNE). Fourteen (43.8%) of enuresis group had persistent symptoms, with 5 MNE and 9 non-MNE each. HbA1c at diagnosis was $11.1{\pm}2.5$, $12.4{\pm}2.1$, higher in non-MNE (P=0.144). Average arousal during sleep was step $3.3{\pm}1.2$, $2.5{\pm}1.0$, higher in improved enuresis group (P=0.059). Conclusion : Nocturnal enuresis among DM patients is underestimated. However, considering psychological and social effects of enuresis in children, extensive and long-term studies are needed in the future to clarify relationship between prevalence and DM control.

  • Diabetes is a major social problem in which the incidence and prevalence of diabetes increase every year around the world. Diabetes, characterized by high blood sugar, which increases the concentration of glucose in the blood, causes various symptoms and signs. As a result, patients should strengthen self-management in their daily lives. However, the biggest factor that patients fail in managing blood sugar is diet failure. Therefore, in this paper, an application was designed and implemented to input a diet through photography or image upload and analyze users' diets through intake indicators and blood sugar statistics.
